1.Effects of Cangfu Daotan Decoction Combined with Clomiphene on Serum Hormone Level and Endometri-al Receptivity in Patients with Polycystic Ovary Syndrome Combined with Infertility

OBJECTIVE:To investigate the effects of Cangfu daotan decoction combined with Clomiphene citrate capsule on sex hormone level and endometrial receptivity in patients with polycystic ovary syndrome (PCOS) combined infertility. METH-ODS:A total of 116 PCOS patients with infertility in the First Affiliated Hospital of Guiyang College of TCM from Jan. 2014 to Dec. 2014 were divided into observation group and control group according to random number table,with 58 cases in each group. Both groups received hormone regulation therapy since the fifth day of menstruation. Control group was given Clomiphene citrate capsule 50 mg,qd;observation group was additionally given one dose of Cangfu daotan decoction,400 mL after decocted with wa-ter,bid,on the basis of control group. Both groups were treated for 3 cycles of menstruation. The levels of serum sex hormone (E2,LH),endometrial thickness(Em),endometrial spiral arterial pulsatility index(PI)and resistance index(RI)were compared between 2 groups before and after treatment. The ovulation rate and pregnancy rate were compared between 2 groups after 1-year follow-up. RESULTS:Before treatment,there was no statistical significance in E2,LH,Em,PI or RI between 2 groups (P>0.05). After treatment,the levels of E2 and LH as well as Em in 2 groups were increased significantly,while PI and RI were de-creased significantly;the observation group was significantly better than the control group,with statistical significance(P<0.06). After 1-year follow-up,there was no statistical significance in ovulation rate between 2 groups (P>0.05). The pregnancy rate of observation group was 41.38%,which was significantly higher than that of control group (22.41%),with statistical significance (P<0.05). CONCLUSIONS:Cangfu daotan decoction combined with Clomiphene citrate capsule can effectively regulate serum sex hormone levels of PCOS patients with infertility,and improve endometrial receptivity so as to increase pregnancy rate.
2.Influence of establishment of a hospital which makes people satisfied on nursing management

Objective We aimed to discuss the influence of establishment of a hospital which makes people satisfied on nursing management. Methods We improved our nursing work by establishing quality-control groups, training staff, checking once a week and making feedback once a month through decomposing and subdividing the standard of building a hospital which makes people satisfied from March 2002 to December 2006. Results The qualified rate of nursing management increased from 69.3% to 97.6% every year, which has statistical difference (P<0.01). Conclusions Establishment of a hospital which makes people satisfied improved the general level of nursing management quality and nursing staff. We acquired good social and economic befenit.
3.Effect of two kinds of hemofiltration on vascular endothelial system

Objective To evaluate the effect of single CVVH and HDF on endothelial system. Methods Group CVVH:20 patients with multiple organ dysfunction syndrome ( MODS) underwent CVVH for 12h.Vascular access was through two-lumen catheter in femoral vein;Group HDF:Twenty patients with end stage renal disease (ESRD) on maintenance hemodialysis underwent HDF for 4h.Vascular access was through internal arteriovenous fistula.Group control:Healthy people were examined at our hospital.The se-rum levels of sVCAM-1, sICAM-1, vWF and MMP-9 were measured by enzyme-linked immunosorbent assay ( ELISA) after centrifugation in treatment group before and after treatment.Four cytokines were also meas-ured in fasting healthy subjects in the morning.Results The levels of serum sVCAM-1, sICAM-1, vWF, MMP-9 were significantly higher in Group CVVH and HDF than control group before and after treatment( P<0.05).Group CVVH:The serum levels of sICAM-1 and sVCAM-1 were significantly lower in post-treat-ment than pre-treatment, But the serum levels of vWF and MMP-9 showed no obvious changes after CVVH;Group HDF:the levels of four cytokines showed no obvious changes after single HDF.Conclusions Endo-thelium becomes impaired in MODS and ESRD patients, Endothelium may be protected by CVVH through the partial removal of cytokines harmful to endothelium in MODS patients.Single HDF therapy does not pro-tect endothelial function obviously in MHD patients.

Objective To investigate the relationship between nodular goiter and thyroid cancer and the causes of postoperative recurrence in patients with nodular goiter(NG) undergoing operative treatment. Methods The clinical date of 199 cases of NG were retrospectively analysed. Results Pathological examination revealed that 7 cases were accompanied by thyroid cancer including 2 micro-cancer, all the 7 cases were papillary carcinomas. Objective To investigate the effect of C peptide on tubulointerstitial oxidative stress injury in rats with diabetic nephropathy (DN) induced by streptozotocin (STZ). Methods Wistar rats with DN induced by STZ were randomly divided into control group and C peptide group. All rats were treated via intraperitoneal micro-osmotic pump. Normal Wistar rats at the same age were used as the normal group. Blood glucose and24 h urinary albumin were measured before treatment and every 4 weeks during treatment period. After 12 weeks of treatment, the expression of transforming growth factor - β1 (TGF - β1) in renal tubulointerstitium was detected by immunohistochemical staining. Fresh tubulointerstitial tissue homogenate was harvested and the activity of superoxide dismutase (SOD) was detected in WST-1 method, the content of malondialdehyde (MDA) was detected in TBA method, and the expression of protein kinase A (PKA) mRNA was detected by Real-time PCR. Results Levels of blood glucose, 24 h urinary albumin, MDA content and the expression of TGF-β1 were higher in the control group than those in the normal group, while SOD activity decreased and PKA mRNA was downregulated. C peptide treatment did not change blood glucose level but slowed down the increasing of 24 h urinary albumin, reduced the tubulointerstitial TGF-β1 expression and MDA content, increased the SOD activity and upregulated the PKA mRNA expression. Conclusions C-peptide can decrease the tubulointerstitial oxidative stress in DN rats by activating PKA pathway and then improve tubulointerstitial fibrosis and attenuate proteinuria.
6.Clinical Observation of Danshen Chuanxiongqin Injection Combined with Flunarizine Hydrochloride in the Prevention of Benign Paroxysmal Positional Vertigo and Lower Extremity Deep Venous Thrombosis in Post-operative Long-term Bedridden Patients with Lower Limb Fractures

OBJECTIVE:To observe therapeutic efficacy and safety of Danshen chuanxiongqin injection combined with flunari-zine hydrochloride in the benign prevention and treatment of paroxysmal positional vertigo (BPPV) and lower extremity deep ve-nous thrombosis (DVT) in post-operative long-term bedridden patients with lower limb fractures. METHODS:300 post-operative long-term bedridden patients with lower limb fractures were selected and randomly divided into observation group and control group,with 150 cases in each group. Control group was given Flunarizine hydrochloride capsules orally 10 mg,qd;observation group was additionally given Danshen chuanxiongqin injection 10 ml+5% Glucose injection 250 ml,ivgtt,qd. The incidence of BPPV and DVT were observed in 2 groups after intervention,and the circumference of lower limb,blood coagulation indexes, blood rheology indexes and inflammatory factor were observed before and after intervention,and the incidence of ADR was com-pared. RESULTS:The incidence of BPPV and DVT in observation group were 18.0% and 16.7%,which were significantly lower than in control group(48.7% and 52.7%),with statistical significance(P<0.05);after intervention,the circumference of lower limb,blood rheology indexes and the levels of inflammatory factors in 2 groups were decreased significantly, while the coagula-tion indicators were significantly improved;the observation group was better than the control group,with statistical significance (P<0.05). There was no statistical significance in the incidence of ADR between 2 groups(P>0.05). CONCLUSIONS:Danshen chuanxiongqin injection combined with flunarizine hydrochloride is effective in the prevention of BPPV and DVT in long-term bed-ridden patients with lower limb fractures,with low incidence of ADR.
7.The study on the lowered tumorigenicity of hepatocellular carcinoma cells cotransfected with chemokine mMIP-1α and costimulatory molecule m4-1BB L in vivo

Objective To investigate the effects of macrophage inflammatory protein-1α (MIP-1α) combined with molecule 4-1BB L on the tumorigenicity of hepatocellular carcinoma cells in vivo. Methods Mouse MIP-1α (mMIP-1α) expressed Hepa 1-6 cells were transfected with m4-1BBL recombinant retrovirus, the anti-histidinol cells clones were selected and amplified. The expression of m4-1BB L was confirmed by flow cytometry. The growth curve of Hepa 1-6 cells transfected with mMIP-1α and m4-1BBL alone or together was drawn and compared. C57B/L Mice were randomly divided into 7 groups, 9 mice in each group, injected with mMIP-1α+m4-1BB L Hepa 1-6 cells, m4-1BB L Hepa 1-6 cells, mMIP-1α Hepa 1-6 cells, Hepa 1-6 cells, pLXSHD Hepa 1-6 cells or PBS respectively. The tumorigenicity of hepatocellular carcinoma cells and the mice survival rate were compared between each groups. Results Hepa 1-6 mMIP-1α+m4-1BB L cells which expressed both mMIP-1α and m4-1BB L were successfully established. The expression of mMIP-1α and m4-1BB L alone or together did not affect the growth curve of Hepa 1-6 cells. Observed for 5 weeks, no tumor developed in Hepa 1-6 mMIP-1α+m4-1BB L injected mice. The tumorigenicity of Hepa 1-6 mMIP-1α+m4-1BB L was lower than that of Hepa 1-6 mMIP-1α or Hepa 1-6 m4-1BB L in vivo. The survival rate of Hepa 1-6 mMIP-1α+m4-1BBL injected mice(9/9) was higher than that of Hepa 1-6 m4-1BB L injected mice (6/9)or Hepa 1-6 mMIP-1α injected mice (1/9). Conclusion Chemokine MIP-1α combined with costimulatory 4-1BB L lowered the tumorigenicity of hepatocellular carcinoma cells in vivo, and prolonged the mice survival period.

Objective To introduce a community-based rehabilitation(CBR) network model in Shijiazhuang,Hebei province.Methods The work model in Qiaodong district in Shijiazhuang city was evaluated according to the national CBR standard and analyzed.Results Their work has met the national standard,the score of management section was the highest among all areas.Conclusion The work was featured by government dominate,strong management network and technical support.It implied the social model of CBR.

Objective: To investigate the effects of andrographolide on extracellular signal-regulated kinase 1/2 (ERK1/2) signaling pathway and tumor necrosis factor-α (TNF-α) expression in lipopolysaccharide (LPS)-activated macrophages. Methods: LPS-activated mouse peritoneal macrophages were cultured in media with different concentrations of andrographolide. Cytotoxicity of andrographolide was detected by cell counting kit-8. The macrophages were lysed, and then expressions of phosphorylated ERK1/2, JNK and p38 and nuclear factor-κB inhibitor (IκBα) protein were detected by Western blotting and TNF-α mRNA expression was detected by reverse transcription-polymerase chain reaction. Supernatants of the macrophages were used to detect content of TNF-α protein by enzyme-linked immunosorbent assay. Results: Andrographolide at 1-100 μg/mL showed no cytotoxicity on LPS-activated mouse peritoneal macrophages. Andrographolide inhibited ERK1/2 phosphorylation in LPS-activated murine peritoneal macrophages, which was concentration-dependent (P<0.01). Andrographolide at 1-25 μg/mL had no effects on phosphorylation levels of JNK and p38 and IκBα degradation in LPS-stimulated mouse peritoneal macrophages. In activated macrophages, TNF-α expression was inhibited by 12 μg/mL andrographolide and 20 μmol/L PD98059 (inhibitor of ERK1/2 signaling pathway) at both mRNA expression and protein secretion levels. Conclusion: In LPS-activated macrophages, andrographolide may inhibit the expression of TNF-α by inhibiting ERK1/2 signaling pathway.
10.Identification and pedigree analysis for an A(W)37B subtype due to c.940A>G variant of ABO gene.

OBJECTIVE:
To delineate the serological and molecular profiles of a patient with A(w)37B subtype.
METHODS:
The ABO bloodtypes of the proband, his wife and daughter were determined with a standard serological method. Their ABO genotypes were determined by sequence-specific primer polymerase chain reaction (PCR-SSP). All exons of the ABO gene were directly sequenced. Exons 6 and 7 of the ABO gene were further analyzed by cloning and sequencing.
RESULTS:
The red blood cells of the proband showed a weak B phenotype. His serum sample contained weak reactive anti-A antibody, which was defined as A(w)B blood group based on the serological characteristics. The A and B alleles were detected by blood group genotyping. Gene cloning and sequencing have identified a characteristic c.940A>G variant (ABO*AW.37) in exon 7 of the ABO gene, which resulted in substitution of Lysine by Glutamate at position 314. The proband's daughter has inherited the ABO*AW.37 allele.
CONCLUSION
The c.940A>G variant in exon 7 of the ABO gene probably underlay the decreased activity of GTA transferase and resulted in the Aw37 phenotype.
