1.The effect ofTanreqing injection combined with conventional western medicine therapy on children with bronchioliti

Objective To detect the effect ofTanreqing injection combined with conventional western medicine therapy on children with bronchioliti.Methods A total of 116 children with bronchioliti were enrolled in this study. Children were randomly divided into the control group (n=58) andTanreqing injection observation group (n=58). The control group received continued low flow oxygen, anti-infection treatment, and salbutamol aerosol to relieve asthma. The observation group receivedTanreqing injection based on the intervention of control group. Each group was treated for 7 d. The Th17 and Treg cells were detected by cytometry analysis and the expression of IL-17 and IL-10 were detected by ELISA analysis. The clinical effect of the two groups was compared.Results After treatment, compared with the control group, the Th17 cells (5.91% ± 0.83%vs. 7.23% ± 1.25%,t=2.346) and IL-17 (14.62 ± 3.24 ng/mlvs. 22.52 ± 5.14 ng/ml,t=2.412) decreased significantly, and the Treg cells (7.32% ± 1.24%vs. 5.32% ± 1.03%,t=2.352) and IL-10 (15.84 ± 2.23 ng/mlvs.11.72 ± 2.12 ng/ml,t=2.365) increased significantly in the observation group (P<0.05). The temperature recovery time (t=2.264,P=0.048), disappearance time of coughing (t=2.437,P=3.406), wheezing(t=2.258,P=2.352), pulmonary rales (t=0.032,P=0.034) and hospital duration (t=0.046,P=0.042) in the control group were significantly higher than those in the observation group. The clinical effective rate (94.8%, 55/58) in observation group was higher than that (79.3%, 46/58) in control group (χ2=2.327,P=0.041). ConclusionTanreqing injection can regulate Th17/Treg cell balance, and its curative effect is superior to conventional therapy in children with bronchiolitis.
2.Effect of Ondansetron on postoperative nausea and vomiting in patients undergoing laparoscopic cholecystectomy with general anesthesia

Objective To investigate the influence of Ondansetron on postoperative nausea and vomiting(PONV)in patient undergoing laparoscopic cholecystectomy with general anesthesia and the nursins care. Methods Ninety patients undergoing laparoscopic cholecystectomy with general anesthesia were randomly divided into 3 groups(n=30):low-dose Ondansetron group(O1 group)received 4 mg Ondansetron before section,high-dose Ondansetron group(O2 group)received 8 mg Ondansetron before section and the control group(C group)received normal saline.All injections were diluted to 4 ml.The number and degree of nausea and vomiting wag recorded during 24 h after operation.Results Number of patients with PONV in O1 group and O2 group were less than that of C group,P<0.05,so was the PONV degree.No statistical difference was seen between O1 group and O2 group.Conclusion Intravenous injection of Ondansetron during operation can decrease the PONV in patient undergoing laparoscopic cholecystectomy with general anesthesia.
3.Survey of acute kidney injury in hospitalized patients

Objective To investigate the incidence of AKI and its relationship to mortality of inpatients by analyzing the changes of serum creatinine(SCr). Methods We collected the data of SCr in Peking Union Medical College Hospital through Jun 2006 to May 2007 and then selected the patients who were subjected to SCr determination more than one time. The relationship between the frequency of SCr determination and gender, age was analyzed. The relationship of increased SCr to gender, age, frequency of determination was also analyzed. The risk stratification based on SCr was investigated. In our study, we investigated the incidence of AKI in different diagnostic groups. The relationship between AKI and mortality in ICU and MICU unit was analyzed. Results There were 36 855 patients in one year, 16 934 patients were subjected to SCr determination only one time, 15 233 patients were subjected to SCr determination at least two times. Elder men were subjected to SCr determination more frequently (P<0.01). Along with the increase of SCr concentration, the frequency of SCr determination were increased significantly (P<0.01). Using the increasing of SCr exceeding 50% as the criteria for diagnosis of AKI, the incidence of hospital-acquired AKI was 8.46%, and it was higher in patients with injury and poisoning (16.7%), infection (16.0%), hematological system diseases (16.1%), neoplasms (12.7%). The incidence of AKI was 27.7% and 55.2% in ICU and MICU, respectively. Mortality of patients in MICU was increased along with the increasing of SCr level Mortality of patients with AKI in ICU was 23.3%, that was significant higher than patients without AKI, the adjust OR was 2.7 (P<0.01). Conclusions The incidence of AKI evaluated by analyzing SCr changing is significantly higher than that using experienced clinical diagnosis. This method is convenient in clinic for early diagnosis of AKI.
4.Tetrasubstituted Sulphonated Aluminum Phthalocyanine-Cat ionic Surfactant System for Fluorimetric Determination of Ethanol in Alcoholic Beverage

A new fluorimetric method was developed for the determi nation of ethanol in alcoholic beverage. The method is based on the principle that tetrasubstituted sulphonated aluminum phthalocyanine (AlS4Pc), a red-r egion fluorescent reagent, is induced to associate in the presence of cationic s urfactant, cetyltrimethylammonium bromide (CTMAB), thus its fluorescence is quen ched, and then the aggregate is disaggregated by the a ddition of ethanol and the fluorescence is recovered. This method has a linear determination range of 0.5%~90.0%(V/V), the detection limit is 0.48%(V /V). The method has been used to determine real alcoholic beverage samples w ith satisfactory results.
5.Practice of primary trauma care training system for undergraduate teaching reform in anesthesiology

The teaching team of undergraduates of anesthesiology in Anhui Medical University applied the primary trauma care system of encourage, heuristic teaching and practical teaching to further deepen the educational reform and improve teaching quality for undergraduate education.They designed the diversified section such as drills, discussion, teaching, questions, feedback and so on, implemented the simulation training of anesthesia crisis management skills and completed the feedback evaluation of comprehensive ability before and after the teaching, and then achieved the effect of improving the actual operation ability and clinical thinking capacity of students.So it is a good method and worth extending.
6.Treatment of posterior wall fractures of acetabulum.

OBJECTIVETo explore the treatment methods and outcome of posterior wall fractures of the acetabulum.
METHODSThe data of 31 patients (25 males and 6 females, aged 19-59 years, mean: 40.5 years) with posterior wall fractures of the acetabulum hospitalized in our department from 2002 to 2006 were analyzed retrospectively in this study. The types of fractures, number of fragments, combined dislocations, and sciatic nerve function were documented before admission. All the fractures were treated with open reduction and internal fixation. Based on the fracture type and site, either screws alone or reconstructive plates were used. The patients were immobilized for an average of 12 weeks before partial weight bearing was permitted. After follow-up for 12-70 months (43.6 months on average), modified Merle d'Aubigne score was adopted to evaluate the outcomes of the operations.
RESULTSThe percentages of the excellent, good, fair and poor results were 48.4%, 41.9%, 6.5%, and 3.3%, respectively, with a good to excellent rate of 90.2%. Idiopathic sciatic nerve injury occurred in only one case.
CONCLUSIONSThe sciatic nerve should be routinely exposed and protected during the surgery. The type of fixation should be based on the fracture type and site. Prolonged immobilization may be helpful in improving the final outcomes.

7.Selection of approach and fixation in the treatment of type C fracture of distal humerus in adults.

OBJECTIVETo study the surgical treatment for distal humeral fractures in adults according to the follow-up results.
METHODSTwenty-one cases (16 males and 5 females) of distal humeral fracture were included in this study. The average age was 42.5 years (range: 37-52 years). Fractures were classified according to the AO classification system. Nine cases of C1, 8 C2 and 4 C3-type fractures were identified. Open reduction and internal fixation were performed in all cases. A tricep-reflecting approach was adopted, and either the AO orthogonal plating or parallel plating technique was chosen, based on the fracture type. The plaster cast was removed 3 weeks after operation. Rehabilitation was encouraged during this period and afterwards. The average follow-up time was 12.2 months (range: 8-28 months). The outcome was scored according to Aitken and Rorabeek system.
RESULTSNo nerve injury, nonunion or failure of fixation was encountered during the operation and follow-up. However, ossifying myositis occurred in one case.
CONCLUSIONSA triceps-reflecting approach can provide adequate exposure to the joint. The use of AO orthogonal plating or parallel plating techniques based on the type of fractures can provide rigid fixation for the fracture.

8.Preliminary screening results of Alzheimer's disease among the elderly in Ningbo City

Objective:
To investigate the preliminary screening results of Alzheimer's disease (AD) among the elderly in Ningbo City, Zhejiang Province and analyze the influencing factors, so as to provide insights into prevention and control of AD in the elderly.
Methods:
A multi-stage stratified proportional sampling method was used to select the registered residents aged 60 years and older in Ningbo City as subjects. Demographic characteristics, lifestyle factors, physical conditions and social activities of the subjects was evaluated with questionnaires. AD was screened using the Mini-Mental State Examination (MMSE), and factors affecting the positive rate of AD preliminary screening were identified using a multivariable logistic regression model.
Results:
A total of 34 027 elderly residents were investigated, with an mean age of (71.71±6.89) years. There were 15 115 males (44.42%) and 18 912 females (55.58%), 16 146 residents living in rural areas (47.45%) and 17 881 residents living in urban areas (52.55%). The positive rate of AD preliminary screening was 7.28% (2 476 cases). Multivariable logistic regression analysis showed that age (70 to 74 years, OR=1.357; 75 to 79 years, OR=1.807; 80 to 84 years, OR=2.782; 85 years and older, OR=4.907), gender (female, OR=1.118), residence (urban areas, OR=0.713), marital status (unmarried, OR=3.078; widowed/separated/divorced, OR=1.301), educational level (primary school, OR=0.629; junior high school and above, OR=0.609), occupation before retirement (others, OR=0.741), family genetic history (no, OR=0.651), household disposable monthly income (1 000 to 1 999 Yuan, OR=0.636; 2 000 to 2 999 Yuan, OR=0.569; 3 000 Yuan and above, OR=0.448), exercise (seldom, OR=0.855; regular, OR=0.780), number of chronic diseases (2~5, OR=1.328), hearing loss (no, OR=0.764), smell loss (no, OR=0.615), chronic constipation (no, OR=0.696), major negative life events (no, OR=0.804), subjective memory loss (no, OR=0.583), sleep quality (general, OR=0.640; good, OR=0.616), living style (living with spouse, OR=0.300; living alone, OR=0.315; living with children, OR=0.350) and social activities (2-3 times/week, OR=0.584; 4 times/week and more, OR=0.409) were factors affecting the positive rate of AD preliminary screening.
Conclusions
The positive rate of AD preliminary screening among the elderly in Ningbo City is relatively high. Age, gender, economic conditions, exercise, social activities and physical conditions were factors affecting the positive rate of AD preliminary screening.
9.Reference intervals of serum calcium, phosphate and alkaline phosphatase of healthy school children and adolescents in Heilongjiang province

Objective To analyze the serum calcium , phosphate and alkaline phosphatase level among apparently healthy school children and adolescents in Heilongjiang Province and establish the reference intervals in different gender and different age groups .Methods Random sample of 2 101 primary and secondary school students from 9 to 18-year-old was collected from different district of Heilongjiang Province including Harbin and Mudanjiang from July 2008 to June 2009 .The fasting venous blood was collected and serum calcium , phosphate and alkaline phos-phatase concentration was measured as soon as possible .After screening outlier individual , a total of 2 091 subjects were enrolled.The reference intervals of serum calcium , phosphate and alkaline phosphatase for healthy school children and adolescents were established by gender and age ( P2.5 to P97.5 ) .Results The change tendency of serum calcium, phosphate and alkaline phosphatase with age and sex were presented .All these parameters had shown inverse relationship with age and were significantly higher in boys than girls (P<0.001).Serum calcium, phosphate of both sexes and serum alkaline phosphatase of girls declined progressively with age .However , the highest alkaline phosphatase level was seen at 13 years in boys.Alkaline phosphatase showed a positive correlation with age before 13 years of age , but after that alkaline phosphatase started to decline .Conclusions Reference in-tervals of serum calcium , phosphate and alkaline phosphatase for healthy school children and adolescents in Hei -longjiang Province in the age group of 9-18 years were presented , which supply good reference value for clinicians especially pediatricians to interpret the results of school children and adolescents and make decision .
10.Study on Gastric Cancer Susceptibility Gene Profiling and Risk Assessment in An Area in Shanghai

Aims:To explore gastric cancer susceptibility gene profiling in an area in Shanghai,and to assess the risk of gastric cancer susceptibility. Methods:A total of 152 patients with primary gastric cancer at Shanghai Pudong Hospital were enrolled,and 152 demographic characteristics matched patients with non-gastrointestinal diseases,non-tumor were served as controls. Gene polymorphism was determined by allele specific polymerase chain reaction. Susceptibility gene of gastric cancer was screened. Multiple genes interactions were analyzed and multiple genes risk was evaluated by DEMCHUK model. Results:Univariate analysis showed that CYP2E1,NAT2M1,NAT2M2,NAT2,XRCC1194,MTHFRA1298C,VDR TaqⅠ were susceptibility genes of gastric cancer. Multivariate analysis showed that CYP2E1(C1/ C1),NAT2M1(T/ T),NAT2M2(A/ A),XRCC1194( T/ T)and MTHFRA1298C(A/ C)were susceptibility genotypes. Synergistic effect was found between genes except MTHFRA1298C (A/ C)with NAT2M1(T/ T)and NAT2M2(A/ A)(P ﹤0. 05). Multiple genes risk analysis showed that combination OR of multiple genes was highly correlated with gene frequency,and the risk of gastric cancer was increased with the increasing number of susceptibility genes. Conclusions:CYP2E1(C1/ C1),NAT2M1(T/ T),NAT2M2( A/ A),XRCC1194( T/ T) and MTHFRA1298C( A/ C) are susceptibility genotypes of gastric cancer. Carrying multiple susceptibility genes can significantly increase the risk of gastric cancer.
