1.Clinical study of tegaserod in treatment of chronic constipation,dyspepsia and reflux overlapping symptoms

Objective To evaluate the efficacy of tegaserod in treating overlapping symptoms of chronic constipation and dyspepsia or reflux.Methods Eighty eight patients with overlapping symptom were enrolled and randomly divided into:tegaserod group(TEG group,received tegaserod 6 mg bid), PPI group(esomeprazole 40 mg qd)and combined group(tegaserod 6 mg bid and esomeprazole 40 mg qd).Esomeprazole was taken 30 min-prior to the meals in the morning.Each group was treated for 4 weeks.Endpoints to evaluate the clinical efficacy including complete relief rates(CRRs)of epigastric symptoms,the total scores of gastrointestinal symptoms,gastric emptying and colonic transit time and gastric sensation.Results The total scores of gastrointestinal symptoms were all significantly decreased in three groups after four-week therapy,which were more decreased markedly in TEG group(7.23?3.13)and the combined group(5.13?2.26)than that in PPI group(13.58?2.02,P

Objective To investigate the role of Treg/CD4~+ T in the pathogenesis and prevention of bowel dysfunction after acute enteritis in rats.Methods Studies were conducted in 40 female Wistar rats with weights of 170-180 g.The rats were divided into normal control,experimental control,antibi- otics-treated,probiotics-treated and combined treated groups.The latter 4 groups were first infected with shigella flexneri by garage to establish the model of acute infectious enteritis,then normal control and experimental control group were administered 1 ml saline once everyday for 10 days.Antibiotics- treated group was given gentamycin 1 ml (8.32 mg?kg~(-)?d~(-1)) for 3 days,then was given saline 1 ml once everyday for 7 days.Probiotics-treated group was given medilac-S 1 ml (52 mg?kg~(-1)?d~(-1)) by gavage for the first 7 days,and 1 ml saline for the latter 3 days.The combined treatment group was given gentamycin for the first 3 days and medilac-S for the latter 7 days using the same dose.All rats were given colon-rectal balloon distention 16 to 22 days after the occurrence of bowel inflammation.The contraction of enteric smooth muscle was measured in vitro,the proportion of Treg cells in blood and mesenteric lymph node were analyzed by flow cytometry,and the histological changes were also observed at the same time.Statistical analyses were performed by using t test or one way ANOVA. Results No obvious changes of distal colon were histological observed in different groups.The perceptual threshold of rat gut decreased and the tension integral of enteric smooth muscle increased,and the ratio of Treg/CD4~+ T in blood and mesenteric lymph node was significantly lower in experimental control group [(4.40?0.43)%;(2.47?1.36)%] than normal controls [(5.15?0.47)%;(4.01?1.12)%,P<0.05],while that of combined treatment group had no difference from normal controls [(4.95+0.41)%;(3.22?1.86)%].Conclusions The ratio of Treg/CD4~+ T may play an impor- tant role in the pathogenesis of post-infectious bowel dysfunction in animals.Treating acute enteritis with both antibiotics and probiotics can normalize the abnormalities,and thus prevent the occurrence of bowel dysfunction.
3.Clinical study on treatment of chronic superficial gastritis with jinghua weikang capsule.

OBJECTIVETo observe the clinical efficacy and safety of Jinghua Weikang Capsule (JWC) on chronic gastritis (CG).
METHODSNinety patients with CG were randomly divided into 3 groups: the control group treated with domperidone 10 mg, 3 times per day orally, the high and low-dose JWC groups treated with JWC 160 mg and 80 mg respectively 3 times per day. The therapeutic course for all groups was 14 days. Dyspeptic symptom score, gastric emptying rate and quality of life (QOL) in patients were observed.
RESULTSDyspeptic symptom score was 19.97 +/- 1.85 and 24.40 +/- 1.85 after treatment in high and low-dose JWC groups respectively, lower than that before treatment (28.33 +/- 1.88 and 27.70 +/- 1.68, P <0.05); QOL improved obviously in all the three groups, among which the high-dose JWC group showed the best effect. The adverse reaction of JWC was very mild.
CONCLUSIONJWC is an effective and safe drug for alleviating dyspeptic symptoms in patients with CG, its effect is more evident when high dose is applied.

4.Genome Shuffling and Its Prospect for Strain Improvement in Ethanol Production from Lignocellulosic Hydrolysates

Commercial production of bioethanol from lignocellulosic hydrolysates requires efficient fermenting strains. The abilities of the strain to converting all types of sugars in the hydrolysate to ethanol in high yield and to effectively tolerating/metabolizing inhibitors are necessary. Genome shuffling is a novel method for breeding, and it has been applied in pharmaceutical and food industry. This review summarized the technique of genome shuffling including principle, process, applications and its prospect for strains improvement in ethanol production from lignocellulosic hydrolysates.
5.Clinical effects of community-based rehabilitation on chronic schizophrenia

ObjectiveTo explore the effect of community based rehabilitation on chronic schizophrenia. Methods60 patients of chronic schizophrenia were randomly divided into the community based rehabilitation group (the study group) and the inpatients group (the control group). The study used prospective design for 1 year with brief psychiatric rating scale (BPRS),nurses' observation scale for inpatients evaluation (NOSIE) and social disability screening schedule (SDSS).ResultsCompared with the control group at the end of 6 month and 1 year, scores of BPRS, NOSIE and SDSS in the study group were significantly different (P<0.05-P<0.001). The relapse rate of the study group (0%) also lowered than that of the control group( 20%).ConclusionsCommunity based rehabilitation therapy can control the chronic schizophrenia effectively. It also promotes the life quality and social function of patients, and lowers the relapse rate significantly. It is an important rehabilitation method for chronic schizophrenia.
6.Efficacy, side effects and blood concentration monitoring of high-dose methotrexate in treatment of 180 children with acute lymphoblastic leukemia.

This study was purposed to investigate the effects of high-dose methotrexate (HD-MTX)-CF + VDT protocol on pediatric acute lymphoblastic leukemia (ALL) by means of retrospective analysis. MTX plasma concentration was dynamically detected and evaluated so as to avoid or reduce the side effects of HD-MTX, and adjust the time and dosage of calcium folinate (CF) or carry out the plasma exchange as occasion requires. Totally 180 cases of ALL were enrolled in this study, and received 380 administration of HD-MTX-CF + VDT protocol, including 122 patients with induction therapy as well as 58 cases during maintenance therapy, among which 68 cases were defined as low risk, 80 cases as middle risk and 32 cases as high risk. 2.0 g/m(2) MTX, 3.0 g/m(2) MTX, and 5.0 g/m(2) MTX were individually used according to low risk, middle risk or T immunohistochemical expression. The results indicated that 36.3% cases showed the side-effects of HD-MTX including mucocutaneous lesions, gastrointestinal reaction, hepatic dysfunction, renal damage, fever, myelosuppression, cardiotoxicity, infection and allergic response. All of these side effects were reversible through treatment. The elimination delay of MTX occurred in 110 cases, out of which 3 cases got MTX concentration > 10 µmol/L at 24 hours, 50 cases > 1.0 µmol/L at 44 hours, the remaining 57 cases > 0.1 µmol/L at 68 hours. CF dosage was adjusted according to the concentration of MTX until it was less than 0.1 µmol/L. 1 case had renal interstitial inflammation and acute renal failure, but finally he was cured. No patients received plasma exchange or died. It is concluded that the extramedullary leukemia control protocol, in which MTX is main drug, is effective therapy for obtaining long-term remission and event-free survival rate in ALL patients, but the side effects and risks increase along with the increase of MTX dose. The metabolic level of HD-MTX has found to be obvious individual, so the dynamic monitoring of MTX concentration in plasma and administration of proper dosage of CF are important factors for HD-MTX protocol application in ALL patients.

7.Analysis of 1125 syphilis cases.

OBJECTIVETo summarize the epidemiological, clinical and serological features of syphilis.
METHODThe clinical data of 1125 patients with syphilis were retrospectively analyzed.
RESULTSNon-marital sexual intercourse is the main route of infection. The 129 cases of primary syphilis were featured by extragenital chancres and the 357 cases of secondary syphilis were featured by scaling macules on the palms and soles and condylomata lata. Early latent syphilis were detected in 178 cases, late latent syphilis in 174 cases, and latent syphilis of unknown duration in 285 cases. Concurrent infection with other sexually transmitted diseases were noted in 107 cases (9.51%). Most patients were treated with benzathine penicillin, and only 31 patients (2.75%) were allergic to this therapy. Except for the primary syphilis, the sensitivity of syphilis serological tests (including rapid plasma regain test, treponema pallidum particle agglutination, treponema pallidum hemagglutination assay, and fluorescent treponemal antibody-absorption) were higher than 97%.
CONCLUSIONSThe incidence of latent syphilis is high, while the incidence of late syphilis is low. Penicillin remains the treatment of choice for syphilis.

8.Expression of c-Cbl, Cbl-b, and epidermal growth factor receptor in gastric carcinoma and their clinical significance.

BACKGROUND AND OBJECTIVEc-Cbl and Cbl-b are two ubiquitous members of the Casitas B-lineage lymphoma (Cbl) family, which play important roles in the downregulation of epidermal growth factor receptor (EGFR) by acting as E3 ubiquitin ligases and multiadaptor proteins. This study investigated the expression of c-Cbl, Cbl-b, and EGFR in gastric carcinoma and its clinical significance.
METHODSThe expressions of c-Cbl, Cbl-b, and EGFR were detected by immunohistochemistry using tissue microarrays consisting of 124 specimens of gastric carcinoma and 16 specimens of normal gastric mucosa. The relationship between the expressions of c-Cbl, Cbl-b, and EGFR and clinicopathologic factors of gastric carcinoma were analyzed statistically.
RESULTSThe positive rates of c-Cbl, Cbl-b, and EGFR were higher in the gastric carcinoma group than in the normal group (71.0% vs. 18.0%, P<0.01; 82.3% vs. 25.0%, P<0.01; 56.5% vs. 12.5%, P<0.01, respectively). The expression of c-Cbl was positively correlated with depth of invasion (r=0.219, P=0.015), and TNM staging (r=0.266, P=0.003). The expression of Cbl-b was positively correlated with lymph node metastasis (r=0.190, P<0.034) and TNM staging (r=0.298, P<0.001). The expression of EGFR was positively correlated with depth of invasion (r=0.286, P<0.001) and TNM staging (r=0.362, P=0.000). The expression of both c-Cbl and Cbl-b was positively correlated with EGFR (r=0.241, P=0.007; r=0.183, P=0.042, respectively). Synchronous strong-positive expressions of c-Cbl, Cbl-b, and EGFR were observed in 27 specimens of gastric carcinoma, most of which were at advanced stage.
CONCLUSIONSOverexpressions of c-Cbl, Cbl-b, and EGFR are closely related to the invasion and progression of gastric carcinoma. c-Cbl and Cbl-b may serve as novel molecular markers for gastric carcinoma.

9.Determination of three constituents in Polygoni multiflori Radix Praeparata in plasma of atherosclerosis rats

AIM To establish an HPLC method for determining the contents of three constituents in Polygoni multiflori Radix Praeparata in plasma of atherosclerosis rats.METHODS After the rats were intragastrically administrated with Polygoni multiflori Radix Praeparata CMC-Na solution,the plasma was collected.The HPLC analysis was carried out on a 30 ℃ thermostatic Hypersil C1s column (250 mm ×4.6 mm,5 μm),with the mobile phase comprising of acetonitrile-0.03% phosphoric acid flowing at 0.9 mL/min in a gradient elution manner,and the detection wavelength was set at 280 nm.DAS2.0 software was applied to drawing concentration-time curves and calculating pharmacokinetic parameters.RESULTS Stilbene glucoside,emodin and physcion showed good linear relationships within the ranges of 61.25-6 125 μg/L (r =0.999 8),12.6-3 150 μg/L (r =0.999 3) and 24.1-6 030 μg/L (r =0.999 5),respectively.The method recoveries were 99.5%-105.8% with the RSDs of 1.3%-3.3%,while the extraction recoveries were 87.2%-96.3% with the RSDs of 3.2%-5.9%.The pharmacokinetic behaviors of three constituents all accorded with two-compartment model,but their contents in plasma were much lower than those in medicinal material.CONCLUSION The bioavailabilities of stilbene glucoside,emodin and physcion are relatively low in plasma of atherosclerosis rats,which may be related to constituents' intestinal absorption after intragastric administration with Polygoni multiflori Radix Praeparata.
10.The study of the expression and the prognostic value of Survivin and Ki67 in pancreatic endocrine tumors.

OBJECTIVETo explore the relationship between the expression of Survivin and Ki67 with prognosis of pancreatic endocrine tumors (PETs).
METHODSImmunohistochemistry for Survivin and Ki67 was performed in 25 cases of normal pancreatic tissues and 81 cases of PETs by tissue microarrays and to observe the expression and evaluate the relationship with prognosis.
RESULTS(1)The expression of Survivin and Ki67 in PETs was significantly higher than that in normal pancreatic tissues (P <0.01); (2)The expression of Survivin and Ki67 in PETs was correlated with tissue grading and the TNM-staging (P < 0.05), but not related with tumor size, location and functional status. In addition, the expression of nuclear Survivin was association with lymph node metastasis (P < 0.05). (3)The high expression of Ki67 was related with the expression of nuclear Survivin, but not related with the expression of cytoplasmic Survivin.
CONCLUSIONSurvivin and Ki67 were both expressed in PETs, which were closely related to the clinical pathological characteristics. They could be used as new indicators in the evaluation of prognosis of PETs. The expression of Survivin in nucleus had more diagnostic significance than that in cytoplasm, and that could be highly correlated with lymph node metastasis, which would be used as a new marker of poor prognosis.
