1.EVALUATION OF THE DIAGNOSIS OF INSULINOMA

The article presents an analysis of clinical signs, pathological findings and results of laboratory examinations of 10 cases of insulinoma. Diagnostic procedures such as blood sugar and serum insulin determinations, elective arteriography and CT scanning are evaluated. Besides, the relationship between the clinical signs and pathological findings were studied. The results of the study showed that the ratio of blood sugar to insulin and elective ateriography were valuable significant in diagnosing insulinoma, and that the diagnostic efficiency of CT left much to be desired. They also showed that duration of the disease had remarkable effects on patients with insulinoma.
2.Treatment of locally advanced breast cancer with breast-conserving therapy after neoadjuvant chemotherapy: Report of 31 cases

12 months postoperatively,and excellent results were obtained in 16.0% of the patients((4/25)),good in 40.0%(10/25),and poor in 44.0%(11/25). Conclusions For patients with locally advanced breast cancer previously treated with downstaging neoadjuvant chemotherapy,breast-conserving therapy offers satisfactory results.Strict adherence to technique and use of postoperative irradiation and systemic therapy are crucial to breast-conserving therapy.
3.Conservative surgery for the treatment of breast cancer:Analysis of 148 cases

Objective To investigate the therapeutic procedures,indications and efficacy of breast-conserving therapy (BCT). Methods BCT was carried out in 148 patients with breast carcinoma in this hospital from January 1996 to October 2003.There were 5 patients in stage 0,85 patients in stage Ⅰ,55 patients in stage Ⅱ,and 3 patients in stage Ⅲ.The surgical treatment was quadrantectomy or local wide excision of tumor,combined with axillary lymph node dissection.Routine adjuvant irradiation,chemotherapy and hormone therapy were given postoperatively. Results Pathological examinations of breast specimens found no positive margins.Follow-up observations for a median of 54 months (range,2 ~ 84 months) revealed a local recurrence rate of 2 7% ( 4/148 ;3 patients in breast and 1 patient in axillary fossa).Distant metastasis occurred in 5 patients (3 4%) and 3 of them died.Sites of metastasis included lung,liver,meninges and bone.The duration from surgery to distant metastasis was 6 ~ 43 months.Evaluation of cosmetic results in 108 patients who had received BCT for at least 12 months showed that good outcomes were achieved in 32 4% of the patients (35/108),fair outcomes,in 49 1% of the patients (53/108),and poor,in 18 5% (20/108). Conclusions For patients with early stage breast carcinoma,and part of those with local advanced breast cancer who have received neoadjuvant chemotherapy and gotten downstaging effects,the effects of BCT is satisfactory.Standard excision,postoperative irradiation and systemic combination treatment are crucial to BCT.Most patients receiving BCT have good cosmetic results.
4.Clinical analysis of sarcoidosis In elderly patients

Objective To improve the recognition of sarcoidosis in elderly patients.Methods Clinical data of 13 elderly patients with sarcoidosis were retrospectively analyzed.Results There were three cases without symptom and five cases with intra-pulmonary and extra-pulmonary manifestations respectively.Eight cases had atypical radiological appearances and five cases showed inflammatory changes through bronchofibercopy.There were three cases with increased percentage of lymphocytes and high ratio of CD4/CD8 in bronchoalveolar lavage fluid (BALF) respectively.Seven cases were diagnosed through biopsy.And the symptoms of all thirteen patients diminished after treatmeit.Conclusions The clinical symptom and radiologieal manifestation of sarcoidosis in elderly patients are varied and usually atypical.The diagnosis should be based on comprehensive analysis including clinical manifestation and examination.The elderly patients with indication of treatment can be given corticosteroid therapy with close clinical monitoring.

6.Clinical observation of oxaliplatin combination with 5-fluorouracil/folinic acid therapy in advanced colorectal cancer

Purpose: To evaluate the effect and safety of combination chemotherapy with oxaliplatin( L-OHP) plus 5-fluorouracil(5-FU)/folinic acid( CF) in advanced colorectal cancer( ACRC). Methods: 31 patients with ACRC entered the study. L-OHP 130 mg/m2 , iv infusion for 4 hours on day 1; CF 200 mg/m2 , iv infusion for 2 hours followed by 5-FU 500mg/ m , iv infusion for 4 hours from day 1 to day 5, repeated every 3 weeks. Results: There were 10 partial responses, 7 stable disease, 9 progressive disease, the response rate being 38. 5% . The main adverse effect was neuro-sensory toxicity which was seen in 90. 3% of the patients. Vomitting and diarrhea was observed in 58. 1% and 45. 2% of the patients respectively. Bone marrow suppression was mild. Conclusions: L-OHP combination with 5-FU/CF yields an encouraging response with acceptable toxicity in our study. Furthermore clinical reseach is worthwhile.

8.Quality Standard of Fengshi Bitong Capsule

Objective To establish the quality standard of Fengshi Bitong Capsule.Methods Notopterygh Rhizoma,Angelicae Pubescentis,and Saposhnikoviae Radix were identified by thin layer chromatography(TLC).Aconitine limit test in this drug also carried out by TLC.The contents of Isoimperatorin in the Notopterygh Rhizoma were determined by high performance liquid chromatography(HPLC).Results The spots in the TLC were fairly clear and no interference was shown in the blank samples.The aconitine limited in conformity with the 《Chinese Pharmacopoeia》 2010.Isoimperatorin showed a good linear relationship within a range of 3.668 96-183.448 00 μg· mL-1(r=1).The average of recovery was 96.42%,RSD=2.64%,respectively.Conclusion The established methods are highly specific,stable and reproducible,which can be used as quality control of fengshi bitong capsule.
9.Evaluation of early alterations of common carotid artery structure and function in patients with hypertriglyceridemia using ultrasound radio-frequency analyzing technique

Objective To evaluate the clinical value of early alterations of carotid arterial intima-media thickness (IMT) and elastic function in patients with hypertriglyceridemia (HTG) using ultrasound radio-frequency technique.Methods 48 patients (triglyceride value ≥ 1.8 mmol/L) were chosen as HTG group.Quantitative intimamedia thickness (QIMT) and quantitative arterial stiffness (QAS) techniques were used to observe the alterations of carotid arterial IMT and elastic index,including diameter of carotid artery (D),distensibility coefficient (DC),compliance coefficient (CC),elasticity coefficient (α),elasticity coefficient (β) and pulse wave velocity (PWV).50 healthy subjects were recruited in control group.Differences of the above-mentioned parameters were analyzed,as well as the elastic index connections between the two groups.Results IMT,α,β,and PWV index of bilateral carotid arteries in HTG group was higher than that of the control group,while the DC and CC index was lower,and the differences was statistically significant (P<0.05).Within the groups there were differences between the elastic indexes of bilateral carotid arteries.In control group and the HTG group:CCL>CCR,αL>αR,βL>βR,PWVL>PWVR.CC was negatively correlated to α,β and PWV (P<0.05).Meanwhile,there was positive correlation among α,β and PWV (P<0.05).The differences of D between two groups had no statistical significance (P>0.05).Conclusions The abnormity of arterial occurs earlier than the morphological change,while the ultrasound radio-frequency technique can provide information of earlier alteration of common carotid arteries structure and elastic function in patients with HTG,which is a significant method of testing early alteration of carotid elastic in patients with HTG.
10.Expression of CXC chemokine receptor 4 in Barrett esophagus, esophageal adenocarcinoma and esophageal squamous cell carcinoma and its clinical significance

Objective To investigate the expressions of CXCR4 in Barrett esophagus (BE), esophageal adenocarcinoma (EADC) and esophageal squamous cell carcinoma (ESCC), and its relationship with pathology, clinical staging and lymph node metastasis. Methods The expressions of CXCR4 in 56 cases of normal esophageal mucosa, 80 BE (including 22 BE with multifocal dysplasia), 25 EADC and 48 ESCC were examined with immunohistochemical method. Results CXCR4 was expressed in most samples of BE (80. 8% ), EADC (68. 0% ) and ESCC (78.4%) without significant difference ( P > 0. 05 ), which was significantly higher than that in normal esophageal mucosa (39. 3%, P <0. 01 ). The level of CXCR4 expression in BE, EADC or ESCC were not related with gender, age, or location of the foci ( P > 0. 05). There was no significant difference in CXCR4 expression between BE without dysplasia or BE with multifocal dysplasia ( P > 0. 05 ). CXCR4 expression level in well-differentiated EADC was significantly higher than that of mild or poorly differentiated (P < 0. 05 ). CXCR4 expression level was higher in EADC with lymph node metastasis than those without ( P < 0. 05 ). CXCR4 level in ESCC with TNM staging grades Ⅲ -Ⅳ was higher than that of grades Ⅰ - Ⅱ, and this variable was also higher in cases with lymph node metastasis than those without (P < 0. 05), so was the case of well and poorly differentiated ESCC (P < 0. 01 ). Conclusion Increased expression level of CXCR4 may be a common feature of EADC and ESCC, which is irrelevant to pathological types. CXCR4 level rises at the stage of BE, which is associated with the degree of tumor differentiation, lymph node metastasis and TNM staging. CXCR4 expression is of guiding significance in the diagnosis of BE, EADC and ESCC, and is the potential drug target.
