ObjectiveTo investigate the efficacy and treatment options of precise electrophysiological diagnosis combined with percutaneous low-frequency electrical stimulation for penile scleroderma in high-altitude hypoxic areas. MethodsA total of 54 patients with electrophysiological diagnosis and treatment of corpora cavernosa were included in the retrospective analysis. Their age ranged from 20 to 70 years with a mean of 45.5±11.6 years and disease duration from 2 to 16 months. PDE5i, levocarnitine oral solution, and traditional Chinese medicine for invigorating blood circulation and eliminating stasis were administered orally before treatment. The visual precision electrophysiological diagnosis was performed after discontinuation of all oral drugs for 1 week before treatment. The electrophysiological diagnostic parameters were recorded and contrasted before and after the set-up treatment, and the corresponding parameters for precision electrophysiology were adjusted for percutaneous low-frequency electrical stimulation treatment, 2-3 hours each time, once daily, and 10 times per course. At the same time was administered assisted nasal cannula oxygen inhalation for 2 to 3 hours (4L/min). Before and after electrophysiological diagnosis and treatment were evaluated by using the numeric assessment scale of pain extent, the international index of erectile function score (IIEF-5), penile curvature measurement, color Doppler measurement of plaque areas and comparison of whole body and disease local infrared thermogram temperature changes. ResultsAll patients successfully completed 2-4 sessions of percutaneous low-frequency electrical stimulation (20 sessions to 40 sessions). There was a significant statistical difference (P < 0.01) between the mean values before and after treatment for the degree of weakness / erection pain (7.1±1.6 vs 2.0±1.0);(8.2±3.2 vs 16.0±5.1) before and after IIEF-5 treatment, respectively (P < 0.01),Pre- and post-treatment of PCDDU measured mean plaque size was 0.23(0.09~0.54)cm2 vs 0.23(0.09~0.54)cm2,with no statistically different change in improvement value (P = 0.189). Four cases had no penile camptodactyly, Before electrical stimulation, 50 (92.6%) patients had different degrees of penile curvature deformity, and there was no change in the improvement of the contrast bending angle before and after treatment (P >0.05). The comparison of TMT images before and after treatment with visual precision electrophysiology diagnosis suggested that temperature change >1.5℃ in the whole body and penile and bilateral inguinal regions, etc. could be considered effective electrical stimulation treatment. There were no recurrences of induration, increased pain and penile curvature at 3~10 months follow-up, and no patient required subsequent surgery. ConclusionThe results of this study show that visualizing the electrophysiological diagnosis situation to set local and holistic combined treatment parameters, and treating PD by means of percutaneous low-frequency electrical stimulation at meridian and neuromuscular acupoints of traditional Chinese medicine could achieve good safety and efficacy, especially in the early stage, which has a clear effect on pain treatment, thus opening new avenues for PD treatment.

Method: One hundred and twenty-three patients were randomly divided into control group (61 cases) and observation group (62 cases) by random number table. Patients in control group got aspirin enteric-coated tablets, 0.1 g/time, 1 time/day, and rosuvastatin calcium capsules, 10 mg/time, 1 time/day. In addition to the therapy of control group, patients in observation group were also given Dazhu Hongjingtian capsule, 4 grains/days, 3 times/days. And the outpatient follow-up was made once every 2 weeks for continued 16 weeks. Intima-media wall thickness (IMT), and number and size of plaque were detected by color Doppler ultrasound. And levels of triglyceride (TG), total cholesterol (TC), high-density lipoprotein (HDL-C), low-density lipoprotein (LDL-C), superoxide dismutase (SOD), malondialdehyde (MDA), hypersensitive C reactive protein (hs-CRP), homocysteine (Hcy), matrix metalloproteinase-2 (MMP-2), MMP-9, tissue inhibitor of matrix metalloproteinase-2 (TIMP-2), platelet alpha granular membrane protein (GMP-140), P-selectin on surface of platelets (CD62 p), lysosome glycoprotein (CD63), and vascular pseudotumor factor (VWF) were detected. Qi deficiency and blood stasis syndrome were graded. Result: After treatment, IMT in observation group was thinner than that in control group, number of plaque was less than that in control group, and size of plaque was smaller than those in control group (P<0.01). After treatment, levels of LDL-C, hs-CRP, Hcy, MMP-2, MMP-9, MDA, GMP-140, vWF, CD62 p and CD63 were lower than those in control group (P<0.01), whereas levels of HDL-C and TIMP-2 were higher than those in control group (P<0.01). And scores of Qi deficiency and blood stasis syndrome were lower than those in control group (P<0.01). Conclusion: In addition to the treatment for resisting platelet and regulating blood lipid, Dazhu Hongjingtian capsule can lessen and reduce arterial speckle regulate lipid metabolism, ameliorate symptoms of traditional Chinese medical (TCM) syndrome, with effects in resisting oxidation, inflammation and thrombotic formation.

Objective To investigate the diagnosis and surgical treatment of co-infection with hepatic cystic and alveolar echinococcosis, so as to provide insights into the diagnosis and treatment of these patients. Methods The clinical data of patients with co-infections of hepatic cystic and alveolar echinococcosis in Qinghai Provincial People’s Hospital between 2017 and 2018 were retrospectively analyzed. Results Three patients were diagnosed with co-infection of hepatic cystic and alveolar echinococcosis. One patient was diagnosed by preoperative CT scan, and confirmed intraoperatively. The other two cases were diagnosed as cystic echinococcosis by preoperative color ultrasonography and imaging examinations, and were definitively diagnosed as co-infection of hepatic cystic and alveolar echinococcosis by intraoperative examination of the lesion morphology and postoperative pathology. Two patients were given radical surgery, and another case was given removal of the internal capsule and subtotal excision of the outer capsule of Echinococcus granulous in the liver following exploration. Conclusions Co-infection with hepatic cystic and alveolar echinococcosis is easy for missed diagnosis and misdiagnosis prior to operation, and the definitive diagnosis may be made by means of imaging examinations combined with postoperative pathology. The surgery is relatively complicated and difficult for patients with co-infection of hepatic cystic and alveolar echinococcosis, and individualized surgical treatment regimen should be employed for patients with various types of infections.

Journal of Chinese Physician 2012;14(5):580-584
Objective Our study was performed to design a drug-sustained capsular tension ring (CTR) to evaluate its potentiality on prevention of PCO in the swine capsular bag model in vitro. Methods Following the continuous capsule curvilinear capsulorhexis ( CCCC),Phacomulsification with capsular tension ring implantation was pedormed.CTR-supported swine capsular bag models were prepared and divided into two groups, group CTR ( n =13 ) implanted with the original CTR without any modification and group CTR-PLGA-MG132 ( n =13) implanted with the CTR covered with PLGA and MG132.The CTRsupported capsular bags were cultured in vitro for up to 3 weeks. The area of lens epithelial cells (LEC) coverage over the posterior capsule surface was quantified every day under microscope. The capsules were treated for histological examination. The change of fibronectin was assessed by ELISA assay kit. Results After 2 ~ 3 days, outgrowth of LEC across the posterior capsule was observed, and the posterior capsule was totally covered by a confluent monolayer of cell after (9.06 ± 1.61 ) days in group CTR.Capsular wrinkles became increasingly apparent as time progressed. An increase in capsular thickness was also observed. In contrast, there was less LEC deposition in group CTR-PLGA-MG132.Histological examination showed LEC layers were closely arranged on the posterior capsular surface in group CTR.In group CTR-PLGA-MG132,there was comparatively looser cell arrangement.Compared with group CTR,the mean fibronectin level of posterior capsule by week 3 in group CTR-PLGA-MG132 was 25.14 μg/ml and 106.09 μg/ml respectively.Statistical analysis showed a significant difference ( P < 0.01 ).Conclusions LEC migration, proliferation, and synthesis of EMT markers were inhibited in Group CTR-PLGA-MG132,compared with Group CTR.Drug-sustained capsular tension rings can effectively inhibit the migration, proliferation of LEC and the change of EMT ( epithelial-to-mesenchymal transition) in swine capsular bag models.Drug-sustained capsular tension rings might be a potential therapy to prevent the posterior capsular opacification in the future.

China Occupational Medicine 2017;44(05):600-603
OBJECTIVE: To explore the relationship between metabolic syndrome( MS) and occupational stress among workers in high altitude area. METHODS: A cross-sectional study was conducted in 940 workers as research subjects who were engaged in petroleum smelting, thermal power generation and financial management et al. in high altitude area. A questionnaire survey was conducted using the Occupational Stress Inventory-Revised for occupational stress, The occupational health examination was performed to measure waist circumference, blood pressure, blood sugar and blood lipids. RESULTS: The abnormal rates in obesity,triglycerides,high-density lipoprotein, blood pressure and fasting blood glucose were 28. 3%,16. 3%,14. 3%,13. 3% and 6. 5%, respectively. The detection rate of MS was 5. 0%( 47/940). The multivariate logistic regression analysis showed that the risk of elevated MS decreased with the increased of occupational stress level( P < 0. 05),and the risk of elevated MS increased with the increased of occupational stress reaction severity( P < 0. 05),after adjusting the confounding factors of gender and labor nature. CONCLUSION: The higher occupational stress reaction of the workers in high altitude area, the higher the risk of MS.
