Background and Purpose:The early prediction of local tumor progression-free survival(LTPFS)after radiofrequency ablation(RFA)for colorectal cancer(CRC)lung metastases has significant clinical importance. The application of radiomics in the prediction of tumor prognosis has been explored. This study aimed to construct a radiomics-based nomogram for predicting LTPFS after RFA in CRC patients with lung metastases. Methods:This study retrospectively analyzed 172 CRC patients with 401 lung metastases admitted to Department of Interventional Radiology,Fudan University Shanghai Cancer Center from August 2016 to January 2019.This study was reviewed by the medical ethics committee of Fudan University Shanghai Cancer Center(ethics number:2402291-24).After augmentation of pre-ablation and immediate post-ablation computed tomography(CT)images, the target metastases and ablation regions were segmented manually to extract the radiomic features.Maximum relevance and minimum redundancy algorithm(MRMRA)and least absolute shrinkage and selection operator(LASSO)regression models were applied for feature selection. The clinical model, the radiomics model, and the fusion model were constructed based on the selected radiomic features and clinical variables screened by the multivariate analysis. The Harrell concordance index(C-index)and area under receiver operating characteristic(ROC)curves(AUC)were calculated to evaluate the prediction performance. Finally, the corresponding nomogram of the best model was drawn. Results:Among all the lung metastases,102(25.4%)had final recurrence, and 299(74.6%)had complete response(CR).The median follow-up time was 21 months(95%CI:19.466-22.534),and the LTPFS rates at 1,2,and 3 years after RFA were 76.5%(95%CI:72.0-80.4),72.1%(95%CI:66.6-76.9)and 69.9%(95%CI:64.0-75.1).In both the training and test dataset, the fusion model based on the final 12 radiomic features through the LASSO regression and 4 clinical variables screened by multivariate analysis achieved the highest AUC values for LTPFS,with C-index values of 0.890(95%CI:0.854-0.927)and 0.843(95%CI:0.768-0.916),respectively.Conclusion:The fusion model based on radiomic features and clinical variables is feasible for predicting LTPFS after RFA of CRC patients with lung metastases, whose performance is superior to the single radiomic and clinical model. At the same time, the nomogram of the fusion model can intuitively predict the prognosis of CRC patients with lung metastases after RFA,thus assisting clinicians in developing individualized follow-up review plans for patients and adjusting treatment strategies flexibly.

OBJECTIVE To s tudy the impr ovement effects of tilianin on the atherosclerosis (AS)model mice and its potential mechanism. METHODS Eight C 57BL/6J mice were taken as the normal group. Forty ApoE-/- mice were randomly divided into model group ,tilianin low-dose ,medium-dose and high-dose groups [ 2.1,3.5,7.0 mg/(kg·d)] and simvastatin group [positive control drug ,3.5 mg/(kg·d)],with 8 mice in each group. Normal group was given normal diet ,and other groups were given high-lipid diet to induce AS model. At the same time ,normal group and model group were given normal saline intragastrically , administration groups were given relevant drug intragastrically ,once a day ,for 12 consecutive weeks. The levels of TC ,TG, LDL-C,HDL-C,Ox-LDL,IL-1β,IL-6,MCP-1 and TNF-α in plasma were determined. The pathomorphological changes of the aorta in mice were observed. The positive rate of ICAM- 1,VCAM-1 and PCNA in the aorta were determined. mRNA expressions of MMP- 2,MMP-9,TGF-β1,Smad2 and Smad 3 as well as protein expressions of TGF-β1,Smad2/3 and p-Smad 2/3 were also determined in aorta of mice. RESULTS Compared with normal group ,the plasma levels of TC ,TG,LDL-C,Ox-LDL,IL-1β, IL-6,MCP-1 and TNF-α in model group were increased significantly(P<0.01),while HDL-C level was significantly reduced (P<0.01). Lipid plaques were formed in the aorta ,and the plaque area was large and caused severe stenosis of the lumen. mRNA expressions of MMP- 2,MMP-9,TGF-β1,Smad2 and Smad 3 as well as positive rate of ICAM- 1,VCAM-1,PCNA and protein expression TGF-β1,Smad2/3,and p-Smad 2/3 in the aorta were significantly increased (P<0.01). Compared with model group , most of above indexes of medication groups were improved significantly (P<0.05 or P<0.01). CONCLUSIONS Tilianin can inhibit the activation of TGF-β1/Smads signaling pathway and then inhibit the proliferation of vascular smooth muscle cells ,reduce , inflammation and regulate lipid metabolism to inhibit the No.81960766) formation of AS.

Objective To analyze the current status of exercise and exercise self-efficacy (ESE) and its influencing factors in middle-aged and elderly patients with high blood pressure, and to provide a basis for the intervention of patients with hypertension in the community. Methods The stratified random sampling method was used to select 546 patients with hypertension who had received hypertension management for one year in the community. The general condition, exercise condition, and ESE scores were collected using survey questionnaires. Chi-square test was applied to analyze the difference of exercise efficacy score among different characteristic objects, and analysis of regression method was applied to analyze the influencing factors of exercise self-efficacy. Results A total of 546 people were surveyed in this study, of which 67.03% of hypertension patients participated in exercise. The main exercise program was walking healthily (87.43%). A total of 262 people (47.99%) had strong performance on ESE score, while 284 people (52.01%) had weak self-efficacy. The univariate unconditional logistic regression analysis found that there were significant differences in the distribution of educational level, family monthly income, years of hypertension, whether they received the guidance from family doctors, whether they exercised and whether they warmed up before exercise (P<0.05). The multivariate unconditional logistic regression analysis showed that educational level of junior high school and low average monthly household income were the risk factors affecting exercise efficacy, and the guidance of family doctors, exercise, and hypertension less than 5 years were the protective factors affecting exercise efficacy. Conclusion The exercise self-efficacy of middle-aged and elderly patients with hypertension in this community is at a medium level, and family doctors should carry out targeted interventions based on influencing factors to improve exercise self-management ability.
