Chinese Journal of General Surgery 1997;0(06):-
Objective To summarize the diagnosis and treatment of 33 cases of Leriche syndrome. Methods A retrospective review of the clinical data of 33 cases of Leriche syndrome was done. Results Claudication and impotence occurred in 79.9% and 70.4% of the cases. Color Doppler ultrasonography, especially combining with CTA or MRA, was helpful for the diagnosis. Aortic angiography or DSA was necessary for the determination of the clinical patterns and selecting the therapeutic methods. Surgical patterns selestion should be considering the patients' general status and conditions of the affected vessels. Surgical treatment was performed on 25 cases, including12 aortoiliac artery bypasses , 6 aortobifemoral artery bypasses , 4 axillo bifemoral artery bypasses, 2 embolectomies by Fogarty tube only and 1 aortal interposition with artificial vessel plus renal artery plasty. Aorta iliac artery bypasses get the best results with 1 year patency rate(100%) in all cases, and 5 year patency rate of 75.0%, which was significantly superior to those axillo bifemoral artery bypass grafts with 5 year patency rate of 37.5%. All the other 8 patients without operation died within 5 months. Conclusions Early diagnosis and comprehensive therapy should be adopted to improve the long term patency rates of grafts transplantation in Leriche syndrome.

Objective To explore the clinical characteristics of primary central nervous system malignant lymphoma (PCNSML). Methods The clinical data of 22 cases of PCNSML were analyzed retrospectively. Results The patients in our study mainly presented with headache (16 cases), vomiting (11 cases), weakness of extremities (5 cases) and numbness (1 case). The tumors were supratentorial in 19cases, infratentorial in 3 cases and multiple in 5 cases. CT scan showed same density or high-density lesions with surrounding mild edema. Brain MRI demonstrated slight-hypointense lesion of 9 cases and isointense lesion of 5 cases on T1-weighted imaging. On the T2-weighted imaging, the lesions were slight-hyperintense in 11 cases and isointense in 3 cases. Enhanced scanning showed that the lesions were contrasted evenly and obviously on CT and MRI. Misdiagnosis was given preoperatively in 17 patients. The pathological examination revealed B cell malignant lymphoma in all cases. In 18 cases of follow-up, 5 patients with multiple lesions who only received stereotactic biopsy all died within 6 months, the mean survival time of 6 patients who were treated with total resection of the tumors and radiotherapy was 27 months, 4 patients treated with partial resection and ?-radiotherapy was 21 months, 4 patients treated with partial resection and radiotherapy was 10 months. The patient who was treated with only total resection of the tumor died in 11 months because of recurrent. Two patients with only partial resection died in 5 and 8 months, respectively. Conclusions There are no specific clinical manifestations of PCNSML and the disease is often misdiagnosed preoperatively. Although certain characteristic performance may be found on CT and MRI scans, the final diagnosis of this disease depends on pathological examination. Comprehensive treatments may prolong the survival time of the patients.

Chinese Journal of Tissue Engineering Research 2015;(33):5318-5321
BACKGROUND:With the research development of disc degeneration at molecular level, transforming growth factor β1 gene has a role in the proliferation and differentiation of intervertebral disc cels, and it is involved in the repair of intervertebral disc injury. But whether transforming growth factor β1 is also involved in pathophysiological process of disc degeneration has not been determined. OBJECTIVE:To discuss the relationship between transforming growth factor β1, inflammatory cytokines and degree of lumbar disc degeneration. METHODS: Twenty-two patients with lumbar disc degeneration (22 mild, 26 moderate and 24 severe cases) were enroled as observation group, and another 30 patients without lumbar disc degeneration served as control group. Levels of transforming growth factorβ1, interleukin-6, interleukin-8, tumor necrosis factor α were detected and compared among patients with mild, moderate and severe lumbar disc degeneration as wel as between the observation and controlgroups. RESULTS AND CONCLUSION: Levels of transforming growth factor β1, interleukin-6, interleukin-8, tumor necrosis factor α were significantly higher in the observation group than the control group (P < 0.01). In the observation group, the above-mentioned indicators were ranked as folows: severe cases > moderate cases > mild cases (P < 0.01). There was a positive correlation between these indicators and the severity of lumbar disc degeneration (r=0.198, 0.312, 0.356, 0.275, 0.724,P < 0.01). These findings indicate that the levels of transforming growthfactor β1, interleukin-6, interleukin-8 and tumor necrosis factor α are increased with the severity of lumbar disc degeneration.
