1.Quality control circle activities to improve patients’compliance to head-of-bed elevation

Objective To evaluate the effect of implementing quality control circle (QCC)activities on nurses’ knowledge about ventilator-associated infection(VAP),as well as head-of-bed (HOB)elevation and incidence of VAP in patients.Methods 100 patients who used ventilators in an intensive care unit(ICU)of a hospital were ran-domly selected,50 patients were selected before the implementation of QCC(January-June,2013),and 50 were se-lected after the implementation of QCC (July-December,2103).Condition of nurses and patients who used ventila-tors before and after implementation of QCC were investigated.Results After implementing QCC,the scores of nurses’recognition on knowledge about causes and risks of VAP,significance and standard of HOB elevation,and HOB elevation≥ 30°were all higher than before implementing QCC;the total score was higher than before imple-menting QCC ([18.32±3.14]vs [7.98 ±1 .12],P <0.05 );patients’compliance to HOB elevation was higher than before implementing QCC([96.00%,n=48]vs [44.00%,n=22],P <0.05);incidence of VAP was lower than before implementing QCC ([4.00%,n =2]vs [36.00%,n =18],P <0.05).Conclusion Implementation of QCC activities can effectively improve ICU nurses’recognition on knowledge about VAP and enhance patients’ compliance to HOB elevation,as well as reduce the incidence of VAP,it is helpful for improving healthcare-associat-ed infection management quality.
2.Application of malnutrition-inflammation score in patients with hemodialysis

Objective: To evaluate the value malnutrition-inflammation score(MIS) for the nutritional status in patients with maintenance hemodialysis.Methods: Eighty patients with maintenance hemodiaqlysis were selected in this study.MIS was used to access the nutritional status in these patients.At the same time,the laboratory indexes including serum albumin(ALB),transferrin(TRF),hemoglobin(Hb),serum creatinine(Scr),blood urea nitrogen(BUN),C-reactive protein(CRP),serum calcium(Ca) and phosphorus(P) were measured,and Kt/v,urea reduction ratio(URR) and protein catabolic rate(PCR) were calculated.The body composition was also measured by bioelectrical impedance analysis after postdialysis,including body weight(WT),body mass index(BMI),fat mass(FAT MASS),fat-free mass(FFM) and total body water(TBW).Results: The abnormal rates of these nutritional marker were: protein malnutrition(ALB1.8 mmol/L) 70.0%,respectively.All patients were mild or moderate malnutrition by MIS.MIS showed a significant correlation with WT,BMI,ALB,TRF,Hb and CRP(P﹤0.001,P

4.Effects of Processed Temperature And Time on Color and Contents of Six Types of Boswellic Acids in Frankinense

This paper focused on factors which affected on different color of northern and southern region vinegar-processed frankincense.Meanwhile,contents of six main boswellic acids were also determined to elaborate the influence of heat in chemical components.Vinegar-processed frankincense from northern and southern region was collected.And different temperature and time were used in the processing of frankincense to receive the vinegar-processed frankincense samples.The color difference meter was utilized combining with the PCA statistic analysis method.The Zorbax ExtendC18 chromatographic column (4.6 mm × 50 mm,1.8 μm) was used with acetonitrile-0.1% phosphoric acid as the mobile phase and gradient elution.The velocity of flow was 1 mL· min-1.The detection wavelength was 210 nm and 250 nm.The column temperature was 30℃.The results showed that the color of northern region processed frankincense was yellow or pale brown.And the southern region processed frankincense was pale brown or dark brown.It showed the difference on processed degree.The L* value of the northern processed frankincense was 75.327 to 80.746 and the L* value of southern processed was 44.321 to 49.527.The a* value of the northern processed frankincense was 5.378 to 6.502 and the a* value of southern processed was 9.423 to 9.978.There was no significant difference on b*.There were certain differences on L* and a* among vinegar-processed frankincense with the same surface color.The color parameter results of self-made vinegar-processed frankincense indicated that along with changes of processing temperature and time,the color,L* and a* change.Even frankincense processed for 30 min with mild fire,it will not achieve the color parameter value of the southern region vinegar-processed frankincense.However,after 11 min processing with medium fire,the color can be achieved.The content determination results showed that four contents,including α-boswellic acid,β-boswellic acid,3-acetyl-α-boswellic acid and 3-acetyl-β-boswellic acid were increased.Contents of 11-carbonyl-3-boswellic acid and 3-acetyl-11-carbonyl-β-boswellic were decreased after being processed.The range of increasing or decreasing by medium fire was higher than mild fire.At the same temperature,as the increasing of processing time,the content has an increasing or decreasing tendency.It was concluded that temperature was the main factor influencing the color of vinegar-processed frankincense from northern and southern regions.Different processing degrees also make influence on the contents of chemical compounds.The color parameter value can be used to evaluate the color of processed frankincense.
5.The urinary beta-trace protein in the diagnosis of type 2 diabetic renal injury

Objective To study the potential use of the urinary beta-trace protein ( βTP) for diagnosis of type 2 diabetic renal injury.Methods 174 patients with type 2 diabetic mellitus (T2DM) were classified into 2 groups according to the ratio of urinary albumin to creatinine (Alb/Cr):diabetes without renal injury group (group A) and diabetes with renal injury group (group B).70 healthy subjects served as normal control group ( group C).The level of urinary βTP and αl microglobulin (α1MG) was measured by latex particle enhanced immunoturbidimetry assay.The urinary Alb and Cr were determined by nephelometry and Jaffe method respectively.The level of uriuary βTP among all groups was compared and ROC curve analysis was performed.The relevant analysis on urinary βTP,urinary α1MG and other related indexes was made.Results The median level of urinary βTP/Cr in group B was 9.1mg/g Cr,significantly higher than 3.1mg/g Cr of group A and 2.0mg/g Cr of group C.The difference had statistical significance ( H =45.5,P < 0.01).The other indexes ( Alb/Cr,α1MG/Cr,SCr) were all higher in group B than in the other 2 groups ( H =110.9,38.3,11.4 respectively,P <0.01).The relevant analysis showed that urinary βTP/Cr was positively correlated with urinary α1MG/Cr (r =0.894,P < 0.01),SCr (r =0.367,P < 0.05 ),HbA(J) C ( r =0.242,P < 0.05 ),systolic pressure ( r =0.162,P <0.05 ),and the course of the disease ( r =0.251,P < 0.05 ).No correlation was found between urinary βTP/Cr and diastolic pressure,fasting blood glucose(FBG) or BMI.ROC curve analysis showed the area under the curve (AUC) was 0.86 (95%CI,0.78-0.93)for urinary βTP/Cr and 0.76 (95% CI,0.67-0.85) for urinary α1MG/Cr.The best cut-off value of urinary βTP/Cr and α1MG/Cr was 4.1mg/g Cr vs 10.9mg/g Cr,the sensitivity was 68.5% vs 59.7%,and the specificity was 89.8% vs 80.3%.The difference had statistical significance (P < 0.05).Conclusions Urinary βTP has better diagnostic value for type 2 diabetic patients with renal injury than urinary α1MG.It can sensitively reflect renal tubular injury and can be used as a novel available biomarker to evaluate the renal tubular injury in clinic.
6.Risk factors of hypertension in IgA nephropathy

Objective To explore the risk factors of hypertension in patients with IgA nephropathy in South China. Methods The clinical and renal pathological data of 280 primary IgA nephropathy patients diagnosed by biopsy were analyzed to extinguish the risk factors of hypertension. Results A total of 96 patients were suffered with hypertension (34.3%). A single-variable analysis showed that the age (≥40 years), body weight (≥60 kg), absence of macrohematuria, duration of disease (≥60 months), blood urea nitrogen≥8 mmol/L, serum creatinine (≥133 μmol/L), hyperuricaemia, degree of 24 h-proteinuria (≥1.5 g), segmental glomerular lesions (≥25% ), globe glomerular sclerosis (≥10%), tubular atrophy (≥25%), interstitial fibrosis (≥25%), interstitial inflammation (≥25% ) and arteriole hypertrophy (≥10% ) were all risk factors related to hypertension; multivariate logistic regression analysis showed that serum creatinine, age, arteriole hypertrophy, body weight and 24 h-proteinuria were the independent risk factors. Conclusion Many factors were related the hypertension in patients with IgA nephropathy, while serum creatinine, age, arteriole hypertrophy, body weight and 24 h-proteinuria were the independent risk factors of hypertension.
7.Investigation and analysis of iodine nutritional statusin healthy adults in Urumqi of Xinjiang Uyghur Autonomous Region

Objective To investigate iodine nutrition status in healthy adults in Xinjiang Urumqi city and their relationship to ethnicity, gender and age. Method A cross-sectional survey was performed in 2 100 residents of Xinjiang Urumqi 2 communities in May 2013, of which 1 835 healthy adults aged 18-84 years, mean age 46.3 ± 14.2 years were enrolled. Urine iodine with arsenic-cerium catalytic spectrophotometry and salt iodine and water iodine of the residents were measured. Result The water iodine content was 3.14 μg/L, salt iodine was 27.75 mg/kg, 1 835 urine samples were collected, the urinary iodine median(MUI) was133.4 μg/L, iodine deficiency accounted for 32.37% (595 cases), iodine sufficient 42.94% (788 cases), iodine super-sufficient 14.44%(265 cases), iodine excess 10.19%(187 cases). The urine iodine level in Han ethnic group was higher than those in Uygur adults, no significant difference was observed between men and women (P>0.05). Urinary iodine levels among different age groups had no statistical difference, but in the older groups the level was lower than that in young or middle age adults. The urinary iodine level decreased gradually with the age increasing. Conclusion There was a sufficient pattern of iodine nutrition levels in healthy adults in Xinjiang Urumqi; there was a correlation between the level of iodine nutrition and the ethnicity but not with gender;iodine nutrition level in older age group was lower than those in younger age groups. Monitoring the urinary iodine continually and decreasing iodine intake may be necessary for part of iodine excess population.
8.Noise and speckle reduction in ultrasound Doppler blood flow spectrograms by using MP-PCNN.

To reduce background noise and Dopplar speckle in the spectrogram of ultrasound Doppler blood flow signals, a novel method, called Matching Pursuit with threshold decaying pulse coupled neural network (MP-PCNN), has been proposed. The proposed method used an iterative algorithm, which decomposed the ultrasound Doppler signals into linear expansion of atoms in a time-frequency dictionary by using the Matching Pursuit (MP) for de-noising the ultrasound Doppler signal. Subsequently, a simplified unidirectional pulse coupled neural network was applied to calculate the firing matrix of the denoised spectrogram. The Doppler speckles were located and removed through analyzing and processing the PCNN firing matrix. Experiments were conducted on simulation signals which SNRs were 0dB, 5dB and 10dB. The result showed that the MP-PCNN performed effectively in reducing noise, eliminating Doppler speckles, and achieved better performance than exiting noise and speckle suppression algorithm for Doppler ultrasound blood flow spectrogram.

9.Assessment of left ventricular mvocardial scar with three-dimensional MRI

ObjectiveTo assess the value of free-breathing 3D phase sensitive inversion recovery (PSIR) turbo FLASH for demonstrating the left ventricular myocardial scar in patients with myocardial infarction.MethodsTwenty-three patients with myocardial infarction underwent 2D and 3D phase sensitive inversion recovery (PSIR) Turbo FLASH sequences 10-15 minutes after injection of dimeglumine gadopentetate.The image quality,the area,location and volume of hyperenhanced scar were independently assessed by two experienced radiologists.Student′s t test or rank sum test and Pearson linear correlation were used for the statistics.ResultsThere were no significant differences in the overall image quality (2.57 ±0.59 vs 2.39 ±0.66,Z=-0.93,P =0.35),the area (24.48 ±10.83 vs 29.00 ± 11.56,Z=-1.41,P =0.16) and location ( 16.78 ± 6.51 vs 18.87 ± 6.76,Z =- 1.17,P =0.24) of hyperenhanced scar between 2D and 3D PSIR sequences.The mean volume of hyperenhanced scar was significantly greater in 3D PSIR sequence than that in 2D PSI R sequence [(23.46 ± 10.61 ) vs (31.65 ± 13.86) cm3,t =-2.25,P =0.03].There was a good correlation in the volume of hyperenhanced scar between 2D PSIR and 3D PSIR sequences (r=0.940,P<0.01 ;y =2.851 + 1.228x,R2 =0.883).ConclusionFree-breathing 3D PSIR Turbo FLASH is a promising new imaging technique for accurate assessment of myocardial scar.
10.Application of the sandwich teaching method in the course of Surgical Nursing

Objective To investigate the application effect of the sandwich teaching method in the course of Surgical Nursing . Methods Two classes of the students majoring in nursing were randomly divided into control group and observation group, with 36 students in each group. The students in the control group received the traditional teaching method, and those in the observation group received the sandwich teaching method . The teaching effect was compared between the two groups by theoretical assessment and questionnaire survey. SPSS 24.0 was used for analysis of variance and the t-test. Results Compared with the control group, the observation group had significantly higher scores of course assessment, learning atmosphere, and learning initiative. The observation group also had significantly higher good rates of learning atmosphere and learning initiative than the control group(P<0.05). Conclusion The sandwich teaching method can significantly improve students' abilities of communication and problem solving, arouse their enthusiasm of learning, and increase the interestingness of classroom, and therefore, it has a practical value in teaching.
