1.Corrected TIMI frame count after coronary intervention as a predictor of myocardial tissue perfusion in patients with acute myocardial infarction

Objective To study the probability of evaluating myocardial tissue perfusion by corrected TIMI frame count (CTFC) after successful percutaneous coronary intervention (PCI) in patients with acute myocardial infarction (AMI). Methods Sixty-three patients with their first acute myocardial infarction (AMI) who accepted primary coronary intervention were recruited. All these patients got TIMI 3 grade flow. CTFC was measured after successful PCI in these patients. Wall Motion Score Index (WMSI) assessed by two-dimensional echocardiography before and one month after PCI and the correlation studied between CTFC and WMSI. Results According to CTFC the patients were divided into two groups. Improvement of WMSI in the TIMI 3 fast group was significantly greater than that of the TIMI 3 slow group. CTFC had a significant correlation with the change in WMSI. Elapsed time from the onset of symptoms to reperfusion in the TIMI slow group was significantly longer than that of the TIMI fast group. Conclusion CTFC is a quantitative, and reproducible index of coronary blood flow. Lower CTFC is associated with greater functional recovery and could supply evidence for additional adjunctive treatment.
2.Perinatal care and Changing Trend of Maternal and Perinatal Mortality

Objective To investigate the effect of perinatal care on the reduction of the maternal and perinatal mortality. Methods The perinatal care and the relationship between the maternal and perinatal mortality during 1980 to 1997 were analysed. The factors that related to the morality were studied. Results The maternal mortality rate was significantly decreased from 31.7/10~5 live births in 1960 to 16.1/10~5 live births in 1992~1997. The main cause of maternal deaths in 1960' was obstetrical factors, but in 1997 it was pregnancy complications. The perinatal mortality rate was 21.2‰ in 1980~1981 ,and was 10.9‰ in 1992~1997. The factors related to the maternal and perinatal mortality including the perinatal care, high risk pregnancy,and birth weight were analysed, the specific mortality rate of each factor was calculated. Conclutions The two mortalities can be used to evaluate the result of perinatal care,and are important feedback information for making health care policy. Pernatal care play an important role in the reduction of maternal and perinatal mortality.
3.Embolization effect of 5-Fu gelatin microsphere(FGM) in external carotid artery branches in dogs

objective: To evaluate the embolization effect of 5 Fu gelatin microsphere(FGM) in oral and maxillofacial region.Methods: FGM was prepared with drug load of 12% and encapsulation of 85%. After irridiated with 60 Co, FGM was suspended into 76% of meglumine diatrizoate, and then perfused into external maxillary artery and lingual artery in 8 dogs under X ray observation. 4 dogs were used in each artery embolization. Blood flow velocity and volume in the arteries were tested at different time after embolization; all the data were analyzed statistically. The sections of embolized tissue at different time were examined microscopically. Results: Before and 1 h, 48 h and l month after ombolization the blood flow volume(ml/min) in external caroted artery was 43?5.0, 31.0?4.5, 26.5?4.0 and 35.0?30; that in external maxillary artery 5.0?1.5, 0.5?1.0, 2.5?1.0 and 3.0?1.5; in ligual artery 3.5?1.0, 0.5?0.5, 0 and 0; in internal maxillary artery 17.0?4.0, 19.5?4.0, 19.0?4.5 and 21.0?3.5, respectively. Stuff of the cavity of the corresponding arteries by FGM were found with histological examination. Conclusion: FGM can embolize artery completely.
4.Enterococci resistance and vancomycin resistant gene detection

Objective To investigate Enterococci resistance to glycopeptides and their genotypes. Methods MICs of antimicrobial agents to Enterococci were determined with agar dilution test and van genes were amplified with PCR. Results 298 E. faecalis and 42 E. faecium clinical isolates were collected from 13 University teaching hospitals around China. The resistant rates of E. faecalis to penicillins were 14.1%~17.8%, which was much lower than those of E.feacium. No glycopeptides resistant Enterococci strain was detected. 8 E. faecalis and 1 E. faecium were intermediate sensitive to vancomycin but sensitive to teicoplanin with MICs of 8~16 and 0.25~2 mg/L, respectively. PCR amplification of van genes gave rise to only vanC2/3 gene positive in E. faecalis B144. Conclusions Glycopeptide resistant Enterococci is not common in China and no vanA or vanB type Enterococci was detected in this study. Penicillins, vancomycin and teicoplanin could be employed in the treatment of Enterococcus faecalis infections. High attention should be paid to future development of vancomycin resistant Enterococcus (VRE) in China.
5.Effect of dexmedetomidine on lipid peroxidation during lung ischemia-reperfusion in rats

Objective To evaluate the effect of dexmedetomidine on lipid peroxidation during lung ischemia-reperfusion (I/R) in rats.Methods Fifty-four pathogen-free healthy male Wistar rats,weighing 250-350 g,were randomly divided into 3 groups (n=18 each) using a random number table:sham operation group (S group),I/R group,and dexmedetomidine group (Dex group).In group Dex,dexmedetomidine 5 μg/kg was injected via the caudal vein once a day for 2 consecutive days.The equal volume of normal saline was given in S and I/R groups.At 30 min after administration on 2nd day,lung I/R was produced by clamping the left hilum of lung for 45 min followed by 120 min of reperfusion in I/R and Dex groups.At 45 min of ischemia,and 60 and 120 min of reperfusion,6 rats selected from each group were sacrificed,and the left lungs were removed for examination of the pathological changes and for determination of wet/dry lung weight ratio (W/D ratio),malondialdehyde (MDA) content and superoxide dismutase (SOD) activity.Results Compared with S group,the SOD activity was significantly decreased,and the MDA content and W/D ratio were significantly increased at 60 and 120 min of reperfusion in I/R and Dex groups (P<0.05).Compared with I/R group,the SOD activity was significantly increased,and the MDA content and W/D ratio were significantly decreased at 60 and 120 min of reperfusion in Dex group (P<0.05).The pathological changes of lungs were significantly attenuated in Dex group as compared with I/R group.Conclusion Dexmedetomidine mitigates lung I/R injury through inhibiting lipid peroxidation in rats.
6.Effect of bacille calmette-guerin polysaccharide nucleic acid (BCG-PSN) on the expression of T cell-derived cytokines in patients with atopic dermatitis

Objective To assess the changes in frequency of peripheral T lymphocytes expressing different cytokines in patients with atopic dermatitis (AD) before and after treatment with BCG-PSN and their relationship with disease severity. Methods A randomized, double blinded and placebo cross-over control study was conducted. A total of 8 patients with AD were recruited in this study. Intramuscular BCG-PSN or placebo was given to patients every other day for 36 days. Flow cytometry was performed to measure the frequency of IL4-, IL5-, IFN-γ- and TNFα-expressing peripheral CD4+ T cells and CD8+ T cells before and after the therapy. Disease severity was evaluated by atopic dermatitis area and severity index score (ADASIS). Results The difference value in IFN-γ+CD8+ T cell frequency before and after therapy was significantly higher in patients treated with BCG-PSN than in those with placebo (8.056 ± 13.962 vs -6.549 ± 10.491, U = 2.26, P< 0.05). There was no statistical difference in the frequency of IL4-, IL5-, TNFa-expressing CD8+ T cells between BCG-PSN- and placebo-treated patients (all P > 0.05). The decrease in ADASIS was 1.56 ± 1.49 in patients treated with BCG-PSN, which was statistically higher than that in placebo-treated patients (-0.05 ± 1.54, U = 2.00, P< 0.05). Conclusion As an immunomodulator, BCG-PSN may control AD by restoring the balance of T-cell subsets.
7.High glucose increases podocyte autophagy through PI3K-AKT-mTOR signaling pathway

Objective To evaluate the effects of high glucose on autophagy and apoptosis of podocyte and explore the signaling pathway in high glucose-induce podocyte autophagy.Methods Differentiated mouse podocytes were exposed to high glucose(30 mmol/L) or rapamycin (autophagy enhancer,1 μg/L) or LY294002 (a selective PI3K inhibitor,50 mmol/L) for 24 h.The formations of autophagy were observed by electron microscopy and acridine orange staining.Apoptosis was evaluated by flow cytometry.The expression of autophagy protein LC3-Ⅱ/Ⅰ and Beclin-1 as well as the phosphorylation of AKT and mTOR were examined by Western blotting analysis.Results High glucose induced podocytes apoptosis,increased autophagy and the expression of autophagy-associated proteins (all P < 0.05).Rapamycin further increased the expression of LC3-Ⅱ and Beclin-1 protein (all P < 0.05),but LY294002 inhibited partialiy the protein expression of LC3-Ⅱ and Beclin-1 induced by high glucose (both P < 0.05).Treatment with rapamycin increased the phosphorylation of AKT,but reduced that of mTOR in podocytes.Moreover,LY294002 inhibited phosphorylation of both AKT and mTOR (both P < 0.05).Conclusions High glucose promotes podocyte autophagy and apoptosis.High glucose-induced autophagy is mediated partly through PI3K-AKT-mTOR signaling pathway.

10.Prenatal diagnosis and perinatal management of 44 cases of duodenal obstruction

Objective To investigate the prenatal diagnosis, perinatal management and standardized treatment protocol for neonates with duodenal obstruction. Methods A network in prenatal diagnosis, perinatal management and monitoring of congenital malformation was founded between Beijing Obstetrics and Gynecology Hospital and the Capital Institute of Pediatrics. Forty-four fetuses were prenatally diagnosed as duodenal obstructions by this network from July,2001 to September, 2010. The data of prenatal diagnosis, treatment after birth and prognosis were analyzed. Results Among 44 patients diagnosed as fetal duodenal obstruction by prenatal ultrasonography, three cases underwent induced abortion, three were in pregnancy, 14 were lost during follow-up and 24 were confirmed by surgical treatments after birth. Within 24 neonates underwent surgery, 21 showed double-bubble sign and 20 combined with polyhydroamnios in prenatal ultrasonography. Twenty-four neonates underwent upright abdominal plain film examination,22 showed double-bubble sign, 1 showed single-bubble sign and 1 showed triple-bubble sign,respectively. Nineteen neonates underwent upper gastroenterography which showed distention of stomach and duodenum, increased stomach peristalsis and an obstacle of duodenum emptying. Within 23 neonates underwent ultrasonographic studies, 10 showed distention and increased peristalsis of duodenum. Following surgical procedures were performed: diamond shape anastomosis was completed in 19 cases with annular pancreas; duodenal vertical resection, across suture and excision of the membrane was done in four cases with duodenum membranate stenosis; end-to-back anastomosis was taken in one case with duodenal separate atresia; Ladd's procedure was applied in 11 cases associated with malrotation. All patients were cured. Conclusions Standardized perinatal management and earlier intervention should be offered to newborns with duodenal obstruction to achieve better effects.
