Objective To analyze the double contrast(DC) imaging of anterior gastric wall cancer,in order to raise the knowledge of the imaging feature.Methods DC examination was confirmed by clinical data check and pathology before the operation in 35 cases of anterior gastric wall cancer.Results Occurence rates of hanging droplets,white foggy phonomen,overhanging white line and linear signs were 78%,94%,66% and 88%,respectively.Multiple mural lines would appear if the stomach greater or lesser curvature was involved with cancer.When one or two signs of them was defined,it has important significance to diagnose.Conclusions DC imaging has characteristic manifestation in diagnosis of the anterior gastric wall cancer.In combination of cautiously observation,fully recognization on all sings,changes of peripheral tissues structure imaging in cancer lesion situs and along with comprehensive clinical analysis could raise the accuracy in diagnosis of the anterior gastric wall cancer,before the stomach cancer operation.

ObjectiveTo investigate the relationship of blood glucose,blood lipids,serum uric acid,fibrinogen level and prognosis of patients with acute lacunar cerebral infarction.MethodsClirical data of 127 cases with acute lacunar cerebral infarction were retrospectively analyed.Patients with normal blood glucose,blood lipid,uric acid and fibrinogen level were enrolled into A group of 64 cases,63 cases with abnormal levels were selected as group B,an average of 2 years of follow-up,observing the two groups of neurologic,re-infarction incidence and prevalence of vascular dementia were obsenved in two groups.ResultsAfter 4 weeks of treatment,A group of neural function defect score(9.6 ±7.3) was significantly lower than group B( 14.3 ±7.4) (t =3.8696,P ＜0.05) ;95.4％ total effective rate in A group was significantly higher than B group of 82.0％ ( x2 =5.8623,P ＜ 0.05 ).After an average follow-up of 2 years,A group of recurrent cerebral infarction rate( 18.8％ ),vascular dementia rates(7.8％ ),were significantly lower than group B(28.6％),17.5％(11/63)(x2 =6.5421,5.9623,P＜0.05).ConclusionA number of abnormal serum biochemical indexes had a significant impact on short and long-term prognosis of acute lacunar cerebral infarction.

Objective To illustrate if the racial coefficient (Rc) be biased by different reference GFR (rGFR) distribution among studies. Methods 1405 white and 321 African American participants in MDRD study and 684 chronic kidney disease(CKD)patients in Chinese eGFR Investigation Study were included.Firstly.the unweighted datasets of white and Chinese were stacked together.rGFR,age and plasma creatinine (Pcr) were log transformed.Linear regression model was constructed using log transformed rGFR as dependent,gender,race and log transformed Pcr and age as independent.Unweighted RC (uRC) for Chinese was calculated.Then.the Chinese CKD distribution of rGFR was weighted to be the same as that in White American.and weighted RC (wRC)for Chinese was calculated.The cases of White.and African-American were stacked together.The cases of African-American were weighted to make the rGFR distribution the same as that in White-American and Chinese population respectively,and RCs for African-American were calculated. Resuits The uRC for Chinese was 1.197(1.180-1.211)and the wRC was 1.130 (1.117-1.143).The two RCs did not overlap with each other.The RCs for African-American were 1.205(1.19-1.219)and 1.233(1.219-1.247)respectively. Conclusions The RCs were influenced by the difference of rGFR distribution.To find out the real RC.an intemational collaborative study iS needed,with the same rGFR measure method.strict control of Pcr measurement,and the same rGFR distribution.

Objective To analyze the distribution of body mass index (BMI) in the patients with thyroid cancer. Methods The clinical data of 1 005 cases of thyroid cancer diagnosed by the postoperative pathology from January 2007 to October 2017 in Xinjiang Military Region General Hospital were retrospectively analyzed. A total of 668 healthy people who underwent ulltrasonic and functional test of thyroid cancer without obvious abnormal response were chosen as healthy controls. The preoperative BMI of thyroid cancer patients and BMI distribution of healthy controls were compared. Results According to the BMI Asia criteria, 1 005 patients with thyroid cancer included 346 (34.43 %) cases of obesity, 296 (29.45 %)normal cases, 275 (27.36 %) chubby cases, 60 (5.97 %) cases of severe obesity, 28 lean cases and the obese people were the most. Among 668 healthy controls, 269 (40.27 %) cases were normal, 168 (25.15 %) cases were chubby, 165 (24.70 %) cases were obesity, and 41 (6.14 %) cases were lean, 25 (3.74 %) cases were severe obesity and the most common was the normal group. There was a statistically significant difference of BMI distribution between the two groups in the lean, normal, obesity, and severe obesity group (χ2= 11.400, P = 0.001; χ 2= 20.991, P = 0.000; χ 2= 17.898, P = 0.000; χ 2= 4.129, P = 0.042). The pathological types of 1005 cases of thyroid cancer were mainly papillary carcinoma [948 cases (94.33 %)], successively followed by follicular carcinoma [42 cases (4.18 %)], medullary carcinoma [9 cases (0.90 %)] and undifferentiated carcinoma [6 cases (0.60 %)]. Patients with papillary carcinoma included 321 (33.86 %) obese cases, 282 (29.75 %) normal cases, 264 (27.85 %) chubby cases, 57 (6.01 %) cases of severe obesity, and 24 (2.53 %) lean cases. Patients with follicular carcinoma included 19 cases (45.24 %) obese cases, 9 (21.43 %) normal cases, 7 (16.67 %) chubby cases, 4 (9.52 %) lean cases, and 3 (7.14 %) cases of severe obesity; Patients with medullary carcinoma included 4 obese cases, 3 chubby cases, and 2 normal cases; Patients with undifferentiated cancer included 3 normal cases, 2 obese cases, and 1 chubby case. There was no significant difference in BMI distribution among different pathological types of thyroid cancer patients (all P＞ 0.05). Conclusions The number of obesity and severe obesity in thyroid cancer patients is more than that in healthy people, while the number of lean and normal groups is less than that in healthy controls. Obesity may be related with thyroid cancer, which contributes to the risk assessment and prevention of thyroid cancer.

Objective By a sequencing panel consisting of 50 targeted genes, aiming at depicting the molecular landscape of ET, PV, and PMF, which are three major subtypes of MPN, to provide valuable information in the diagnosis and prognosis of MPN.Methods A retrospective study was conducted of 53 patients from Huashan hospital and Changhai hospital. All patients were diagnosed in accordance with the 2016 WHO diagnostic criteria for MPN, including 31 cases of ET(11 males, 20 females, median age 55 years), 17 cases of PV(12 males, 5 females, median age 65 years), and 5 cases of PMF(4 males, 1 females, median age 67 years), and underwent next-generation of DNA sequencing of their bone marrow or blood samples. The genetic analyses were performed on bone marrow or peripheral blood. Referring to COSMIC, dbSNP, Clinvar and other public databases, we analyzed the sequencing data, and elucidated the mutation profile of MPN patients, combining with their clinic information. Results In addition to the typical JAK2, CALR, and MPL mutations, pathogenic mutations in other 11 genes were detected, as well as 4 SNPs that confer individual susceptibility to MPNs (rs4858647, rs9376092, rs58270997, rs621940). The average rate of mutated genes was 2.3 genes per patient. In all patients (53 cases), the mutated genes detected were TET2, EZH2, ASXL1, MIR662, SF3B1, BARD1, DNMT3A, KIT, RUNX1, TP53, NRAS according to their mutational frequency. Conclusions Applying next-generation sequencing technology, multi-gene sequencing of a bunch of typical BCR-ABL-negative MPN patients can be performed at one time within 2 working days, and pathogenic mutations other than JAK2, CALR, MPL can be found, which has a bright prospection in clinic.

Objective:To explore the value of nursing checklist in posterior surgery for thoracolumbar fracture with general anesthesia under prone position.Methods:A retrospective case series study was conducted to analyze the clinical data of 106 patients with thoracolumbar fracture admitted to Sir Run Run Shaw Hospital，Zhejiang University School of Medicine from June 2018 to May 2020. There were 80 males and 26 females，with age range of 25-57 years［（48.6 ± 11.9）years］. Segments of injury were located at T 11 in 18 patients，at T 12 in 26，at L 1 in 21，at L 2 in 25 and at L 3 in 16. All patients were treated with thoracolumbar posterior screw fixation under general anesthesia. Of all，51 patients received conventional postural nursing from June 2018 to May 2019（control group），and 55 patients received prone position nursing scheme for general anesthesia on the basis of conventional postural nursing from June 2019 to May 2020（verification group）. The incidence of postoperative complications including stress injury，brachial plexus injury，ulnar nerve injury and ocular discomfort as well as length of hospital stay and patients’ satisfaction were compared between the two groups. Visual analogue scale（VAS）and Oswestry disability index（ODI）were also used to measures outcome at postoperative 3 months. Results:All patients were followed up for 8-12 months［（10.5±0.9）months］. Verification group and control group showed significant differences in the incidence of stress injury（4%∶29%），brachial plexus injury（4%∶16%）and ocular discomfort consisiting of tears（2%∶12%），foreign body sensation（0%∶4%）and dryness（4%∶16%）（ P < 0.05），not in ulnar nerve injury and blurred vision. Length of hospitalization in verification group was（7.0±1.3）days，significantly shorter than that in control group［（9.9±1.9）days］（ P < 0.05）. Satisfaction of patients in verification group and control group was 85%（47/55）and 69%（35/51），respectively（ P < 0.05）. At 3 months postoperatively，VAS in verification group［（1.9 ± 0.8）points］was significantly lower than that in control group［（3.5±1.1）points］（ P < 0.05），and ODI was similar between the two groups（ P > 0.05）. Conclusions For patients with thoracolumbar fracture treated by posterior surgery with general anaesthesia under prone position，nursing checklist helps reduce occurrence of the related complication，shorten length of hospital stay，improve patient satisfaction，reduce postoperative pain and promote rehabilitation.
