1.Cost-effectiveness Analysis of Different Slow-release Isosorbide Mononitrate Preparations in the Treatment of Coronary Heart Disease

OBJECTIVE:To evaluate the cost-effectiveness of different slow-release isosorbide mononitrate preparations in the treatment of coronary heart disease.METHODS:The random open study,three factors crossover trial and own control clinical test were conducted on3different Isosorbide mononitrate preparations.Then the curative effects of each were observed and the cost-effectiveness analyses were conducted.RESULTS:For the three kinds(A,B and C)of slow-release isosorbide mononitrate capsule,their total effective rates were87.9%,69.7%and57.6%respectively;the average daily medical expenses of which were10.25,15.14and14.04respectively;their cost-effectiveness ratio were69.97,130.33and146.25yuan respectively.CONCLUSION:The cost-effectiveness of the slow-release isosorbide mononitrate preparations is superior to the control medicines in the treatment of coronary heart disease.
2.Dynamic changes in apoptosis of polymorphonuclear neutrophil in rabbit with acute lung injury induced by seawater drowning

Objective To explore the dynamic changes in apoptosis of polymorphonuclear neutrophil (PMN) during the course of seawater drowning-induced acute lung injury (SWD-ALI), and the possible roles of cytokines in the process. Methods Thirty-six NZ rabbits were randomly divided into 6 groups by using a random number table: control group (0h, as baseline value) and 5 seawater drowning groups (S1h, S3h, S6h, S12h and S24h). Blood gas analysis and hemogram were observed at different time points. The ratio of wet to dry lung weight (W/D) and lung permeability index (LPI) were calculated. The changes in PMN apoptosis were analyzed by flow cytometer. The concentrations of tumor necrosis factor alpha (TNF-?), interleukin-1 (IL-1 ) and IL-10 in serum were detected by ELISA. Meanwhile, the pathological changes in lung tissue were examined with hematoxylin-eosin (HE) staining, and lung pathologic scores (LPS) were evaluated. Results The W/D ratio reached its peak value at 3h, while LPI at 6h. The histopathologic findings indicated that inflammatory cell infiltration was most massive at 6~12h. LPS increased at 1h, significantly higher than that of control group (P0.05). Conclusions The apoptosis of PMN, the main inflammatory effector cells, is significantly inhibited in the early stage of SWD-ALI. The serum cytokines, TNF-?, IL-1 and IL-10, play important roles in the pathogenesis of SWD-ALI, but have no evident effects on the inhibition of PMN apoptosis.
3.Inhibitory effects of leuprolide acetate microsphere on experimentally induced endometriosis in rats

AIM: To study the inhibitory effects of domestic leuprolide acetate microsphere (LE-ms) on the growth of explanted endometrium in the rat models of EMT and its possible mechanisms. METHODS: The 60 rats models of EMT were induced surgically by the Jones method. Then the animals were treated with LE (20 ?g? -1?d -1,28 d,sc) , enanton(20 ?g? -1?d -1,sc) and domestic LE-ms (2, 20, and 200 ?g㎏ -1?d -1,sc), respectively; another 30 rats were divided into sham group(N.S, 1 ml?kg -1?d -1,21 d,sc), EMT+ LE group(100 ?g?kg -1?d -1,21 d,sc)and EMT group(N.S, 1 ml?kg -1?d -1,21 d,sc). At the same time, estrous cycle was monitored daily by examination of vaginal cytologic smears. After 3 weeks, blood was drawn and the serum estradiol concentration was assayed. The volume of endometrial implant was assessed. Lateral uterus, bilateral ovary, thymus and spleen were weighed. The NK cell cytotoxicity in the spleen was evaluated with lactate dehydrogenase (LDH) release assay. RESULTS: Implants in control group continued to grow, while those in groups treated with the drugs showed remarkable atrophy. The inhibitory rates were 87.2%, 78.3%, 57.3%, 89.0% and 94.7%, respectively. AIM: To investigate the effects of nitric oxide (NO) inhalation on nitric oxide synthase (NOS) and endothelin-1 (ET-l) of patients with hypoxic pulmonary hypertension. METHODS: Examined 13 pulmonic blood samples to determine the concentration of NOS in leukocyte and ET - 1 in plasma before NO inhalation, 30 minutes after inhalation, 2 and 12 hours after stopping of inhalation respectiviy. RESULTS: The values taken before inhalation was NOS (0.70 ? 0.21 )mol/min?mg-l, ET-1 (78.89 ? 46.59) Pmol/L; 30 minutes after inhalation (0.74?0.14)mol/min.mg-l, ET - 1 (88 .27 ? 45 .41 )pmol/L; 2 hours after stopping of inhalation NOS (0.64 ? 0.22)mol/min.mg-1, ET - 1 (80.76?42.66)pmol/L; and 12 hours after stopping of inhalation NOS (0. 63? 0. 17)mol/min.mg-1, ET-1(61.07?29.44)pmol/L. NO significant difference was found in the values of NOS and ET- 1 before and after inhalation, P> 0. 05. CONCLUSION: The effects of NO inhalation on NOS and ET-l in patients with hypoxic pulmonary hypertension are not significant according to the above investigation.
6.RET proto-oncogene mutation analysis in a pedigree with multiple endocrine neoplasia 2A

ObjectiveTo discuss the clinical diagnosis and treatment of multiple endocrine neoplasia ( MEN ) 2A, and report the mutation of the RET proto-oncogene in a pedigree of three patients with MEN 2A.MethodsBilateral adrenalectomy was performed on two of the three patients with hypertension and bilateral adrenal-conserving adrenal pheochromocytoma resection was performed on the other patient. All three patients were treated by total thyroidectomy and neck lymphadenectomy. Twelve family members were recruited to the study. Peripheral blood was collected and total genomic DNA was prepared for polymerase chain reaction (PCR). PCR products of exon 10 and exon 11 of the RET proto-oncogene were purified and a direct DNA sequence analysis was performed.ResultsThe pathological diagnosis of the specimens was bilateral adrenal pheochromocytoma and medullary thyroid carcinoma in all the three patients. There was no tumor recurrence or distant metastasis after 1.5 - 5 years of follow-up. A missense mutation of TGC (Cys)to CGC (Arg) at codon 634 in exon 11 of the RET proto-oncogene was detected in all three patients. Genetic screening identified two mutation carriers in the other members of this pedigree.ConclusionGenetic mutation screening and surgical intervention may be helpful to the members of high-risk families.

10.Simple bone cyst of the jaw: a retrospective study of 11 cases.

OBJECTIVEThis study described the clinical, surgical, and radiographic findings of simple bone cysts.
METHODSA retrospective study was conducted for patients diagnosed with simple bone cysts in the Department of Oral and Maxillofacial Surgery of the Affiliated Stomatological Hospital of Nanchang University from March 2005 to March 2015. Clinical, radio-graphic, surgical, and follow-up data were gathered. Results were statistically analyzed by central tendency and dispersionusing SPSS 20.0 software.
RESULTSEleven cases of simple bone cysts were collected, including three male and eight female patients. Ten cases (90.9%) were asymptomatic and one case developed symptoms of swelling. All of the cases had no history of trauma in the affected area, and all were solitary; ten cases (90.9%) were unilocular, and one (9.1%) was multilocular.The shape of each lesion could be assigned to four categories: cone (3 cases), round (2 cases), oval (4 cases), and irregular (2 cases). The treatment in 10 cases consisted of surgery to explore the cavity and curettage of the bone walls. During surgery,the bone cavity in seven cases (70%) was vacant, whereas serous fluid was found in two cases (20%) and serous-bloody fluid in one case (10%). Of the ten cases, three cases exhibited complete bone healing and seven cases showed new bone formation.
CONCLUSIONSimple bone cysts of the jaws are usually asymptomatic and appear incidentally on routine radiographies. The prevalence is higher in the mandible and young people. The patient usually has no history of trauma, and the bone cavity of lesion is mostly vacant. Curettage of the bone walls of the lesion is suggested for simple bone cysts. Systemic clinical and radiologic follow-up are necessary to ensure successful treatment.
