1.Acupuncturists and their academic contributions in Lingnan area: a review.

The academic thoughts of Lingnan acupuncture and moxibustion have been an essential part of Lingnan medicine. By exploration and arrangement of Lingnan medicine and books, journals and literatures regarding acupuncture and moxibustion, the ancient and modern acupuncturists and their academic contributions in Lingnan area were reviewed. As a result, the number of Lingnan acupuncturists and their works was low before Qing Dynasty, while from the Republic of China era to People's Republic of China, a considerable amount of acupuncturists emerged with quite a lot of works. By exploration and arrangement of Lingnan acupuncturists and their works and academic opinion, the acupuncture-moxibustion school characterized by Lingnan could be formed and developed.

2.One-stage reconstruction of dorsal hand soft-tissue defects with tendon allograft and free anterolateral thigh flaps

Objectives To evaluate the clinical effect of one-stage reconstruction of dorsal hand softtissue defects using tendon allograft and free anterolateral thigh flaps.Methods From July 2006 to July 2011,fifteen cases of complex soft-tissue defects in dorsal hands were repaired using tendon allografts and anterolateral thigh flaps,sizing from 9 cm ×5 cm to 14 cm× 11 cm,in one stage.Two to 4 digital extensor tendons were reconstructed.With a brace on,early finger exercises were started after 2 weeks postoperatively when the flaps had survived.Results All the 15 flaps survived uneventfully.Twelve of the 15 patients were available for follow-up from 12-24 months (averaged 16 months).Two cases received tendolysis due to poor finger movement 6 moths postoperatively.At the end of follow-up,the range of wrist joint active flexion is from 40 to 70degrees and extension is from 25 to 50 degrees.The motion range of the related metacarpophalangeal joints was from 60 to 85 degrees,that of the interphalangeal joints from 80 to 90 degrees.The overall effective rate was 92 percent.Conclusion Simultaneous composite repair using tendon allografts and skin flaps proved to be reliable and cost-effective for complex dorsal hand soft-tissue defects.Allograft tendon reconstruction can not only obliterates new morbidities of autografting but also facilitates timely rehabilitation,avoiding extension loss of hand joints.
4.Application of the new method for the detection of the aromatic esterase activity of PON1 in coronary heart disease

Objective To observe the effect of paraoxonase 1 (PON1) application of the new method of arylesterase activity in patients with coronary heart disease,analysis of paraoxonase 1 (PON1) of the clinical value of arylesterase activity in the new testing method.Methods From January 2014 to January 2016 in our hospital 86 patients with coronary heart disease as the research object,and then select the healthy people at the same time to the hospital physical examination of 50 as the control group to take the research object,spectrophotometric method for the determination of coronary heart disease patients and control subjects serum PON1 arylesterase activity,PON1 arylesterase activity at the same time with statistics the different degree of coronary heart disease,PON1 arylesterase activity between patients with coronary heart disease and control group comparison study and different severity of coronary heart disease patients,the patients with coronary heart disease PON1 aromatic ester enzyme activity,age,gender,BMI,TC,total cholesterol,low density lipoprotein cholesterol LDL-C and glycerin three greases TG included in the analysis of factors of coronary heart disease multiple linear regression equation,to determine the changes of patients with coronary artery disease by PON1 arylesterase activity,to provide a reference for clinical treatment.Results The activity of PON1 in patients with coronary heart disease was significantly lower than that of the control group,and the difference was statistically significant (P<0.05).Single branch lesions in patients with PON1 arylesterase activity was significantly higher than that of double vessel lesions and three lesions were double branch lesions in patients with PON1 arylesterase activity was significantly higher than that in three patients,the differences were statistically significant (P<0.05).According to the multiple linear regression analysis showed that coronary heart disease and the patient′s age,gender,BMI,TC,LDL-C,TG and PON1 arylesterase activity (P<0.05),which was related with age,gender,BMI,TC,LDL-C and TG were positively correlated,negatively correlated with PON1 arylesterase activity.Conclusion The PON1 activity of in patients with coronary heart disease is significantly decreased,and the extent of the disease is more severe,the more obvious the decline of PON1,the activity of PON1 shows a negative correlation with coronary heart disease.
6.Clinical outcomes of thoracoscopic thymectomy versus open surgery for early-stage thymoma

From June 2008 to December 2012,video-assisted thoracoscopic surgery (VATS group) was performed for 24 cases of thymoma.Their operative procedures,postoperative complications,remission of myasthenia gravis (MG),perioperative mortality and oncological outcomes were compared with 19 cases of trans-sternal open thymectomy (OT group).Compared with OT group,VATS group had significantly shorter operation duration,less blood loss,shorter postoperative hospital stay,shorter chest tube duration and less chest drainage amount (P < 0.05).The total rate of postoperative complications of VATS group was 4% (1/24) and it was less than OT group (4/19) without significant difference (P >0.05).The effective remission rate of thymomatous MG was 7/9 in VATS group and it was similar to that in OT group (6/8,P > 0.05).VATS group had a recurrent or metastatic rate of 0% (0/23) and it was better than OT group with 1/17.And there was no significant inter-group difference (P > 0.05).The total survival rate of VATS group was 100% and it was similar to that of OT group (16/17,P > 0.05).Thoracoscopic thymectomy is safe,feasible,efficacious and mini-invasive for thymoma.With fewer complications and a quicker recovery,it has similar short-term outcomes to conventional open thymectomy.
7.Surgical treatment of acetabulum top compression fracture with sea gull sign

Objective To investigate the surgical method and clinical curative results of acetabulum top compression fractures with sea gull sign. Methods Data of 14 patients who had acetabulum top compression fractures with sea gull sign were retrospectively analyzed and were accepted surgical therapy and followed up. There were 5 females and 9 males, aged from 28 to 71 years (average, 49.9 years). The pre-operative time was from 4 to 14 days (average, 9 days). Letournel-Judet fracture classifica-tion:eight cases for the anterior column+posterior semi-transverse, six cases for a simple anterior column. Three patients with lum-bar compression fracture, two patients with tibial plateau fracture, one patient with multiple rib fractures. All patients were accept-ed open reduction and bone graft and internal fixation through ilioinguinal approach or ilioinguinal approach+Kocher-Langenbeck approach. The quality of fracture reduction was assessed according to the Matta reduction criterion after operation. The hip func-tion was evaluated according to Matta. Results 14 patients were followed up from 6 to 60 months (average, 36 months). All pa-tients reached bone healing;healing time was from 3 months to 4 months (average, 3.4 months). According to the Matta reset stan-dard after operation, there were 6 excellent cases, 5 good cases, 2 fair cases, 1 poor case, the excellent and good rate was 78.5%(11/14). The Matta was from 10 to 18 scores (average, 16.4 scores), 5 excellent cases, 5 good cases, 3 fair cases, 1 poor case, the ex-cellent and good rate was 71.4%(10/14). Traumatic arthritis occurred in 3 patients. Pain was serious in two patients and disap-peared after total hip joint replacement. Pain was mild for one patient and disappeared after accepted non steroidal painkiller. One patient had heterotopic ossification after operation, which was not treatmented because of no obvious symptom. Conclusion The appropriate operation time of acetabular roof compression fracture with sea gull sign was from 5 to 10 days after fracture, which was no more than two weeks had best. Through ilioinguinal approach or ilioinguinal approach+Kocher-Langenbeck approach, the articular surface could be reduced excellent and got sufficient bone graft. The clinical efficacy was satisfactory after operation.
8.Current status of frailty in elderly patients undergoing cardiac surgery and establishment of frailty prediction model

Objective:To analyze the current status of frailty in elderly patients undergoing cardiac surgery, explore its risk factors, and establish a predictive model of frailty in order to provide targeted and predictive nursing programs.Methods:A total of 205 cardiac surgery patients admitted to the first affiliated hospital of Xinjiang Medical University from March 2015 to January 2019 were selected as the study subjects. Patients were divided into 2 groups according to whether they were frail: the frailty group ( n=78) and the control group ( n=127). Logistic regression was used to analyze the risk factors that affect the frailty of elderly patients undergoing cardiac surgery. The receiver operating characteristic curve (ROC) was used to evaluate the effectiveness of model X [consisting of body mass index (BMI), Mini-Mental State Examination (MMSE) score, number of diseased patients, and number of drugs] in diagnosing frailty in elderly patients undergoing cardiac surgery. Results:Of the 205 patients, 78 (38.05%) showed frailty. The proportion of high school education level and above, Tinetti Gaitassessment (TGA) score≥24 and MMSE score≥27 in the frailty group was lower than that in the control group, and the proportion of Geriatric Depression Scale-15 (GDS-15) score≥7, the number of diseases≥3 and the number of drugs≥5 in the frailty group was higher than that in the control group ( χ2 value was 9.254-26.061, P<0.05). Logistic regression analysis showed that BMI, MMSE score, number of diseased, and number of medications were independent risk factors for frailty in elderly patients undergoing cardiac surgery ( OR value was 0.032-5.275, P<0.05). The area under the ROC curve, sensitivity and specificity of frailty in elderly cardiac surgery patients assessed by model X were 0.913, 75.61% and 96.77%, respectively. Conclusion:The incidence of frailty is higher in elderly patients undergoing cardiac surgery. Model X can diagnose the frailty of elderly patients undergoing cardiac surgery and help clinical nurses to carry out targeted care.
9.Relationship of hyperuricemia with all-cause mortality in patients with atrial fibrillation

Objective:To explore whether hyperuricemia was an independent risk factor for all-cause mortality in patients with atrial fibrillation.Methods:Patients with atrial fibrillation who were confirmed by 12-lead electrocardiogram in 11 hospitals of Kailuan Group from 2006 to 2007 were selected as the research objects.All patients were followed up by prospective cohort study, and all-cause deaths were observed.The last follow-up time was December 31, 2013.Kaplan-Meier curve and Cox proportional hazards model were used to analyze and compare the risk of all-cause mortality in patients with atrial fibrillation in the hyperuricemia group compared with the normal uric acid group.Results:A total of 388 community-based patients with atrial fibrillation were included in the final statistical analysis, with 136 all-cause deaths occurred during an average follow-up period of 6.93 years.The incidence of all-cause mortality was 9.24% per year(36/390)in the hyperuricemia group, whereas 5.16% per year(100/1 937) in the normal uric acid group.In the univariate Cox proportional risk model analysis, the risk ratio (95% CI) of all-cause death in patients with atrial fibrillation in the hyperuricemia group (95% CI) was 1.84(1.26-2.69) times that in the normal uric acid group ( P<0.01). After adjusting for potential confounding variables, the adjusted risk ratio (95% CI) of all-cause death in patients with atrial fibrillation in hyperuricemia group was still 1.94(1.32-2.85) times of that in normal uric acid group ( P<0.01). After adjustment for potential confounding variables, for each 0.01 g/L increase in uric acid (1 g/L=5 950 μmol/L), the risk of all-cause death in patients with atrial fibrillation increased by 1.15 (1.05-1.26) times ( P<0.01). Conclusion:Hyperuricemia was an independent risk factor for all-cause death in patients with atrial fibrillation in community.
10.Comparison of five kits for quantitation of hepatitis B virus DNA

Objective To compare the sensitivity, specificity and consistency of five kits for quantitative detection of hepatitis B virus (HBV) DNA. Methods Four domestic fluorescence kits, A, B, C, and D, and Roche Cobas Ampliprep/Cobas TaqMan HBV test for quantitation of HBV DNA in serum, were applied to detect the National Standards, an additional plasma for sensitivity (7 times dilution), 15 plasma samples from healthy blood donor and 45 plasma samples from chronic hepatitis B patients. Results All five kits showed the correct results for the 9 positive specimens and 8 negative specimens from the National Standards. The quantitative results of specimens for sensitivity met requirements of the National Standards. The lowest concentration of HBV DNA by these three kits was 15.6 IU/mL. The lowest detection level of HBV DNA for domestic kit B was ≤500 IU/mL. There was linear correlation between the results by Roche kit and domestic kit C (r> 0.98). Kit D showed 2 false-negatives results in the 15 healthy blood donor samples. The coincidence rates between 4 domestic kits and the Roche kit ranged from 50% to 96% (A: 61%, B: 50%, C: 96%, D: 83 %). The consistency rate of kit C with the Roche kit was significantly higher than those of kit D, A and B (X2=5.62, P<0.05, X2=28. 93, P<0.01, X2=44.31,P<0.01, respectively). The consistency rates among these 5 kits were highest when testing samples with HBV DNA levels between 1×104-1×107 IU/mL. Conclusions The qualities of domestic kits vary remarkably and kit C has the greatest consistency rate with the Roche kit. Therefore, the qualities of domestic kits need to be further improved.
