1.The correlation between hyperinsulinemia and pulse wave velocity

Objective To study the correlation between hyperinsulinemia (HIns) and arteriosclerosis in one community in Beijing. Methods Subjects who received arteriosclerosis screening in physical examination annually were studied. All subjects were received 75g oral glucose tolerance test (OGTT) to evaluate glucose metabolic level, and brachial-ankle pulse wave velocity (baPWV) examination to evaluate arteriosclerosis. The correlation between hyperinsulinemia and pulse wave velocity was analyzed. Results Among all the 1046 subjects under investigation, baPWV of subjects with HIns was higher than subjects with normoinsulinemia (NIns) in different glucose metabolism status [normal glucose tolerance, ( 1381.2 ±280. 8) cm/s vs ( 1280. 3 ±218. 7) cm/s; imparied glucose regulation, ( 1557. 5 ±319.3) cm/s vs (1474.7 ±305. 1) cm/s; diabetes, (1764.3 ±476.6) cm/s vs (1664.2 ±374.6)cm/s], especially in subjects with normal glucose tolerance ( P < 0.01 ). The prevalence of cardiovascular risk factors in subjects with HIns was much higher than subjects with NIns ( P < 0.01 ). Multiple logistic regression analysis showed that hyperinsulinemia was the risk factor of arteriosclerosis, and the OR (95%CI) of subjects with HIns was 1.91 (1. 169-3. 105, P <0.01 ) as compared to the subjects with NIns. Conclusion The subjects with HIns suffered from much more metabolic risk factors than NIns.Hyperinsulinemia that closely correlated with baPWV was a risk factor of arteriosclerosis.
2.The effects and mechanisms of BTBD10 on the proliferation of islet beta cell

ObjectiveTo explore the role of BTBD10 overexpression in the proliferation of insulinoma cell line INS-1and its mechanism. MethodsThe recombined expression plasmid of pcDNA4.0-BTBD10 was constructed by gene cloning technique and was transfected into INS-1 cell by lipofectamine 2000. The stable overexpression BTBD10 of INS-1cell was selected at 48th hour after transfection.INS-1 cell proliferation activity was measured by MTT method.The expression of BTBD10,protein kinase B(Akt),phospho-Akt(p-Akt),mammal target of rapamycin (mTOR) and phospho-mTOR (p-mTOR) were determined by Western blot.ResultsThe stable overexpression BTBD10 of INS-1 cell wassuccessfullyconstructed.OverproductionofBTBD10promotedbetacellproliferation.The phosphorylation of Akt and mTOR was increased and the ratio of p-Akt/Akt and p-mTOR/mTOR was enhanced in the INS-1 overexpressed by BTBD10.But the expression of total Akt and mTOR presented no obvious changes. Conclusion The overexpression BTBD10 of INS-1 cell could activate of Akt/mTOR signalling pathway via stimulating phospho-mTOR and Akt,and enhance overall cell protein translation,so as to promote proliferation of INS-1 cell.

The heterogeneity of high-density lipoprotein-cholesterol (HDL-C) in hyperuricemia was analyzed in Uygur,Han,and Kazak ethnic groups in Xinjiang.A total of 2 396 subjects were surveyed.The HDL-C levels were significantly decreased in hyperuricemia group compared with control group in Han and Uygur,while the reverse occurred in Kazak.The serum uric acid (SUA) level in Kazak was the lowest.The SUA was negatively correlated with HDL C in Kazak (P < 0.05).HDL-C was a recognized protective factor for cardiovascular disease.With the lowered SUA level in the Kazak group,HDL-C level was increased.When SUA level was higher than a certain level (such as the SUA-4),there existed a negative impact on cardiovascular disease
4.Regulating Effects of Shen-Xiong Hua-Yu Capsule Preconditioning on Expression Subtype of NOS in Cerebral Ischemia-reperfusion Injury among Rats

This study was aimed to observe the regulating effect of Shen-Xiong Hua-Y u (SXHY) capsule precondi-tioning on the expression of subtypes of nitric oxide synthase (NOS), including endothelial nitric oxide synthase (eNOS), nervous nitric oxide synthase (nNOS), inducible nitric oxide synthase (iNOS), in cerebral ischemia-reperfu-sion injury (CIRI) among rats, and to further clarify the mechanism of protective effect by SXHY capsule on acute CIRI rats. Rats were randomly divided into the sham operation group, CIRI group, and SXHY capsule of high-, medium-, and low-dose preconditioning group (480, 240, 120 mg·kg-1). Each group was further randomly divided into different subgroups, which were the 3, 6, 12, 24, 48, 72 h group after 2 h CIRI (n=6). Intragastric administration was given once a day for 7 days. The middle cerebral artery occlusion (MCAO) and reperfusion model was repro-duced by an intraluminal filament method on the 7th day. The protein expressions of eNOS, nNOS and iNOS were measured by immunhistochemical method. The results showed that compared with the sham operation group, expres-sions of eNOS, nNOS and iNOS in the CIRI group were increased at different time points (i.e., 3, 6, 12, 24, 48, 72 h, P< 0.05 or P< 0.01). Compared with the CIRI group, eNOS expression increased at different time points in SX-HY capsule group (P< 0.05 or P< 0.01). The nNOS and iNOS expression decreased at different time points (P<0.05 or P < 0.01). Among them, the high-dose group was the group with the most obvious effect. It was concluded that SXHY capsule preconditioning had protective effect on CIRI. Its mechanism may be related to the regulation on protein expression of NOS subtypes.
5.Nursing experience of inhalation of steroid for patients after endoscopy surgery

Objective To explore the nursing experience of inhalation of steroid for patients after endoscopy surgery. Methods From September to December 2016, we selected 200 patients with transnasal endoscopic frontal surgery of Allergy Department at Beijing Tongren Hospital, Capital Medical University as subjects. All of the patients were treated with the inhalation of glucocorticoid. Nurses provided the guidance of inhalation of steroid for patients and observed the effects. Results A total of 200 patients accepted inhalation. After inhalation, patients' subjective symptoms improved. A total of 182 patients were very satisfied with the nursing service and 18 patients were satisfied with the nursing service. Three months after surgery, there were 152 patients without edema, 24 with the mild edema and 24 with obvious edema in operation cavity of patients. Conclusions Reasonable guidance by nurses can optimize the effects of inhalation of steroid, promote the recovery of patients after surgery and improve patient satisfaction.
6.Correlation between subjective symptoms and self-management efficacy in patients with chronic rhinosinusitis and nasal polyps after endoscopic sinus surgery

Objective:To understand the current status of subjective symptoms and self-management efficacy in patients with chronic rhinosinusitis and nasal polyps after endoscopic sinus surgery, and analyze the correlation between the two.Methods:This study was a cross-sectional study. From July 2018 to June 2020, a total of 228 patients with chronic rhinosinusitis and nasal polyps who underwent endoscopic sinus surgery in a ClassⅢ Grade A hospital in Beijing were selected as the research object. The survey was conducted using the General Information Questionnaire, Visual Analogue Scale (VAS) , and Chronic Disease Self-Efficacy Scale (CDSES) . Pearson correlation and Spearman rank correlation were used for the correlation analysis.Results:Among 228 patients with chronic rhinosinusitis and nasal polyps, there were still 184 cases (80.70%) of nasal congestion, 134 cases (58.77%) of runny nose, 116 cases (50.88%) of olfactory symptoms, 112 cases (49.12%) of head and face pain symptoms, and 208 cases (91.22%) of general discomfort. Among 228 patients with chronic rhinosinusitis and nasal polyps, the symptom management self-efficacy dimension in CDSES score was (6.19±1.04) , and the disease common management self-efficacy dimension score was (7.42±0.95) , and there were 32 cases with a total score of ≥7, accounting for 14.04% (32/228) . The VAS score of various subjective symptoms was negatively correlated with the total score of CDSES and the score of each dimension, and the difference was statistically significant ( P<0.05) . Conclusions:Patients with chronic rhinosinusitis and nasal polyps still have disease symptoms after endoscopic sinus surgery, and the self-management efficacy needs to be further strengthened. Clinical nurses should pay attention to subjective symptoms of these patients after surgery, teach them how to manage disease symptoms, and improve the patient's self-management efficacy.
7.Basic research in sepsis: advances and challenges

Although the understanding of sepsis has evolved from "sepsis 1.0" to "sepsis 3.0", and the consensus on clinical management of sepsis has been continuously updated, the incidence rate and mortality of sepsis remain high. Therefore, in-depth investigation of the pathogenesis and related influencing factors of sepsis is of great significance for revealing the nature of sepsis and improving the clinical outcome of sepsis patients. This review will focus on the key issues in the basic research of sepsis, and summarize the recent advances and challenges in this field, mainly including genetic polymorphism, microorganisms, high-mobility group box 1 (HMGB1), endothelial dysfunction, immunotherapy, and biomarkers, aiming to provide new insights for the diagnosis and treatment of sepsis.
8.Cutpoint and clinical significance of HbA1C for diabetes diagnosis in a cross-sectional study

Objective To compare the difference of cutpoint and clinical significance of HbA1C for the diagnosis of abnormal glucose metabolism in two population groups with different ages.Methods According to oral glucose tolerance test(OGTT),the cutpoint and clinical significance of HbA1C for the diagnosis of type 2 diabetes and impaired glucose regulation(IGR)were investigated in the two population groups.Results The mean HbA1C of 1 064 young subjects in an academy and 1 671 aged subjects in a community were 5.31% ±0.41% and 5.79% ±0.71%,respectively.The cutpoints of HbA1C for diagnosis of diabetes were 5.7%(specificity 86.7%,sensitivity 66.7%)and 5.9%(specificity 73.8%,sensitivity 80.1%)in the two population groups,and 5.6% for diagnosis of IGR (specificity 82.8%,sensitivity 55.8%)and 5.7%(specificity 60.9%,sensitivity 64.3%),respectively.87.8%,78.7%,and 38.5% were diagnosed diabetes by current OGTT criteria at HbA1C levels of ≥5.7%,≥5.9%,and≥6.5%,IGR being 61.6%,39.6%,and 4.1%,and normal glucose tolerance being 24.4%,10.0%,and 0.4%.Conclusion The cutpoints of HbA1C for diagnosis of diabetes and IGR are different in populations with different ages and HbA1C levels.As one of diagnostic criteria for diabetes,HbA1C 6.5% with relatively higher specificity and lower sensitivity must be combined with fasting blood glucose,random blood glucose,and OGTT.
9.Relationship between childhood maltreatment and suicide ideation of college students

Objective:
To explore the relationship between childhood maltreatment and suicide ideation of college students,so as to provide basis for physical and mental health of college students.
Methods:
Students from 6 universities in Shanxi Province(3 854 college students provided eligible questionniare for childhood abuse and 3 882 for suicidal ideation) were selected by multi-stage random cluster sampling from May to July 2018, the Chinese Version of the Childhood Trauma Questionnaire and the Self-rating Idea of Suicide Scale were used in the survey.
Results:
Of all subjects,42.4% had at least one kind of abuse experience in childhood and 3.9% had suicidal ideation in the past week. By chi square test,the detection rate of suicidal ideation of college students who suffered physical abuse,emotional abuse,sexual abuse,physical neglect,emotional neglect in childhood was higher than that of college students without maltreatment (χ2=13.78,12.97,17.10,56.56,66.58,P<0.01);Spearman correlation analysis showed that there was a positive correlation between each type of maltreatment in childhood and college students’ suicidal ideation(r=0.06,0.06,0.07,0.12,0.13,P<0.01);Multivariate Logistic regression model found that after controlling other influencing factors,physical neglect and emotional neglect in childhood were independent risk factors for suicide ideation of college students (OR=2.18,2.07,P<0.05).
Conclusion
Physical neglect and emotional neglect in childhood can increase the risk of college students’ suicidal ideation. Paying enough attention and care to them in early childhood may help to prevent their suicidal ideation in adulthood.
10.Clinical significance of serum glial fibrillary acidic protein level in middle-aged and elderly patients with Parkinson's disease

Objective:To investigate the clinical significance of serum glial fibrillary acidic protein(GFAP)level in middle-aged and elderly patients with Parkinson's disease(PD).Methods:In the prospective study, a total of 39 patients with PD hospitalized in the Department of Neurology of Zhejiang Hospital affiliated to Zhejiang University School of Medicine, and 17 healthy subjects from January 2017 to May 2021 were collected.Serum GFAP levels in the PD group and healthy control(CT)group were detected by an ultra-sensitive Simoa hypersensitive protein detection technology.The correlations of serum GFAP level with age, gender, clinical presentation type, depression score, Montreal Cognitive Assessment Scale(MOCA)score and Mini-Mental State Examination(MMSE)score were analyzed.Results:The level of serum GFAP was significantly higher in PD group(219.6±166.2)ng/L than in CT group(109.9±56.6)ng/L( P< 0.01). In PD group, there was no correlation of serum GFAP with age, gender, clinical classification, depression and MOCA score(age: r=0.042, gender: r=-0.142, depression score: r=0.076, MoCA score: r=0.014, all P>0.05); but there was a significant negative correlation between serum GFAP and MMSE score( r=-0.433, P< 0.05). Conclusions:There is a negative correlation between serum GFAP level and MMSE score, suggesting that the increase of serum GFAP might be suggestive of cognitive decline in Parkinson's disease patients to some extent, which should be paid attention to in clinical work.
