1.Double-index Method for Quality Control of Tribulus Terrestris L

Objective:To establish the quality control method for Tribulus terrestris L. by colorimetry and HPLC. Methods:The HPLC method was with a Welch Ultimate LP-C18 column(250 mm × 4. 6 mm,5μm),the mobile phase was methanol-water(80:20) and the detection wavelength was 203 nm. The colorimetry was with a perchloric acid method. The saponins of Tribulus terrestrist as the index,the determination method for total saponins and saponins of Tribulus terrestris L. was established. Results:The results of the HPLC and colorimetry methods showed saponins of Tribulus terrestris had good linear relationship within the range of 0. 820-7. 380 μg and 24. 600-86. 100 μg with the average recovery of 99. 3% and 99. 5%,respectively. Total saponins and saponins of Tribulus terres-tris in Tribulus terrestris from 18 habitats were measured by the methods. Conclusion:The methods are sensitive,accurate and repro-ducible,and can be used as the quality control methods for Tribulus terrestris.
2.Studies on the optimization of extraction method and content of different parts in XinJiang Bergenia crassifolia (L.)

Objective To extraction method optimization and the content of the different parts of Xinjiang Bergenia crassifolia (L.), provide the basis for efficient extraction of Bergenia crassifolia pigment. Method Using the orthogonal experiment, the alcohol extraction process of ethanol concentration, dosage of ethanol, extraction time, extraction times, as well as to Bergenia crassifolia pigment content of the different part. Results Of optimization, the best ethanol extract of xinjiang Bergenia crassifolia pigment process conditions is 8 times the amount of 50%ethanol, extracting 3 times, each time 0.5 h. In Bergenia crassifolia pigment content of different parts exists obvious difference. Bergenia crassifolia pigment content of Taproot and fibrous root is higher, at 8.65%to 9.58%, while Bergenia crassifolia pigment content of the leaves is relatively low, at just 0.15%. Bergenia crassifolia pigment content of leaves significantly higher than the old leaves. Conclusion This experimental study on efficient extraction of xinjiang Bergenia crassifolia pigment to provide strong technical support and theoretical basis.
3.Speciation analysis of harmful elements in Rhei Radix et Rhizoma, Chrysanthemi Flos.

OBJECTIVETo investigate the state and distribution of Pb, Cd, Hg, As, Cu in Rhei Radix et Rhizoma and Chrysanthemi Flos.
METHODThe samples were prepared by modified Tessier sequential extraction; The elements of Cu, Pb, and Cd in the samples were analyzed by atomic absorption spectrometry (AAS), while Hg, and As were analyzed by atomic fluorescence (AFS).
RESULTCu,Pb,Cd,Hg,As in Chrysanthemi Flos were 12.806, 10.478, 0.436, 0.231, 1.531 mg x kg(-1), respectively. Cu, Pb, Hg in Chrysanthemi Flos mainly existed in residual and organic states; Cd was priority to ion exchange state; the residual state was the main form of As, and ion exchange state and water soluble state also had a large proportion. Cu, Pb, Cd, Hg, As in Rhei Radix et Rhizoma were 10.530, 4.926, 0.478, 0.260, 0.750 mg x kg(-1), respectively. Cu, Pb and Hg of residual state were the highest ratio; Cd and As mainly existed in ion exchange state.
CONCLUSIONThe experiment results showed that sequential extraction can be applied in speciation analysis of the harmful elements of traditional Chinese medicine. The method can speciate the state and distribution of harmful elements and provide the information of harmful elements. It will provide reference to the production of Chinese traditional patent medicine and herb extracts, processing of medicine herbs, the development of new drugs and safety evaluation of traditional Chinese medicine.

4.Efficacy and related factors of pegylated interferon α-2a plus ribavirin therapy for chronic hepatitis C in non-responders

Objective To evaluate the efficacy of pegylated interferon ( PegIFN ) α-2a plus ribavirin ( RBV) therapy for chronic hepatitis C ( CHC) in non-responders, and to investigate the related influencing factors.Methods A prospective, open, multicenter and randomized study was conducted.A total of 81 CHC non-responders were recruited from 10 clinical centers during February 2009 to November 2011.Patients were randomly assigned into two groups:group A (n=37) was given PegIFNα-2a plus RBV treatment for 72 weeks and group B (n=44) was given PegIFNα-2a plus RBV treatment for 96 weeks.Both groups were followed up for 24 weeks after treatment.Virological responses in two groups were observed, and treatment efficacies among patients with different genotypes, and among those with different previous treatment were compared.SAS software was used for statistical analysis.Results Fifty-two patients ( 28 from group A and 24 from group B) completed the study in total.The rates of rapid virological response ( RVR) , complete early virological response ( cEVR ) , end of treatment viral response ( ETVR ) and sustained virological response (SVR) in group A were 25.0% (7/28), 60.7% (17/28), 67.9%(19/28) and 60.7%(17/28), respectively; while those in the group B were 41.7% (10/24), 70.8%(17/24), 70.8%(17/24) and 70.8% (17/24), respectively; and there were no significant differences between two groups (P>0.05).SVR was observed in 82.9%(29/35) of patients with CC genotype of IL-28B, which was higher than that in patients with other genotypes ( 3/13 ) , and the difference was of statistical significance (P<0.01).There was no significant difference in viral responses between patients previously treated with IFN plus RBV and those treated by IFN only (P>0.05).The rates of RVR, cEVR, ETVR and SVR in patients who were previously treated with IFN were 36.4%(12/33), 81.8%(27/33), 81.8%(27/33) and 75.8%(25/33), and the rates of cEVR, ETVR and SVR were higher than those in patients who were previously treated with PegIFN (P<0.05), but no significant difference was observed in RVR (P>0.05).Adverse events occurred in 38 patients (46.9%), but no severe ones were observed. Conclusion The efficacy of PegIFNα-2a plus RBV therapy for CHC in non-responders is satisfactory, which may influenced by IL-28B genotypes and previous treatment.

6.High sustained virological response to optimized therapy for refractory chronic hepatitis C treatment-na(i)ve patients: a multicenter randomized study.

OBJECTIVETo perform a prospective,multicenter,open,randomized study to determine a treatment regimen for treatment-naive patients with refractory chronic hepatitis C (RHC) using the predictive value (PV) of early virological response (EVR).
METHODSA total of 438 patients from 18 hospitals were recruited between December 2008 and December 2010 and administered peg-interferon/ribavirin treatment for 12 weeks. Patients who achieved complete EVR (cEVR) were assigned to group A for a 48-week course of treatment, while patients without cEVR were randomly allocated to either group B 1 for a 72-week course of treatment or to group B2 for a 96-week course of treatment. Serum hepatitis C virus RNA levels at baseline,treatment weeks 4, 12 and 24, end of treatment, and post-treatment week 24 were measured and used to evaluate the efficiency of therapy.
RESULTSThe overall sustained virological response (SVR) rate was 85.1%. In all, 91.0% of patients achieved cEVR and were assigned to group A, which had an SVR rate of 90.8%. There was no statistically significant difference in the SVR rates of groups B1 and B2 (29.4% vs. 25.0%, P more than 0.05). The positive PV of rapid virological response (RVR), cEVR and delayed virological response (DVR) for SVR was 93.4%, 90.8% and 77.8% respectively, and the negative PV of RVR, EVR and DVR for SVR was 28.0%, 93.3% and 100% respectively. Overall, 66.9% of the patients experienced adverse events (AEs), but only 1.9% of patients experienced sevcre AEs.
CONCLUSIONThe majority of Chinese RHC treatmentna(i)ve patients (91.0%) can achieve cEVR and a high SVR rate with a low rate of severe AEs using the cEVR guided personal treatment regimen.

7.Renal re-transplantation in a pre-sensitized small infant and literature review

Objective To explore the feasibility and safety of kidney transplantation for pre-sensitized infants using deceased donors and summarize the relevant literature reports .Methods A second kidney transplantation was successfully performed for an 8-month-old pre-sensitized girl in July 2017 .She had a low level of donor specific antibody (DSA ) against human leucocyte antigen (HLA ) B62 due to severe acute rejection (AR) after her first kidney transplantation .For desensitization , plasmapheresis and intravenous immunoglobulin plus anti-CD20 antibodies were offered on operative day .Clinical data and outcomes were retrospectively analyzed .Results Renal graft regained immediate function after transplantation .Preformed DSA could be detected at 1 week .However ,there was no de novo DSA .At 1 year post-transplantation ,preformed DSA turned negative .During a follow-up period of 2 years ,renal graft showed an excellent function with a serum creatinine of 31 μmol/l and eGFR of 110 ml/min/1 .73m2 .No AR episode or proteinuria occurred .DSA stayed negative .Simultaneously physical development also caught up .Her height of 93 cm tall and weight of 13 .5 kg at month 24 & 8 months corresponded to normal grow th curve of her age .Conclusions Pre-sensitized infant could tolerate desensitization therapy well and achieve satisfactory outcomes .With surgical precisions and optimized managements ,kidney transplantation provides excellent renal functions and survivals for infants with organs from deceased donors .
8.Treatment of Respiratory Diseases with Banxia Houputang: A Review

Respiratory diseases are common, frequently-occurring clinical diseases. As the prevalence rate is increasing year by year, they have become a problem that seriously affects public health. The diseases are mainly located in the lung by traditional Chinese medicine (TCM) syndrome differentiation. Lung governs Qi and controls breathing and is also an organ for the storage of phlegm. Clinically, phlegm and Qi are often used for the treatment. Banxia Houputang (BHT), originated from Synopsis of the Golden Chamber (《金匮要略》), was used to treat plum-stone Ai (globus hystericus) at first. It is composed of Rhizoma Pinelliae, Cortex Magnoliae Offcinalis, Poria, Rhizoma Zingiberis Recens, and Folium Perillae, and treats diseases with the core pathogensis of mutual obstruction of phlegm and Qi. BHT has the effects of moving Qi, dissipating mass, descending adverse Qi, and resolving phlegm, which basically correspond to the pathological characteristics of the lungs. Clinical studies have confirmed that modified BHT can be used either alone or in combination with western medicine to treat chronic pharyngitis, asthma, chronic obstructive pulmonary disease, pneumonia, obstructive sleep apnea, upper airway cough syndrome and other respiratory diseases, with significant effects. It effectively improves the symptoms and signs of the diseases and reduces the recurrence rate. Basic research has shown that BHT plays anti-inflammatory, anti-oxidative stress, anti-apoptotic, autophagy-regulating, and iron overload-regulating roles by regulating the targets in multiple pathways. This paper, by combing the relevant literature in recent years, conducted a systematic review on BHT from the three aspects of syndrome analysis, clinical treatment research and mechanism research, with a view to providing theoretical basis and reference for the mechanism research of BHT in treating respiratory diseases and for expanding its clinical application.
9. Comparison of clinical values in decision making between cone beam CT and conventional approach in furcation involvement of maxillary molar

Objective:
To compare the use of cone beam computed tomography (CBCT) and conventional approach (clinical probing and periapical radiograph) in assessing furcation involvement (FI) of maxillary molars and making treatment decisions.
Methods:
Thirty-two subjects were selected from the pool of patients at the Department of Periodontology, Tianjin Stomatology Hospital & Hospital of Stomatology, NanKai University from January 2015 to December 2016. All the patients have a diagnosis of generalized chronic periodontitis. Ninety-five maxillary molar with FI from the radiography database were analysed. Two doctors used conventional approach and CBCT images to assess FI and make treatment decision respectively. If the clinical and radiographic findings did not clearly indicate a distinct periodontal therapy, two treatment options were considered.
