6. Silybin-derived flavonolignans and their structure-activity relationships: Research advances

Silybum marianum (L.) Gaertn. is recorded by the pharmacopoeias of many countries to have such bioactivities as antioxidant, anti-hepatocirrhosis, anti-inflammation, immunomodulation and liver regeneration. It has been used clinically to treat hepatitis, fatty liver, liver cirrhosis, ischemic damage, radiation injury, etc. With silybin entering the phase II clinical trial as an anti-cancer drug, its correlative investigations including derivatives preparation, structure-activity relationship (SAR) detection, and the detailed action mechanisms have been developing rapidly. Recently, the new synthetic strategy of silybin and dehydrosilybin (DHS) along with their anti-HSV (herpes simple virus) activity has also been reported. This paper reviews about one hundred preparative derivatives of silybin and DHS which appeared in recent years, with the emphasis on their pharmacological activity and the results of their SAR. The future development direction of structural modification of silybin and DHS is also discussed and predicted.
7.Comparison of clinical effect on locking plate for proximal humeral fracture with or without application of inferomedial screws.

OBJECTIVETo compare clinical outcomes of locking plate for proximal humeral fracture whether application of inferomedial screws.
METHODSFrom January 2012 to July 2013, 46 patients with proximal humeral fracture underwent locking plates were retrospectively analyzed. There were 25 males and 21 females aged from 29 to 80 years old with an average of 55.1 years old. Among them, 25 patients were treated with inferomedial screws (support group), including 13 males and 12 females aged from 38 to 80 years old with an average of (55.8 ± 11.8) years old; 8 cases were part two fracture,10 cases were part three fracture and 7 cases were part four fracture according to Neer classification. Twenty-one patients were treated without inferomedial screws (non-support group), including 12 males and 9 females aged from 29 to 79 years old with an average of (54.2 ± 14.8)years old; 6 cases were part two fracture, 9 cases were part three fracture and 6 cases were part four fracture according to Neer classification. Operative time, fracture healing time and complications were observed and compared, Neer scoring of shoulder joint were used to evaluate clinical effect.
RESULTSAll patients were followed up from 12 to 41 months with an average of 15.6 months. Operative time and fracture healing time in support group was (1.6 ± 0.4) h and (3.0 ± 0.6) months, and (1.5 ± 0.4) h and (3.1 ± 0.6) months in non-support group, while there was no statistical difference in operative time and fracture healing time between two groups. There was significant differences in Neer score between support group (89.7± 4.9) and non-support group (83.1 ± 7.1). No complication occurred in support group,while 4 cases occurred complications in non-support group.
CONCLUSIONLocking plate with inferomedial screws for proximal humeral fracture has advantages of stable fixation, less complications, quick recovery of function and satisfied clinical effect.

10.Effect of BRL-37344 on ion channel in guinea pig cardiac myocytes

Objective To study the biological effect of ?3 adrenoceptor-activated electrophysiological activities in cardiac myocytes. Methods Changes of transient outward potassium ( Ito) and L-type calcium current ( ICa-L) in primary cultured guinea pig cardiacmyocytes were detected with the standard whole-cell patch clamp technique about 5 -10 min before and after administration of ?3 adrenoceptor agonist,4--2-hydroxy-( 3-chlorophenyl) ethyl-aminopropylphenoxyacetate ( BRL-37344; BRL). Results BRL could significantly increased the Ito in guinea pig cardiac myocytes with the I-V curve up-moved. When the membrane potential reached to + 80 mV,the density current increased from 6. 11 ? 1. 03 pA/pF to 8. 46 ? 2. 07 pA/pF ( n = 6,P =0. 013 064). BRL ( 10 -6mol/L) could significantly increase the ICa-Lin guinea pig cardiac myocytes,which was ( 2. 30 ?0. 75) -fold higher than in control group( n =5,P =0. 0063).
