1.Value of contrast-enhanced ultrasound and enhancement MRI in differetial diagnosis of the orbital neoplasms

Objective To evaluate the ability of contrast-enhanced ultrasonography (CEUS ) compared to enhanced MRI scan in differential diagnosis of benign and malign tumor of orbit.Methods CEUS and enhanced MRI was performed in 74 patients with tumor of orbit.These patients were divided into two groups:benign and malignant tumor depended on histological diagnosis.The results were analysed with SonoLiver software.The coincidence rate of diagnosis and enhancement intensity among CEUS,MRI and CEUS+ MRI were compared and the quantitative parameters of CEUS were analysed.Results (1 )The diagnostic accordance rate of benign group by three methods were 90.0%,80.0% and 96.0% respectively, there was statistical difference for diagnostic coincidence rate between the MRI and CEUS+ MRI(P <0.05).The diagnostic accordance rate of malignant group by the above three methods were 75.0%,79.2%and 95.8% respectively,there was no statistical difference for diagnostic coincidence rate between the three methods(all P > 0.05 ).(2 )The enhancement intensity of CEUS was different between benign and malignant groups (P <0.05).There was no statistical difference between benign and malignant groups by MRI method (P >0.05).(3)The parameters of tumor imaging obtained time intensity curve(TIC):rise time (RT),half elution time and time to peaking (TTP)were significantly different between benign and malignant groups(P <0.05).While there was no significant difference of parameters as mean transit time (mTT)and arrived time (AT)(P >0.05).Conclusions CEUS had higher diagnostic accuracy than MRI in benign orbital tumor.The contrast enhancement intensity and time-intensity curves provide valuable diagnostic information for differential diagnosis of benign and malignant orbital tumor.
2.Application of contrast-enhanced ultrasound in differential diagnosis of cervical lymph nodes

Objective To evaluate the ability of contrast-enhanced ultrasonography (CEUS)in differentiate reactive lymph node,metastatic lymph node and lymphoma.Methods In a prospective study CEUS was performed in 129 patients with cervical lymph node enlargement.The entire process were recorded and preserved in DICOM format.The results were registered with Sonoliver.The receiver operating characteristic curve (ROC curve)analysis was performed to find the corresponding cutoff values. The selected node was removed surgically and submitted for histology.Results Of all the nodes,26 were reactive nodes,85 were metastases and 1 8 were lymphoma.Enhancement pattern was the most accurate way to characterize lymph nodes.The enhancement pattern of reactive lymph nodes was homogeneous and most of them were enhanced by lymphatic type while the metastatic lymph nodes were inhomogeneously enhanced or weakly enhanced by peripheral type.Lymph node lymphoma usually had no fixed enhancement pattern. Arrive time (AT),rise time (RT),time to peak (TTP),mean transit time (mTT),maximum intensity (IMAX),under the curve (AUC),rising slope(Kup),semi descending slope(Kdown)and perfusion index (PI)were significantly different in the three groups(P <0.05).RT,TTP and mTT of reactive lymph nodes were the shortest,which had significant difference compared with those of metastatic lymph nodes and lymphoma (P < 0.05 ).Kup,Kdown,IMAX%,AUC,PI in the reactive lymph nodes were significantly decreased compared with the metastatic group (P <0.05),but there was no significant difference compared with those in lymphoma (P >0.05 ).When TTP≥ 7.74 s,mTT≥26.54 s,metastatic lymph nodes were considered.When RT≥4.62 s,TTP ≥ 7.74 s,mTT ≥ 28.32 s,reactive lymph nodes were not considered. Conclusions Dynamic contrast-enhanced ultrasound image and enhancement pattern can distinguish neck lymph nodes while the optimal cut-off point time of the time-intensity curve parameters can further contribute to the identification of lymph nodes.
3.The value of contrast-enhanced ultrasonography in differential diagnosis of thyroid microcarcinoma of TI-RADS grade 3-4

Objective To provide a reference for differential diagnosis of thyroid microcarcinoma classified as TI-RADS 3-4 grade using contrast-enhanced ultrasound.Methods SonoLiver software was used to analyze the process of contrast-enhanced ultrasound of 112 thyroid small nodules classified as TI-RADS 3-4 grade.The dynamic vascular pattern curve(DVPC)and the time-intensity curve(TIC)were automatically acquired for summarization of the types and characteristics of the two curves.The relevant parameters such as the rise time(RT),time to peak(TTP),maximum intensity(IMAX%),mean transit time(mTT),area under curve(AUC)and perfusion index(PI)were acquired and calculated using the software.Receiver operating curve (ROC) analysis was used to achieve the cut-off value of these parameters for differential diagnosis.Results For thyroid microcarcinoma which TI-RADS assessment of grade 3-4,TIC showed as slow rise and slow drop wave or slow rise and fast drop wave,DVPC showed negative waveform or negative-positive bi-directional waveform.ROC analysis demonstrated that IMAX of 96.5%,AUC of 443.08 and PI of 47.64 were the best cut-off values for the differential diagnosis of benign and malignant nodules.When IMAX<96.5%,AUC<443.08,PI<47.64,the nodules were highly likely to be thyroid microcarcinoma.Conclusions The contrast-enhanced ultrasound parameters can provide effective reference for the differential diagnosis of thyroid carcinoma with grade 3-4 in TI-RADS classification.
4. The value of contrast-enhanced ultrasonography in differential diagnosis of orbit tumors in extraconal compartment

Objective:
To provide a reference for differential diagnosis of orbit tumors in extraconal compartment by analyzing the parameters of contrast-enhanced ultrasonography (CEUS).
Methods:
A total of 76 patients with orbit tumors in extraconal compartment who underwent CEUS examination were retrospectively analyzed. These patients were divided into two groups depending on histological diagnosis including 27 cases with malignant tumor and 49 cases with benign tumor. CEUS images were analyzed using Sonoliver software. The time-intensity curve (TIC) and dynamic vascular pattern curve (DVPC) were acquired and the characteristics of the arterial flow infusion of these were analyzed. The related parameters such as time of arrival time(AT), rise time (RT), time to peak(TTP), mean transit time(mTT), slope of rise(K1), the absolute value for slope of down (K2) were also obtained. The differences of DVPC, TIC and quantitative parameters between the two groups were compared. Receiver operating curve analysis was used to acquire the cut-off values of these parameters for differential diagnosis.
Results:
①TICs of the benign tumors were mainly fast rise and slow drop and DVPCs were mainly positive biphasic, while TICs of the malignant tumors were mainly fast rise and fast drop and DVPCs were mainly biphasic. ②The differences of quantitative parameters IMAX, K1, K2, mTT and RT of TICs were statistically significant between the two groups (
5.Three-dimensional finite element analysis of extrusion of the maxillary canine during orthodontic treatment with invisible aligner without brackets

BACKGROUND: With the advancement of science and technology, invisible aligner without brackets have emerged. However, this new type of digital orthodontic technology is rarely reported on its biomechanics. OBJECTIVE: By the use of rectangular attachments of different sizes and different bonding positions, to obtain different force of canines and the stress supporting of periodontal tissue during the extrusion movement, providing reference for the clinical use of rectangular attachment in the orthodontic treatment of low canine tooth. METHODS: A three-dimensional finite element model of appliance-attachments-canine-periodontium-cancellous bone-cortical bone was established based on the scan data of the isolated canine. The attachments were set in different directions (vertical, horizontal), sizes (3, 4, 5 mm in height) and positions (in the occlusal side of the crown center, in the gingival side of the crown center). The 0.25-mm forced occlusal displacement was loaded to observe the deformation and equivalent stress of the tooth and periodontal membrane. RESULTS AND CONCLUSION: (1) If the size of rectangular attachment increases, the force of the tooth will grow in a certain range and the tooth will have better effect of extrusion with a larger level of tilt motion in other directions. (2) The extrusion effect of the horizontal attachment is better than that of the vertical attachment, which is more difficult to have distal incline, but is prone to lingual incline.
6.Analysis of " Internet plus healthcare" related policies in the eastern, central and western regions of China before and after the COVID-19 pandemic

Objective:To analyze the commonalities and differences of " Internet plus healthcare" related policies in the eastern, central and western regions of China before and after the COVID-19 pandemic and to provide references for further improving China′s " Internet plus healthcare" policy.Methods:Collecting the policies of " Internet plus healthcare" promulgated by 31 provinces(autonomous regions, municipalities)in China from April 2018 to December 2020 on the websites of various government departments, extracting keywords based on content analysis methods, and calculating degree centralities using UCINET 6 software. A comparison of the policies of the eastern, central and western regions between the pre epidemic stage(April 2018 to December 2019)and the COVID-19 pandemic stage(January 2020 to December 2020)was carried out.Results:In the pre epidemic stage, Internet diagnosis and treatment, " Internet plus medical service price" and quality supervision were the focus of attention in all regions. But the eastern region paid more attention to the policy planning of Internet hospital operation and Internet medical application, the central region focused on the transformation and efficiency improvement of medical service process by Internet technology, and the western region paid more attention to encourage the application of telemedicine to solve the difficulty of seeing a doctor. In the pandemic stage, the " Internet plus service price" was the policy focus in all regions, mostly in medical insurance payment, and the policy difference was not obvious among the eastern, central and western regions.Conclusions:Before and after the COVID-19 pandemic, the policy of " Internet plus healthcare " promulgated by the local governments in eastern, central and western regions had different emphasis and different characteristics. Our suggestion was to improve the policy system, standardize industry supervision, strengthen information security, orderly interconnect medical insurance, and actively promote the exchange of policy experiences among the provinces.
7.Research progress of stroke self-management assessment tools and interventions

The concept of stroke self-management, assessment tools, influencing factors and intervention measures and methods based on influencing factors were reviewed in this paper. The purpose of this study was to provide some reference for the formulation of stroke health management intervention program and the development of rigorous stroke health management practice research.
8.Contrast-enhanced ultrasonography characteristics of liver metastasis correlation with different pathological type of lung cancer

Objective:To provide objective basis for the diagnosis on liver metastasis from lung cancer by analysing contrast-enhanced ultrasonography(CEUS) characteristics.Methods:In a reprospective study, CEUS was performed in 78 cases with liver metastasis from lung cancer in Renmin Hospital of Wuhan University from January 2014 to March 2019. Patients were divided into three groups: the adenocarcinoma group( n=30), squamous cell carcinoma group ( n=20), and neuroendocrine neoplasm group( n=28). Conventional ultrasound and CEUS features were analyzed and the distinctions among the three groups were compared. Results:CEUS showed that 85.9%(67/78) of the liver metastasis from lung cancer with pattern of rapid wash-in and 89.7%(70/78) of rapid wash-out, so there was no statistical difference among the three groups( P>0.05). In the adenocarcinoma and squamous cell carcinoma group, 53.3%(16/30) and 55.0%(11/20) of cases were rim-like enhancement. And 71.4%(20/28) of the neuroendocrine neoplasm group with global enhancement that was different from the other groups(all P<0.05). At the peak of enhancement, most cases were hyperenhancement among the three groups, and the difference was not statistically significant( P>0.05). During the lesions of hyperenhancement, 73.7%(14/19) was global enhancement in the neuroendocrine neoplasm, compared to 75.0% (12/16) of the adenocarcinoma group and 72.7%(8/11) of the squamous cancer group with rim-like enhancement, which the differences was statistically significant(all P<0.05). At the peak of enhancement, 67.9%(19/28) lesions of the neuroendocrine neoplasm group were homogeneous enhancement, and only 21.4%(6/28) of the lesions was observed non-enhanced area, which was different from the other groups(all P<0.05). The times to iso-echogenity and hypo-echogenity were (33.2±7.6)s and (45.1±10.8)s respectively in the neuroendocrine neoplasm group, which were more than the other groups(all P<0.05). Capsule enhancement appeared in 32.1%(9/28) of the neuroendocrine neoplasm group in the delay phase, which was higher than 13.3%(4/30) of the adenocarcinoma group and 10.0%(2/20) of squamous cell carcinoma group (all P<0.05). Conclusions:CEUS can provide more diagnostic information for liver metastasis from lung cancer and may be a beneficial technique for differential diagnosis.

Among these parameters ,mT T with 28 .1 s had the highest diagnostic value ( sensitivity :81% ,specificity :77% ) . Conclusions The quantitative CEUS parameters can provide effective reference for differential diagnosis of orbit tumors in extraconal compartment .
10. In vitro study of the effect of human antigen R on lysosomal acidification during autophagy in mouse cardiomyocytes

Objective:
To investigate the effect of human antigen R on lysosomal acidification during autophagy in mouse cardiomyocytes cultured in vitro.
Methods:
The hearts of 20 C57BL/6 mice aged 1-2 days no matter male or female were isolated to culture primary cardiomyocytes which were used in the following experiments. (1) The cells were divided into 5 groups according to the random number table (the same grouping method below), i. e., normal control group and sugar-free serum-free 0.5, 1.0, 3.0, and 6.0 h groups. The cells in normal control group were routinely cultured for 54.0 h with Dulbecco′s modified Eagle medium/nutrient mixture F12 (DMEM/F12) medium (the same regular culture condition below), and the cells in sugar-free serum-free 0.5, 1.0, 3.0, and 6.0 h groups were firstly regularly cultured for 53.5, 53.0, 51.0, 48.0 h and then cultured with replaced sugar-free serum-free medium for 0.5, 1.0, 3.0, and 6.0 h, respectively. The protein expressions of microtubule-associated protein 1 light chain 3 Ⅱ (LC3Ⅱ), autophagy-related protein 5, and adenosine triphosphatase V1 region E1 subunit (ATP6V1E1) were detected by Western blotting. (2) The cells were divided into normal control group and sugar-free serum-free 3.0 h group. The cells in corresponding groups were treated the same as those in experiment (1), and the cell lysosomal acidification level was observed and detected under a laser scanning confocal microscope. (3) Two batches of cells were grouped and treated the same as those in experiment (1). The protein expression of human antigen R in the whole protein of cells of one batch and its protein expression in the cytoplasm and nucleus protein of cells of the other batch were detected by Western blotting. (4) The cells were divided into normal control group, simple control small interfering RNA (siRNA) group, simple human antigen R-siRNA1 (HuR-siRNA1) group, simple HuR-siRNA2 group, sugar-free serum-free 3.0 h group, sugar-free serum-free+ control siRNA group, sugar-free serum-free+ HuR-siRNA1 group, and sugar-free serum-free+ HuR-siRNA2 group. After 48 hours of regular culture, the cells in simple control siRNA group and sugar-free serum-free+ control siRNA group were transfected with negative control siRNA for 6 h, the cells in simple HuR-siRNA1 group and sugar-free serum-free+ HuR-siRNA1 group were transfected with HuR-siRNA1 for 6 h, and the cells in simple HuR-siRNA2 group and sugar-free serum-free+ HuR-siRNA2 group were transfected with HuR-siRNA2 for 6 h. Hereafter, the cells in these 8 groups were continuously cultured for 48 h with regular conditon, and then the cells in normal control group and each simple siRNA-treated group were replaced with DMEM/F12 medium, the cells in the other groups were replaced with sugar-free serum-free medium, and they were cultured for 3 h. The protein expression of human antigen R in the whole protein of cells was detected by Western blotting. (5) Two batches of cells were divided into sugar-free serum-free+ control siRNA group and sugar-free serum-free+ HuR-siRNA1 group, and the cells in corresponding groups were treated the same as those in experiment (4). The distribution and expression of human antigen R in the cells of one batch were observed and detected by immunofluorescence method, and the lysosomal acidification level in the cells of the other batch was observed and detected under a laser scanning confocal microscope. (6) Three batches of cells were divided into sugar-free serum-free 3.0 h group, sugar-free serum-free+ control siRNA group, sugar-free serum-free+ HuR-siRNA1 group, and sugar-free serum-free+ HuR-siRNA2 group, and the cells in corresponding groups were treated the same as those in experiment (4). The protein expressions of cathepsin D in the whole protein of cells of one batch, human antigen R in the cytoplasm protein of cells of one batch, and ATP6V1E1 in the whole protein of cells of the other batch were detected by Western blotting. (7) The cells were divided into normal control group, sugar-free serum-free 3.0 h group, sugar-free serum-free+ control siRNA group, and sugar-free serum-free+ HuR-siRNA1 group, and the cells in corresponding groups were treated the same as those in experiment (4). The mRNA expression of ATP6V1E1 in cells was detected by real-time fluorescent quantitative reverse transcription polymerase chain reaction. The sample number of each experiment was 3.
