OBJECTIVE To analyze the isolation rate and four consecutive years′(2004-2007) antibiotic-resistance trend of extended-spectrum ?-lactamases(ESBLs) producing Escherichia coli in urinary tract infection and to provide evidence for anti-infection therapy.METHODS The anti-microbial susceptibility test results of E.coli cultured from the midstream urine of outpatients and inpatients of our hospital from 2004 to 2007 were collected.Whonet 5.4 statistical analysis software was used for analysis.RESULTS During four years,no apparent change has been shown about antibiotic resistant rate of ESBLs producing E.coli.The isolation rate of this kind of E.coli increased year by year from 31% to 44% and showed high resistance to third generation cephalosporins and quinolones antibiotics, but was 100% sensitive to imipenem.CONCLUSIONS With the ESBLs-producing E.coli increasing year by year, the antibiotic-resistance rate of E.coli isolated from urinary tract infection is apparently increased and thus screening of antibiotics sensitive to enzyme-producing bacterial is the key point when the empirical therapy especially when the third and fourth generation cephalosporins are ineffective.

Objective To explore the effect of ulinastatin on coagulation function of elderly patients after hip replacement.MethodsTotally 120 patients undergoing elective hip replacement in our hospital were chosen.According to the stratified randomization method, all patients were divided into observation group and control group with 60 in each. The control group received oral Xarelto combined with other conventional treatment, while the observation group received ulinastatin on the basis of the treatment regimen of the control group. The coagulation parameters and clinical manifestations were compared between two groups.ResultsTwo days after operation, the TT, APTT, PT, DD and FIB between the two groups showed statistically significant difference (t=-5.300,-2.319,-2.409,-2.325,-3.567;P<0.05).The hospital stay,CRP,TXB2 and total drainage volume in observation group 5d after operation were lower than those in control group, between the two groups showed statistically significant difference (t=-2.529,-2.082,-3.388,-2.887;P<0.05).The patients with elevated blood urea and serum creatinine in observation group were obviously less than that of control group, but the patients with nausea in observation group were obviously higher than those in control group, between the two groups showed statistically significant difference (χ2=5.217,5.926,8.571;P<0.05).The patients with deep vein thrombosis in observation group were obviously higher than those in control group, between the two groups showed statistically significant difference (χ2=8.571, P<0.05).ConclusionUlinastatin can significantly improve the hypercoagulable state of patients after hip replacement and reduce the incidence of deep vein thrombosis, and plays an important role in promoting wound healing and immunity recovery.

Objective:To detect the expression of miR-106b~25 cluster in glioma cell line and tissues. Methods:Real-time PCR was performed to determine the expression of miR-106b~25 cluster members (miR-106b, miR-93, and miR-25) in different human glio-blastoma cell lines. Different pathological grade glioma specimens were surgically removed. In-situ hybridization was performed to de-tect the expression of miR-106b~25 cluster members in different pathological levels of glioma tissues. Results:In the expression of the benchmark on normal brain tissues, three kinds of miRNAs in all test cell lines have a tendency to increase. Based on the expression of the pathological level I average rate in 43 cases of glioma specimens collected after neurosurgical operations, the real-time PCR results showed that the average expression quantity of the three kinds of miRNAs in each group gradually increase. The increase in tumor path-ological levels results in statistically significant expression differences of miR-106b and miR-93 between the groups (F=4.479, P=0.018 and F=3.493, P=0.040, respectively). However, miR-25 expression differences between the groups have no statistically signifi-cant differences (F=2.766, P=0.075). In situ hybridization results show that the expressions of three miRNAs in high grade gliomas are significantly higher than that in the low-level tumor. Spearman rank correlation analysis results indicate that the expression of these miRNAs signal-intensity distribution is positively correlated with glioma, in accordance with WHO pathology classification. The corre-lation coefficient for miR-106b, miR-93, and miR-25 are 0.617, 0.438, and 0.463, respectively (P<0.001). Conclusion:The expression of miR-106b~25 cluster members is up-regulated in the glioma and is positively correlated with tumor grade.

Objective To evaluate the expression of CD47 and calreticulin and their relationships with clini-copathologic features, prognosis in infiltrating ductal breast carcinomas. Methods The expression of CD47 and calret-iculin was evaluated in 128 cases with infiltrating ductal breast carcinomas by means of immunohistochemistry, and the relationship between CD47 and other clinicopathologic factors as well as prognosis were analyzed. Results The expression level of CD47 in cancer nest was 81/128,which was lower than 106/128 in its adjacent normal epithelium (χ2 =12.400,P<0.05),but calreticulin expression was 50/128 and was higher in cancer nest(χ2 =21.510,P<0.05 ).In addition, the expression level of calreticulin had certain difference in different molecular subtype (χ2 =21.510,P<0.05),and an increased expression in Basal-like and Her-(2 +)/ER(-)subtypes.The expression level of CD47 was associated with tumor sizes, histological tumor grade and lymph node involvement(χ2 =11.400, 4.732,5.432,all P<0.05),and the expression level of calreticulin was associated with histological tumor grade and distant metastasis(χ2 =10.810,6.770,all P<0.05).The 5 -year survival rate of patients performed following-up was 68.37%.Univariate analysis indicated the survival rate in 5 years of high level CD47,calreticulin expression was higher than that of low level CD47,calreticulin expression(χ2 =5.231,13.069,all P<0.05).Conclusion The expression of CD47,calreticulin is associated with index of dysprognosis and survival rate in 5 years,so combined CD47 and calreticulin detect may be of prognostic value in patients with breast caner.
