1.Application of PET-CT in efficacy evaluation of neoadjuvant chemotherapy for breast cancer

Objective To evaluate the value of PET-CT in patients with breast cancer and to determine if the PET-CT can provide additional information to predict early response to neoadjuvant chemotherapy (NAC).Methods NAC was given to 20 patients with breast cancer confirmed by biopsy puncture from September 2009 to March 2012.The PET-CT was carried out for all patients before NAC.TEC program with three weeks for one cycle was selected.After 6 days of the first cycle,the PET-CT was performed again.The changes of standard uptake value before and after the first cycle were compared.At the same time hand palpation was selected to detect the changes in tumor size before and after the first cycle of NAC.The changes of the standard uptake value and in tumor size need to refer to the pathology Miller & Payne classification methods to evaluate the efficacy of the NAC.Results The SUV were (7.51±1.76) Bq/ml and (4.98±1.61) Bq/ml before and after chemotherapy (t =7.916,P < 0.05) the maximum tumor diameters were (9.62±4.38) cm and (8.89±4.08) cm before and after NAC (t =2.154,P> 0.05).SUV had highly correlated with pathological MP classification (r =0.725,P =0.000); while for the tumor size there was no significant change (r =0.026,P =0.824).Conclusion PET-CT can predict the efficacy earlier and is more accurate than clinical efficacy standard for the NAC.
2.A cross-sectional survey and analysis of hospital infection in Pukou Hospital in 2012

Objective To investigate and analyze the prevalence rate as well as antibiotics use in this hospital and to improve the awareness of the hospital infection administration for the medical staff .Methods The hospital infection prevalence rate in hospital and the risk factors were analyzed by prevalence survey and bed-side and case records investigation .Results In all 320 patients sur-veyed ,25 patients were diagnosed with hospital infection and the rate was 7 .81% ;27 cases of hospital infection was reported and the cases prevalence rate was 8 .44% .The main infection site was respiratory tract and it accounted for 48 .14% ,moreover ,the in-tensive care unit ,urology ,and neurology were the high incidence departments .The day of usage rate of antimicrobial drug was 62 .50% ,prevention of drug use was 46 .50% ,a kind drug use was 74 .00% ,and the two kind drug uses was 22 .00% .The positive rate of hospital infection pathogenic detection was 46 .87% .Hospital infection rate was related to the age ,urinary tract intubation , and ventilator factors .Conclusion Hospital infection administration should be strengthened to reduce invasive procedures for key depart-ments and focus groups ,meanwhile ,the rational application of antibiotics should be regulated to reduce hospital infection rates .
3.Establishment and implementation of disease oriented integrated curriculum system for stomatology

The traditional discipline-centered curriculum design can neither keep up with developments of modern medical science nor reach requirements of the education reform in the new century.Since 2011,College of Stomatology in School of Medicine in Shanghai Jiao Tong University had developed ‘ disease oriented integrated curriculum system reform’ for students of long-term stomatology education.In view of the problems existing in the original curriculum system,the integrated curriculum system was set up by coalescing clinical medicine curriculum according to the related systems and oral medicine curriculum according to the developmental rules of diseases.Lectures were combined with discussion classes in the reform and performance appraisal system was changed from simplex judgments into comprehensive evaluations.At last,further considerations of promoting the reform based on the practice were proposed.
4.The relationship between serum leptin and fibrosis in patients with chronic hepatitis B incorporate alcoholic liver disease

Objective To study the relationship between serum leptin and fibrosis in chronic hepatitis B incorporate alcoholic liver disease(CHB + ALD) patients.Methods Select 17 male patients with CHB+ALD,15 male patients with liver cirrhosis(LC) ,19 male patients with chronic hepatitis B(CHB) and 12 male healthy persons as normal control(NC).Serum leptin is detected by ELISA in 63 patients.Serum levels of fibrosis markers(HA,LN,PC HI ,IV-C) are determined at the same time.Results Serum leptin level in CHB +ALD are significantly higher than those of the NC(6.79±24.12) μg/L vs (4.27±7.18 ) μg/L (P<0.05).There is no significant difference between patients with CHB and the NC.The four fibrosis markers in CHB + ALD patients are significantly higher than that of men both in the CHB and the NC.There are significant correlations between both serum leptin level and quadrinomial fibrosis markers(P<0.05 or P<0.01 ).Conclusion That serum leptin level is one aecelerate gene in the fibrosis formation in patients with CHB+ALD.
5.Tensile mechanical properties of human tracheal cartilage

BACKGROUND:Tracheal injury suture and the development of a new tracheal prosthesis all need to understand the tensile mechanical properties of tracheal cartilage.so as to repair and rebuild tracheal function.Previous researches on the biomechanics of artificial trachea are many reported.while the biomechanics of human tracheal cartilage is reported less.OBJECTIVE:To investigate the mechanical properties of tracheal cartilage using one-dimensional tensile test method.METHODS:Two fresh cadaver specimens of normal human trachea.with the informed consents of their families.were involved.The specimens were thaw at room temperature,and then tracheal cartilage specimens were cut using scalpel into 20 samples at the length of 25 mm,width 5 mm,thickness 1.8-2.2 mm.The 20 tracheal cartilage samples were subjected to one-dimensional tensile test with Shimadzu electronic universal testing machine Japan,at the tensile test speed of 5 mm/min.The tensile maximum load,maximum displacement,maximum stress,maximum strain,elastic modulus and stress-strain curve of the specimens were observed.RESULTS AND CONCLUSION:The maximum load of human cadaver tracheal cartilage was(60 946±10.377)N,maximumdisplacementwas(1.973±0.159)mm,maximum stresswas(6.229±1.125)Mpa,maximum strainwas(32.825±2.776)%.Tracheal cartilage stress-strain curves was changed along with the index,the initial low slope of the curye was due to the direction of imposed tension was similar with the arrangement of collagen structure,the steep slope represented the tensile strength of collagen.To describe the tracheal cartilage stress-strain relationship in a one-dimensional tension,15 stress-strain data of the tracheal cartilage experimental data adopted polynomial by the least square fitting method,to obtain stress(δ)-strain(ε)relationship forrnula:σ(ε)=-0.111 3e~5+1.602 1e~4-7.821 6e~3+17.995 1e~2+3.624e.It is indicated that tracheal cartilage has a strong capacity to bear Joad and resist deformation,reflecting their viscoelastic mechanical properties of both flexibility and viscosity,supporting that the mechanical properties of cartilage is positively related to cartilage collagen content.

7.Construction of a murine model of chronic graft-versus-host disease by haplo-identical spleen cell infusion

Objective For providing experimental platform of chronic graft-versus-host disease (cGVHD),to establish a mouse model by haplo-identical spleen cell infusion.Methods The donor male mice (Balb/cH-2d) and the recipient (Balb/c C57BL/6) F1 H2-d/b (CB6F1) female mice were randomly divided into four groups:3 experimental groups injected with 3 107,6 107 and 9 107 spleen cells,respectively,while the control group received RPMI 1640 solution.H-2d and H-2b were checked to analyze the chimerism in bone marrow cells.Body mass,figure,cutaneous manifestation and survival of recipient mice were observed and scored every 3 days.Pathologic changes of target organs were observed and scored.Results Injection of 6 107 and 6 107 splenocytes in the recipient mice resulted in a chronic disease with a low level of parental cell engraftment steadily.As compared with 3 107 group,the incidence of cGVHD in 6 107 and 9 107 groups were significantly increased (P <0.01).But there was no significant difference between 6 107 and 9 107 groups (P>0.05).Conclusion A murine model of cGVHD after haplo-identical spleen cell infusion of donor is successfully established by injection of 6 107 and 9 107 spleen cells.

Objective To study mental status of recovered two year after in shake area crowd of gansu wen-xian. Methods Fifty former disaster crowd were assessed with SCL-90, Coping Style Questionnaire and Social Support Questionnaire two year after their discharge, 50 healthy medical staffs of gan-su CTM Affiliated Hospital of logistics ensure center were assessed with same instruments as control. Results The former disaster crowd and 5 subscores of SCL-90 with higher average scores than Chinese norm and results of control, they were somatization( F =4. 31) , phobia ( F = 5. 25) , obtrude ( F =5. 91) , P<0. 01, depression ( F = 3. 11) anxiety ( F = 3. 74) , P < 0. 05; The former disaster crowd felt more objective support, used more rationalization, but were less capable in problem solving. In former disaster crowd, problem-solving, objective social support and utility of the support were negatively correlated with subscores of SCL-90 ( r = -0. 31~-0. 40, P <0. 05) ; while immature coping, such as fantasy, withdrawal and rationalization were positively correlated with subscores of SCL-90 ( r = 0. 40 ~ 0. 60, P < 0. 05).Conclusion Former disaster crowd still have psychosomatic symptoms and need social and psychology intervene and support.
9.Influence of 4-aminopyridine on voltage-activated K(+) current and cell proliferation in small cell lung cancer.

OBJECTIVETo study the inhibition of voltage-activated K(+) conductance and cell proliferation by 4-aminopyridine (4-AP) in the human small-cell lung cancer (SCLC).
METHODSInhibition of voltage-activated K(+) current by 4-AP through the whole-cell patch-clamp technique in SCLC cell line was studied. The influence on the cell-cycle by 4-AP was observed by flow cytometry to identify the in vitro inhibition by 4-AP to the cell proliferation of the SCLC cell line.
RESULTSExposure of the tumor cells to 5 mmol/L 4-AP reduced the peak outward K(+) current (evoked by a depolarization to +80 mV) from 1.22 +/- 0.11 nA (n = 30) to 0.59 +/- 0.10 nA (n = 28). Flow cytometry results showed that cell population accumulated in the G(0)/G(1) phase and a significantly reduced proportion in the S phase and G(1)/G(2) phase cells after having been exposed to 4-AP for three days. Incubation of the SCLC cells with 0.1, 5, 10, 15, 20 mmol/L 4-AP resulted in a concentration-and time-dependent reduction in the number of viable cells as compared with the control.
CONCLUSIONThe voltage-activated K(+) channels expressed by SCLC play an important role in SCLC cell proliferation. The proliferation of the SCLC cells is inhibited by K(+) channel antagonists.

10.Effects of education level of men who have sex with men on their high risk sexual behaviors and the infection of HIV and syphilis.

OBJECTIVETo study the effects of educational background of men who have sex with men (MSM) on their high risk sexual behaviors and the HIV/STI infection rates.
METHODSDuring July to November of 2009 and March to October of 2010, snowball and convenience sampling methods were adopted to recruit MSM from MSM venues and via the internet in Changzhou city of Jiangsu province, and finally 659 MSM were conducted a questionnaire survey and serological testing. According to the educational background of MSM, they were divided into 3 groups, that is, junior high school group (206 cases), high school group (254 cases), and university group (199 cases). The questionnaire mainly includes information on social demography, sexual behaviors, condom use, etc. Blood samples were collected for HIV and syphilis spirochete detection, and urine samples were also collected in 291 MSM who were recruited during July to November of 2009 for neisseria gonorrhoeae and chlamydia trachomatis nucleic acid detection. χ(2) test and other statistical analysis methods were used to compare the characteristics of sexual behaviors and HIV/STI infections in 3 groups.
RESULTSOf the 659 valid questionnaires returned, junior high school group, high school group, and university group accounted for 31.3% (206 cases), 38.5% (254 cases) and 30.2% (199 cases). Places where MSM of different education levels most often to seek sexual partners, were significantly different. Junior high school group and high school group mostly went to bath house/sauna club (56.3%, 116 cases) and bar (34.8%, 88 cases) for partners, respectively, while the university group sought partners mainly through the internet (41.1%, 81 cases) (χ(2) = 99.35, P < 0.05). 53.4% (109/204) of the junior high school group had anal sex with men in the last 6 months, which was higher than that of high school group (67.7%, 172/254) (χ(2) = 9.74, P < 0.05) and university group (72.7%, 144/198) (χ(2) = 16.04, P < 0.05) . A total of 54.4% (111/204) of the junior high school group had sex with women in the last 6 months, which was higher than that of university group (38.6%, 76/197) (χ(2) = 10.10, P < 0.05) , but was not statistically significantly different from that of high school group (46.9%, 119/254) (χ(2) = 2.59, P = 0.11) . The rates of condom use with men at the last anal sex in junior high school group (73.4%, 80/109) , high school group (78.0%, 131/168) and university group (73.9%, 105/142) were similar. The rates of condom use with women in the last intercourse in junior high school group, high school group and university group were 51.8% (57/110), 54.6% (65/119) and 61.8% (47/76), respectively(χ(2) = 1.88, P = 0.39) . In junior high school group, high school group and university group, the infection rates of HIV were 9.2% (19/206), 10.6% (27/254) and 5.6% (11/197) (χ(2) = 3.68, P = 0.16), the positive rates of neisseria gonorrhoeae were 3.8% (3/79), 3.4% (4/117) and 0.0% (0/95) (χ(2) = 3.85, P = 0.14), the positive rates of chlamydia trachomatis were 5.1% (4/79), 9.4% (11/117) and 4.2% (4/95) (χ(2) = 2.70, P = 0.26). The infection rate of syphilis in junior high school group was 19.9% (41/206), which was higher than high school group (12.2%, 31/254) (χ(2) = 5.11, P < 0.05) and university group (10.2%, 20/197) (χ(2) = 7.45, P < 0.05 ).
CONCLUSIONThere was no obvious correlation between education level and high risk sexual behaviors;MSM with lower education level were at higher risk of infection of syphilis.
