1.The value of ultrasound in thyroid nodules management: analysis of ultrasonographic features in 123 patients with thyroid nodules

Objective To evaluate whether ultrasonographic features of thyroid nodules are associated with histological malignancy, and to set up helpful criteria in identifying those who should accept subsequent fine-needle aspiration biopsy (FNAB). Methods 146 thyroid nodules of 123 patients accepted ultrasonographic evaluation and FNAB. All patients but one with suspicious or malignant cytology were operated. Results Out of total 146 nodules, 43 were histologically malignant, 102 were cytologically benign, and 1 nodule with suspicious cytology was not followed. Hypoechoic pattern, blurred margins, irregular shape, microcalcification, and type Ⅲ vascularity were significantly more frequent in malignant than benign nodules (P < 0. 05). No one of the ultrasonographic features could independently predict malignant nodules. A better compromise is probably to apply FNAB in nodules with at least one or two malignant ultrasonographic signs. Conclusion No single parameter could satisfactorily identify a subset of patients to be selectively examined by FNAB. A cost-effective approach to FNAB should depend on combination of specific ultrasonographic features.

Objective To investigate the efficacy and safety of combination of transcatheter arterial chemoembolization (TACE)and percutaneous radiofrequency ablation (RFA)in treatment of hepatocellular carcinoma (PHC).Methods 70 cases of PHC were divided into combined group (TACE +RFA,n =37) and control group (only TACE,n =33).Patients were followed up for1 to 2 years,and the therapeutic effects and side effects were compared between the two groups.Results The patients with tumor completely necrosis and AFP level lower than >50% in combined group were significantly more than those in control group (P <0.05);the half of year,one year and two years survival rate in combined group were greatly higher than those of control group;no severe side effect was observed in two groups.Conclusions TACE+RFA is effective and safe in treatment of PHC of more than 5 cm in diameter.
3.A study on the influence of CRT brightness for image reading

Objective To study the influence of CRT brightness on image reading. Methods Both physical testing and compared viewing were used in the study. (1) Setting the CRT brightness at high and low grade (the blank brightness of CRT was 112 cd/m 2 and 82 cd/m 2, respectively). (2) 21 films with the density ranging from 0.27 to 2.71 were digitized by a scanner and displayed on the CRT. The brightness of CRT that varied with film density was measured by a bright meter, then the relation chart between CRT brightness and the film density was drawn. (3) Thirteen films on which there were many small and low contrast signals were viewed on CRT by three doctors. Objective To discuss the clinical effect of artificial pneumothorax combined with intercostal nerve block in alleviating chest pain occurring during and after percutaneous microwave ablation (MWA) for subpleural lung malignancy.Methods A total of 30 patients with subpleural lung malignancy were randomly and equally divided into group A (n=10),group B (n=10) and group C (n=10).The patients in group A received both artificial pneumothorax and intercostal nerve block before MWA.The patients in group B only received artificial pneumothorax before MWA,and the patients in group C only received intercostal nerve block before MWA.The degree of pain was evaluated by visual analogue scale (VAS) score during MWA,immediately after MWA and at 6 h,12 h and 24 h after WMA.The side effects after MWA were recorded.Results During MWA,no statistically significant differences in VAS scores existed between each other among the three groups (P=0.885).The VAS scores determined at 6 h,12 h and 24 h after MWA in group C were significantly increased (P=0.014,P=0.006 and P=0.006 respectively).No patient in group A and group B developed symptoms of chest tightness after artificial pneumothorax was performed.After treatment,a small amount of asymptomatic residual pneumothorax was still observed in 6 patients of group A and group B,which disappeared spontaneously in about one week.Another patient still showed massive pneumothorax even after thoracic gas suction,and the patient recovered after thoracic closed drainage for three days.No other serious complications related to artificial pneumothorax occurred.Conclusion Artificial pneumothorax combined with intercostal nerve block can effectively relieve the chest pain occurring during and after MWA in patients with subpleural lung malignancy,and clinically this technique is quite safe.(J Intervent Radiol,2017,26:269-273)
5.Clinical value of serum IL-2,IL-6,IL-10,ChE and HA changes in patients with chronic hepatitis B and liver cirrhosis

Objective To study the changes and clinical significance of serum interleukin-2 (IL-2) ,interleukin-6 (IL-6) ,interleu-kin-10 (IL-10) ,cholinesterase (ChE) and hyaluronic acid (HA) in the patients with chronic hepatitis B (CHB) and liver cirrhosis (LC) .Methods The serum concentrations of IL-2 ,IL-6 ,IL-10 ,ChE and HA were detected in 92 cases of CHB(CHB group) ,63 cases of LC (group LC) and 46 cases of healthy populations (control group) .Results The serum concentrations of IL-2 ,IL-6 and HA in the CHB group and the LC group were higher than those in the control group ,while the serum concentrations of IL-10 and ChE were lower than those in the control group ,the differences had statistical significance (P<0 .05) .The serum concentrations of IL-2 ,IL-6 and HA in the patients with HBV DNA copy number of 6 .0-6 .9 log10 copy/mL were higher than those with the copy number of 3 .0-4 .9 log10 copy/mL and 5 .0 -5 .9 log10 copy/mL ,while the serum concentrations of IL-10 and ChE were lower than those with HBV DNA copy number of 3 .0-4 .9 log10 copy/mL and 5 .0-5 .9 log10 copy/mL ,the differences had statistical significance(P<0 .05) .The serum concentrations of IL-2 ,IL-6 and HA in the LC patients with the Child-Pugh grade C were higher than those with the Child-Pugh grade A and B ,while serum concentrations of IL-10 and ChE were lower than those with the Child-Pugh grade A and B ,the differences were statistically significant (P<0 .05) .Conclusion The simultaneous detection of serum IL-2 ,IL-6 ,IL-10 ,ChE and HA concentration changes can provide certain clinical reference value for the evaluation of severity and prognosis in the patients with CHB and LC .
6.Effects of Ligu Capsules in Preventing and Treating Menopausal Osteoporosis of Rats

Objective To investigate the effect of Ligu Capsules in preventing and treating menopausal osteoporosis in ovariectomized rats.Methods Rat models with osteoporosis were established by ovariectomy. Bone mineral density of femur,dry and wet bone weight and blood biochemical parameters were measured before and after treatment.Results Compared with the model group, Ligu Capsules increased femur bone mineral density,elevated the serum levels of Ca,P,Mg ,Zn and CT, decreased serum BGP level and hydroxyproline/creatinine(Hyp/Crea)ratio,increased dry weight, wet weight and ash weight of femur, as well as the bone length, bone volume, and bone diameter.Conclusion Ligu Capsules can prevent and treat osteoporosis in ovariectomized rats.
7.Clinical observation of highly active antiretroviral therapy in 70 pediatric patients with acquired immunodeficiency syndrome

Objective To observe the treatment response and potential issues related to antiretroviral therapy (ART) in pediatric patients with acquired immunodeficiency syndrome (AIDS) and to provide a basis for revising the treatment guideline and improving the clinical practice. Methods Children eligible for ART according to the current treatment guideline were recruited from Dehong area. Enrolled patients were provided with ART and followed up for regular clinical check and laboratory tests. Results By the end of March 2009, a total of 70 children had received ART. Among them, 60 patients were treated with regimen including zidovudine (AZT)+ lamivudine (3TC)+nevirapine (NVP). Twelve, eighteen, twenty-three and nineteen patients have tested for HIV viral load after 3 month, 6 month, 12 month and 18 month treatment, respectively. Among them, 7, 12,14 and 14 patients respectively achieved HIV viral load lower than 1000 copy/mL. Average CD4+ Tlymphocyte count increased from (317.8±288.8) × 106/L at baseline to (1037.2±1086. 1) × 106/Lafter 18 month treatment. Side effects mainly occurred within the first 3 months of treatment. Nearly 50% of children had gastrointestinal symptoms. Resistance to 3TC, NVP and efavirenz (EFV) were found in five patients who have completed 12 months of treatment. Conclusions Pediatric AIDS patients show good compliance and treatment response to ART. Most side effects happen in the first 3months of treatment and the most common side effects are gastrointestinal symptoms.

Objective To evaluate the imaging characteristics of chondromalacia patellae(CMP)in low-field MR and the value of MR in diagnosis.Methods 37 cases with CMP proved by arthroscopy and/or operation were carefully examined by 0.3 Tesla permanent magnet MR unit.The results of MRI were retrospectively analyzed according to that of arthroscopy and operation to search for the MR imaging manifestations of different degrees of patellae cartilage injuries in CMP.Results According to the golden-standard by arthroscopy and operation as for CMP,the overall diagnostic accuracy of MRI was 79.4%,with 28.6% accuracy in the early phase,92.6% in the late phase of CMP.Conclusion Low-field MR imaging is accurate in diagnosing CMP in the late phase and should be suggested as a useful nontraumatic imaging modality in the assessment of CMP before arthroscopy and operation.
9.Protective effects of high concentration fentanyl on brain slice injury induced by oxygen glucose deprivation

Aim To investigate the protective effects of high concentration fentanyl on the brain slice injury induced by oxygen glucose deprivation(OGD).Methods Rat brain slices were made and randomly assigned to four groups:control(n=10),OGD(n=10),fentanyl 50 ?g?L~(-1)(F_(50),n=10) and fentanyl 500 ?g?L~(-1)(F_(500),n=10).Changes of the neuron injury and apoptosis were observed with TTC staining,LDH releases,TUNEL staining,immunohistochemistry and electromicroscope.In addition,changes of intracellular calcium were measured with confocal laser-scanning microscopy.Results F_(50) and F_(500) attenuated the decrease of TTC staining and the increase of LDH release induced by OGD in brain slices.Neuronal apoptosis and changes of neuronal ultrastructures were attenuated by F_(50) and F_(500).Bcl-2 and Bax protein expressions were increased after OGD.Bax protein expression was decreased by F_(50) and F_(500),while Bcl-2 protein expression was increased by F_(50)and F_(500).Intracellular calcium concentration was increased by OGD and then it was lowered by F_(50) and F_(500).The protective effects of F_(50) were more obvious than that of F_(500).Conclusions High concentrations of fentanyl have neuron protective effects against OGD injury in rat brain slices,and fentanyl 50 ?g?L~(-1) has more obvious protective effects than fentanyl 500 ?g?L~(-1).
10.Analysis of post-pancreatoduodenectomy diarrhea:a report of 71 cases

Objective To explore the etiology,mechanism and treatment of diarrhea after(pancreatoduodenectomy).Methods Based on the clinical data of 159 cases of pancreatoduodenectomy(performed) in the recent one and half years,the pathogenesis of post-pancreatoduodenectomy diarrhea was(analyzed) and the effect of different treatments was observed.Results Seventy-one cases had diarrhea,with an incidence of 44.7%.Tweenty-two cases had bacterial infection of the intestinal tract and 4 cases had fungus infection.The incidence of infection was 36.6%.In 64 cases diarrhea was relieved with effective treatment,accounting for 90.1% of all cases.Seven cases with chronic diarrhea had additional treatment with oral pancreatic enzyme and symptoms were relieved 2 weeks after treatment.Conclusions Diarrhea is frequently observed in patients after pancreatoduodenectomy,and the majority of them can be cured with(treatment) selected according to the pathogenesis.
