1.Primary total hip arthroplasty using the modular S-ROM prosthesis for the treatment of severe developmental dysplasia of the hip

[Objective]To research short-term results of primary total hip arthroplasty using the cementless modular S-ROM prosthesis for the treatment of severe developmental dysplasia of the hip.[Method]Eighten patients(19 hips) with severe developmental dysplasia of the hip underwent primary total hip arthroplasty using the S-ROM prosthesis.Of the patients,6 were male and 12 were female.The average age was 32 years.According to the Crowe classification system,there were 11 hips in type Ⅲ and 8 hips in type Ⅳ.The acetabulums were deepened at the level of the true acetabulum.The S-ROM modular stems were used on the femoral side.Subtrochanteric shortening osteotomy was performed when it was difficult to reset.The Harris hip score(HHS) and radiography were used to evaluate the hip function preoperatively and at the follow-up.[Result]One proximal femur tissure fracture occurred,and was bundled up with circle wire.There were 2 postoperative sciatic nerve irritation,which disappeared completely within 3 months.All patients were available for follow-up.There was one heterotopic ossification above greater trochanter.But the mobility of the hip was normal.There was no loosening or infection during the follow-up.Average follow-up period was(2.8) years(range,6 months-5 years).The HHS increased from 38.4 preoperatively to 85.2 at the time of final follow-up.[Conclusion]S-ROM is a perfect choice for the teatment of severe developmental dysplasia of the hip.Short-time clinical results were excellent.
2.Effect of S100A8/A9 Protein Complex on F-actin Network in Human Cervical Carcinoma Cell Line,CasKi Cells

Objective To investigate the effect of S100A8/A9 protein complex on the surface morphology and the F-actin network in human cervical carcinoma cell line,CasKi cells.Methods After being cultured with 20 ?g/ml S100A8/A9 protein complex,the cell skeleton of the CasKi cells were observed under a confocal scanning fluorescence microscope by staining the F-actin network.Atomic force microscopy(AFM) was employed to reveal the change of ultrastructure of the cell surface in vivo.ResultsAfter being cultured with the S100A8/A9 protein complex for 24 hours,the F-actin network disorder was revealed.Most of the F-actins distributed peripherally.The OD value of the F-actin decreased significantly from 92.42?5.16 to 57.67?3.70 after been treated with the S100A8/A9(t=5.268,P=0.000).The AFM showed a withdrawing morphology with reduced pseudopodia and destruction of stress fibers. Conclusion S100A8/A9 protein complex can change the ultrastructure of the surface of CasKi cells and its stress fibers by re-distributing of the F-actin in the cells.
3.Serum level of MMP-3 and the expressions of VEGF-C and MMP-3 in gastric cancer

Objective: To investigate the level of MMP-3 in the serum of gastric cancer pa-tients and to examine the expression of VEGF-C in gastric cancer, to evaluate the effects of the two factors on the development process of cancer. Methods: The enzyme linked immunosorben assay (ELISA) method was used to detect the serum levels of MMP-3 in 50 patients with gastric cancer. The immunohistochemical method was utilized to analyze the expressions of VEGF-C and MMP-3 in 50 resected specimens. 4.Treatment of knee arthropathy with rotating-platform knee prosthesis in 62 cases

OBJECTIVE: To study the therapeutic effect of treating knee arthropathy with total knee arthroplasty using rotating-platform knee prosthesis. METHODS: A total of 62 patients (74 knees) in Provincial Hospital Affiliated To Shandong University from January 2005 to January 2009 were selected, including 36 males (41 knees) and 26 females (33 knees), aged 52-79 years, mean aged 61.5 years; 28 cases with left knee, 22 cases with right knee and 12 cases of both knees. The cases were composed of 34 cases with osteoarthritis, 21 cases with rheumatoid arthritis and 7 cases with traumatic arthritis. All the patients received the total knee arthroplasty for the first time using rotating-platform knee prosthesis, no replacement of the patella. The clinical results of pre-operetion and after-operation were evaluated by the HSS knee rating score system. RESULTS: All the patients were followed-up for averaged 22 months (3-48 months) after operation. Incisions of all the cases were healed, and there was no infection and hematoma. Lower extremity deep venous thrombosis occurred in 2 cases at 3 days after operation. Swelling disappeared through drug treatment 10 days later. Pain and function of all the patients were obviously ameliorated after operation. The average HSS knee rating score was 44.99 (from 34 to 58) before total knee arthroplasty, which was improved to 88.78 (from 69 to 98), and the good-to-excellent rate was 98% (excellent 45 cases, good 16 cases, average 1 case). There were no signs of prosthetic dislocation or loosening in the X-ray films. CONCLUSION: Total knee arthroplasty using rotating-platform knee prosthesis owns advantages of easy surgical operation, tess complications, good recovery and satisfactory results in a short-term.
5.Quality of life following radical gastrectomy in patients with gastric carcinoma

Objective:To compare the quality of life following total gastrectomy(TG) with that folliwng subtotal gastrectomy(SG) in patients with gastric carcinoma.Methods:Sixty-six patients underwent gastrectomy between 2003 and 2005 at the Department of General Surgery of the Qilu Hospital were enrolled into this study.Among them,15 underwent TG,42 accepted distal end subtotal gastrectomy(DSG) and 9 underwent proximal end(PSG) .The EORTC quality of life questionnaire QLQ-STO20 was used in the present retrospective study to assess follow quality of life of thes patients.The food-intake,local symptom and quality of life of the 3 groups were compared.Results:Patients recovered worse in short term after TG compared with PSG,but there was no statistical difference in the near future.But both of them were not as good as DSG.The contraflow was common and serious after PSG.The incidence rate of abdominal distension was high.Conclusion:The indication of TG for carcinoma of the superior part of stomach can be extented properly.Evidence-Based Medicine should be used to screen better reconstruction mode.On the base of radical cure,quality of life should be improved as far as possible.
6.Repair of cartilage defects using chondrocyte and osteoblast composites in vitro cultured in bioreactor

Objective To assess the feasibility of chondrocyte and osteoblast composites in vitro cultured in bioreactor in repairing cartilage defects.Methods Marrow mesenchymal stem cells were isolated and cultured in vitro,and then were induced to chondrocytes and osteoblasts by growth factor.Chondrocytes and osteoblasts were cocultured in bioreactor for 21 days to form the composites.The adhesion,extension and proliferation of chondrocytes and osteoblasts were observed under scanning electron microscope.The cartilage defects on canine model were repaired with the chondrocyte and osteoblast composites.Results The induced chondrocytes and osteoblasts had fine adhesion,extension and proliferation in the β-TCP scaffold.The repaired tissues in experimental group maintained their thickness to the full depth of the original tissues.A statistical difference was observed between negative control group and experimental group(q=12.337 0,P < 0.01)and between blank control group and experimental group (q=31.539 3,P <0.01).Conclusion Perfusion bioreactor makes chondrocyte and osteoblast survive and proliferate in a three-dimensional scaffold and increases the composition rate of the chondrocyte and osteoblast.
7.Bicon-threaded cup for treating hip rheumatoid arthritis in 24 cases

OBJECTIVE:To observe the clinical effect of Zweymüller bicon-threaded cup in total hip arthroplasty for treating hip rheumatoid arthritis.METHODS:A retrospective analysis was performed on 24 cases (39 hips) of rheumatoid arthritis selected at the Department of Joint surgery of the Affiliated Provincial Hospital of Shandong University,including 8 males (14 hips) and 16 females (25 hips),aged from 32 to 64 years (average 49.2 years old),course of disease being from 4 to 15 years (average 8.5 years).All the cases the state of wound healing and perioperativ complications were examined,and the position of acetabulum prosthesis was observed by X-ray film.RESULTS:All patients were followed up,and the follow-up time ranged from 14 to 62 months (average 37.5 months).They were all obtained first-stage wound healing,and two cases of gastrocnemius vein thrombus were found among them,occurred by non-operation.All patients were free from pain with stable fixation and mobility increasing.The process of osseo integration being well,the radiolucent line and radiographic loosening of acetabulum prosthesis and sedimentation of stem were not found on antero-posterior pelvis-radiograph and lateral radiograph of ipsilateral hip.The Harris hip score increased from preoperative 33.5 points (17-46 points) to postoperative 86.5 points (79-96 points) at 3 months.There were no complication appeared such as dislocation,fracture and nerve injury.rheumatoid arthritis.
8.Clinical application of rotating platform knee prosthesis in the total knee arthroplasty

Objective To research clinical application of rotating platform knee prosthesis Gemini MK II in the total knee arthroplasty. Methods From August 2010 to July 2013, 87 patients (106 knees) underwent total knee arthroplasty using rotating platform knee prosthesis Gemini MK II. The average ages of the cases were 65.3 years (52-81 years). The clinical results were evaluated by the HSS knee rating score system as well as X-ray films. Results The patients were followed up for mean 24.2 months (7~40 months). The pain caused by the illness of knee obviously alleviated and the function of knee significantly improved. The complications such as osteolysis around the prosthesis, prosthesis loos-ening and dislocation of meniscus bearings were not found. The HSS knee rating score was (51.5±9.7) before total knee arthroplasty, which improved to (87.3±6.4), The difference was significant(P<0.01). Conclusion There are satisfactory results in a short time for rotating platform knee prosthesis Gemini MK II in the total knee arthroplasty. A long term follow-up is obviously necessary to make more precise statement about the knee replacement system.
9.Clinical results of 100 cases of modified eversion carotid endarterectomy

Objective:The primary goal of this study is to explore the safety and effectiveness of a new modified eversion carotid endarterectomy (MECEA).Methods:This is a retrospective case series study. One hundred patients were consecutively treated with MECEA by the same operator at Department of Vascular Surgery,Peking Union Medical College Hospital from January 2019 to December 2023. There were 77 males and 23 females. The age was (66.0±8.6)years (range: 39 to 85 years). Twenty-four (24.0%) patients were symptomatic with the degree of carotid stenosis over 50%,76 patients (76.0%) were asymptomatic with the degree of stenosis over 70%. All these patients meet the indication of carotid endarterectomy. The main difference between MECEA and traditional eversion carotid endarterectomy was the anterior,lateral,and posterior walls of the internal carotid artery were incised obliquely from the origin of the internal carotid artery toward the common carotid artery,leaving the wall of internal carotid artery intact at the bifurcation. The surgical process,cardiovascular and cerebrovascular complications and other surgical complications were recorded. The incidences of complications,restenosis of intraoperative target lesions and re-intervention were collected during follow-up.Results:All procedures were performed successfully under general anesthesia. The total operation time was (36.5±10.1)minutes (range: 22 to 65 minutes),and carotid clamping time was (15.0±6.3)minutes (range: 7 to 31 minutes). One patient (1.0%) occurred postoperative cerebrovascular accident,1 patient (1.0%) developed cerebral hyperperfusion syndrome (CHS),and another 1 patient (1.0%) suffered myocardial infarction. All these patients were recovered after medical treatment within a week. The follow-up time( M(IQR)) was 24 (28) months (range: 6 to 62 months). Two patients (2.0%) were reported to have hemodynamically significant restenosis within 2 years,with one patient requiring intervention. No patient suffered from ipsilateral ischemic stroke. Conclusions:MECEA is a safe and effective surgical method of treating carotid artery stenosis. This method can reduce carotid clamping time and lowers the risk of ischemic stroke. Meantime,it preserves the integrity of the adventitia at the bifurcation of carotid artery,reduces the chance of restenosis. Moreover,it might be helpful to prevent postoperative CHS due to reducing damage to the carotid body and carotid sinus nerve.
