1.Study on the efficacy tolerability of topiramate in transferring monot herapy for the patients with partial seizures

Objective To observe the possibility and efficacy tolerability of topiramate(TPM) in transferring monotherapy for the patients wi th partial seizures with or without secondary general seizure,as well as to find out the optimum way of the transferring.Methods After T PM add-on therapy 64 patients with seizures reduced ≥50%, it reduced about 1/ 4 of AEDs or one drug each time according to every two weeks until gradually to stop fully and transferring monotherapy.Results 18 patients we re successful with TPM monotherapy (28.13%), 35 patients were half success(54.69 %), 11 cases for failure (17.19%). The less AEDs combined, the higher possibilit y of transferring monotherapy would be. The rate of success for cabamazepine see med higher than valproate.Conclusion It was possible to transfe r monotherapy by TPM. The rate of success was 28.13%. It was easier to reduce ca bamazepine than valproate. In the transferring monotherapy it was better to redu ce one after another, in principle the one of the little dose was reduced first, and then the hepatic enzyme inducing agents was reduced. Even if the transferrin g was half success,the kinds of concomitant AEDs could be reduced at least, it w as in line with the policy of medicine application.

Objective To observe the therapeutic efficacy and safety of Topiramate (TPM) as monotherapy in patients with partial epilepsy.Methods The seizure rate in baseline phase of 30 patients with partial epilepsy (treated by Topiramate as monotherapy for 20 weeks) were observed and recorded before treatment.The TPM starting dosage was 25 mg/d, and then increased 25 mg/d each week for 8 weeks until effective dosage or 200 mg/d for 12 weeks,the change of seizure frequency and bad reaction were found.Results The seizure in 16 cases (53.3%) was totally controlled,seizure reduction ≥75% was in 6 cases (20%),≥50% was in 2 cases (6.7%),

5.Clinical manifestation and MRI feature on toxic encephalopathy after inhaling and injecting heroin

Objective To study the clinical manifestation and MRI feature on toxic encephalopathy after inhaling and injecting heroin. Methods 4 patients with toxic encephalopathy induced by inhaling, injecting heroin were observed with clinical and MRI. Results The 4 patients were male, they all had definite addiction histories (the time of addiction for 4 months to 7 years), there were special language and retropulsive dystonia besides there were common characteristics of toxic encephalopathy: reaction retardation, disturbance of intelligence, MRI showed the change of generalized demyelination in bilateral symmetrical cerebral hemisphere, posterior limbs of internal capsule, corpus callosum, medial lemniscus, cerebellum hemisphere dentate nucleus. The clinical symptom of 1 patient treated by adrenocortical steroid disappeared nearly 11 months after the onset of disease, 2 patients’ condition was remitted 3 months after treatment, 1 patient with 7 years inhaling history died the 55th day after the deterioration of disease. Conclusion Clinical manifestation and MRI on toxic encephalopathy after inhaling and injecting heroin were known, so we could diagnose the patients of the disease in time.

7.Nursing care to patients undergoing NovaSure endometrial ablation

Objective To summarize the experience of nursing patients undergoing NovaSure endometrial ablation. Methods Two hundred and eleven patients with abnormal uterine bleeding claiming no need of giving birth,who were hospitalized in our hospital during August 2011 to April 2013,were managed with NovaSure endometrial ablation and nursing care was performed.Follow-ups were conducted for investigating the curative effects.Results All the patients lived through the operations,their life signs were stable.As for the adverse effects,124 of them had mild abdominal pains,7 medium-level or severe hypogastralgia,1 dizziness and nausea,10 uterial hematocele,and 1 cystitis.All of them got recovered after treatment.One-month follow-up after hospitalization showed a success rate of 98.0%.Conclusions NovaSure is a new approach to abnormal uterine bleeding.It is advantageous for its simplicity,shortness in operation duration,less trauma to patients and good effects.Meanwhile,preoperative mental care and cooperative care during operation may facilitate their recoveries.
8.Effects of mild hypothermia combined with edaravone on expressions of tumor necrosis factor-α and interleukin-6 in cerebrospinal fluid of patients with severe traumatic brain injury

Objective To study the effect of mild hypothermia combined with edaravone on the expressions of tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)in cerebrospinal fluid(CSF)of patients with severe traumatic brain injury(sTBI)and on their prognoses. Methods A prospective randomizd controled trial was conducted. Seventy-seven patients in the Center for Neurology and Neurosurgery of Affiliated Hospital of Logistics University of Chinese People's Armed Police Forces were randomly assigned into control group(38 cases)and treatment group(39 cases)according to random number table. All the patients were treated with routine treatments such as dehydration of intracranial pressure(ICP),neural nutrition,anti-infection,mechanical ventilation and maintenance of water and electrolyte balance in control group,while in treatment group,the patients received mild hypothermia combined with edaravone on the basis of routine treatment within 24 hours after injury. The contents of TNF-αand IL-6 in CSF were measured by radio-immunoassay(RIA)at different time points in both groups. In the meantime,the ICP was also measured. The prognosis was evaluated after 6 months of injury according to Glasgow outcome scale(GOS). Results Compared to control group,in the treatment group,the expression levels of TNF-αand IL-6 in CSF had no significant difference(both P>0.05)on the 1st day after injury,but they were significantly increased on the 3rd day after injury,began to decline on the 7th day,and reached to the valley value on the 14th day after injury,the degree of descent in treatment group being more significant than that in control group〔TNF-α(μg/L):2.43±0.39 vs. 3.12±0.47,IL-6(ng/L):83.53±11.48 vs. 101.69±13.64,both P<0.01〕. Before the treatment,the level of ICP in treatment group had no significant difference from that of control group(P>0.05),but it was gradually increased on the 1st day after injury in both groups,it reached the peak value on the 3rd day after injury,and began to decline on the 7th day after injury,the degree of descent being more significant in treatment group〔mmHg(1 mmHg=0.133 kPa):14.88±3.73 vs. 21.76±4.78,P<0.01〕. The favorable prognosis rate was significantly higher〔35.9%(14/39)vs. 21.1%(8/38),P<0.05〕,and the mortality was obviously lower in treatment group than those of control group〔28.2%(11)vs. 42.1%(16),P<0.05〕. Conclusion In patients with sTBI,mild hypothermia combined with edaravone can protect brain tissue through alleviating high ICP and decreasing the expression levels of TNF-αand IL-6 in CSF, resulting in promoting the recovery of nerve functions and improving prognosis.
9.Literature Analysis of Moxifloxacin-induced Adverse Drugs Reactions in 3 445 Cases

OBJECTIVE:To investigate the characteristics and regularity of moxifloxacin-induced adverse drug reaction(ADR) and provide reference for the rational clinical use of drugs. METHODS:Retrieved from CNKI (2005-2014) about the moxifloxa-cin-induced ADR,3 445 cases and the related ADR information were statistically analyzed. RESULTS:Moxifloxacin-induced ADR had a certain relationship with gender,and the male had a high incidence,especially the elderly patients. ADR could appear within 10 min;the clinic features were allergic reaction,the nervous system and digestive system at most. CONCLUSIONS:Great impor-tance should be attached to ADR monitoring and rational use of drug to reduce or avoid the occurrences of moxifloxacin-induced ADR.
10.Establishment and evaluation of secretory otitis media model induced by endotoxin

OBJECTIVE To establish secretory otitis media(SOM) model induced by endotoxin and evaluate its reliability. METHODS Healthy 50 Sprague-Dawley rats of either sex( 250-300g)were randomly divided into experimental group(ET group) and control group(NS group),with 30 and 20 rats respectively. 35ul volume of 1mg/ml endotoxin from Pseudomonas aeruginosa and same amount of saline(NS) were transbullaly injected into the right tympanic bullas of the two groups. Six rats of ET group and four rats of NS group were killed postoperative on 6h,1d,3d,7d,14d respectively. The temporal bones were harvested for histological study. The left tympanic bullas served as control. Another four rats(each group 2 rats) were used for SEM study with the same method. One rat was killed and harvested for SEM observation of mucocilliary transporting system on the tympanic ostium of Eustachian tube. RESULTS ①LM(light microscope):The epithelial layer and subepithelial space(SES) were thicked mildly in ET group at 6 hours,and more heavily at 1 day with inflammatory cells infiltration (mostly polymorphonuclear leukocytes,PMNs). At 3 days,the same changes were observed most severely. By 14 days,the mucosal lining returned to normal. No obvious changes were observed in the NS group. ②SEM(scan electrical microscope):In ET group,the impairment of mucociliary transport system such as lodging,shedding of cilias were observed at 3 days and returned to normal at 14 days. No abnormal changes were observed in NS group. CONCLUSION Endotoxin can induce SOM with inflammatory changes peak at day 3 and return to normal by day 14.
