1.Therapeutic Effects of Magnesium Sulfate Combined with Budesonide and Terbutaline Sulfate Inhalation for the Treatment of Bronchiolitis

Objective To observe the clinical efficacy of magnesium sulfate inhalation combined with budesonide and terbutaline sulfate nebulizer for the treatment of bronchiolitis.Methods Sixty children with bronchiolitis were randomly divided into observation group and control group with 30 cases in each group.Children in the control group received routine treatment and the observation group were inhaled with magnesium sulfate combining budesonide and terbutaline sulfate.Curative effects were evaluated in the aspects of the clinical symptoms,serum inflammatory factors and overall clinical efficacy.Results The symptoms in children including asthma,shortness of breath,coughing,wheezing and lung auscultation were significantly alleviated within a shorter period of time than that in the control group (P<0.05).The serum levels of TNF-u,IL-6,IL-8 and TIgE were significantly reduced in both groups after the treatment (P<0.05).But in the observation group,the alleviation of the symptoms was significantly greater than that in the control group (P<0.05).The overall effective rate in the observation group and the control group was 93.3% and 70.0% respectively (P<0.001).Conclusion The clinical efficacy of inhaling magnesium sulfate with budesonide and terbutaline sulfate to treat bronchiolitis is satisfactory and worth clinical application.

3.Effects of different doses of dexmedetomidine on etomidate in inhibiting the EC50 of laryngeal mask placement

Objective To investigatethe EC50 of different doses of dexmedetomidine on etomidate inhibited responses tolaryngeal maskinsertion in patients.Methods 88 with breast cancerfrom surgical department in Zhuji Hospital of Traditional Chinese Medical from August 2014 to August 2015were selected and randomly divided into the control group(groupA)and the experiment group(group B1,group B2 and group B3)with 22 casesin each group,respectively,intravenous pump 0.9%sodium chloride solution and DEX(dose followed by 0.3,0.6,0.9μg/kg).The next sequential intravenous infusion of etomidateafter 10 min.The EC50 and the 95%confidence interval of etomidate were determined by sequential method in each group of patients,the changes of vital signs and adverse reactionsin patients were monitored.Results Four groups of patients with LMA EC50 and 95%confidence interval of etomidate respectively:0.78(0.723~0.835)μg/mL in group A,0.66(0.612~0.711)μg/mL in group B1,0.58(0.532~0.627)μg/mL in group B2,0.46(0.416~0.521)μg/mL in group B3.Four groups of patients with laryngeal mask insertion immediately before MAP and HR were lower than the baseline value,the difference was statistically significant(P<0.05),but the LMA elevated after one min.The control group of four patients with respiratory depression,three cases of patients with bradycardia in group B3,two cases of patients with hypotension were improved after symptomatic treatment.Conclusion In a certain range,increasing the dose of dexmedetomidine reduces the effective concentration of etomidate,which inhibits the laryngeal mask placement reaction.
4.Symbolic dynamics of heart rate variability in myocardial ischemia

Objective To investigate the autonomic modulation and influence during myocardial ischemia based on analysis on heart rate dynamics with heart rate variability(HRV). Methods From 13 free download ECG records in long-term ST-T(LTST)database, 193 ischemia episodes were extracted for analysis, and 5 min intervals before and after each selected isehemia episode were selected as control ones. Using symbolic dynamic analysis method, the indexes during different stages were calculated. The repeated measurement analysis of variance was introduced for statistic test. And the distribution of autonomic reflex mode to isehemia duration was analyzed by linear fitting. Results Symbolic analysis detected an increase in the percentage of non-varlable patterns (0 V%) and a decrease in the percentage of very variable patterns(2 UV%), indicating a more sympathetic predominance compared with the baseline. But the trend of the shift in autonomic balance towards sympathetic predominance reduced with the extension of ischemic duration, suggesting a possible protection for heart. Conclusion Symbolic dynamic analysis is capable of identifying changes in autonomic modulation and seems appropriate for elucidating the neural pathophysiological mechanisms occurring in myocardial ischemic episodes.
5.Effect of lentinan injection in different treatment administration of oxaliplatin combined with capecitabine in gastric cancer and the effect on the peripheral blood of patients with CEA and CA199 levels

Objective To investigate the effect of lentinan injection in different treatment administration of oxaliplatin combined with capecitabine in treatment of gastric cancer and the effect on the peripheral blood of patients with CEA and CA199 levels.Methods 90 cases in our hospital from September 2009 to September 2012 were selected as research subjects, according to patients with lentinan injection treatment, the patients were divided into A, B and C three groups, the effect and the adverse effects of chemotherapy in three groups of patients, detection of serum CEA and CA-199 levels. Results CR, PR, SD and PD of A groups were 12, 7, 8, 3 cases, RR was 63.3%, obviously higher than that of B, C two groups, the difference was statistically significant (P<0.05), group A patients with leukopenia I,II and III degree were 15, 10, 5 cases of gastrointestinal reaction, I, II, III were 12, 10, 8 cases of abnormal liver function, I, II and III degree were 14, 9, 7 cases.A group, leukopenia, gastrointestinal tract, liver function abnormal lesion was lower than the B group and C group, the difference was statistically significant (P<0.05), CEA, CA199 level before treatment compared to no difference, one course of chemotherapy, after two courses of A, B, C three groups of patients with CEA and CA199 level were before treatment decreased, and the level of group A was significantly lower than that of B, C two B, C group, no significant difference between the two groups. Conclusion Lentinan injection helps to improve the sensitivity of chemotherapy in patients with gastric cancer, reduce adverse drug reactions, reduce the level of tumor markers, improve the prognosis of patients with gastric cancer.
6.Risperidone inhibits 3T3-L1 pre-adipocytes differentiation

Objective To investigate the influence of risperidone on differentiation of 3T3‐L1 pre‐adipocytes .Methods 3T3‐L1 pre‐adipocytes were induced to differentiate into mature adipocytes by adopting the classic hormone cocktail method and observed by the oil red O staining .Meanwhile ,the inducing medium was added with risperidone for studying its influence on 3T3‐L1 pre‐adi‐pocytes differentiation .Results 3T3‐L1 pre‐adipocytes were successfully differentiated into the mature adipocytes ,0 .1 ,1 ,10μmol/L risperidone all could inhibit the differentiation of 3T3‐L1 pre‐adipocytes .Conclusion Risperidone can inhibit the differentiation of 3T3‐L1 pre‐adipocytes .
7.Analyzing Present Situation and Countermeasures of Incentive Mechanism of Scientific research at the Hospital

Objective To analyze current situation of incentive mechanism and investigate the countermeasures.Provide the evidence for improving the incentive mechanism of scientific research at the hospital.Methods Questionnaire survey was conducted to investigate current situation and countermeasures of incentive mechanism of scientific research.Database was building using EpiData3.1,and was performed by descriptive analysis.Results 194 questionnaires were available.For the current incentive mechanism,86.5% respondents thought the content is comprehensive,The over 50 years old,bachelor or below,and clinical examination group thought the content were less comprehensive.Respectively,81%,82.5%,76.2% of respondents thought stimulation on achievements has positive effect to research,team work,and individual accounted for.The over 41 years old college or bellow,clinical treatment group felt less effect.For improving measures of incentive mechanism,the top three positions of contents were technological reward,high level project and SCI paper;and incentive methods were promotion,material rewards and training.Conclusions Incentive mechanism can promote scientific research to some extent.To further promote scientific research development,individual incentive measures need to be taken,the contents need to be expanded,variety incentive methods need to be used.
8.Influence of Astragalus and Thoroughfare Oral Liquid on immunological functions of immunosuppressive mice induced by hydrocortisone

Objective To study the effects of Astragalus and Thoroughfare Oral Liquid(ATOL) on the immunological functions of the immunosuppressive mice induced by hydrocortisone (HC). Methods A total of 50 mice were divided into 5 groups randomly. Mice in 4 groups were immunosuppressive animal model induced by HC, then ATOL (0.2, 0.1 and 0.05mL/10g) was administered to 3 groups of them. The phagocytotic function of peritoneal macrophage, the proliferation rate of T cells in spleen, and the cytotoxicity of NK cells in spleen, and the production of antibodies and IL-2 by spleen cells were examined to evaluate the immunological function of mice. Results Different doses of ATOL could improve the phagocytotic function of peritoneal macrophage, the proliferation rate of T cells in spleen and the cytotoxicity of NK cells. The drug could also promote antibody formation as well as IL-2 in the immunosuppressive mice. Conclusion ATOL can evidently improve some of the immunological functions in HC-induced immunosuppressive mice.

10.Effect of separating drug sales from medical services on hospital revenue and medical services in the county public hospitals of Guangxi Province

Objective:To evaluate the effect of separating drug sales from medical services on hospital revenue and medical services in the county-level public hospitals of Guangxi. Methods:The controlled before and after study design was employed. 2009 to 2012 was the pre-intervention period;2013 was the intervention period. Pilot people’s hospitals were included in the intervention group;non-pilot people’s hospitals were included in the control group. Da-ta came from hospitals and new rural cooperative medical statistics from 2009 to 2013 and the Guangxi Statistical Yearbook from 2010 to 2014. The analysis method of difference-in-differences based regression was employed. Re-sults:Separating drug sales from medical services included cancelling medicine markups, increasing price of inspec-tion and nursing services, reducing price of large equipment inspection services and increasing financial assistance. In terms of hospital revenue, compared with non-pilot hospitals, for pilot hospitals, the reform reduced medicine rev-enues by 3. 326 million yuan and increased medical revenue by 10. 75 million yuan. There was no significant change in financial assistance. In terms of medical expenses, compared with non-pilot hospitals, the reform reduced per-visit outpatient drug expenses in pilot hospitals by 3. 51 yuan, increased per-visit outpatient inspection fees by 2. 23 yuan, reduced per-visit inpatient drug expenses by 133. 5 yuan, increased per-visit inpatient inspection fees by 62. 01 yuan, and increased per-visit inpatient nursing fees by 69. 72 yuan. There were no significant change in outpatient and inpa-tient visits, length of stay, outpatient expenses per-visit and inpatient expenses per-visit. Conclusion:County hospi-tals can offset losses due to cancelling medicine markups by medical service pricing adjustment in inpatient departments;in outpatient departments, they can offset losses due to cancelling medicine markups by both medical service pricing ad-justments and medical service utilization adjustments beyond policy adjustments. The reform did not reduce the operating revenue of pilot hospitals or the medical expenses per visit. The reform had little effect on hospital and doctor incentives.
