1.Effects of Vitamin B6 on the Efficacy and Related Indexes of Patients with Peripartum Cardiomopathy

OBJECTIVE:To observe the effects of Vitamin B6 on efficacy and related indexes of patients with peripartum car-diomyopathy(PPCM). METHODS:64 PPCM patients were randomly divided into control group(32 cases)and observation group (32 cases). Control group received diuretic drugs,angiotensin converting enzyme inhibitors or angiotensin Ⅱ receptor antagonist,β-blockers,digitalis,low molecular weight heparin and other conventional treatment. Observation group additionally received Vita-min B6 tablet 50 mg,orally,twice a day. They treated for 2 weeks. Clinical efficacy,serum prolactin(PRL),brain natriuretic pep-tide (BNP),left ventricular end-diastolic diameter (LVEDD) before and after treatment,milk back rate and the incidence of ad-verse reactions in 2 groups were observed. RESULTS:There was no significant difference in the total effective rate in 2 groups (P>0.05). Before treatment,there were no significant differences in PRL,BNP and LVEDD in 2 groups (P>0.05). After treat-ment,PRL,BNP and LVEDD in 2 groups were significantly lower than before,and observation group were lower than control group,with statistical significances (P<0.05). And the milk back rate in observation group was significantly higher than control group,with statistical significance(P<0.05). There were no obvious severe adverse reactions during treatment. CONCLUSIONS:Based on conventional treatment,Vitamin B6 shows similar efficacy and safety with conventional treatment in the treatment of PPCM,though it is superior to conventional treatment in terms of decreasing PRL level,it has high milk back rate.
2.Clinical efficacy of transurethral drugs perfusion combined with ultrashort wave for treatment of chronic bacterial prostatitis

Objeetive To explore a new therapeutic way of transurethral drugs perfusion combined with ultrashort wave on treating chronic bacteria prostatitis.Methods 241 cases with chronic bacteria prostatltis were randomly divided into two groups.the treatment group were treated with transurethral perfusion combined with ultrashort wave.the control group were treated with transurcthral drugs perfusion.Results Total effective rate in the treatment group was 85.9%.that of the control group was 36.7%.It had a significant difference between two groups(P<0.01).Conclusion Transurethral drugs perfusion combined with ultrashort wave could promote the curative effect of antibiotic maximumly,so it is an effective method to treat chronic bacterial prostatitis.
3.Posterior approach microendoscopic surgery in the treatment of lumbar spinal stenosis

Objective To study the clinical application of posterior approach microendoscopy in the treatment of lumbar spinal stenosis. Methods One hundred and forty-two cases of degenerate lumbar spinal stenosis were treated by posterior approach limited decompression, total or semi laminectomy and decompression, or fenestration decompression from February 2000 to December 2001. Results Out of the 142 cases, posterior approach microendoscopic limited decompression was completed in 87 cases with full decompression obtained.Follow-up for a mean of 18 months in 82 cases revealed a rate of excellent-and-good of 92.7% (58 cases, excellent; 18 cases, good).No complications occurred. Conclusions Degenerate lumbar spinal stenosis, resulted from simple lumbar intervertebral disc herniation, hypertrophy of ligamentum flavumcan, and facet hypertrophy, can be effectively treated by posterior approach microendoscopic limited decompression. Proper selection of patients, full decompression and experienced operative skills are the key to satisfactory surgical results.
4.Clinical investigation of contrala teral C_7root transfer to the radial nerve for total brachial plexus root avulsion injury

Objective To observe the effect of contralateral C 7 root transfer to the radial nerve for the treatment of total brachial plexus root avulsion injury.Methods One hundred and seven patients with total brachial plexus root avulsion injury were treated by contralatera l C 7 root transfer to the radial nerve fro m1993to 1999.Among them 36patients were followed-up for more than 2years;there were 31male and 5female patients,with age ranged fro m 7to 44years,and 27years in average.The time of initial surgery from the time of injury was between 3to 36months,among which 4patients were tr eated between 3to 6months,29between 6to 12months,2between 12to24months and 1between 24to 36months;9.4months on the average.The whole surgical procedure was divided into three phases.Phase I:The exploration of the brachial plexus both above and below the clavicle;the phrenic nerve was transferr ed to the anterior branch of the su-perior trunk or was anastomosed with the musculocutaneous nerve;the accessory nerve was transferred to the supraclavicular nerve;the median nerve was previously placed subcutaneously at the anterior re gion of the arm-pit.PhaseⅡ:The contralateral C 7 nerve root was transferred to the uln ar nerve which was used for the bridging.PhaseⅢ:The intercostal nerve was transferred to the median nerve and the thorac odorsal nerve,the ulnar nerve and the radial nerve was a nastomosed.Patients were followed-up for 24months to 84months,38months on average.The follow-up sta ndards included:the recovery of the motor function of the muscles in-nervated by the radial nerve;the rec overy of the regeneration potential of the muscles;the change in the sensory function in the cutaneous re gion innervated by the radial nerve.Results Twenty-nine patients(80.6%of all)was improved in motor function,and 27patients(75.0%)in sensory function.Conclusion The overall effect of contralateral C 7 root transfer to the radial nerve for total brachial plexus root avulsion injury was satisfactory,while the functio nal recovery of triceps was not as exp ected.[
5.Determination of Cinobufagin and Resibufogenin in Qianglijiuxin Dripping Pills by HPLC

Objective: To determine the contents of cinobufagin and resibufogenin in Qianglijiuxin Dripping Pills. Methods: The HPLC method was used at Kromasil C 18 column of 250mm?4.6mm, 5?m, 0.5% Dipotassium Hydrogen Phosphate Acetonitrile (50∶50) as the mobile phase, 1.0mL/min as the flow rate at detection wavelength of 296nm. Results: For cinobufagin, the linear range was 0.67?g~4.7?g and the average recovery rate was 97.78% with RSD=1.2%. For resibufogenin, the linear range was 0.33?g~2.3?g and the average recovery was rate 99.61% with RSD=2.0%.Conclusion: The method can be applied to content determination of cinobafagin and Resibufogenin in Qianglijiuxin Dripping pills.
6.A Fluorescence Biosensor for Detection of Mercury Ion Based on Oligonucleotide

A simple,fast and highly sensitive fluorescence analysis method for detection of mercury ion was developed based on N-methyl-mesoporphyrin IX (NMM)/G-quadruplex DNA system and specific T-Hg-T mismatches.In this strategy,a large number of thymine was introduced into guanine-rich oigonucleotides which could form G-quadruplex.In the presence of Hg2+,guanine-rich oigonucleotides and complementary strand could form double-stranded DNA molecule by specific T-Hg-T mismatch pair,leading to destruction of G-quadruplex DNA structure.In the absence of Hg2+,guanine-rich oigonucleotides spontaneously formed G-quadruplex DNA structure that could bound NMM to generate intense fluorescence.Based on the above facts,a sensitive fluorescence biosensor for determination of Hg2+ was fabricated.And the optimal conditions for Hg2+ determination were as follows:buffer solution pH of 6.7,20 mmol/L KCl and 2.5 μmol/L NMM in buffer and incubation for 2 h.Under the optimal conditions,the fluorescence intensity signal change (F0-F) and the Hg2+ concentration exhibited a linear correlation within 50 nmol/L to 1000 nmol/L range with a low detection limit of 22.8 nmol/L (3σ).The biosensor exhibited good selectivity toward common metal ions.The developed method was successfully employed to detect Hg2+ in tap water with recovery of 106.1%-107.8%.
7.Drug Resistance Analysis of Pseudomonas aeruginosa in Intensive Care Unit and Molecular Mechanism of Its Resistance to Fluoroquinolones

OBJECTIVE To study the Pseudomonas aeruginosas drug resistance in intensive care unit and its fluoroquinolone-resistant molecular mechanism,and provide scientific basis for rational employment of antibiotics in clinic. METHODS E test was used to determine the minimal inhibitory concentrations(MIC) of 13 antibiotics against 83 P. aeruginosa strains. Twenty-eight fluoroquinolone-resistant strains were selected with standard sensitive strain-ATCC27853 as control. The quinolone resistance-determining region(QRDR)of the gyrA and parC genes was amplified by PCR and sequenced. RESULTS The positive rate of P. aeruginosa in sputum specimen was the highest from 83 strains (71.08%). gyrA Genes of all resistant strains had an ACC to ATC mutation in codon 83,leading to the amino acid substitution of threonine for a an isoleucine and 11 high level resistant strains also showed a GAC to GGC mutation in codon 87,leading to the substitution of an aspartic acid a glycine for. In addition,14 resistant strains also had an TCG to TTG mutation in codon 87 of parC gene,leading to the amino acid substitution of a serine for a leucine. We didn't find parC gene mutation existing independent in fluoroquinolone-resistant P. aeruginosa. CONCLUSIONS Meropenem remains highly active against P. aeruginosa. But the abuse of imipenem and other fluoroquinolones leads to rise in their drug-resistance rate. Fluoroquinolone-resistance has increased rapidly,the mechanism of resistance is gene mutant. It displayed that gyrA and parC gene mutation is associated with fluoroquinolone resistance.
8.Clinical Study on Changqin No. 1 Combined with Western Therapy in Treating Severe Traumatic Brain Injury

Objective To investigate the clinical efficacy of Changqin No. 1 combined with Western therapy for severe traumatic brain injury and its effects on prognosis. Methods Totally 65 cases of severe traumatic brain injury were randomly divided into the treatment group (n=32) and the control group (n=33). The control group was treated with basic Western therapy (dehydration reduction of intracranial pressure, nutrition nerve, scavenging oxygen free radicals, improve cerebral circulation, nutritional support and maintain the internal environment stability, anti-infection, prevention and treatment of complications, hyperbaric oxygen). The treatment group was treated with Changqin No. 1 plus the basis of routine treatment, 1 dosage per day, 2 times stomach tube nasal feeding or blunt, for 14 d. One month after treatment, awake rate, awake time, the incidence of complications and MMSE in both groups were compared. Three months after treatment, the prognosis of two groups were compared by GOS. Results One month after treatment, the awake rate in treatment group was 77.4% (24/31) and 53.1% (17/32) in the control group, with statistical significance (χ2=4.089, P=0.043), and the death rate was 0. The awake time in the treatment group was significantly shorter than that of the control group (t=2.458, P=0.017). The incidence of pulmonary infection in the treatment group was lower than the control group (P=0.001). There was no statistical significance in urinary tract infection, epilepsy, liver and kidney dysfunction rate of the two groups (P>0.05). The number of awake case was 24 in the treatment group and 17 in the control group. The number of normal MMSE cognitive function was 3 in the treatment group and 2 in the control group, and the treatment group was better than the control group (Z=-2.205, P=0.027). Three months after treatment, the good prognosis was 58.08% (18/31) in the treatment group and 28.12%(9/32) in the control group, with statistical significance (χ2=5.763, P=0.016). Conclusion Changqin No. 1 combined with Western basic treatment can help patients with severe traumatic head injury awake early, reduce pulmonary infection complications, and improve the cognitive function of sober patients and improve the prognosis.
9.CT Value in the Diagnosis of Non-intestinal Lesions on the Right Lower Abdomen (A Report of 70 Cases)

Objective To evaluate the value of CT scan in diagnosing non-intestinal lesions on the right lower abdomen.Methods CT scan was performed in 70 patients with clinically supected non-intestinal lesions in the right lower abdomen.The diagnosis was confirmed by surgery or biospy in all cases,except 1 case of nephroptosis.Results 41 cases were benign lesions,which appeared as cystic or solid masses with definite bound on plain CT scans and were slight or middle enhanced on contrast enhanced CT scans,but abscess and tuberculosis were showed peripheral or multilocular enhancement with indistinct bound.The remaining 29 cases were malignant masses which presented as large irregular soft tissue mass and inhomogenous enhancement.Conclusion CT scan of certain is value in diagnosis and differential diagnosis of non-intestinal lesions of right lower abdomen.
10.Analysis of influencing factors leading to major birth defects in six counties of Shanxi province

To explore the influencing factors of major birth defects.The children of 0 -3 years and their mother from six different districts were surveyed from November 2009 to December 2009.The incidence rate was 134.27/10 000 (493/36 716)in 6 counties. And the first five birth defects were as follows:congenital heart disease,neural tube defects,cheilopalatognathus,polydactylia and hypophrenia.The risks and protective factors of the first 5 birth defects were different.Anamnesis of birth defects and defective children from relatives,multiple pregnancy & birth and intake of antipyretic analgesics during pregnancy were the common risk factor for the first 5 birth defects.The key points for the prevention of birth defects are to reinforce the pre-pregnancy guidance,the screening and evaluation of risk factors and disease prevention during pregnancy.
