1.Clinical characteristics of dopa-responsive dystonia

Objective To analyze the clinical characteristics of dopa-responsive dystonia (DRD). Methods The clinical characteristics and the causes of misdiagnosis in 15 patients with DRD were reviewed retrospectively.Results 4 male and 11 female patients were investigated in this study. The age at DRD onset was from 3~25 years old,the average age was 11.7 years old.The courses ranged from 2 to 29 years and the average was 12.1 years. The symptoms in female patients often aggravated during gravidity and delivery. Main clinical features included gait irregularity, postural instability and tremor, with marked diurnal fluctuation. The disease may be misdiagnosed as cerebral palsy, idiopathic torsion dystonia, essential tremor, parkinson's disease, neurosis and so on. All the patients in this study improved obviously in 2 weeks after therapy with low dose of Madopar, and recovered in 2~4 years of follow up.Conclusions Clinical diversity and diurnal fluctuation are the characteristics of DRD. Early trial treatment with low dose of Levodopa should be highly recommended for the generalized dystonia in children and untypical adults.
2.Detection of Chromosome 6 Aberration in Lung Carcinoma Using Fluorescence in Situ Hybridization(FISH)

Objective To study the relationship between chromosome aberration and the pathogenesis,development and prognosis of lung carcinoma. Methods Tissue speciments from 30 cases of lung carcinoma and 10 cases of normal lung tissue were detected using FISH with chromosome 6 probe,respectively. Results All of the 30 cases of lung carcinoma were found chromosome 6 aberration,involved monosomy,trisomy,and even heptasomy.But there was no chromosome 6 aberration found in normal lung tissues.The significant difference was between these two groups. Conclusion Chromosome 6 aberration may occur in the early stage of lung carcinoma,which may closely relate with the process of pathogenesis,development of lung carcinoma.The chromosome 6 aberration may have the significant association with clinical phase and pathological differentiation among the patients.
3.Extraction Technology of Volatile Oil from Qingjie Granules and Optimization of Inclusion Technology by Box-Behnken

Objective To optimize the extraction of volatile oil from Qingjie Granules and process of inclusion compound of beta-cyclodextrin. Methods Water Distillation was used for extraction. Extraction time, grinding degree, and the amount of water were set as inspection factors, and volatile oil volume was set as the evaluation index to inspect extraction process of volatile oil from Qingjie Granules. With inclusion rate as the evaluation index, the single factor test and the Box-Behnken combined with the response surface method were used to choose the optimum inclusion process. Results The optimum extraction process for Schizonepetae Herba, Saposhnikoviae Radix and Forsythiae Fructus coarse powder should be with 10 times amount of water, extracting 3 h. Inclusion method should be saturated water solution method, and the inclusion process of volatile oil as feed and beta-cyclodextrin inclusion ratio was 1:12; the temperature was 40 ℃; inclusion time was 3.5 h. By means of TLC, UV and IR spectra, the formation of the inclusion compound of volatile oil in clear solution particles was preliminarily proved. Conclusion The optimum extraction and inclusion process of volatile oil from Qingjie Granules are stable and feasible, which can be used in industrial production.
4.Obervations on the clinical efficiency of autologous bone marrow stem cell transplantation on chronic ischemic disease of lower limb

Objective To explore the application of autologous bone marrow stem celI transplantation in the treatment of chronic ischemic disease of lower extremity and evaluate the effect.Methods Forty patients with chronic ischemic limbs were treated from January 2008 to April 2009.The bone marrow of each patient was stimulated by an injection of the recombinant human granulocyte macrophage colony-stimulatory factor(GCSF),300?g /d,for 2-3 days.Then,200 mL of bone marrow was withdrawn from the iliac spine and the stem cells were obtained.After the bone marrow separation and purification,they were transplanted into the more severely ischemic limb.After the transplantation,they were foIlowed up by phone once a month for six months.The skin temperature,transcutaneous oxygen pressure(TcPO2),digital subtraction angiography(DSA)and ankle/brachial index(ABI)were rechecked at 6 months later,and the symptoms of pain,cold,numbness,intermittent claudication and physical signs of ulcer and gangrene were also evaluated.Results All of the 40 patients were followed-up.At 6 months post-transplantation,the various indices were compared with before transplantation.(1)The skin temperature and TcPO2 had significante improvement (P0.05).(3)DSA and all of the clinical symptoms showed general improvement in more than 60% of cases.(4)There were no serious complications.Conclusions Transplantation of stimulated autologous bone marrow stem cells is effective in the treatment of ischemic disease of lower limb,which is a simple,safe and effective treatment for ischemic disease of lower limb.
5.HPLC Characteristic Chromatogram and HPLC-MS/MS Analysis ofKanglao Qingfei Granules

Objective To establish HPLC characteristic chromatogram ofKanglao Qingfei Granules.Methods HPLC analysis of samples was performed on Kromasil C18 column (4.6 mm × 250 mm, 5μm), with acetonitrile-1% glacial acetic acid as the mobile phase of gradient elution (0-50 min, 5%→15% acetonitrile;50-70 min, 15%→25% acetonitrile;70-80 min, 25%→40% acetonitrile;80-90 min, 40%→65% acetonitrile, 90-120 min, 65%→95% acetonitrile);the volume flow rate was 1.0 mL/min;detection wavelength was set at 290 nm;column temperature was 30℃. Chromatographic peaks were identified by HPLC-MS/MS method.Results The similarity degrees of 10 batches of samples were all greater than 0.995, and 13 chromatographic peaks were determined as common characteristic peaks, of which 10 peaks were confirmed in the source attribution and 8 peaks were identified in chemical component.Conclusion The established HPLC characteristic chromatogram can be used for the quality control ofKanglao Qingfei Granules.
6.Effect of angong niuhuang wan on serum lactate dehydrogenase isoenzymes in rats with cerebral infarction

BACKGROUND: Angong niuhuang wan consists of mercury and arsenic, which has drawn the attention on its safety. It is necessary to carry on the research on evaluation in its availability and safety.OBJECTIVE: To study the mechanic differences of angong niuhuang wan in the organic body physiologically and pathologically.DESIGN: Randomized controlled experiment based on the experimental animals.SETTING: Institute of clinical pharmacology affiliated to a university.MATERIALS: The experiment was performed in Institute of Clinical Phar macology affiliated to Guangzhou University of Traditional Chinese Medicine from March to April in 2001. Guangdong Medical Experimental Animal Center provided 24 SD male rats, weighted varied from 250 g to 300 g.METHODS: SD rats were randomized into 4 groups, named as normal group, normal & angong niuhuang wan group(normal & wan group), model of cerebral infarction group(model group) (The middle cerebral artery embolism was induced in rats by photochemistry. ) and model of cerebral in farction group & angong niuhuang wan group (model & wan group), 6 rats in each group. Medication instruction: gastric feeding was applied once daily, 0. 13 g/kg, totally for 7 days. MAIN OUTCOME MEASURES: contents of lactate dehydrogenase (LDH) isoenzymes, LDH1-5.RESULTS: Contents of serum LDH1-3 in normal & wan group were significantly increased than those in normal group( P < 0.01 ), of which, the values of LDH1, LDH2, LDH3 were(17.02 ±0.46)%, (14.70 ±0. 18)%,(15.47±0.13)% successively in normal & wan group, were(11.25 ± 0. 70) %, (8.26 ± 0. 90) %, ( 12. 86 ± 0.90) % successively in normal group. Content of serum LDH3[(15.51 ±2.60)% [ in model & wan group was significantly increased compared with that in model group[(10.93±2.10)% ] (P < 0.01) . The contents of LDH4 were[(22.62 ±3.00)% ] and(28. 18 ±0. 80)% respectively in two groups, indicating significantly reducing( P < 0.01 ).CONCLUSION: The organic injury of angong niuhuang wan is less in pathological state of focal cerebral infarction than that in physiological state,suggesting that the mechanic differences of angong niuhuang wan in organic body present in physiological and pathological states.
7.Inhibitory effect of scopolamine on withdrawal syndromes in morphine dependent rats

Objective: To observe the inhibitory effect of scopolamine(Spm) and chlopromazine (Clo) on withdrawal syndromes in morphine dependent rats. Methods: The intensity of withdrawal syndromes on the model of morphine dependent rats was recorded after single or muiltiple subcutaneous administration(sc) of Spm and Clo at different doses. Results: Withdrawal syndromes were markedly decreased when single Spm 1 mg/kg and Clo 0.5 mg/kg combined with morphine were injected (P<0.05). Spm+Clo(sc) had much stronger effects on inhibiting withdrawal syndromes after intraperitoneal (ip) naloxone in morphine dependent rats (P<0.01). Conclusion: Spm can act on Ach-receptor and relieve morphine withdrawal syndromes. Clo may have a synergistic action with Spm via α2-receptor in the locus coeruleus of the rat brain stem.
8.Short-term Effect of Phyllanthus Compound and Lamivudine in Treating Chronic Hepatitis B: An Observation of 30 Cases

[Objective] To investigate the therapeutic effect of Phyllanthus compound (PC, mainly composed of Herba Phyllanthus, Radix Astragali, Radix Notoginseng, Radix Glycyrrhizae, etc.) and lamivudine on chronic hepatitis B. [ Methods ] Sixty cases of chronic hepatitis B were randomized to two groups: group A (the combination of PC and lamivudine) and group B (lamivudine 100mg/d). After a six-month treatment, the therapeutic effect was evaluated. [Results] After treatment, symptoms and signs were improved in group A. The recovery rate of alanine aminotransferase (ALT) was 83.3% and 53.3%, the rate of hepatitis B E antigen (HBeAg) turning negative was 73.3% and 13.3% , the percentage of HBeAg ( - ) or anti-HBe ( + ) was 53.3% and 6.7% and HBV-DNA-turning-negative rate was 93.3% and 70.0% in groups A and B respectively (P
9.Determination of Drug Release and in vitro Transdermal Rate of Tetrahydropalmatine in Xiaoji Aitong Cat-aplasmas in Rats

Objective:To determine the release and in vitro transdermal rate of tetrahydropalmatine in Xiaoji Aitong cataplasmas in rats. Methods:Using the self-made drug release determination devices and the modified Franz diffusion cells, and the skin of rats as the barrier, the release and in vitro transdermal rate of tetrahydropalmatine in Xiaoji Aitong cataplasmas were detected by HPLC. Re-sults:Tetraydropalmatine within the range of 0. 51-10. 22 μg showed a good linearity (r=0. 999 7), and the average recovery was 98. 49%(RSD=0. 84%,n =9). The release of tetrahydropalmatine in 80 min was 63. 60%, and the skin permeation rate was 23. 15% in 24h. Conclusion:Xiaoji Aitong cataplasmas have good drug release and transdermal performance.
10.Study of glycemic fluctuation impact on angiopoietin-2 and endoplasmic reticulum stress in type 2 diabetes complicated with acute phase of acute coronary syndrome

Objective To investigate the changes in angiopoietin 2 and endoplasmic reticulum stress,and the prognosis of acute phase of coronary syndrome (ACS) in type 2 diabetic patients with glycemic fluctuations.Methods Seventy-eight cases of consecutive diabetic patients with ACS within 7 days were enrolled.Another 78 cases of non-diabetic patients with ACS were selected as control.Risk assessment with global acute coronary events (GRACE) score in patients with ACS,dynamic blood glucose monitoring system (CGMS) for three days,realtime PCR analysis of angiopoietin-2,within the reticulum stress-related heavy chain binding protein (Bip),inositol kinase demand Ⅰ (IREI),endoplasmic reticulum class should PKR kinase (PERK),oxygen-regulated protein 150 (ORP150),activating transcription factor 6 (ATF6),X-box binding protein 1 (XBP1)mRNA expression change,phosphorylation of eukaryotic translation initiation factor 2oα (p-eIF2α) by western blotting angiopoietin-2,parameters were compared between the two groups.Correlation analysis with GRACE score ; while angiopoietin-2 parameters and glycemic fluctuation,endoplasmic reticulum stress-related genes correlation analysis were made.Results (1) ACS group average daily blood glucose fluctuations(MAGE),mean absolute difference daytime blood glucose (MODD),and postprandial blood glucose fluctuation (MPPGE) and the maximum amplitude of glycemic excursions (LAGE) were significantly higher [MAGE(5.13 ± 1.19) vs (3.19 ± 0.55) mmol/L,MODD (2.59 ± 0.72) vs (1.72 ± 0.63) mmol/L; MPPGE (3.51 ± 1.01) vs (2.58 ± 0.55) mmol/L and LAGE (7.75 ± 2.39) vs (4.34 ± 0.85) mmol/L,all P<0.05].(2)In ACS group angiopoietin-2,endoplasmic reticulum associated genes Bip,IREI,PERK,ORP150,ATF6,XBP1,and monocyte chemoattractant protein 1 (MCP-1) mRNA expression levels as compared with the control group were statistically significant(all P<0.05) ; Angiopoietin-2,protein p-eIF2α were higher(P<0.05).(3) In the ACS group with pearson correlation analysis,angiopoietin-2 and MAGE,LAGE,MPPGE correlation (r =0.432,0.279,0.386,all P<0.05),Bip and MAGE,LAGE,MPPGE correlation(r =0.783,0.589,0.887,all P< 0.05) ; IREI and MAGE,LAGE,MPPGE,MODD correlation (r =0.567,0.783,0.569,0.823,all P<0.05) ; PERK and MAGE,MPPGE,MODD correlation(r =0.687,0.902,0.709,all P<0.05) ; ORP150 and MAGE,LAGE,MPPGE,MODD correlation(r=0.779,0.871,0.775,0.689,all P<0.05) ; ATF6 and MAGE,LAGE,MPPGE,MODD correlation(r =0.873,0.675,0.893,0.884,all P<0.05),while XBP1 with no correlation with glycemic fluctuations (P>0.05).(4) The endoplasmic reticulum stress gene was related to MCP-1 and blood glucose fluctuation parameters.(5) Multiple Logistic regression analysis revealed that LAGE,MPPGE,Bip,IREI,PERK,ORP150,ATF6 were risk factors affecting angiopoietin-2,and angiopoietin-2,ages,MAGE,homeostasis model assessment for insulin resistance,Bip,ATF6 were risk factors affecting GRACE.(6) The ejection fractions of the ACS patients showed negative correlation with MAGE and LAGE,multiple linear regression analysis showed that age,HOMA-IR,Ang-2,and MAGE,Bip,ATF6 established the linear regression relation with ejection fraction.Conclusion Glycemic fluctuations cause angiopoietin-2 to rise and lead to increased endoplasmic reticulum stress and affect the prognosis of diabetic patients with ACS.
