Five-zang is the nucleus of viscera state doctrine. The nonequilibrium of Five-zang's different properties guides syndrome differentiation and treatment in TCM. This article discusses five-zang's nonequilibrium from lung being a delicate viscus,correlation between lung and five-zang,physiology and pathology of lung and so on. It attaches the importance to five-zang's nonequilibrium and clinical level of syndrome differentiation.

A simple,fast and highly sensitive fluorescence analysis method for detection of mercury ion was developed based on N-methyl-mesoporphyrin IX (NMM)/G-quadruplex DNA system and specific T-Hg-T mismatches.In this strategy,a large number of thymine was introduced into guanine-rich oigonucleotides which could form G-quadruplex.In the presence of Hg2+,guanine-rich oigonucleotides and complementary strand could form double-stranded DNA molecule by specific T-Hg-T mismatch pair,leading to destruction of G-quadruplex DNA structure.In the absence of Hg2+,guanine-rich oigonucleotides spontaneously formed G-quadruplex DNA structure that could bound NMM to generate intense fluorescence.Based on the above facts,a sensitive fluorescence biosensor for determination of Hg2+ was fabricated.And the optimal conditions for Hg2+ determination were as follows:buffer solution pH of 6.7,20 mmol/L KCl and 2.5 μmol/L NMM in buffer and incubation for 2 h.Under the optimal conditions,the fluorescence intensity signal change (F0-F) and the Hg2+ concentration exhibited a linear correlation within 50 nmol/L to 1000 nmol/L range with a low detection limit of 22.8 nmol/L (3σ).The biosensor exhibited good selectivity toward common metal ions.The developed method was successfully employed to detect Hg2+ in tap water with recovery of 106.1％-107.8％.

Objective To investigate the correlation between the relapse of condyloma acuminatum(CA)and the potential capability of tumor necrotic factor (TNF) production of the host′s peripheral blood leukocytes. Methods Forty-two CA patients and 58 normal controls were enrolled in this study. CA relapse was diagnosed clinically. EB virus-transformed B lymphoblastoid cell line(LCL)were used as TNF producing cells. The TNF producing capability of LCL was measured by bioassay using L929 (a TNF sensitive tumor cell line) as target cells. The LCL were stimulated with LPS to produce TNF. Results The average level of TNF production of LCL from all CA patients (including recurrent and non-recurrent CA patients) was similar to that of normal controls (30.14% ? 12.27 vs 34.06% ? 12.06,P = 0.1136). However, the level of TNF production of LCs from recurrent CA patients was significantly less than that from non-recurrent CA patients (24.75% ? 7.51 vs 36.62% ? 10.96,P = 0.00016). Compared with that of normal controls, recurrent CA patients showed a lower capability to produce TNF (24.75% ? 7.51 vs 34.06% ? 12.06,P = 0.00054), whereas non-recurrent CA patients showed a similar capability to normal controls (36.62% ? 10.96 vs 34.06% ? 12.06,P = 0.3517). Conclusions These results indicate that the cellular immune mechanism might play an important role in the clearance of the residual HPV from the host, in which TNF is involved.

OBJECTIVE To investigate the relationship between phenotypes and genotypes of clinical isolates of ESBLs-producing Klebsiella pneumoniae.METHODS Agar dilution method was used to test the MICs of 11 antibiotics against 67 ESBLs-producing K.pneumoniae strains.PCR was performed for amplifying ?-lactamase-encoding genes of SHV-,TEM-,and CTX-M-type,and the PCR products of some strains were cloned and sequenced to identify their gene serotypes.RESULTS With no imipenem-resistant strains among 67 strains,their resistant rates to 10 kinds of antibiotics were 10.45-89.55% The cross-resistant rates to aminoglycosides of 60 strains and to ?-lactams of 44 strains were 88.33% and 40.91%,respectively.The positive rates of SHV-,TEM-,and CTX-M-type for 67 strains were 91.04%,56.72% and 28.36%,respectively,and SHV-12,TEM-1 and CTX-M-3 genotypes were found in 7 strains by cloning and sequencing.CONCLUSIONS Sixty seven strains of ESBLs-producing K.pneumoniae present a clear feature of multi-resistance and cross-resistance to most of antibiotics except imipenem,among them there are 7 strains producing SHV-12 and CTX-M-3 extended-spectrum ?-lactamase coexistent with TEM-1 broad-spectrum ?-lactamase.

OBJECTIVE To investigate the epidemiological features of pregnant women with syphilis, their pregnant outcomes, and perinatal and neonatal prognosis. METHODS Fifty-eight pregnant women with syphilis were retrospectively studied from Jan 2003 to Dec 2007. RESULTS There were 3 cases in 2003, 8 in 2004, 11 in 2005, 15 in 2006 and 21 in 2007. There were 19 dominant syphilis cases and 39 latent syphilis cases among 58 pregnant women with syphilis. OBJECTIVE To study the distribution and drug resistance of the cancer patient′s pathogenic bacteria.METHODS The clinical nosocomial infection of 1451 cancer patients was analyzed by using the soft WHONET-5.RESULTS Of 955 strains isolated from sputa,the G-bacilli were 31.2%,and their main bacteria were Pseudomonas aeruginosa and Klebsiell pneumoniae,the G+ coccis were 31.2%,and their main bacteria were Staphylococcus epidermidis,and the fungi were 43.3%,and their main molds were Candida.albicans.Of 284 strains isolated form stool,the G-bacilli were 64.4%,and their main bacteria were Eschericha coli,the G+ cocci were 10.2% and their main bacteria were S.epidermidis,and fungi were 25.5%.Of 72 strains isolated from blood,the G-bacilli were 62.5%,and their main bacteria were E.coli,the G+ cocci were 30.5% and the fungi were 7.0%.Of 140 strains isolated from pharyngeal swab,the G-bacilli were 15.0%,and the G+ cocci were 43.0% and the fungi were 42.0%.The results of sensitivity tests showed as followed: The G-bacilli had a highly sensitive to imipenem,and had a high drug resistance against the first and second generation cephalosporin,ampicillin,piperacillin.The G+ cocci were highly sensitive to vancomycin and had a high drug resistance against oxacillin,penicillin,and erythromycio,the fungi had an obvious drug resistance against azoles.CONCLUSIONS It is high prevalence of ESBLs among MRS,and Staphylococcus.The application and selection of antibiotics must be based on the results of sensitivity tests,and the drug resistance of pathogenic bacteria must be controlled.

To observe the expression of CD40/CD40L on peripheral blood mononuclear cells (PBMC) in patients with condyloma acuminatum (CA), flow cytometry was employed to examine the expression of CD40 and CD40L on PMBC in 36 patients with CA and 20 healthy controls. Our results showed that mean level of CD40 expression in CA patients was significantly lower than that in the controls (6.58% +/- 2.74% vs 14.81% +/- 6.12%, t = 5.703, P < 0.05); the average level of CD40L in CA patients was also significantly lower than that in the controls (0.73% +/- 0.54% vs 2.67% +/- 2.43%, t = 3.532, P < 0.05). Our results suggest that the reduced costimulatory interaction of CD40 and CD40L in CA patients may be one of the important factors responsible for the low cellular immunity.

Objective:To evaluate the clinical value and the prognostic factors of postoperative radiotherapy in type B3 thymoma pa-tients. Methods:A total of 159 patients with thymoma were treated by surgery and postoperative radiotherapy. According to Masaoka staging system, 12, 33, 62, and 52 patients had stageⅠ,Ⅱ,Ⅲ, andⅣlesions, respectively. Myasthenia gravis existed in 38 patients. Altogether 58 patients underwent chemotherapy. Overall survival, disease-free survival, and local control rates were calculated by Ka-plan-Meier method. Prognostic factors were analyzed by Cox regression model. Results:With a median follow-up of 52 months (8-125 months), the overall 5-year survival rate was 81.6%. The 5-year progression-free survival rate was 76.2%. The 5-year local control rate was 82.6%. The recurrence rate was 32.6%, and the metastatic rate was 9.3%. In the univariate analysis, tumor size, Masaoka stage, re-section margin, radiotherapy, and chemotherapy were significantly associated with 5-year overall survival and progression-free surviv-al (P<0.05). In the multivariate analysis, Masaoka stage, resection margin, and radiotherapy were independent prognostic factors of 5-year progression-free survival (P<0.05). Radiotherapy could improve the regional control rate and the overall survival of patients in Ma-saoka stagesⅢ-Ⅳ. Conclusion:The major failure mode for type B3 thymoma is the recurrence of pleure. Radiotherapy can improve the regional control rate and the overall survival of patients in advanced stages. Masaoka staging, surgical margin, and radiotherapy are the independent prognostic factors for type B3 thymoma treated by postoperative radiotherapy.

Objective To investigate the geometric feature of femoral medullary cavity through CT scanning and it's matching with femoral intramedullary nail,and to analysis the reason for difficulties in implanting distal lock pin.Methods Thirty dried femur specimens were measured through CT scanning,and twenty images were taken evenly according to the total length of shaft of femur from each femur.In each image,the data of the central axis point and eight points around medullary cavity wall in X and Y axes were obtained,which were used to reconstruct the three-dimensional models of medullary cavity,central axis,intramedullary nail by computer software.And the models were overlapped for comparison.Then the coronal and sagittal fold line charts for central axis of medullary cavity were drawn.The inserting process of intramedullary nail was simulated to observe whether the nail would punch out of the medullary cavity wall.Results The central axis of femoral medullary cavity and intramedullary nail were overlapped and compared.They matched well in the coronal plane,but the curves of femoral medullary cavity were larger than those of intramedullary nails in sagittal plane.While simulating the inserting process of intramedullary nail,6 nails punched out of the medullary cavity wall in coronal plane (20％,6/30),so did 13 nails in sagittal plane (43％,13/30).Conclusion Intramedullary nails match well with most of the femoral medullary cavities.However,the anatomic structure of the femoral medullary cavity differs individually.Curves of some femoral medullary cavity are large,which can cause deformation of intramedullary nail,and this is the main reason for the failure of distal locking.
