1.Clinical Analysis of Revision Hip Arthroplasty in 16 Patients

Objective To research the cause of loosening of the prosthesis in advanced complication and after total hip joint replacement the technology in revision hip arthroplasty. Methods Using bone cement artifical prosthesis,16 patients were performed revision because the pain or the function failure in hip joint after total hip arthroplasty.Results ⑴Interfacial membranes were found around prosthesis in revision hip arthoplasty. ⑵Patients were followed up for an average 2.6 years.Conclusions ⑴Loosening of the prosthesis of late complication after total hip arthroplasty may relate to interfacial membranes. ⑵Throughout debriding and reconstrucling proper bone bed are key process in revision hip arthroplasty.
2.Role of phosphatidyl-inositol 3-kinase-Akt signal pathway in attenuation of ischemia-reperfusion injury by sevoflurane preconditioning in isolated rat hearts

Objective To investigate the role of phosphatidyl-inositol 3-kinase-Akt (PI3k-Akt) signal pathway in the attenuation of ischemia-reperfusion (I/R) injury by sevoflurane preconditioning in isolated rat hearts. Methods Ninety-six adult male SD rats weighing 220-280 g were randomly divided into 6 groups ( n = 16 each): sham operation group (group S); I/R group; sevoflurane preconditioning group (group SP); wortmannin group (group W); dimethyl sulfoxide (DMSO) group (group D) and sevoflurane preconditioning + wortmannin group (group SW) . Their hearts were excised and perfused in a Langendorff apparatus with K-H solution saturated with 95%O2-5%C02 at 37 ℃ . The hearts were continuously perfused for 180 min in group S. After 15 min of equilibration, the isolated hearts were subjected to 30 min of ischemia followed by 120 min of reperfusion in SP, W, D and SW groups. Croups SP, W, D and SW received 10 min of perfusion with K-H solution containing 2. 4% sevoflurane, 100 nmol/L wortmannin, 20 μmol/L DMSO, and 2.4% sevoflurane + 100 nmol/L wortmannin, respectively, followed by 5 min washout before I/R. Eight hearts in each group were selected and HR, left ventricular end-diabetic pressure (LVEDP), left ventricular developed pressure (LVDP), and ± dp/dtmax were recorded at the end of equilibration and at 15 min of reperfusion, Myocardial tissues were obtained at 15 min of reperfusion for determination of apoptosis (by TUNEL) and phosphorylated Akt (p-Akt) expression (by Western blot) . Another 8 hearts were selected at 120 min of reperfusion for determination of myocardial infarct size by TTC staining. Result Compared with group S, LVDP and ± dp/dt,^ were significantly decreased and LVEDP was significantly increased in groups I/R, SP, W, D and SW, and myocardial p-Akt expression was up-regulated in groups I/R, SP and D ( P < 0.05). Compared with group I/R, LVDP and ± dp/dtmax were significantly increased, LVEDP and apoptosis index were significantly decreased, myocardial p-Akt expression was up-regulated, and myocardial infarct size was significantly reduced in group SP (P <0.05) . Conclusion Activation of PI3K-Akt signal pathway is involved in the attenuation of I/R injury by sevoflurane reconditioning in isolated rat hearts.

4. Dynamic simulation and finite element analysis of gunshot wounds to the center of mandible mental region in human

Objective: To establish a finite element model of gunshot wound on the maxillofacial region in human, and to Conduct dynamic simulation and analysis of the injury process and degree of gunshot wounds to the center of mandible mental region, so as to explore a new method for establishing a gunshot wound model of human mandible. Methods: Based on the existing modeling method and internal parameters, digital CT scan data of human mandible were used to establish a three-dimensional finite element model of human mandible, which was used to dynamically simulate the process of gunshot wounds to the center of mandible mental region under different shot conditions. The residual velocities of the shootings with different projectiles at different entry angles and different impact velocities were calculated; the energy loss of the projectile and therate of energy loss after exiting the mandible were also calculated; and the obtained data were compared and analyzed. Results: The dynamic processes of gunshot wounds to the center of mandible mental region with two projectiles, three impact velocities, and three entry angles were successfully simulated. Meanwhile, thes imulation was also done for the stress distribution in different parts of mandible after injury. Computation and the analysis of the modeling data showed that these verity of injuries and the injury efficiency of projectiles differed with different injury conditions. Conclusion: Finite element model can dynamically simulate the injury process of gunshot wounds to the center of mandible mental region. It is found that the injury severity on mandible and the injury efficiency vary under different injury conditions. The finite element model is a potentially ideal model for studying the maxillofacial gunshot wounds.
5.In vitro induction of immune response by dendritic cells pulsed with TRAG-3-derived cytotoxic T lymphocyte epitope.

OBJECTIVETo explore the in vitro immune response to taxol resistance associated antigen 3 (TRAG-3)-derived cytotoxic T lymphocyte (CTL) epitope-pulsed dendritic cell (DC).
METHODSThe HLA-A2.1 restricted CTL epitope of TRAG-3 was previously identified to be a nonameric peptide sequence from 58 to 66 amino acid residues (TRAG-3(58-66)). The peptide was synthesized according to Fmos procedure, purified and molecular weight determined. Peripheral blood mononuclear cells (PBMC) of normal HLA-A2.1(+) individuals were used to isolate DC and DCs' phenotype identified by flow cytometry. Induction of CTL was achieved by in vitro stimulation of HLA-A2.1(+) PBMC with peptide-pulsed DC. CTL activity against HLA-A2.1(+)/TRAG-3(+) melanoma cell line LB373-MEL was assessed by (51)Cr release assay and IFN-gamma released by ELISA.
RESULTSThe synthetic nonameric peptide was above 90% pure and the determined values of molecular weight were conformed to their theoretical values. The phenotype of the isolate DCs was characteristic for mature ones. PBMC stimulated in vitro by TRAG-3-derived epitope-pulsed DCs for three times could specifically kill the target cells and secreted high concentration of IFN-gamma.
CONCLUSIONTRAG-3-derived epitope-pulsed DC can elicit specific anti-tumor immune response in vitro. Clinical trials using it as tumor vaccine may be feasible as specific immunotherapy for patients with TRAG-3 expressing cancers.

6.The change in IL-13 mRNA expression in rat lungs with acute pulmonary injury induced by lipopolysaccharide.

OBJECTIVETo observe the change in IL-13 mRNA expression in rat lungs with acute lung injury (ALI) induced by lipopolysaccharide (LPS), so as to investigate the significance of anti-inflammatory mechanism in the development of ALI.
METHODS(1) Different doses (2 approximately 8 mg/kg) of LPS were injected via the tail vein to the rats to establish ALI model. (2) The IL-13 mRNA content in pulmonary tissue was determined by RT-PCR.
RESULTS(1) Different degrees of ALI was induced in the rats by different doses of LPS. ARDS was produced in the rats by LPS of 6 mg/kg. (2) The IL-13 mRNA expression in the pulmonary tissue could be enhanced by the induction of LPS, especially when its dose was larger than 6 mg/kg.
CONCLUSION(1) Rat ALI model could be produced by LPS injection via the tail vein. (2) LPS in dose of 6 mg/kg was the threshold for the production of rat ARDS. (3) The incidence of rat ARDS might be closely related to the enhanced expression of IL-13 mRNA in rat pulmonary tissue.

7.Effects of Hesperidin on Osteoporosis through Anti-Inflammatory and Antioxidant Activity in Rat Models

Objective To study the anti-osteoporotic effect of different concentrations of hesperidin on ovariectomized rat model of osteoporosis(Ovariectomy,OVX)by reducing inflammatory molecules and oxidative stress response mechanism and the relationship between hesperidin and dose.Methods SD female rats were randomly divided into OVX group and Sham group,and OVX operation and sham operation were performed respectively.OVX group was randomly divided into 5 groups with 6 rats in each group,and 6 rats in Sham group were randomly selected as normal control group.They were Sham group(Sham rats+1%CMC suspension),control group(OVX rats+1%CMC suspension),HP1 group(OVX rats+5 mg·kg-1 HP),HP2 group(OVX rats+10 mg·kg-1 HP),HP3 group(OVX rats+20 mg·kg-1 HP)and AN group(OVX rats+2.5 mg·kg-1 AN).After 4 weeks of ovariectomized or sham operation,the drug was given by oral administration every morning for 10 weeks.After 10 weeks of administration,the change of body weight of rats was measured.Before the rats were killed,the serum of abdominal aorta was obtained and placed in the specimen cabinet at-20℃,the levels of Ca2+ and P5+,the levels of inflammatory factors TNF-α,IL-6 and IL-1β,and the levels of periosseous transition markers ALP,ACP,β-CTx and OC were also measured.After the rats were killed,the left femur was also placed in the specimen cabinet at-20℃.After treatment,the levels of oxidative stress factor TRARS and antioxidant enzymes GSH,GST,GR,GPx and CAT in the left femur were measured,and the bone mineral density,Young's modulus,maximum load,energy absorption and maximum stress in the middle part of the left femur were measured,and the expression level of SIRT1 gene was obtained.Results Compared with the control group,after 10 weeks of treatment with different concentrations of HP,the weight gain was significantly reduced(P<0.001),the levels of Ca2+ and P5+ in OVX rats were significantly decreased(P<0.001),and the levels of periosseous transition markers ALP,ACP,β-CTx and OC were significantly decreased(P<0.001).It significantly increased bone mineral density,Young's modulus,maximum load,energy absorption,maximum stress and stiffness(P<0.001),significantly decreased the level of OVX-induced thiobarbituric acid reactive substances and increased the levels of antioxidant enzymes GSH,GST,GR,GPx and CAT(P<0.001),and significantly decreased the levels of inflammatory molecules TNF-α,IL-6 and IL-1β(P<0.001).The expression level of SIRT1 gene was significantly decreased(P<0.001).Compared with the control group,the above parameters in the high concentration HP group were significantly different from those in the control group(P<0.001).Conclusion It is suggested that hesperidin can inhibit osteoporosis by reducing pro-inflammatory cytokines and oxidative stress in ovariectomized rats in a concentration-dependent manner.
8.Differences in Clinical Characteristics and Surgical Outcomes of Patients with Ischemic and Hemorrhagic Pituitary Adenomas

Objective:
: Ischemia and hemorrhage of pituitary adenomas (PA) caused important clinical syndrome. However, the differences on clinical characteristics and surgical outcomes between these two kinds apoplexy were less reported.
Methods:
: A retrospective analysis was made of patients with pituitary apoplexy between January 2013 and June 2018. Baseline and clinical characteristics before surgery were reviewed. All patients underwent transsphenoidal surgery and were followed up at least 1 year.
Results:
: Total 67 cases (5.8%) among 1147 pituitary tumor patients were enrolled, which consisted of 28 (~2.4%) ischemic PA and 39 (~3.4%) hemorrhagic PA. There were more male patients in the ischemic group compared with hemorrhagic group (78.6% vs 53.8%, p=0.043). However, the mean age, tumor size and functional tumor ratio were significant higher in the hemorrhagic group. Headache was more common in ischemic PA (82.1%) than that of hemorrhagic PA (51.3%, p=0.011). Magnetic resonance imaging findings found that mucosal thickening and enhancement of the sphenoid sinus was observed in 15 ischemic PA patients (n=27, 55.6%), but none in patients with hemorrhagic PA (n=38, p<0.0001). It was worth noting that the rate of pre-surgical hypopituitarism in ischemic PA patients were seemed higher than that in hemorrhagic PA patients, but not significant. The two groups got a total tumor resection rate at 94.1% and 92.9%, independently. No significant difference on the operative time, blood loss in operation and complications in perioperative period was observed in two groups. After operation, cranial nerve symptoms recovered to normal at 81.8% of ischemic PA patients and 82.6% of hemorrhagic PA patients. Importantly, the incidence of postoperative hypopituitarism partially decreased in both groups, among which the rate of hypothyroidism in ischemic PA patients significantly decreased from 46.4% to 18.5% (p=0.044).
Conclusion
: Patients with ischemic PA presented different clinical characteristics to the hemorrhagic ones. Transsphenoidal surgery should be considered for the patients with neuro-ophthalmic deficits and might benefit for pituitary function recovery of the apoplectic adenoma patients, especially pituitary thyroid axis in ischemic PA patients.

Objective: Chronic graft-versus-host disease (cGVHD) is a major long-term complication after allogeneic hematopoietic stem cell transplantation (allo-HSCT) . It is important to study the changes of serum biomarkers expression in patients for early diagnosis and treatment. Methods: The expression levels of five serum protein markers (IL-1b, IL-16, CXCL9, CCL19, CCL17) in patients with or without cGVHD after allo-HSCT were detected by liquid suspension microarray. Results: Compared with the control group without cGVHD, the expression levels of CXCL9 and CCL17 in serum of patients with cGVHD were significantly increased (P<0.05) . CCL17 was correlated with the severity of cGVHD (P<0.001) . CXCL9 was significantly increased in the serum of patients with skin lesion (P<0.01) , and CCL17 was significantly expressed in cGVHD patients with liver as the target organ (P<0.01) . Conclusion: The combination of CXCL9 and CCL17 can be used as serum biomarkers of cGVHD, which has certain reference value in assisting the diagnosis and evaluation of cGVHD severity.
