1.Correlation analysis of bispectral index,spectral edge frequency of electroencephalogram with midazolam-induced sedation

Objective: To analyse correlation of bispectral index,spectral edge frequency of electroencephalogram with midazolam-induced sedation. Method: 30ASA grade Ⅰ-Ⅱ adult patients, undergoing elective surgery under regional anesthesia were randomly devided into three groups according to intravenous bolus doses of midazolam,i, e. group Ⅰ:0.05mg?kg~(-1),group Ⅱ:0.1mg?kg~(-1),group Ⅲ:0.2mg?kg~(-1). After an intravenous bolus dose of mida zolam was administered,both bispectral index (BIS), 95% spectral edge freguency (SEF) of electroencephalogram were monitored and their correlation with midazolam induced sedation was analysed. 2.Anterior debridement bone graft fusion and posterior fixation in treatment of intervertebral infection

Objective To explore the clinical outcome with anterior debridement,bone graft and posterior internal fixation with short nail of treating intervertebral infection.Methods Ninteen cases (11 male and 8 female,aged from 34 to 63,46 on average) of lumbar intervertebral space infection treated in our hospital from June 2008 to June 2013 were retrospectively analyzed.These infections occurred at L2-L3 in 2 cases,L3-L4 in 5 cases,L4-L5 in 8 cases and L5-S1 in 4 cases.All patients had history of disc surgery or puncture.The clinical outcome after operation was analyzed according to the low back paine after operation which analyzed by visual analog scale (VAS),and the bone fusion.Erythrocyte sedimentation rata and C-reactionprotein were aslo recorded before and after surgery.Results All cases were followed up,average 24 months.The lower back pain was improved obviously between preoperation and postoperation (P <0.05).Also erythrocyte sedimentation rata and C-reactionprotein returned to normal after the operation and all cases had complete bone union at the end of follow-up with no instrument failure noted.Conclusion Anterior debridement,bone graft and posterior internal fixation with short nail in of treating intervertebral infection not only can effectively reconstruct the stability of the spine,but also retain range of motion segment of spine.
3.Therapeutic effect of polymethyl methacrylate augmented pedicle screw instrumentation and fusion for old osteoporotic vertebral compression fractures complicated with spinal instability

Objective To explore the clinical efficacy of polymethyl methacrylate (PMMA) augmented pedicle screw instrumentation and fusion for old osteoporotic vertebral compression fractures complicated with spinal instability. Methods The clinic data of 26 old patients with osteoporotic vertebral compression fractures complicated with spinal instability treated from June 2011 to June 2015 were retrospectively analyzed. Among them, there were 5 male patients aged from 61 to 72 years and 21 female patients aged from 56 to 75 years. All patients had history of chronic low back pain. The clinical outcome after operation was analyzed according to visual analog scale (VAS) and Oswestry dysfunction index (ODI). The status of bone fusion were aslo recorded before and after surgery. Results There were no pulmonary embolism, infection, toxic reaction of PMMA, and no nerve root compression and damage occurred in patients. The operation time was (105.0 ± 20.5) min, and bleeding volume was (200 ± 55) ml. The operative incision was healed in Ⅰ stage. All patients were followed up for 12-27 months, and average was 14 months. The levels of VAS scores and ODI scores after operation and 1, 3, 6, and 12 months after operation were significantly increased compared with those before operation (P<0.05). Conclusions PMMA augmented pedicle screw instrumentation and fusion is an effective method for old osteoporotic vertebral compression fractures complicated with spinal instability.
4.Usage of left internal thoracic artery in the patients aged over 70 years during coronary artery bypass graft

Objective To review the outcome of coronary artery bypass grafting (CABG) using left internal thoracic artery (LITA) grafts in these patients aged more than 70 years old.Methods 1471 patients aged more than 70 years old[mean (73.3 ± 3.9) years] from July 2010 through August 2012,who underwent CABG in Anzhen hospital,form the cohort of this study.Among them,1395 cases underwent off-pump CABG,while 76 cases underwent on-pump CABG which includes onpump beating heart CABG in 12 cases.The average number of the grafts is 3.12 ±0.68.All of the patients were divided into two groups on base of the graft:Group A:only saphenous vein graft was used in 564 cases; Group B:The left internal thoracic artery to the left anterior descending artery was used in 907 cases,which also include total arterial grafting,facilitated by the use of the radial artery,right gastroepiploic artery was operated in 42 cases,beside these artery grafts,saphenous vein graft was used to anastomosis with other vessels.Results Operative mortality was 2.12% in group A and 2.09% in group B.There was no significance between the 2 groups.The volume of blood drainage was larger in group B than that in group A.At the same time,there was no significance in the incidence rate of second thoractomy,malignant arrhythmia,stroke,poor wound healing,and usage of IABP between the 2 groups.Conclusion It did not increase the mortality and morbidity of serious complications using the left internal thoracic artery to the left anterior descending artery in those elderly patients aged more than 70 years old during the CABG operation.With consideration of the obvious advantages in the long term patency,LITA was proposed to be used in CABG even in those age > 70 patients.
5.Ischemic mitral valve reconstruction and biological valve replacement in elderly patients: comparison of long-term survival and complications

Objective This study reviews the 198 consecutive mitral valve operations for elderly patients(> 65 y) with ischemic mitral insufficiency performed at Anzhen Hospital between January 2000 and june 2016.The results for mitral valve reconstruction are compared with those for biological mitral valve replacement.Methods From January 2000 to June 2016,198 elderly patients with ischemic mitral insufficiency underwent mitral valve reconstruction (n =150) or biological mitral valve replacement(n =48).All of them coronary artery bypass gafting was performed at the same time.Preoperative clinical characteristics,procedural characteristics,major and minor complications after surgery,preoperative and postoperative left ventricular ejection fraction (LVEF) by echocardiography,and outcome (survivor or death,mitral regurgitation,NYHA degree) were assessed.Results There was no significant difference between the two groups in the rate of mortality during hospitalization and early postoperative cardiac function.The proportion of severe MR in 1,3,and 5 years after MVP were 5.1% 、6.3%、7.7% respectively.There was no MVR due to the recurrent moderate-to-severe reflux.There were 2 SBE but no reflux appeared after MVR.There was no significant difference in cardiac function and mortality between the two groups within 5 years.Conclusion There is no significant difference between CABG + MVP and CABG + MVR(BV) in cardiac function and mortality in the treatment of elderly patients with IMR.,There was a possibility of MR recurrence after CABG + MVP.There was no MR after CABG + MVR(BV),but the risk of SBE is higher than MVP.We can select operation mode personalized according to the type of mitral valve disease and cardiac function.
7.Portal vein-superior mesenteric vein resection and reconstruction during pancreaticoduodenectomy using the perivenous occlusion management strategy

Objective:To explore the perivenous blocking management strategy for portal vein-superior mesenteric vein (PSMV) resection and reconstruction and its effect on postoperative complications in patients undergoing pancreaticoduodenectomy (PD).Methods:The data of 137 patients with pancreatic cancer treated with PD in Beijing Chaoyang Hospital Affiliated to Capital Medical University, Chaoyang Central Hospital, the Second Hospital of Chaoyang, Rizhao Central Hospital, the Second People's Hospital of Binzhou from January 2010 to December 2020 were collected. There were 83 males and 54 females with an average age of 61.8 years. There were 42 patients in the reconstruction group and 95 patients in the control group. The main intraoperative indexes and postoperative complications were compared between the two groups with the aim to review our experience in PSMV resection and reconstruction by using the perivenous blocking management strategy.Results:PD was successfully completed in 137 patients in the reconstruction group, the PSMV blocking time was 15-120 min, with a median of 30 min. The operation time 380 (330, 465) min, intraoperative blood loss 725 (500, 1000) ml, and postoperative hospital stay 21.0 (16.0, 28.0) d in the reconstruction group were significantly higher than those of control group [305 (280, 340) min, 400 (300, 500) ml and 18.0 (14.0, 24.5) d] (all P<0.05). The reoperation rate and perioperative mortality were 4.8% (2/42) and 2.4% (1/42) in the reconstruction group, while 2.1% (2/95) and 1.0% (1/95) in the control group, respectively. There was no significant difference between the two groups (both P>0.05). The incidence of pancreatic fistula, peritoneal effusion and infection, pulmonary infection of the reconstruction group was significantly higher than those of the control group ( P<0.05). There was no significant difference in the incidence of postoperative bleeding, delayed gastric emptying, biliary fistula, incision infection, reoperation between the two groups ( P>0.05). Conclusions:PSMV resection and reconstruction significantly increased the incidences of complication after PD, including pancreatic fistula, peritoneal effusion/infection and pulmonary infection. The perivenous blocking management strategy significantly promoted smooth postoperative recovery and effectively reduced morbidity rates of postoperative bleeding and mortality after PSMV resection and reconstruction in PD.
8.Curcumin protects against the intestinal ischemia-reperfusion injury: involvement of the tight junction protein ZO-1 and TNF-alpha related mechanism.

Present study aimed to investigate the eff ect of curcumin-pretreatment on intestinal I/R injury and on intestinal mucosa barrier. Thirty Wistar rats were randomly divided into: sham, I/R, and curcumin groups (n=10). Animals in curcumin group were pretreated with curcumin by gastric gavage (200 mg/kg) for 2 days before I/R. Small intestine tissues were prepared for Haematoxylin & Eosin (H&E) staining. Serum diamine oxidase (DAO) and tumor necrosis factor (TNF)-alpha levels were measured. Expression of intestinal TNF-alpha and tight junction protein (ZO-1) proteins was detected by Western blot and/or immunohistochemistry. Serum DAO level and serum and intestinal TNF-alpha leves were signifi cantly increased after I/R, and the values were markedly reduced by curcumin pretreatment although still higher than that of sham group (p<0.05 or p<0.001). H&E staining showed the significant injury to intestinal mucosa following I/R, and curcumin pretreatment signifi cantly improved the histological structure of intestinal mucosa. I/R insult also induced significantly down-regulated expression of ZO-1, and the eff ect was dramatically attenuated by curcumin-pretreatment. Curcumin may protect the intestine from I/R injury through restoration of the epithelial structure, promotion of the recovery of intestinal permeability, as well as enhancement of ZO-1 protein expression, and this eff ect may be partly attributed to the TNF-alpha related pathway.

9.The auxiliary application strategy of radiofrequency ablation in laparoscopic anatomical hemihepatectomy

Objective:To analyze the auxiliary application strategy and efficacy of radiofrequency ablation (RFA) in laparoscopic anatomical hemihepatectomy (LAH).Method:The clinical data of consecutive patients who underwent RFA-assisted LAH from 5 hospitals including Beijing Chaoyang Hospital, Capital Medical University between January 2016 and January 2020 were retrospectively reviewed.Results:Among the 32 patients, there were 21 males and 11 females. The age range is 32 to 77 years, with a median age of 52 years. There were 18 cases of hepatocellular carcinoma, including 16 cases of single lesion and 2 cases of two lesions, with the maximum tumor diameter of (5.6±1.2) cm. There were 12 cases of metastatic liver cancer, including 8 cases of single lesion, 3 cases of two lesions, 1 case of three lesions, and the maximum tumor diameter (4.7±1.6) cm. Primary hepatolithiasis: 2 cases. Right hemihepatectomy was performed in 23 cases and left hemihepatectomy in 9 cases. No conversion to laparotomy. The operation time of right hemihepatectomy was (310.0±22.0) min, and left hemihepatectomy was (285.0±25.0) min. Intraoperative hemorrhage (330.0±28.0) ml. No patients received intraoperative infusion of human red blood cell suspension. Postoperative reactive pleural effusion occurred in 6 cases, biliary fistula in 3 cases, wound infection in 1 case, and cross section effusion in 1 case, all of which recovered after conservative treatment. No postoperative abdominal bleeding and liver insufficiency. Postoperative hospital stay (9.5±3.2) d. The follow-up time was 1-42 months, and the median follow-up time was 20 months. During the follow-up period, 17 (56.7%) of the 30 patients with malignant tumor experienced tumor recurrence, no sectional tumor recurrence, and no death.Conclusions:RFA has a variety of auxiliary applications in LAH, including prevention and treatment of liver cancer rupture during mobilization of liver, treatment of small bleeding blood vessels during liver transection, and help in securing safe and adequate resection margins.
10.Efficacy of radiofrequency ablation in treatment of complicated pyogenic liver abscess

Objective To evaluate the efficacy of radiofrequency ablation (RFA) in treatment of complicated pyogenic liver abscess (PLA).Methods A retrospective analysis was conducted on 20 patients with complicated PLA treated from August 2010 to January 2018 in the West Campus of Beijing Chao-yang Hospital which is Affiliated to the Capital Medical University and Rizhao Central Hospital of Shandong Province.These patients were treated with RFA based treatment modalities using CT-guided or laparoscopy combined with Ultrasound-guided RFA.The successful rate,efficacious rate and safety of RFA were respectively analyzed.Routine follow-up was carried out after treatment.Results The successful rate of RFA was 100%.The median operation time was 45 minutes.All the 20 patients were cured by one session of RFA.The multilocular lesion in one patient was converted to a unilocular lesion after one session of RFA.The patient was then cured by percutaneous hepatic abscess drainage.No serious complications were observed.Conclusions RFA was safe and efficacious.It should be considered as an alternative treatment for patients with complicated PLA who were not responsive to,or not applicable to antibiotics treatment and radiological intervention.
