1.Experience of the use of prosthetic materials to repair incisional hernia in 37 cases

Objective To investigate therapeutic ways a nd effects of using prosthetic materials to repair incisional hernia of the abdo minal wall.Methods Marlex mesh was used in 32 cases and Com posix mesh in 5 cases. Positions of mesh implanted: Retromuscular placement in 1 5 cases, premuscular placement in 12 cases and inlaying mesh into abdominal wall defeat in 10 cases.Results 33 patients were followed for 3 months to 28 months postoperatively, 32 patients were cured without reccurrence. One reccurented at 6 months postoperatively.Conclusion App lication of prosthetic materials to repair incisional hernia of abdominal wall i s safe, and has a low reccurrent rate.
2.APPLICATION OF VARIOUS CONCENTRATIONS OF BRANCHED CHAIN AMINO ACID SOLUTIONS FOR SUB-TOTAL GASTRECTOMY

Forty patients who underwent sub-total gastrectomy were divided into four groups, one of them for comparison, the other three groups received 10%, 30% and 100% branched-chain amino acid (BCAA) respectively. The results showed that in BCAA-10 and BCAA-3H group, negative balance was partially corrected, but in BCAA-10 group nitrogen remained an obvious negative balance on the 3rd postoperative day. There were more than ten of plasma AA lower than preoperative day in these two groups. In BCAA-30 group, after surgery, negative nitrogen balance was significantly corrected, and changes of plasma AA had no statistic significance compared with that before operation.Creatinine of urine had no significant difference between each group.In BCAA-30 group, transferrin (Tf) was increased compared with that before operation. It is suggested that the protein synthesis is corrected but the catabolism is not restrained.BCAA may play a regulatory role in muscle protein metabolism, BCAA can serve as fuel substrate to promote synthesis and reduce breakdown of muscle protein to correct negative nitrogen balance.Application of large quantities of BCAA should be based on isotonic AA solution.
3.Clinical evaluation of toothbrushing effects of the manual toothbrush with the bristles made of 3 different materials

Objective: To compare the effects of ordinary nylon toothbrush,bamboo fiber toothbrush and silicone rubber toothbrush. Methods: 30 and 156 college students were respectively included by the enrolment criterion and were respectively divided into 3 groups. Tooth brushes with the same specification and with the 3 materials were respectively used in the 3 goups with the same methods. Plaque index(PI) (n = 10) was examined before and 1 month after use of the brushes; gingival index(GI) was examined(n = 52) before,1 and 3 month after use of the brushes. Results: 1 month after use of the brushes full mouth PI removal rate(n = 10) and GI reduction(n = 52) were not statistically different among the 3 group(between each 2 groups,P> 0. 05). 3 months after use of the brushes GI reduction of the silicone rubber toothbrush group was more than that of ordinary nylon toothbrush and bamboo charcoal toothbrush groups(P < 0. 05). Conclusion: Plaque removal efficiency of the 3 kinds of toothbrush is similar. The silicone rubber toothbrush is more effective for gigiva health than ordinary nylon toothbrush and bamboo charcoal toothbrush.
4.Comparison of tensionfree herniorrhaphy with conventional herniorrhaphy for inguinal hernia

ObjectiveTo evaluate the value of tensionfree herniorrhaphy for inguinal hernia. Objective To discuss the medication rules of formulas containing Longgu (Os Draconis,Bone Fossil of Big Mammals) by national TCM master-YAN Zhcnghua.Methods On the base of collecting the formulas containing Longgu used by YAN Zhenghua and a database was established based on Traditional Chinese Medicine Inheritance Support System (TCMISS).Apriori algorithm and other data mining methods were built in the data analysis module in TCMISS,and then the module was used for analyzing frequency of single medicinal,frequency of medicinal combinations,association rules and core medicinal combinations.Results The formulas containing Longgu were commonly used for treating insomnia,vertigo and stomachache.The medicinal with high frequency included Muli (Concha Ostreae,Oyster Shell),fried Suanzaoren (Semen Ziziphi Spinosae,Spine Date Seed),Shouwuteng (Caulis Polygoni Multiflori,fleeceflower stem),Danshen (Radix Salviae Mihiorrhizae,Salvia Root) and Chishao (Radix Paeoniae Rubra,Red Peony Root).The medicinal combinations with high frequency included Muli + Longgu,Muli+ fried Suanzaoren,and Longu + fried Suanzaoren.The association rules with support degree ≥ 35 % and confidence coefficient ≥ 0.98 included Longgu→Muli,Fuling (Poria,Indian Bread,Poria),Longgu → Muli,Muli,Fuling → Longgu,and Longgu,fried Suanzaoren → Muli.Conclusion The medicinal,used by YAN Zhenghua,mostly have the effects of tranquilizing mind and activating blood,and his formula constructing laws are clear and precise.
6.Establishing cases library of renal diseases and the feedback after the application of the cases library

Clinical teaching is the critical stage for medical students turning to qualified doctor.In order to overcome the objective problems of insufficient clinical case resources,using the electronic medical record system to collect cases,the real case library of kidney disease was initially constructed,which was presented in the form of network resources.This case database was applied to assist teaching in the probation of eight year medical students at the undergraduate stage,and the application of the case database was evaluated in the form of questionnaire.It is found that case database is helpful to students' clinical learning,and has the necessity of further improvement and good prospects for popularization.It provides a new idea for clinical teaching.
7.Clinical observation of supracondylar fracture of humerus requiring surgical nerve exploration

Objective:To investigate the clinical effect of minimally invasive treatment of supracondylar fracture of humerus with nerve injury and the need for surgical exploration of nerve.Methods:From August 2017 to September 2020, 34 cases of children with supracondylar fracture of humerus with nerve injury in the Department of orthopedics of Shanxi Children′s Hospital were selected for retrospective analysis.Closed reduction, Kirschner wire fixation and small incision surgery were used to explore the injured nerve.The follow-up period ranged from 6 months to 3 years, with a follow-up time of (2.15±0.49) years.The elbow function, radial nerve, median nerve and ulnar nerve function were evaluated according to the trial standard of upper limb peripheral nerve function evaluation of Hand Surgery Society of Chinese Medical Association.Results:The elbow function of children in this group: excellent 31 cases, good 2 cases, fair 1 cases, poor 0 cases, excellent 33 cases.Three patients failed to take early functional rehabilitation exercise.Two patients recovered after 2.5 months of treatment by professional orthopedic rehabilitation therapists and physical therapy.One of them did not cooperate with functional rehabilitation training.After 3.5 months of treatment by orthopedic rehabilitation therapists, elbow and finger functions were not affected.Two weeks after nerve injury: radial nerve function evaluation: excellent in 15 cases, good in 3 cases, fair in 4 cases, poor in 0 cases, excellent in 18 cases.Median nerve function evaluation: excellent in 5 cases, good in 2 cases, fair in 6 cases, poor in 0 cases, excellent in 7 cases.Ulnar nerve function evaluation: excellent 5 cases, good 0 cases, fair 1 cases, poor 0 cases, excellent 5 cases.At 3 months after injury, the excellent and good rate of radial nerve, median nerve and ulnar nerve was 100%.Conclusion:The supracondylar fracture of humerus with nerve injury is treated by minimally invasive surgery, and the nerve exploration is still a few.The recovery of nerve injury after surgery is smooth, and the clinical effect is good.

Objective This paper aims to construct a scientific and standardized index system for evaluating the applica-tion effect of Diagnosis-Intervention Packet(DIP)in medical institutions.It seeks to determine the weights of indicators at all lev-els to provide a basis for evaluating the effectiveness of DIP used in various regions as well as a reference for improving DIP poli-cy.Methods This paper used literature analysis to develop an indicator pool preliminarily.The Delphi method was used to con-duct a questionnaire survey with 22 experts to establish the index system.It also used the Analytic Hierarchy Process(AHP)to determine the weight of each index.Results A total of two rounds of expert consultation questionnaires were conducted.The questionnaire exhibited high expert positive coefficients by 91.00%and 100.00%and higher authority coefficients of both 0.91 for two rounds.The coordination coefficients of the two rounds of expert consultation met the standard,and the expert opinions reached a consistence.Additionally,the questionnaire established comprised three primary indicators(efficiency of medical in-surance fund,medical service management,patient benefit,and satisfaction),9 secondary indicators,and 38 tertiary indicators.The AHP was used to determine the weight of indicators at all levels.Among the primary indicators,patient benefit and satisfac-tion had the highest weight(0.446 9).Among the secondary indicators,patients'medical cost burden had the highest weight(0.287 6).Among the tertiary indicators,the inpatient satisfaction had the highest weight(0.1592).The Cronbach's α coeffi-cients of the two rounds were>0.7.Conclusion This paper utilized Delphi method and AHP to establish the evaluation system for assessing the implementation effectiveness of DIP.The approach is highly scientific and authoritative.It can be used to evalu-ate the application effect of DIP and provide an effective tool for medical insurance and hospital managers at all levels to assess DIP payment policies.
9.Polycyclic aromatic hydrocarbons monohydroxy metabolites level in urine of general population in eight provinces of China.

OBJECTIVETo assess the levels of polycyclic aromatic hydrocarbons monohydroxy metabolites in urine of general population in China among 8 provinces, provide the baseline of the metabolites in the general population.
METHODSFrom 2009 to 2010, 18 120 subjects of general population aged 6-60 years old were recruited from 24 areas among 8 provinces in east, west and central areas of China mainland by cluster random sampling. The information of the living environment and health condition were collected by questionnaire and spot urine samples were collected, 4 680 urine samples were analysed by high performance liquid chromatography with tandem mass spectrometry, and monohydroxy metabolites distribution in urine among groups of gender and ages were analysed.
RESULTSGeometric means (GM) of 2-naphthol, 1-naphthol, 3-phenanthrol and 1-hydroxypyrene concentration in urine (95%CI) were 1.85 (1.75-1.95), 1.55 (1.50-1.61), 0.57 (0.54-0.59) and 0.82 (0.78-0.85) µg/L, respectively;and median are 2.44, <0.50, 0.72 and 0.90 µg/L, respectively. The concentration between male and female were significantly different (P < 0.01), and the concentration among the groups of population were significantly different (P < 0.01), the GM of 2-naphthol among the groups of population aged 6-12, 13-16, 17-20, 21-30, 31-45 and 46-60 years old were 1.60, 1.56, 1.69, 2.23, 1.91 and 1.86 µg/L (χ(2) = 17.90, P < 0.01), the GM of 1-naphthol in the groups were 1.30, 1.16, 1.53, 1.68, 1.80 and 1.52 µg/L (χ(2) = 76.22, P < 0.01), the GM of 3-phenanthrol in the groups were 0.78, 0.76, 0.55, 0.42, 0.50 and 0.99 µg/L (χ(2) = 66.48, P < 0.01), the GM of 1-hydroxypyrene in the groups were 0.77,0.64, 1.00, 0.84, 0.84 and 0.57 µg/L (χ(2) = 51.48, P < 0.01), respectively.
CONCLUSIONThe distribution of monohydroxy metabolites levels in urine of general population were different, it provided a basic data for the further study of polycyclic aromatic hydrocarbons biomonitoring in the population.

10.Immunogenicity of mucosal COVID-19 vaccine candidates based on the highly attenuated vesicular stomatitis virus vector (VSVMT) in golden syrian hamster.

COVID-19 is caused by coronavirus SARS-CoV-2. Current systemic vaccines generally provide limited protection against viral replication and shedding within the airway. Recombinant VSV (rVSV) is an effective vector which inducing potent and comprehensive immunities. Currently, there are two clinical trials investigating COVID-19 vaccines based on VSV vectors. These vaccines were developed with spike protein of WA1 which administrated intramuscularly. Although intranasal route is ideal for activating mucosal immunity with VSV vector, safety is of concern. Thus, a highly attenuated rVSV with three amino acids mutations in matrix protein (VSVMT) was developed to construct safe mucosal vaccines against multiple SARS-CoV-2 variants of concern. It demonstrated that spike protein mutant lacking 21 amino acids in its cytoplasmic domain could rescue rVSV efficiently. VSVMT indicated improved safeness compared with wild-type VSV as the vector encoding SARS-CoV-2 spike protein. With a single-dosed intranasal inoculation of rVSVΔGMT-SΔ21, potent SARS-CoV-2 specific neutralization antibodies could be stimulated in animals, particularly in term of mucosal and cellular immunity. Strikingly, the chimeric VSV encoding SΔ21 of Delta-variant can induce more potent immune responses compared with those encoding SΔ21 of Omicron- or WA1-strain. VSVMT is a promising platform to develop a mucosal vaccine for countering COVID-19.
