Objective To observe the food intolerance of irritable bowel syndrome with diarrhoea(IBS-D) patients and healthy physical examinees and the clinical effect of the treatment of IBS-D food rejectingtrend.Methods Sixty-eight cases IBS-D patients meeting the diagnostic criteria of RomeⅢ were selected and 40 cases of healthy as control group,tested 14 kinds of food specific IgG antibody by enzyme-linked immunosorbent assay,treated the IBS-D patients with food intolerance for 12 weeks by the application of food rejectingtrend and observed the change of main symptoms.Results Among the 68 cases of IBS-D patients,55 cases had at least one of food intolerance,the positive rate was 80.88%;the control group had 13 cases of food intolerance,the positive rate was 32.50%,the difference between the two groups was statistically significant(χ2=25.28,P<0.01).The food with the highest positive rates of two groups were both wheat,egg white/egg yolk,milk and soybean.After 12 weeks of diet treatment,the total score of the symptoms of IBS-D patients decreased from (8.25±2.97) points to (3.13±3.52) points(t=11.107,P<0.01),self scores of the degree 0and frequency of abdominal pain,frequency of diarrhea,degree of abdominal distension,stool shape and general feelings of distress were significantly improved compared with that before treatment(t=12.429,11.016,5.685,5.558,7.681,11.065,P<0.01).Among 54 cases IBS-D patients(1 patient lost to follow-up),61.11%(33/54) significantly improved,24.07%(13/54) improved,14.81%(8/54) invalid,the total effective rate was 85.19%(46/54).Conclusion There is correlation between food intolerance and IBS-D.According to the Results of the detection of food specific IgG antibody,food rejectingtrend treatment can effectively improve the symptoms of IBS-D.

BACKGROUND:Clinical studies have shown that early use of methylprednisolone can promote neurological functional recovery, reasonable initial dose, interval time and treatment duration are the key to the methylprednisolone treatment of acute spinal cord injury. OBJECTIVE:To observe the differential protein expression profile in spinal cord tissue after intrathecal injection of high-dose methylprednisolone was given in rat model of acute spinal cord injury. METHODS:Eight Sprague-Dawley rats were included in this study to establish acute spinal cord injury model and the models were randomly divided into two groups, receiving intrathecal injection of methylprednisolone 7.5 mg/kg at 0 and 8 hours after modeling. The injured spinal cord tissue was harvested after 24 hours of injection. The differentialy expressed proteins and nerve regeneration-related differential proteins in two groups were analyzed using isotope labeling and quantitative technical analysis. RESULTS AND CONCLUSION: Totaly 87 differentialy expressed proteins were identified in this study. Compared with 0 hour group, there were 43 up-regulated differential proteins and 44 down-regulated differential proteins in the 8-hour group. Eighteen differential proteins were related to neural regeneration, including 8 up-regulation proteins and 10 down-regulation proteins. OMgp as a potential neural axon growth inhibitory factor specificaly bound with NgR/P75/TROY/Lingo-1 to form receptor complexes and activated RhoA through the second messenger cAMP, thus inhibiting the colapse of axon growth cone. Folowing intrathecal injection of methylprednisolone for treatment of acute spinal cord injury in rats, differential proteins and nerve regeneration-related factors in spinal cord are identified and analyzed for protein database retrieval and protein function analysis, their expression may serve as the indicator of monitoring nerve regeneration after acute spinal cord injury.

AIM: Previous studies paid more attention to the effect of fibroblast immigration in normal surrounding tissue or blood on wound healing. This study explored the morphological character of fibroblasts after heat injury to support the new operation of autogenous epidermis overlapping denatured dermis. METHODS: The experiment was performed at the State Key Laboratory of Trauma, Burn and Combined Injury in the Third Military Medical University of Chinese PLA from April 2006 to April 2007. The dermal fibroblast from the infant prepuce disposed after surgery was cultured in vitro, and divided into 2 groups. In normal group, the dermal fibroblasts were put in water bath at 37 ℃ for 30 seconds; the cells in experimental group were stimulated at different temperatures (50, 51, 52, and 53 ℃) for different time (30, 60, 90, and 180 seconds). Cell survival was detected by MTT; morphology of injured cells were observed under inverted microscope and transmission electron microscope at 3 hours, 1, 2, 3, 4, 5, and 7 days; cell cycle was detected by flow cytometry. Objective To assess willingness to pay(wtp)for typhoid vi vaccine in typhoid epidemic area in Guangxi Zhuang Autonomous Region and to provide evidence for introduction of the vaccine.Methods Applying the method of wtp was investigated in typhoid epidemic area,the curve of "price-accept ratio model"was drawn up with Probit regression.Results The formula of "price-accept ratio model" was described as following:Probit(p)=0.88952-0.46296X.The WTP for typhoid vi vaccine was $10.41,with a 95% confidence interval of $6.67～16.24.Conclusions WTP for typhoid vi vaccine was around $10 in Guangxi typhoid epidemic area.The Contingent Valuation Method was applicable in the investigation of WTP.

Objective To retrospectively evaluate the diagnostic accuracy of endoscopic ultrasonog raphy ( EUS) in pancreatic cancer, analyze the value of EUS in measurement of tumor diameters, TNM staging, and investigate the effectiveness of endoscopic ultrasonography-guided fine needle aspiration ( EUS-FNA). Methods All the data adopted from the patients underwent EUS in PUMC Hospital between 1997 and 2003. Eighty-eight patients with a final diagnosis of pancreatic cancer (56 is proven by surgical pathology, 10 by surgical findings and 22 by clinical diagnosis) and 47 patients with a final diagnosis of benign pancreatic disease were included and analyzed. Results The sensitivity, specificity, accuracy, positive and negative predictive values of EUS in the diagnosis of pancreatic cancer were 95. 5% , 59. 6% , 83. 0% , 81. 6% and 87. 5% respectively. Tumor diameters measured by EUS (2. 8?1. 0)cm were obviously smaller than those by surgical findings (6.5 ? 2. 9) cm. For TNM stage, compared with surgical findings, the accordance rates of EUS in T and N staging were 43. 1% and 61. 1% respectively. The accuracy of EUS-FNA in the diagnosis of pancreatic cancer was 76. 5% ( 13/17). Conclusions The sensitivity of EUS in pancreatic cancer is highest among all imaging procedures. Tumor diameters measured through EUS are smaller than those by surgery.

Objective To investigate the expression and significance of BCAR1 protein in endometrial carcinoma.Methods The level of BCAR1 was measured by immunohistochemistry in 50 endometrial carcinoma samples and 30 normal proliferative endometrium samples.Estrogen receptor (ER) and progesterone receptor (PR) expression was investigated in endometrial carcinoma samples.Results The level of BCAR1 was higher in endometrial carcinoma than that in normal proliferative endometrium (64.0％ (32/50) vs.40.0％ (12/30) ;x2 =4.364,P =0.037).In endometrial cancer,increased BCAR1 expression was significantly correlated with relatively higher age,poor differentiation,positive expression of ER (x2 =6.272,r =0.354 ; x2 =5.640,r =0.366 and x2 =4.217,r =0.290 ; P ＜ 0.05).Conclusion BCAR1 promote carcinogenesis and positively correlate with poor differentiation and ER status in endometrial carcinoma.

Objective To investigate the effect of estrogen on expression of matrix GLA protein (MGP)in ovariectomized Sprague-Dawley(SD)rats and the role of estrogen in improving postmenopausal osteoporosis.Methods Thirty-six SD female rats were allocated into 3 groups randomly,every 12 rats in ovariectomized group(OVX group),estrogen group(E group)and control group(sham group).Rats in OVX and E group all underwent bilateral ovariectomy,those rats in E group were given by estradiol benzoate intramuscularly after 3 weeks of ovariectomy.Rats in sham group underwent bilateral lipectomy near the ovary.All rats were kept the urine and the serum every three weeks and were sacrificed after 15 weeks.The pathology changes of uterus,lumbar vertebral bones were observed by immunohistochemistry.Bone mineral density(BMD)of lumbar vertebra of rats was determined by dual energy X ray absorptiometry(DEXA).The content of MGP in serum and urine was determined by ELISA.Expression of underearboxylated matrix GLA Protein(MGP)was detected by immunohistochemistry.Relative quantification of MGP mRNA expression in lumbar vertebra bone was detected by Fluorescent real-time quantitative polymerase chain reaction.Results(1)After 15 weeks of ovariectomized,the endometrium of uterus and lumbar vertebra exhibit remarkable pathologic changes in OVX group.The serum estrogen of(454±66)pmol/L in OVX group were lower than in(527 ±77)pmol/L in sham group and(556 ±80)pmol/L in E group significantly (P ＜ 0.05).The BMD of lumbar vertebra of(0.263 ± 0.030)g/cm2 in OVX group were lower than (0.295 ±0.024)g/cm2 in sham group and(0.279 ±0.024)g/cm2 in E group significantly(P ＜0.01).(2)The serum MPG protein in OVX group and E group showed decreased trends after ovariectomized,which were(104 ±64)ng/L in OVX group and(134 ±6)ng/L in E group at 9 weeks,which reached statistical difference(P ＜ 0.05).However,MGP in urine in sham group did not exhibit significant difference after 15 weeks of surgery(P ＞0.05).The MGP in urine of E group showed increased trends after 12 weeks of surgery,which reached(110.0 ±3.4)ng/L at 15 weeks,in the mean time,it was found that(86.5 ±2.5)ng/L of MGP in urine in OVX group,which showed significant difference(P ＜ 0.05).(3)MGP could be observed in lumbar vertebra in OVX group by immunochemistry staining.In the other two groups,the expression of MGP was not dominant.(4)Relative quantification of MGP mRNA expression in lumbar vertebra was defined as 1 in OVX group,when compared with 0.289 ±0.260 in E group and 0.103 ±0.098 in sham group,the difference showed statistically significant(P ＜ 0.01).Conclusion Estrogen could increase the expression of MGP mRNA and protein in ovariectomized rats and might play an important role in improving postmenopausal osteoporosis.

Objective To further elaborate the clinical characteristics,diagnosis,treatment and prognosis of primary cutaneous invasive epidermo-tropic CD8 positive cytotoxicity T-cell lymphoma.Methods The clinical characteristics,diagnosis and treatment of a patient with primary cutaneous invasive epidermotropic CD8 positive cytotoxicity T-cell lymphoma were summarized,and diagnosis and treatment of the disease from published literature were reviewed.Results After one year long,recurrence of exudative maculopapule on his body and limbs,a middle-aged male patient visited our hospital.Then,the patient was diagnosed as T2N0M0 Ⅰ phase of primary cutaneous invasive epidermo-tropic CD8 positive cytotoxicity T-cell lymphoma based on results of skin biopsy,immunohistochemistry,T-cell gene rearrangement and PET-CT.Through integrative medicine treatment (combination of bruceolic oil emulsion and chemotherapy with GDP regimen),the patient achieved complete remission (CR).The treatment and follow-up continues till now and his condition was stable.Conclusions Clinical occurrence of primary cutaneous invasive epidermo-tropic CD8 positive cytotoxicity T-cell lymphoma is rare and early diagnosis is difficult.It could be diagnosed with histopathology combined with T-cell gene rearrangement.In order to avoid misdiagnosis,great caution should be taken during disease history inquiry and physical examination,should be early lesion for delayed healing of recurrent lesions,biopsy should be conducted as soon as possible and immunohistochemistry should be performed simultaneously,which will provide a good basis for further treatment.
