OBJECTIVE To study the distribution and drug resistance supervision of infection pathogens of Gram-negative bacterial pneumonia in lower respiratory tract of senile patients and draft preventive and control measures.METHODS The sputum and lower respiration secretion of 368 senile patients were cultured to detect pathogens which were confirmed by routine methods.The disk diffusion test was adopted to analyze drug resistantce.RESULTS Pseudomonas aeruginosa rated the top one,reached 25.0%;the drug resistance supervision manifested that the resistant rates of Gram negative organisms to the routine drugs were high except for Haemophilus influenzae.And extensive-spectrum beta-lactamases(ESBLs) occurred with rate of 30.2%.CONCLUSIONS It is very necessary for clinical microbiology department to detect pathogens of lower respiratory tract pneumonia among senile patients and then to give reasonable advice for medicine in time.

OBJECTIVE To investigate the spread status and drug-resistance of pathogenic bacteria causing chronic bacterial prostatitis among the senile patients,and provide the reference for clincal diagnosis and treatment.METHODS Prostatic fluid from the senile patients with chronic bacterial prostatitis was collected,and bacterial cultures and susceptibility tests were performed.The data were analyzed statistically.RESULTS Among 216 prostatic fluid specimens from the senile patients with chronic bacterial prostatitis,140 isolates of pathogenic bacteria were detected.In these strains,Gram-positive cocci accounted for 60.0% and most of them were Staphylococcus aureus.Gram-negative bacilli accounted for 40.0% and most of them were Escherichia coli.Pathogens causing prostatitis have been resistant to the common antibiotics.Among the total staphylococci,meticillin-resistant Staphylococcus(MRS) accounted for 34.8%.And among the total Enterobacteriaceae,ESBLs producing bacteria accounted for 35.6%.CONCLUSIONS Chronic bacterial prostatitis in the senile patients possesses intractability and persisting.It is important to monitor the pathogens and their drug-resistance.

AIM:To develop a method for determining cholic acid by HPLC-ELSD and GC was applied to determing muscone;in Jawei Xihuang Soft Capsule(Calculus Bovis,Moschus,Venenum Bufonis,Olibanum,Myrrha).METHODS:AC_(18) column(Kromasil C_(18),5 μm,4.6 mm×250 mm)was used as stationary phase,the mobile phase was methanol-0.01% glacial acetic acid(73:21)at a flow rate of 1.0 mL/min.The parameters of ELSD were set as follows:evaporation temperature was 40℃,carrier gas(N_2)pressure was 200 kPa.The GC system consisted of DB-1 capillary column(30 m×0.32 mm×0.25 μm)and FID as the detector.The programmed temperature-GC and internal standard method were employed to determine the content of muscone.RESULTS:The linear ranges of cholic acid and muscone were in the range of 45.2 ng-904 ng and 0.05 mg/mL-0.5 mg/mL respectively.The average recoveries were 99.06% and 99.40% with RSD of 1.56% and 0.95% respectively.CONCLUSION:The method is convenient and accurate,and it can be used for the quality evaluation of Jawei Xihuang Soft Capsule.

Objective To investigate the expression level of Annexin Ⅱ in bronchial lung cancer tissue,alveolar lavage fluid(BALF) and pleural effusion and its value in the diagnosis of lung cancer.Methods From January 2014 to March 2016 in Jingzhou Infectious Disease Hospital,80 cases of patients with pleural effusion of lung cancer (Lung Cancer Group),40 patients with benign pleural effusion (benign group) were chose.The expression level of Annexin of two groups of patients with pleural effusion,BALF were detected and compared.Sixty specimens of department of pathology past (after lung cancer surgery cancer) and 30 cases of previously collected normal lung tissues (normal lung tissue) were collected,differential expression by immunohistochemical staining were compared between the two groups of Annexin Ⅱ protein.Results The positive expression of Annexin Ⅱ in lung cancer was 61.67％ (37/60),higher than that in normal lung tissue of 23.33％ (7/30),the difference was statistically significant(P＜0.001).The positive expression of Annexin Ⅱ in lung cancer patients was significantly correlated with rate of lymph node metastasis and clinical stage of lung cancer(P =0.039,0.029).Group of patients with pleural effusion,BALF expression of Annexin Ⅱ the level was higher than that in benign group(P＜0.001).When the critical value of Annexin in pleural effusion of lung cancer diagnosis was 10.94 μg/L,the sensitivity of diagnosis of lung cancer was 83.47％,the specificity was 80.66％,AUC value was 0.837.The critical value when BALF Annexin Ⅱ in the diagnosis of lung cancer was 52.08 μg/L,the sensitivity of diagnosis of lung cancer was 86.19％,the specificity was 89.22％,the AUC value was 0.898.Conclusion The expression level of Annexin Ⅱ in the BALF and pleural effusion of patients with bronchial lung cancer and lung cancer is increased,and it has a certain clinical value for the differential diagnosis of lung cancer.

Objective To explore the effect of bispectral index (BIS) monitoring on hemodynamics patients undergoing laparoscopic cholecystectomy under laryngeal mask anesthesia.Methods 110 patients with laparoscopic cholecystectomy were randomly divided into the control group and the observation group according to digital table method.The control group was administered according to the experience of anesthesiologists,the observation group was administered according to BIS monitoring.The hemodynamic indexes,the dosage of anesthetic drug and recovery time of two groups were analyzed.Results After pulling out the laryngeal mask,the HR (74.13±6.99)beats/min,MAP (78.54±8.01)mmHg and COR (321.23±15.35)nmol/L of the observation group were significantly higher than HR (94.21±9.53)beats/min,MAP(98.67±10.21)mmHg and COR (463.24±20.01)nmol/L of the control group,the differences were statistically significant (t=12.60,11.50,41.76,all P<0.05).The amount of propofol induction (1.65±0.21)mg/kg,dosage of propofol (323.13±15.64)mg and remifentanil (401.21±20.11)μg of the observation group were lower than those of the control group [(2.39±0.34)mg/kg,(414.32±20.34)mg,(469.54±22.39)μg],and the recovery time of the observation group (6.89±1.01)min was significantly better than (10.21±1.42)min of the control group,the differences were statistically significant (t=13.73,26.36,16.84,14.13,all P<0.05).Conclusion The effect of BIS monitoring in laryngeal mask anesthesia for laparoscopic cholecystectomy is significant,which can reduce intraoperative anesthetic dosage,promote hemodynamic stability,reduce the stress response of patients,shorten recovery time,the anesthesia is more safe and reliable.

After an analysis of the matching for library and teaching model of MOOC, MOOC-related problems were pointed out, such as insufficient supplementary resources, no soft environment for attending MOOC, limita-tions of traditional service model, and how to construct a MOOC-based service model for medical teaching in library was elaborated with suggestions proposed for changing the traditional service model in library, such as bringing into full play of the advantages of intensive information and the supplementary role of library in medical teaching , and providing new types of service.

There are many ways to treat pathological scars.Silicon gel sheeting as a noninvasive and convenient method, are widely used in clinic.The article reviews the lituratures on theoretical research of silicon gel sheeting and its efficacy in the past 26 years, and analyzes the effectiveness of silicon gel sheeting for comparison with other methods.

Objective To investigate the value of combined detection of GGT,blood lipid and hs-CRP in patients with coronary heart disease.Methods Totally 134 patients with coronary heart disease and 38 patients without coronary heart disease were selected after coronary angiography.Coronary heart disease patients were divided into mild stenosis group (group A,n =40),Moderate stenosis group (group B,n =38) and severe stenosis group (group C,n =56) according to Gensini score.The age,sex,GGT,blood lipid and hs-CRP levels were detected in all groups,and t-he statistical analysis was performed by using SPSS 22.0 software.Results The level of GGT,TC,TG,LDL and hs-CRP in CHD group were all higher than those in non CHD group (P ＜ 0.05).The level of HDL was lower than that in the non CHD group (P ＜ 0.05).The level of GGT,TC,TG,LDL,hs-CRP in group B and C were higher than group A (P ＜0.05).The level of HDL in group B and C were lower than group A(P ＜0.05).The level of GGT,TC,TG,LDL,hs-CRP in group C were higher than group B (P ＜ 0.05).Logistic regression analysis showed that serum GGT,TG,hs-CRP were independent risk factors for coronary heart disease (P ＜ 0.05),HDL was an independent protective factor for coronary heart disease (P ＜ 0.05).Conclusion The combined detection of GGT,blood lipid and hs-CRP has important value in the diagnosis of coronary heart disease.
