OBJECTIVE: To seek for the farnesoid X receptor (FXR) inhibitors from the TCM database using chemoinformatics.METHODS: Three-dimensional structure active fraction of FXR-fexaramine compound (PDB code: 1OSH) was analyzed using Cerius2 4.10 software package developed by Accerlrys company.Ligandfit module was applied for molecular docking.RESULTS: 10 compounds which were predicted to have good interactions with FXR were screened using docking score of original inhibitor (fexaramine) and the receptor as threshold value.CONCLUSION: The study could be helpful for development of new lipid-lowering drug and further study of mechanism of FXR and pharmaceutical molecular.

Objective To explore the expression of matrix metalloproteinase-11 (MMP-11) gene in gastric cancer cells and its correlation with transcription factor Sp1.Methods T In the present study,the expression of the MMP-11 was studied and the importance of Sp1 in the expression of the MMP-11 was proved with reverse transcription polymerase chain reaction (RT-PCR),differential PCR,Western-Blot,and DNA sequencing analysis in 10 kinds of the gastric cancer cell lines.Results MMP-11 cDNA was amplified with RT-PCR and differential PCR.In 34 cycle,the MMP-11/β-actin of SNU16,BGC823,MKN45,SGC7901,MGC803,SNU5 and PAMC82 were 3.32 ±0.12,2.22 ±0.11,1.41 ±0.12,1.35 ± 0.05,1.19 ± 0.05,0.97 ± 0.05,and 0.79 ± 0.05 (F =371.54,P ＜ 0.001).DNA sequencing analysis and BLAST showed the MMP11/β-actin of SNU16 DNA was 2 ～2.5 times as much as that of others.This result indicated that MMP-11 gene was amplified on the SNU16 DNA.Differential PCR and DNA sequencing analysis were carried out to amplify the MMP-11 gene in the SNU16 cell.Toward a better understanding of transcription of the MMP-1 1 gene,bioinformics technology was used to demonstrate the existence of two GC-boxes of the 5'-flanking region of MMP-11 DNA.Sp1 protein and MMP-11 protein were over-expressed in tumor cells and tended to have correlation with them (r =0.951,P ＜ 0.05).Conclusions The present results described the expressions of MMP-11 and Sp1 in gastric cancer cells.Moreover and clarified the correlation between MMP-11 and Sp1.It will establish the basic theory in order to confirm further whether Sp1 is a significant promoter of the 5'-flanking region of MMP-11 DNA.

Objective:To investigate the efficacy of pidotimod in the treatment of elderly patients with bronchial pneumonia and its effect on immune function. Methods: Totally 85 cases of elderly patients with bronchial pneumonia were randomly divided into two groups:the pidotimod treatment group (43 cases) and the conventional therapy group (42 cases). The conventional therapy group was given moxifloxacin 400 mg·d-1 , ivd, the pidotimod treatment group was additionally with pidotimod oral solutions 800 mg,po,bid, and the treatment course was 10 days. The changes of immune function in the two groups were detected, and the symptoms including fever, cough, asthma, rale and lung spots in X-ray radiation and the symptom duration in the two groups were also compared. Results:After the treatment, the levels of CD4, CD4 / CD8 and NK cells in the two groups were significantly higher than those before the treat-ment, the level of CD8 cells and IgA, IgG, IgM expression levels were significantly lower than those before the treatment (P<0. 05), and the changes in the pidotimod treatment group were much notable than those in the conventional therapy group(P<0. 05). The in-cidence of fever, cough, asthma, rale and lung spots in X-ray radiation in the pidotimod treatment group was lower than that in the con-ventional therapy group(P<0. 01), and the symptom duration was shorter than that in the conventional therapy group(P<0. 01). Conclusion:The mechanisms of pidotimod in the effective treatment of bronchial pneumonia in elderly patients are relative to the im-provement of T cell subsets, NK cells and humoral immunity levels and the enhancement of immunity.

Objective To investigate the effect of operation timing on patients with rheumatic mitral stenosis underwent valve replacement during perioperative period.Methods Retrospective analysis were performed on 122 cases diagnosed as rheumatic mitral stenosis (mitral stenosis,MS) underwent valve replacement,during January 2012 through December 2013 in the Department of Cardiovascular Surgery,General Hospital of Shenyang military region.According to the mitral valve area (mitral valve area,MVA) measured by echocardiography before operation,patients were respectively divided into three groups,mild stenosis (＞ 1.5 cm2),moderate stenosis (1.0 ～ 1.5 cm2),and severe stenosis (＜ 1.0 cm2).Through the single factor and multi-factor logistic regression analysis on all clinical data collected from patients,the effects of operation timing on patients were confirmed in perioperative period in each group.Results The results were satisfied in the group.Heart function was significantly raised compared to that before surgery,and the life quality of the patients was improved.Two cases died during hospitalization,New York Heart Association Functional Class (NYHA) reached between Ⅰ ～ Ⅱ,Ⅱ,perhaps Ⅰ for survival.Single factor analysis showed that the pulmonary arterial systolic pressure (pulmonary arterial systolic pressure,PASP),left ventricular end diastolic volume,mitral valve area,cardiothoracic ratio,intensive care unit (ICU) staying,ventilator-assistant during hospitalization were influence factors on surgical opportunity of patients with rheumatic mitral stenosis and perioperative outcomes.Multivariate logistic regression analysis showed that pulmonary artery pressure and mitral valve orifice area were the independent influence factors on surgical timing by collecting perioperative outcomes in the groups.Conclusions Mitral valve replacement (mitral valve replacement,MVR) is an effective method in the treatment of rheumatic mitral stenosis.Timely operation can improve the prognosis of patients in the perioperative period,and help to improve the quality of life.Early treatment should be an important factor affecting the perioperative prognosis.Timely operation,early treatment may improve the patient's survival during perioperative period.

Objective To investigate the time-course of mRNA and protein expression of inducible nitric oxide synthase (iNOS) in pulmonary artery and thoracic aorta and the difference in mRNA and protein expressions of iNOS between pulmonary artery and thoracic aorta induced by endotoxemia in rats. Methods Forty-eight Wistar rats of both sexes weighing 350-450g were randomly divided into 2 groups: control group in which animals received normal saline intraperitoneally (ip) and were killed 3h later by dislocation of cervical vertebrae (n=12); test groups in which animals were injected intraperitoneally lipopolysaccharide (LP) 4mg-kg-1 in 2ml of normal saline and were killed after 3h ( n = 12), 8h ( n = 12) , and 48h( n = 12) . Chest was opened immediately and pulmonary artery (PA) and descending thoracic aorta ( TA) were removed. mRNA and protein expression of iNOS in PA and TA were measured by RT-PCR and Western blot.Results The mRNA and protein expressions of iNOS in PA peaked at 8h and that in TA at 3h after ip LPS injection. The levels of mRNA and protein expressions of iNOS in both PA and TA returned to control level at 48h after ip LPS injection. The levels of mRNA and protein expressions of iNOS in PA at 3h and 8h after ip LPS injection were significantly higher than those in TA.Conclusion The mRNA and protein expressions of iNOS in PA and TA after LPS injection are time-dependent. The mRNA and protein expressions of iNOS in TA after LPS injection are earlier than those in PA but the level of mRNA and protein expressions of iNOS in PA after LPS injection is higher than that in TA.

Objective To investigate clinical significance of the changes in the peripheral blood T lymphocytes subsets in patients with diseases related to HBV infection .Methods 257 cases of inpatients and outpatients were selected from Jan .to Dec .2013 ,and were divided into hepatitis B carriers (ASC)group ,chronic hepatitis B(CHB)group ,hepatocirrhosis(LC)group and primary liver cancer(PHC)group according to types of diseases related to HBV infection .Other 50 healthy individuals conducted physical exami‐nation were enro1led in the control group .The absolute CD3+ ,CD4+ and CD8+ T‐cell count ,and CD3+ ,CD4+ and CD8+ percent‐age and CD4+ /CD8+ value were detected in all subjects by using flow cytometer .These data were compared and analyzed .Results Compared with the control group ,there were no significant differences of the absolute CD3+ ,CD4+ and CD8+ T‐cell count ,and CD3+ ,CD4+ and CD8+ percentage and CD4+ /CD8+ value in ASC group ,CHB group and LC group(P>0 .05) .Compared with the control group ,the absolute CD4+ T‐cell count ,CD4+ percentage and the CD4+ /CD8+ value were decreased in the PHC group , while the CD8+ percentage were increased in the PHC group ,there were statistical significant differences between the two groups (P<0 .05) .Conclusion The peripheral blood T lymphocyte subsets could be monitor indexes of cell immune function in diseases related to HBV infection .

Objective To evaluate the effects of a comprehensive intervention including regular aerobic exercise and health education on knowledge,attitude,belief,practice (KAP) scores,body mass index (BMI),waist circumferences,blood pressure,serum glucose and lipid levels in middle-aged and elderly people with overweight and obesity.Methods Three resident communities were chosen randomly from Wangdingdi community in Nankai district in Tianjin.Totally 125 overweight people were involved in this study,62 and 63 people were randomly assigned to intervention group and control group respectively.The members of intervention group were asked to participate in aerobic exercise and health education program 3 times per week within 6 months.And control group members received no intervention.Then to evaluate the effects of intervention of regular aerobic exercise and health education by physical examination and questionnaire.Comparison between intervention group and control group was performed using a repeatedmeasures analysis of variance.Results The comprehensive intervention resulted in significant improvement of the KAP scores [13.82 (95％CI:12.42-15.23)] and high-density lipoprotein cholesterol [0.22(95％ CI:0.17-0.28) mmol/L] and reductions in intervention group in body mass index [-0.47 (95％ CI:-0.58-0.36) kg/m2],waist circumference [-4.00 (95％ CI:-5.01--2.99) cm],systolic blood pressure [-11.69 (95％ CI:-13.84--9.55) mmHg] (1 mmHg=0.133 kPa),fasting plasma glucose [-1.21 (95％ CI:-1.55--0.88) mmol/L],triglyceride [-0.06 (95％ CI:-0.25-0.13) mmol/L],low-density lipoprotein cholesterol [-0.48 (95％ CI:-0.62--0.34) mmol/L].Conclusion Regular aerobic exercise and health education program in community can successfully reduce body mass index,waist circumference and some other risk factors related to overweight and obesity.It is effective to improve awareness,physical fitness,change lifestyle for middle-aged and elderly people in the community.

Objective To study the effects of controlled hypotension with sevoflurane on hymo-dynamics and cerebral metabolic rate of oxygen in neurosurgery.Methods Thirty-seven ASA Ⅰ or Ⅱpatients undergoing elective intracranial surgery were enrolled.The patients were treated with general anesthesia and assigned to two experimental groups,sevoflurane group (group S,n=20)and sodium nitroprusside group (group N,n=1 7).HR,BP,SpO2 and ECG were continuously monitored during operation.Blood samples were taken from radial artery and internal jugular bulb to determine cerebral arteriovenous oxygen differences (Da-jvO2 ) and cerebral metabolic rate of oxygen (CMRO2 ). Results The HR in group S were decreased during hypotension,5 min,10 min,20 min,and 30 min compared with group N (P <0.05).Da-jvO2 and CMRO2 both in two groups were decreased during hypotension(P < 0.05 ),but there was no difference between the two groups.RPP was lower in group S than in group N during hypotension.Conclusion Controlled hypotension with sevoflurane does not increase HR and improves cerebral blood flow-metabolism coupling.

Objective To evaluate the causes,treatment,and prevention of left ventricular rupture after mitral valvereplacement.Methods From May 1981 to November 2010,11 cases occurred in 4 male and 7 female patients aged 28 - 71 years old.There were 2 cases of New York Heart Association (NYHA) functional class Ⅱ and 9 cases of class Ⅲ.Echocardiography showed that the left ventricular end diastolic internal diameter (LVEDI) was 32 -62 mm.Results Rupture probably occurs after endocardial damage to a thin myocardium that has lost the internal buttress of the subvalvar apparatus.With the rise in intraventricular pressure at the end of bypass blood dissects into the myocardium,resulting in a large haematoma and eventual rupture.According to the onset of ruptures,there were 4 cases of delayed rupture which occured at several hours till days post operation after the patients had been back in the ICU,and 7 cases of early ruptur,which occured at the early stage of surgery,while the patient was still in the operation room.There were no cases of ruptures.The types of the ruptures were identified by operation or necropsy as 5 cases of type Ⅰ,4 cases of type Ⅱ,and 2 cases of type Ⅲ.Four patients were saved after emergency treatment,and 7 died.Conclusion It is difficult to repair left ventricle rupture,but effective prevention for onset can decrease its occurrence.The risk factors for left ventricular ruptures are female,advanced age,pathological changes characterized by mitral stenosis,small left ventricle (LVEDD ＜ 35 mm),and low weight( ＜50 kg ).The following actions raise the risk of let ventricular rupture.Isolated replacement of the mitral valve than after double valve replacement or mitral valve replacement and coronary artery bypass graft; aggressive removal of calcification at the mitral valve; undue selection and replacement of mitral valve prostheses; large size of prosthetic valve with a high bioprosthetic angle in particular; deep sutures at the valve ring; aggressive traction or removal of papillary muscle,hematoma,and heart move.Also we should continue correcting unstable hemodynamic especially with very high blood pressure.Once left ventricular rupture occurs,extracorporeal circulation should be established as soon as possible,and direet suture or intracardiac and extracardiac repair suited to actual conditions are reliable ways to save the patient's life.

Objective To explore the expression of matrix metalloproteinase-11 (MMP-11) gene in gastric cancer and its correlation with transcription factor Sp1.Methods The expressions of the MMP-11 and Sp1 in level of RNA and protein in 11 specimens from the gastric cancer patients s were detected by RT-PCR,differential PCR and Western-blot,respectively.Results Electrophoresis illustration showed MMP-11 mRNA expression had significant differences in eight pairs of gastric cancer tissues and normal tissues,including 7 pairs of over expressed in gastric cancer tissues and low or no expression in normal tissues,and 1 pair of low expression in cancer tissues but high in normal tissues.The relative molecular weight of Sp1 protein was mainly 95×103 in cancer tissue and 106×103 in normal tissues.Activated MMP-11 protein mainly was over expressed in gastric cancer tissues,and low or no expression in normal tissues,but degradable MMP-1 1 protein was not observed.The enzyme prototype of and activated type of MMP-11 protein were also associated with Sp1 protein.Conclusions The MMP-11 mRNA expression in gastric tissue is higher than that in the adjacent tissues.Sp1 protein and MMP-11 protein are expressed in gastric cancer,but are low or no expression in normal tissue.There is relationship between Spl and MMP-11.
