1.Clinical Observation of Electroacupuncture plus Needle-cupping for Lumbar Intervertebral Disc Herniation

Objective To observe the clinical efficacy of electroacupuncture plus needle-cupping in treating lumbar intervertebral disc herniation (LIDH). Method Eighty patients with LIDH were divided into an observation group and a control group by random number table, 40 in each group. The observation group was intervened by electroacupuncture plus needle-cupping, while the control group was by electroacupuncture plus cupping. The clinical efficacy, Modified Japanese Orthopaedic Association Scores (M-JOA) and Visual Analogue Scale (VAS) were observed and comparatively analyzed. Result The total effective rate was 95.0% in the observation group, significantly different from that of the control group (P<0.05); the M-JOA scores increased significantly (P<0.05) and VAS scores dropped significantly (P<0.05) in both groups, and the changes of the scores were markedly different between the two groups (P<0.05), indicating that the improvement in the observation group was more significant. Conclusion Electroacupuncture plus needle-cupping can produce a significant therapeutic efficacy in treating LIDH.
2.Clinical study of reformed laparoscopic-assisted vaginal hysterectomy of large uterus with sutureless of cardinal and uterosacral ligament

Objective To compare the clinical efficacy between reformed laparoscopic-assisted vaginal hysterectomy with sutureless of cardinal and uterosacral ligament(LAVH)and trans abdominal hysterectomy(TAH)for large uterus.Methods A retrospective study was performed using 106 reformed LAVH patients(observation group)and 100 TAH patients(control group)whose uterus were big(≥ 12weeks of gestation),the information was observed in perioperative period.Results Anal exsufflation time,hospitalization time after operation and postoperative pain time in observation group[(25.4 ± 3.3)h,(6.5 ±1.5)d,(16.9 ± 2.4)h]were significantly shorter than those in control group[(42.4 ± 5.3)h,(8.5 ± 1.5)d,(30.6 ± 5.3)h](P < 0.05),the rate of complication in observation group[5.7%(6/106)]waa lower than that in control group[10.0%(10/100)](P< 0.05).Conclusion As to large uterus,reformed LAVH has several advantages over TAH,the technique is easy to learn,operation time is short,and the rate of complication is low.
3.Progress in the study of HIV-1 Vif and related inhibitors

Human immunodeficiency virus type 1 (HIV-1) viral infectivity factor (Vif), one of the accessory proteins, which is a small basic phosphoprotein, is essential for viral replication and pathogenesis. The best well-characterized function of Vif is its ability to neutralize the host cell antiviral factor, apolipoprotein B mRNA editing enzyme catalytic polypeptide like 3G (APOBEC3G), which makes the viral particles more infective. In addition, Vif can regulate the reverse transcription and the advanced stage of replication of the virus particle, as well as induce the termination of cell cycle at G2 stage and so on. The designed drug aimed directly at Vif can efficiently block the maturation and infectivity of HIV-1. In this review, the structure, function and especially the related inhibitors of Vif are reviewed.
4.The effect of simvastatin and dexamethasone on the expression of endothelial nitric oxide synthase mRNA and the proliferation in osteoblast

Objective To study the effect of simvastatin and dexamethasone on the expression of endothelial nitric oxide synthase(eNOS)mRNA and the proliferation in osteoblast.Methods The osteoblast was collected from newborn SD rats,skull and cultured,and then randomly divided into 3 groups(8 samples for each group):in group A,the cells were treated with simvastatin of 1?10-7 mol/L,dexamethasone of 1?10-7 mol/L and DMSO of 1‰;in group B,the cells were treated with dexamethasone of 1?10-7 mol/L and DMSO of 1‰;in group C,the cells were treated with only DMSO of 1‰.The cells were collected and total RNA were obtained 96 h after culture.The expression of eNOS mRNA in the cells was analysed with Reverse Transcription-Polymerase Chain Reaction(RT-PCR).The amount of nitric oxide(NO)in culture medium was detected by the method of nitric acid reduction enzyme.The ratio of proliferation of osteoblast was measured using MTT reduction assay.Results The expression of eNOS mRNA in group A,B,C was 0.491?0.014,0.421?0.018,0.489?0.014.The level of NO in the culture medium in group A,B,C was(14.37?1.24)?mol/L,(12.94?0.69)?mol/L,(14.14?1.23)?mol/L.The ratio of osteoblast proliferation in group A,B,C was 0.3055?0.0178,0.2659?0.0105,0.3044?0.0213.There were significant differences of the expression of eNOS mRNA,level of NO and ratio of osteoblast proliferation between group A and group B,group B and group C(P 0.05).Conclusion It demonstrates that the lower expression of eNOS in the osteoblasts induced by dexamethasone may be an important pathogenesis of glucocorticoid-induced osteoporosis.Simvastatin may inhibit the effect of dexamethasone and regulate the expression of eNOS mRNA in the cells and protect the proliferation of osteoblast.

Objective To resolve the difficulty in controlling the transfusion speed by medical personnel.Methods By using liquid drop sampling module,pulsing signals of liquid drop were obtained,and then they were enlarged,shaped and the wrong signals from the top and bottom surface of every drop were excluded before the standard triggered signals were sent out to SCM processing circuit,from which the real-time liquid drop speed and the average drop speed in every minute were obtained.Results Anti-interference and portable transfusion-drop counter could show the real-time liquid drop value and the average liquid drop value in every minute.Conclusion Anti-interference and portable transfusion-drop counter is convenient with simple structure and has been used in clinic.
7.Development of the Wounded Information Collection and Management System

Objective 70 realize the fast collection and management of the basic wounded information in the field treatment. Methods Two-dimensional electronic medicine tag was used as a massage loader. The wounded information was analyzed with a program reader, and the basic personal and cure information were shared with the information management system. Result The system was used in the massive discipline for the first time, fulfilled the wounded information record, analysis, transfer, and the management of the cure information with 80 items. Conclusion The correctness of the basic massage record and analysis is full percent. The manage progress of the system accord with the field condition.

Conclusion The device usage is convenient, the function is stable, and the rate of collection and analysis is 100%.
9.Onlay mesh in situ hernia repair for parastomal hernia: a report of 42 cases

Objective To evaluate hernia repair with Onlay (premuscular positioning of the prosthesis) for the treatment of parastomal hernia.Methods In this study 45 patients with parastomal hernia were treated with a tension-free manner of hernia repair in situ by Onlay using prolene mesh between February 2006 and April 2012.There were 24 males and 21 female cases,including 6 recurrent parastomal hernia cases.The average age was (49 ±8) years ranging from 43 to 80.The parastomal defect ranged from 4 cm to 9 cm.Results All operations were successful.There was no hospital death or severe postoperative complications.The mean operating time was (109 ± 23) min (96-148),and the average time of pulling out the gastrointestinal decompression tube was (2.0 ± 2.0) day (12 h-3 d) and the drainage tube was removed after (5.0 ± 2.0) days (4-9 d).The mean postoperative hospitalization was (11 ± 3) days (9-16 d).42 cases achieved primary healing.The serofluid swelling in 7 cases was cured by puncturing and pressing the incision.41 cases were followed-up from 8 to 48 months,and 4 patients suffered from hernia recurrence respestively after 3 months,4 months,4.5 months and 7 months.One recurrent case was healed by conservative therapy and the other three were healed by reoperation.Conclusions Hernia repair in situ with Onlay method for parastomal hernia with prolene mesh was a safe and effective.
10.SpyGlass-guided laser lithotripsy versus laparoscopic common bile duct exploration for large common bile duct stones: a non-inferiority trial

Objective:To evaluate the clinical efficacy and safety of SpyGlass-guided laser lithotripsy for large common bile duct (CBD) stones with diameter>2 cm.Methods:From August 2015 to August 2018, a total of 157 patients with large CBD stones at the First Affiliated Hospital of Shandong First Medical University who met the inclusion criteria were randomly divided into SpyGlass group ( n=78, underwent SpyGlass-guided laser lithotripsy) and laparoscopic common bile duct exploration (LCBDE) group ( n=79, underwent LCBDE) by using random numbers. Non-inferiority test was used for rates of one-time stone removal and total stone removal, and the non-inferiority margin was set to 10%. The transform rate, incidence of short-term complications, hospital stay, and quality of life (assessed by the gastrointestinal quality of life index) were compared between the two groups. Results:The total success rates of stone clearance were 92.3% (72/78) and 96.2% (76/79) in the SpyGlass group and LCBDE group, respectively ( P=0.023), with valid non-inferiority hypothesis. The one-time stone removal rates were 83.3% (65/78) and 96.2% (76/79), respectively ( P=0.124), with invalid non-inferiority hypothesis. There were no significant differences in the incidence of transform [7.7% (6/78) VS 3.8% (3/79), P=0.294] or short-term complications [5.1% (4/78) VS 10.1% (8/79), P=0.246] between the two groups. Compared with the LCBDE group, the SpyGlass group had a shorter hospital stay (5.65±0.94 d VS 8.84±1.54 d, P=0.001) and higher scores of gastrointestinal quality of life index (1 month after operation: 99.85±4.36 VS 91.51±5.47, P=0.001; 3 months after operation: 131.24±3.32 VS 112.32±7.77, P=0.001). Conclusion:For large CBD stones, the efficacy of SpyGlass-guided laser lithotripsy is not inferior to LCBDE, and it is less invasive. In the future, SpyGlass-guided laser lithotripsy could be an important option for the treatment of large CBD stones.
