1.Prelimenary study on pathogenesis of syringoma

Objective To explore the pathogenesis of syringoma in order to cure this disease. Methods The clinical manifestation was observed in 39 cases of syringoma. The serum level of estradiol (E 2), progesterone (Pro), luteinizing hormone(LH) and follicle stimulating hormone (FSH) were detected with radioimmunoassay in all the 30 female cases and 25 normal controls. The expression of estrogen and progesterone receptor (ER and PR) was determined in syringoma tissues using ABC method and anti ER and anti PR monoclonal antibodies. Results Syringoma mainly involved middle aged women. The average level of serum E 2 in female patients was (73?28) ng/ml and its control group, (42?19) ng/ml ( P
2.Effects of different doses of caffeine in the treatment of neonatal apnea

Objective To investigate the therapeutic effect of different doses of caffeine on premature apnea.Methods 80 patients with apnea were divided into observation group and control group,each group in 40cases.The two groups applicated caffeine citrate.The control group received the basal dose, first application of 20 mg/kg, intravenous injection, intravenous injection of 5 mg/kg after 24 h.The observation group was treated with intravenous injection of 10 mg/kg after 24 h.The treatment period was 7 d.The therapeutic effect, blood gas and oxygenation index and adverse reactions were observed in the two groups.Results The effective rate of the observation group was 92.5%,which was significantly higher than 75.0% of the control group(x2=4.501,P<0.05).After treatment, the duration[(9.05±1.82)s vs (13.53±2.63)s]and lost time[(2.52±0.94)h vs (3.42±1.13)h]of apnea in the observation group were longer than those of the control group.The number of seizures[(22.15±4.16) vs (33.53±8.48)]was less than that of the control group,the differences were statistically significant(t=8.767,5.434,9.231,all P<0.05).After treatment, PaO2[(84.74±9.74)mmHg vs (71.46±8.53)mmHg],PaCO2[(42.62±4.15)mmHg vs (48.54±5.18)mmHg]and PaO2/FiO2[(221.52±26.51)mmHg vs (197.76±21.71)mmHg]in the observation group were significantly better than those of the control group(t=7.574,4.765,8.643,all P<0.05).The incidence rate of adverse reactions include tachycardia, hyperglycemia, hypoglycemia and restlessness had no significant difference between the two groups (15.0% vs 10.0%, P>0.05).Conclusion In the treatment of premature apnea, advanced caffeine citrate treatment can better improve the clinical symptoms and blood gas index, the curative effect is reliable.
3.Efficacy of Xiaojin Pill Combined with Docetaxel,Epirubicin and Cyclophosphamide in the Treatment of Breast Cancer in StageⅢand Its Effect of Immune Functions

OBJECTIVE:To investigate the efficacy and safety of Xiaojin pill combined with docetaxel,epirubicin and cyclo-phosphamide in the treatment of breast cancer in stage Ⅲ and its effect on immune functions. METHODS:102 patients with breast cancer in stage Ⅲ were randomly divided into control group (51 cases) and observation group(51 cases). All patients were given symptomatic treatments as hydration,alkalization and antiemetic treatments in the duration of chemotherapy,and were given conven-tional treatments as 0.75 mg Dexamethasome oral tablets in 12 and 6 hours before the chemotherapy lespectively. Control group was given 75 mg/m2 Docetaxel injection,d1,intravenous infusion+60 mg/m2 Epirubicin hyclrochloride for injection,d1,intravenous in-fusion+600 mg/m2 Cyclophosphamide for injection,d1,intravenous infusion;observation group was additionally given 6 g Xiaojin pill,orally,twice a day. 21 d was regarded as a treatment course. Short-term clinical efficacy,immunoglobulin,T lymphocyte sub-sets(CD4+/CD3+,CD8+/CD3+,CD4+/CD8+)and the incidence of adverse reactions before and after chemotherapy in 2 groups after 2 courses were observed. RESULTS:There were no significant differences in the short-term effective rate and incidence of adverse reactions between 2 groups (P>0.05). Before chemotherapy,there were no significant differences in the immunoglobulin and T lymphocyte subsets between 2 groups(P>0.05). After chemotherapy,immunoglobulin levels in 2 groups were significantly lower than before,the differences were statistically significant (P<0.05);but there were no significant differences between 2 groups (P>0.05);CD4+/CD3+,CD8+/CD3+,CD4+/CD8+ in control group and CD8+/CD3+ in observation group were significantly lower than before,and CD4+/CD3+ and CD4+/CD8+ in observation group were higher than control group,the differences were statistically significant (P<0.05),but there was no significant difference in the CD8+/CD3+ between 2 groups (P>0.05). CONCLUSIONS:Based on the conventional treatment,Xiaojin pill combined with docetaxel,epirubicin and cyclophosphamide shows significant effi-cacy in the treatment of breast cancer in stageⅢ,and can improve immune functions,with good safety.
4.Effect of video-assisted thoracoscopic surgery in thoracic disease

Objective To investigate the effect of video-assisted thoracoscopic surgery( VATS) in thoracic disease,and the feasibility to carry out VATS for basic hospital. Methods The data of VATS treatment were collected to compare the differences between study group and control group,and evaluate the the feasibility to carry out VATS for basic hospital. Results The operation time was (100. 75±22. 72) min, blood loss was (54. 27±26. 21) mL,postoperative drainage was (920. 67±171. 99) mL. The postoperative complications were fewer,post-operative hospital stay was shorter,incision time was shorter(P=0. 000) and pain scores was lower(P=0. 000) in study group than that in control group. Basic hospital has the capacity to conduct this technical. Conclusion VATS is feasible to carry out in basic hospital.

Objective To study the relationship between energy metabolic changes tested by phosphorus-31 magnetic resonance(MR)spectroscopy(31P MRS)and the liver damage score(LDS)in rabbit models and investigate the diagnostic value of 31P MRS in acute hepatic injury.Methods A total of 30 rabbits were received different radiation dose(ranging from 5 Gy to 20 Gy)to establish acute hepatic injury models.31P MRS was than carried out 24 hrs after radiation.The rabbits were divided into mild(LDS≤3 U),moderate(LDS 3-6 U)and severe(LDS>6 U)hepatie injury groups.Ten healthy rabbits were served as controls.MR examination was performed on a 1.5 T imager using a 1H/31P surface coil by the 2D chemical shift imaging technique.The relative quantification of phosphomonoesters(PME),phosphodiesters (PDE), inorganic phosphate (Pi) and adenosine severe hepatic injury groups were 1.83±0.33,1.55±0.24,1.27±0.07 and 0.98±0.18,respectively,with significant differences(P<0.05).The ATP level was progressively decreased with the increase of severity Of hepatic injury.The relative quantification of PME and Pi were decreased in severe hepatic injury group compared to control group(P<0.05).There was no significant difference higher in moderate(1.94±0.50)and severe(1.96±0.72)hepatic injury groups compared to control group(1.43±0.31)and mild hepatic injury group(1.40+0.38)(P<0.05).No significant difference was found in ratio of relative quantification of other phosphorus metabolites.Conclusions 31P MRS is a useful method in evaluating acute hepatic injury.The relative quantification of hepatic ATP level,which can reflect the severity of acute hepatic injury,is correlated with LDS.
6.Quantitative Susceptibility Mapping of Brain Iron Deposition in Patients With Recurrent Depression

Objective:
Recurrence is the most significant feature of depression and the relationship between iron and recurrent depression is still lack of direct evidence in vivo.
Methods:
Twenty-one patients with depression and twenty control subjects were included. Gradient-recalled echo, T1 and T2 images were acquired using a 3.0T MRI system. After quantitative susceptibility mapping were reconstructed and standardized, a whole-brain and the regions of interest were respectively analyzed.
Results:
Significant increases in susceptibility were found in multiple recurrent depression patients, which involved several brain regions (frontal lobes, temporal lobe structures, occipital lobes hippocampal regions, putamen, thalamus, cingulum, and cerebellum). Interestingly, no susceptibility changes after treatment compared to pre-treatment (all p>0.05) and no significant correlation between susceptibility and Hamilton Depression Rating Scale were found. Besides, it was close to significance that those with a higher relapse frequency or a longer mean duration of single episode had a higher susceptibility in the putamen, thalamus, and hippocampus. Further studies showed susceptibility across the putamen (ρ2=0.27, p<0.001), thalamus (ρ2=0.21, p<0.001), and hippocampus (ρ2=0.19, p<0.001) were strongly correlated with total course of disease onset.
Conclusion
Brain iron deposition is related to the total course of disease onset, but not the severity of depression, which suggest that brain iron deposition may be a sign of brain damage in multiple recurrent depression.
7.Diagnostic and therapeutic value of endoscopic retrograde appendicitis therapy for atypical acute appendicitis

Objective To investigate the value of endoscopic retrograde appendicitis therapy ( ERAT) in the diagnosis and treatment of atypical acute appendicitis. Methods All the 48 patients suspected of atypical acute appendicitis in Jiangsu Province Hospital from January 2015 to December 2016 were randomly divided into ERAT group and conservative treatment group according to the treatment method. The final appendectomy rate of the two groups was analyzed. Results Only 17 of the 24 patients in the ERAT group received endoscopic treatment because of complex conditions or personal wishes, and 16 cases were diagnosed as acute appendicitis. Surgical resection was performed in 5 cases because of recurrence of the disease after ERAT, and the appendectomy rate was 31. 2% ( 5/16 ) . In the conservative treatment group, all 24 patients were treated with antibiotics. Twenty of them underwent surgical resection with appendectomy rate of 83. 3% ( 20/24) , and 1 of them had appendiceal perforation. The appendectomy rate of the ERAT group was significantly lower than that of the conservative treatment group (χ2=11. 111, P<0. 05) . Conclusion ERAT has a high diagnostic and therapeutic value for atypical acute appendicitis.
8.Therapeutic effect of children's growth patch combined with recombinant human growth hormone in treatment of idiopathic short stature

Objective To investigate the therapeutic effect of children's growth patch combined with recombinant human growth hormone on idiopathic short stature. Methods A retrospective analysis of 45 children patients with idiopathic short stature from February 2016 to February 2017 was carried out, according to the treatment method, they were divided into the control group, the observation group 1 and the observation group 2, with 15 cases in each group. The control group was those who rejected all medications. The observation group 1 was those who were given children's growth patch; the observation group 2 was those who were given growth patch combined with recombinant human growth hormone therapy. The treatment lasted for 12 months. The height growth, genetic height, and predicted adult height were compared. Results The control group, the observation group 1 and the observation group 2 were compared, and the height growth values were (3.87±1.25) cm, (7.93±1.53) cm, and (10.20±1.97) cm, respectively, and the difference was significant (F=59.48, P<0.01); the genetic heights were (156.80±4.94) cm, (156.07±3.97) cm, (156.13±4.58) cm, respectively, and the difference was not significant (F=0.12, P>0.05); the predicted adult heights were (155.73±3.31) cm, (164.80±5.24) cm, and (167.87±5.68) cm, respectively, and the difference was significant (F=25.35, P<0.01). Conclusion Children's growth patch combined with recombinant human growth hormone in the treatment of idiopathic short stature can significantly increase the height growth value and increase the predicted value of adult height.
9.The associations of muscle size and density with handgrip strength and timed up and go test

Objective:The aim of this study was to explore the associations of muscle size and density with handgrip strength(HGS)and the Timed Up and Go(TUG)test.Methods:Totally 301 participants living in the Xinjiekou community near Beijing Jishuitan Hospital were recruited for CT imaging of the hip and a 1-cm slice of the mid-thigh.The cross-sectional area and density of the gluteus maximus and the mid-thigh muscles were estimated by the Osirix viewer based on CT images.HGS and TUG were also performed in these subjects.Logistic regression analysis was used to evaluate the correlations of muscle density and size with TUG and grip strength.Results:In women, after adjustment for age and BMI, the density of the gluteus maximus was negatively correlated with TUG( P trend=0.0366), while the size of the gluteus maximus and the mid-thigh muscles was not correlated with TUG.In men, the density or size of these muscles was not correlated with TUG.After adjustment for age and BMI, the density of the gluteus maximus was positively correlated with grip strength( P trend=0.0334)and the size of the mid-thigh muscles was also positively correlated with grip strength( P trend=0.0155)in men, but they were not correlated with grip strength in women. Conclusions:There were sex differences in the relationship between muscle size or density and grip strength or timed up and go.The density of the gluteus maximus is associated with muscle strength and physical performance while the size of the mid-thigh muscles is correlated with muscle strength.
