1.Analysis of autopsy pathology and medical tangle concerning of causes of mortality in 58 infants

Objective To investigate the causes of infant mortality and analyze the reasons of misdiagnosis and medical disputes,and to provide help for improving clinical diagnostic accuracy in future in order to decrease the infant mortality and prevent the occurrence of medical tangle.Methods To collect and reorganize the autopsy pathology reports of medical tangle of infant deaths in the autopsy files of the Pathology Department of Affiliated Hospital of Zunyi Medical College within 5 years from Jan.2008 to Dec.2012,and a total of 58 cases were collected,of whom 39 cases were male and 19 cases were female,and the ages of these cases ranged from 0 to 3 years old.Results In 5 years,the number of medical tangle of infant deaths were 8 cases,10 cases,10 cases,15 cases and 15 cases,respectively,which showed an increasing trend on the whole.Infant mortality showed a significant decrease with the increase of age,as the highest mortality was the baby,which accounted for 77.59% (45/58 cases) of infant deaths.The most common cause of infant death was pneumonia,which accounted for 15.52% (9/58 cases) of all diseases,and neonatal asphyxia was the top cause of newborn infant deaths (27.27%,6/22 cases).In the cases of misdiagnosis and missed diagnosis of infant deaths,the respiratory system disease was the most common disease (57.89%,22/38 cases).Conclusions Infant mortality decreased year by year in clinical medicine,but there was an upward trend of medical disputes in infant deaths.Various types of pneumonia was the most common cause of infant deaths,and it was also the main cause contributing to medical tangles of infant deaths.
2.Effects of compatibility circumstances on guizhi's effect in dispelling cold and relieving exterior

Objective:To study the effects of compatibility circumstances on guizhi's effect in dispelling cold and relieving exterior in complex prescriptions. Methods:202 formulae that effected on Sanhanjiebiao were collected from 1425 formulae with guizhi,and the frequency of herbs combining with guizhi in 202 formulae was ranked from high to low. Results:According to the compatibility frequency from high to low,herbs are:Radix Ledebouriellae(60.89%) ,Herba Ephedrae(36.63%) ,Notopterygium Incisum Ting ex H.T.Chang(27.23%) ,Radix Angelicae Dahuricae(14.85%) ,Rhizoma Zingiberis Recens(11.39%) ,Herba Schizonepetae(5.94%) ,Radix Ledebouriellae and Herba Schizonepetae(4.46%) successively. Conclusion:It showed that guizhi should combine with corresponding herbs to form compatibility circumstances to exert the effect of Sanhanjiebiao in formula,and the function exertion will be effective.
3.Androgen receptor expression in molecular subtypes of breast cancer and its relationship with the clinicopathological features

Purpose To investigate the expression of androgen receptor ( AR) in molecular subtypes of breast cancer and its relation-ship with clinicopathological features. Methods The expression of estrogen receptor (ER), progesterone receptor (PR), HER-2, CK5/6, CK14 in breast cancer was detected by EnVision immunohistochemical staining, and they further be divided into 84 Luminal A subtypes, 27 Luminal B subtypes, 14 HER-2 overexpression subtypes, 11 Basal-like subtypes, and 17 Normal-like subtypes, respec-tively. The distribution of AR in 5 molecular subtypes and its relationship with clinicopathological factors, such as age, histopathologi-cal grading, tumor size, lymph node status and clinical staging, were analyzed. Results In 153 cases of breast cancer, the positive rates of AR expression were 73. 20%, Luminal A was the majority ( 62. 50%) in 5 molecular subtypes. In the HER-2 negative and positive breast cancer, the positive rates of AR expression were 75. 00% and 68. 29% and the difference between the two groups was no statistical significant (P=0. 196). Of the 153 breast cancers, 23 (15. 03%) were triple-negative breast cancers (ER-, PR- and HER-2-), 15 (9. 80%) breast cancers were negative for ER, PR and AR, 8(5. 23%) breast cancers were quadruple negative (ER-, PR-, AR- and HER-2 -) . There was significant difference of the expression of AR in ER/PR positive and negative breast cancer (P=0. 001),it was positively correlated with ER/PR protein expression (rs =0. 874, P=0. 024). The difference of AR ex-pression between histological grading was statistically significance ( P=0. 022 ) , the expression of AR was negatively correlated with histopathologic grading (rs = -0. 201, P=0. 013). The positive expression of AR was related to tumor size, but not correlated with lymph node metastasis ( P=0. 211 ) and clinical staging ( P=0. 065 ) . Conclusion The expression of AR in breast cancer tissues and its relationship with clinicopathological features may provide a basis for the further improvement molecular typing of breast cancer, and also provide the theoretical basis for clinical treatment and judging prognosis.
4.Study on some mechanism of leucite microcrystallization to reinforce dental glass ceramics.

OBJECTIVETo research the influence of K(2)O composition in the raw material on leucite microcrystallization and to study the effect of leucite content on compressive strength of the dental glass ceramics reinforced by leucite microcrystallization.
METHODSThe raw materials with different K(2)O content were treated by a decided thermal treatment system. The products were analyzed by polaring microscope and X-ray diffractometer, and their compressive strength was also tested.
RESULTSThe microstructure of products from high K(2)O component was remarkably good, at microcrystal size of 0.8 micro m and the compressive strength was 206.6 MPa. A positive correlation was found between leucite volume and the compressive strength when leucite volume was less than 50% (Vol%).
CONCLUSIONSThe component of K(2)O has a great effect on the microstructure and the properties of the leucite-microcrystal-reinforced dental glass ceramics and the content of leucite microcrystals has a notable influence on the compressive strength of the dental glass ceramics.

5.The using status quo and analysis of POCT glucose meter

Objective To investigate the problems during using, standard and quality management of the POCT glucose meters in hospital, to analyze the solutions, and to provide reference data for improving the test level of POCT in hospital.Methods The amount, brand and application of portable glucose meters in the hospital were obtained by 3 rounds of surveillance from May to July in 2013.All of those glucose meters were taken part in external quality assessment of Clinical Laboratory Center of National Health and Family Planning Commission.The test results of those glucose meters were compared with that of automatic biochemical analyzer, the comparison results were then analyzed.Results The POCT glucose meters possessed 5 brands in our hospital, and the amount and type of glucose meters in some clinical departments were often changed.When 4 brands which were detected as quality control samples by ministry of health, the accuracy of detection results of 3 concentration of brand Ⅲ were substandard, the CV% of two levels were 11.9%and 10.1%respectively, the remaining 3 brands were in line with the requirements.The qualified percentages of 3 times of comparison were 85.0%, 92.0%and 97.4%.Conclusions The hospital should select the brand of portable glucose meters reasonably, correct use of glucose meters, and it is very necessary to build indoor and interstitial quality evaluation system, at the same time, suggesting the hospital to establish the POCT quality management team, to carry out the instrument comparison periodically, so to guarantee the accurate, reliable results of POCT in hospital.
6.Radiologic Study of Talocalcaneal Coalition

20 years)was 8.60%(48/558) , which was more than that in the second decade of life ( 5.22% ). The incidence of painful group was 9.79%(14/143),which was more than that in foot trauma group 6.78%(40/630).The positive C sign was presented in 61.1%(33/54),posterior-type coalition in 38.9%(21/54),short talar neck sign in 61.1%(33/54)and talar beak sign in 22.2%(12/54). Conclusion The talocalcaneal coalition is a common development abnormality in our country. We must pay attention to the diagnosis of talocalcaneal coalition for painful foot adulthood.
7.Radiologic Study of Calcaneonavicular Coalition

Objective To study the incidence and radiologic findings of calcaneonavicular coalition.Methods CR films of foot andankle in 1361 cases were presented,which were evaluated for acute trauma or chronic pain.There were 588 cases of foot CR and 773 cases of ankle CR,age ranged from 10 years to 91 years(984 cases of 20~40 years).The prevalence of calcaneonavicular coalition was determined and the different significance of both male and female,acute trauma and chronic pain group were analysed.Results In 1361 cases,72 cases of calcaneonavicular coalition(5.3%) were demonstrated,8.7%(47/588)on foot CR films and 3.2%(25/773) on ankle CR films.Calcaneonavicular coalition was more dipicted on foot CR films than on ankle CR films(P0.05).Conclusion The foot CR films is more superexcellent than the ankle CR films on demonstrating calcaneonavicular coalition.
8.Diagnostic Significance of C Sign

Objective To study the clinicoradiologic significance of C sign.Methods 773 cases of standard ankle CR radiographs were presented for diagnosis of acute trauma and ankle pain. There were 565 males and 208 females, ages ranged from 10 to 81. There were 630 cases of acute trauma and 143 cases of painful ankle. C sign incidence, C sign figuration, distance of the medial facet of talocalcaneal joint, and the figuration of sustentaculum tali were studied. The relationship between C sign and talocalcaneal coalition was analysed.Results The incidence of C sign was 54.6%(422/773). UNLABELLEDWe designed a pair of specific primers and a TaqMan fluorescent probe targeting the toxR gene of Vibrio parahaemolyticus (VP). After optimizing the conditions, the specialty, sensitivity and reproducibility of the detection method were evaluated.
RESULTS(1) the developed real-time PCR assay protocol detected only VP and was not affected by other normal food pathogens such as Staphylococcus aureus, Salmonela, Listeria monocytogenes. (2) the limit of detection was 25 copies of toxR gene in the detected samples, and the sensitivity of pure cultures and simulated food samples was 21 cfu/mL and 210 cfu/g. (3) the developed protocol of real-time PCR assay had a high reproducibility, and the sample's variation was 0.9% and 1.3% within the same sample and between tests. (4) the standard curve had a good linearity when the gene quantity was between 2.5x10(1) and 2.5x10(6) copies. The developed detection assay targeting the toxR gene can quantitatively detect VP in only 3 hours, and thus is an efficacious method for the detection of Vibrio parahaemolyticus.

We formulated a selective enrichment broth (SVV) for simultaneous growth of Salmonella, Vibrio parahaemolyticus, and Vibrio cholerae by single factor experiment and response surface method. We evaluated the enrichment effect of SVV by conventional culture method and real-time PCR assay. We obtained the SVV broth by supplementting the Buffered Peptone Water (BPW) with bile salt no. 3, potassium tellurite, and sodium citrate as inhibitors, and glucose, mannitol, snhydrous sodium sulfite and sodium pyruvate as accelerants. We also modified the concentration of sodium chloride in BPW. When mixed at equal or varied proportions, the target pathogens had a great accumulation (10(5)-10(8) CFU/mL) after incubated in SVV for 18 h at 37 degrees C with shaking. It can also effectively inhibit the competitive microflora. We detected 10 artificial simulated samples and 608 real samples using SVV with real-time PCR. After enriched in SVV for 18 h, the quantity of the bacteria in samples were above the detection limit. The SVV with PCR assay showed higher tested positive (4.06%) compared to that of the conventional detection method (3.78%) and there was no false report. In summary, SVV is a promising new multiplex selective enrichment broth that can be used in detection of seafood.
