1.Effects of Lidocaine Epidural Block Combined with General Anesthesia on Postoperative Related Indexes of Lung Cancer Patients

OBJECTIVE:To investigate the effects of lidocaine epidural block combined with general anesthesia on postopera-tive related indexes of lung cancer patients. METHODS:In retrospective analysis,108 patients with lung cancer were divided into observation group (56 cases) and control group (52 cases) according to anesthesia method. Both groups were given intravenous dripping of Propofol injection combined with Fentanyl injection,inhaled 1%sevoflurane for inhalation to assist anesthesia and inter-mittent intravenous drip of Vecuronium for injection to maintain muscle relaxation. On this basis,observation group was treated with epidural infusion of 2.0% Lidocaine hydrochloride injection 5 mL/h to maintain anesthesia;control group was given epidural infusion of 0.9% Sodium chloride injection 5 mL/h to maintain anesthesia. The anesthesia time,operation time,intraoperative bleeding volume,postoperative recovery time,the amount of fentanyl,propofol,vecuronium bromide and sevoflurane,the occur-rence of postoperative anesthesia complications (nausea,vomiting,dizziness,drowsiness,skin itching) and other complications (cardiac function insufficiency,cardiovascular events,atrial fibrillation,infection,transient cerebral ischemia,acute coronary com-prehensive syndrome,cerebral apoplexy)were observed in 2 groups as well as active and calm VAS score 4 h and 1,2,3,4,5 after surgery. RESULTS:The anesthesia time,operation time,intraoperative bleeding volume,postoperative recovery time,the amount of fentanyl,propofol,vecuronium bromide and sevoflurane,calm VAS score 4 h,1 d,2 d after surgery and active VAS score 4 h,1 d,2 d,3 d after surgery,the incidence of drowsiness in observation group were significantly shorter or lower than control group,while the incidence of dizziness was significantly higher than control group,with statistical significance(P<0.05). There was no statistical significance in the amount of vecuronium,calm and active VAS score in other time,the incidence of nau-sea,vomiting,skin itching,cardiac function insufficiency,cardiovascular events,atrial fibrillation,infection,transient cerebral ischemia,acute coronary syndrome and stroke between 2 groups(P>0.05). CONCLUSIONS:Lidocaine epidural block combined with general anesthesia can reduce the perioperation bleeding amount,postoperative pain,shorten postoperative recovery time and do not increase postoperative complications.
2.Addition and Subtraction Therapy of Buyang Huanwutang Combined with Si Junzitang to Poststroke Fatigue and Antioxidant and Anti-inflammatory with Syndrome of Qi Deficiency and Blood Stasis

Objective:To evaluate effect of addition and subtraction therapy of Buyang Huanwutang combined with Si Junzitang and acupuncture to poststroke fatigue (PSF) and syndrome of Qi deficiency and blood stasis, at the same time we studied the antioxidant and anti-inflammatory effects. Method:One hundred and forty-four patients were randomly divided into control group and observation group (1∶1) by random number table. 66 patients in control group completed the therapy (4 patients were falling off or missing visit, 2 patients were eliminate), 67 patients in observation group completed the therapy (2 patients were falling off or missing visit, 3 patients were eliminate). In control group, patinets got acupuncture, 1 time/day, 6 times/week, they also got Geqi Tongmai grain, 10 g/time, 3 times/day. Patients in observation group got acupuncture (the same as which in control group), and addition and subtraction therapy of Buyang Huanwutang combined with Si Junzitang, 1 dose/day, and courses of treatment in two groups were 4 weeks. Before and after treatment, fatigue severity scale (FSS), NIH stroke scale (NIHSS), syndrome of Qi deficiency and blood stasis, stroke specific quality of life scale (SS-QOL), and scores of ability of daily life (ADL) were recorded. 3.Application of convolute manipulation for the treatment of femoral fractures in children.

OBJECTIVETo discuss the application of convoluted manipulation in pediatric femoral fractures.
METHODSFrom March 2015 to October 2016, 12 children with femoral fractures were treated by Chinese traditional manipulation including 8 males and 4 females with an average age of 6 years old ranging from 1 to 12 years old. The causes of injury were falls in 10 cases and traffic accidents in 2 cases. Of which 1 case was transverse fractures, 4 cases were oblique fractures and 4 cases were spiral fractures, 2 cases were comminuted fracture, 1 case was greenstick fracture. All patients underwent manual reduction within 1 to 2 days, plus small splint with cedar bark, and parallel lower limb traction.
RESULTSAll the 12 patients were followed up for 1-3 months, with an average of 2 months. All the 12 patients achieved clinical union, and the average healing time was 6 weeks. There was no obvious shortening and rotational angulation. At the last follow-up, Schatzker-Lambert distal femoral fracture evaluation results were excellent in 11 cases, good in 1 case.
CONCLUSIONSConvoluted manipulation is very important for reduction of femoral fractures in children, with the splint of cedar bark, satisfactory therapeutic effect can be achieved.

7.The role and mechanism of CXCR4 in renal injury and fibrosis caused by calcium oxalate crystals

Objective:To investigate the role and mechanism of chemokine receptor type 4 (CXCR4) in renal injury and fibrosis caused by calcium oxalate crystals in mice.Methods:In June 2021, Fifteen male C57/BL6 mice were divided into control group (5 mice), model group (5 mice), and AMD3100 intervention group (5 mice) by random number table method. In model group and AMD3100 intervention group, glyoxylate (100 mg/kg) was injected intraperitoneal for 7 consecutive days for modeling. Meanwhile, the AMD3100 intervention group was also given intraperitoneal injection of AMD3100 (1 mg/kg) for 7 days. The control group was continuously injected with equal volume saline intraperitoneally. After 7 days, peripheral blood was collected from each group to determine the levels of serum urea nitrogen (BUN) and creatinine (Scr) to assess the renal function; HE, Von-Kossa, Picrosirius Red staining was also taken from the left kidney to observe the pathological changes of renal tissue. CXCR4, transforming growth factor β1 (TGF-β1) were detected by immunohistochemistry and western blot. The expression levels of PI3K/AKT pathway-related proteins were detected by western blot.Results:The results of biochemical indexes showed that the serum Scr [(108.03±13.56) μmol/L vs. (39.50±4.48)μmol/L, P<0.01)] and BUN[(5.66±0.48)mmol/L vs. (0.77±0.10)mmol/L, P<0.01) levels were significantly increased in model group compared with the control group. The AMD3100 intervention group was significantly lower than the model group in terms of Scr [(65.77±3.27)μmol/L vs. (108.03±13.56)μmol/L, P<0.05) and BUN [(2.97±0.44)mmol/L vs. (5.66±0.48)mmol/L, P<0.05) levels. The results of kidney pathology in mice showed that renal tubules were significantly dilated with inflammatory cell infiltration in the model group compared with the control group, and a large number of calcium oxalate crystals and collagen fibers were deposited. The extent of kidney damage, calcium oxalate crystals and collagen fibers deposition were significantly reduced in the AMD3100 intervention group compared with the model group. The results of western blotting showed that the relative expression of CXCR4(0.639±0.019 vs. 0.158±0.012, P<0.01) and TGF-β1(0.698+ 0.018 vs. 0.314+ 0.015, P<0.05) was significantly increased in the model group compared with the control group. The relative expression of CXCR4(0.322±0.231) in the AMD3100 intervention group compared with the model group (0.322±0.231 vs. 0.639±0.019, P<0.05) and TGF-β1(0.445+ 0.017 vs. 0.698+ 0.018, P<0.05) were significantly decreased. The results of immunohistochemical staining showed the trend of CXCR4 and TGF-β1 expression in each group consistent with the results of protein blotting assay. Western blotting results showed that the expression of p-PI3K (0.613±0.016 vs. 0.213±0.011, P<0.01) and p-AKT(0.149±0.013 vs. 0.047±0.014, P<0.01) was significantly increased in the model group compared with the control group. The expression of p-PI3K in the AMD3100 intervention group compared with the model group (0.292±0.020 vs. 0.613±0.016, P<0.05) and p-AKT (0.098±0.021 vs. 0.149±0.013, P<0.05)was significantly decreased. Conclusion:CXCR4 inhibits calcium oxalate crystal-induced kidney injury and interstitial fibrosis in mice by targeting the PI3K/AKT pathway.
8.Analysis of imagery motor effective networks based on dynamic partial directed coherence.

The research on brain functional mechanism and cognitive status based on brain network has the vital significance. According to a time-frequency method, partial directed coherence (PDC), for measuring directional interactions over time and frequency from scalp-recorded electroencephalogram (EEG) signals, this paper proposed dynamic PDC (dPDC) method to model the brain network for motor imagery. The parameters attributes (out-degree, in-degree, clustering coefficient and eccentricity) of effective network for 9 subjects were calculated based on dataset from BCI competitions IV in 2008, and then the interaction between different locations for the network character and significance of motor imagery was analyzed. The clustering coefficients for both groups were higher than those of the random network and the path length was close to that of random network. These experimental results show that the effective network has a small world property. The analysis of the network parameter attributes for the left and right hands verified that there was a significant difference on ROI2 ( = 0.007) and ROI3 ( = 0.002) regions for out-degree. The information flows of effective network based dPDC algorithm among different brain regions illustrated the active regions for motor imagery mainly located in fronto-central regions (ROI2 and ROI3) and parieto-occipital regions (ROI5 and ROI6). Therefore, the effective network based dPDC algorithm can be effective to reflect the change of imagery motor, and can be used as a practical index to research neural mechanisms.
9. Bioinformatics analysis of leptin regulating gallbladder contraction and secretion in mice

Objective:
To identify the key pathogenic genes of leptin regulating gallbladder contraction and secretion in mice and to reveal the potential molecular mechanism by comprehensive bioinformatics.
Methods:
The expression profile of GSE3293 was downloaded from Gene Expression Omnibus (GEO) database. The data contained 8 samples, including 4 leptin-treated gallbladder samples and 4 saline-treated gallbladder samples. The most valuable 250 differentially expressed genes (DEGs) were obtained by grouping analysis of GEO online GEO 2 R-TOP 250 software or tools, and further analyzed by bioinformatics. The GO function and KEGG pathway enrichment of DEGs were analyzed by DAVID online software. The protein-protein interaction (PPI) network of DEGs was constructed from STRING database.
Results:
A total of 250 differentially expressed genes were identified from the GSE3293 dataset, of which 197 genes were up-regulated and 53 genes were down-regulated. GO analysis showed that the biological functions of DEGs were mainly concentrated on MHC class II protein complexes, plasma membrane, extracellular exosome. KEGG pathway analysis showed that these DEGs were mainly involved in tuberculosis, leishmaniasis, cell adhesion molecules, bacteriophages, infection and other signaling pathways. PPI network showed that these DEGs coded proteins interacted strongly, and the first five pairs of DEGs with the strongest correlation were screened out.
Conclusions
The molecular mechanism of cholelithiasis is predicted from gene level by bioinformatics analysis of function enrichment and PPI of DEGs in mouse gallbladder. However, the function of DEGs still needs a lot of clinical and molecular biological experiments to confirm.
10.Characteristics and management mechanism of imported malaria from overseas during the malaria elimination phase in Wuhan City

Objective To analyze the characteristics of imported malaria epidemic from overseas in Wuhan, to explore the management mechanism of on-site cases, and to accumulate experience for the treatment of imported malaria in large cities after malaria elimination. Methods The epidemiological data on imported malaria from abroad during the period of malaria elimination (2010-2019) in Wuhan were collected. The gender, age and severe illness-related factors of the cases were analyzed. Based on the characteristics of the epidemic and the current situation of prevention and control, the content and experience of the “Municipal-District 24-7” case mechanism were discussed. Results The medical resources in Wuhan were the best in the central region, resulting in a large number of imported malaria cases, with a total of 474 cases reported from 2010 to 2019 (40.79% of the total number of cases in Hubei Province), including 359 cases of falciparum malaria, 36 severe cases and one death (the death rate was 0.28%). The patients were mainly young and middle-aged men aged 20 to 49 years old (97.26%). There were many referral cases (40.30%), and there was no seasonal clustering of cases reported. The undiagnosed proportion at the first visit was 44.85%, and the time of attack-diagnosis was 4 days or more in 61.00% of cases. The occurrence of severe cases was related to unconfirmed diagnosis at the first visit (χ2=35.46, P<0.001) and attack-diagnosis time (Z=-6.49, P<0.001). Conclusion Imported malaria occurs frequently in Wuhan, mainly falciparum malaria. However, “Municipal-District 24-7” case mechanism has effectively curbed the occurrence of severe and death cases and provided valuable experience for case management in similar cities in China.
