Objective To explore main risk factors of post-operative severe complications in elderly patients with gastric cancer.Methods From January 2006 to June 2011,a total of 236 elderly patients to receive gastrectomy for gastric carcinoma were retrospectively analyzed in this casecontrol study.The factors affecting post-operative severe complications were analyzed by multivariate logistic regression analysis.Results Age was younger in patients without than those with postoperative complications [(72.9±5.8) years vs.(80.6±6.7) years,t=-9.194,P=0.000].Serum levels of hemoglobin and albumin were higher in patients without than with post-operative complications [(109.94±18.72) g/L vs.(98.75±18.82) g/L,(38.37±5.52) g/L vs.(33.69± 3.35) g/L,t=4.391,7.080,both P=0.000].Operation time was shorter in patients without than with post-operative complications [(3.1 ± 1.0) hours vs.(3.6 ± 1.1) hours,t =-3.507,P =0.001)].Intraoperative blood transfusion volume was less in patients without than with post-operative complications [(2.5±4.4) L vs.(2.8±5.5) L,t=-4.901,P=0.000)].The long diameter of tumor was shorter in patients without than with post-operative complications [(4.5 ± 2.8) cm vs.(5.3± 2.7) cm,t =-2.111,P =0.036)].Logistic regression analysis revealed that chronic obstructive pulmonary disease (COPD),serum albumin level,age,operation time and blood transfusion volume were associated with postoperative severe complications.Logistic regression equation was as follow:P=1/[1+e(16.22-033X1+0.99X3-390X4-192X5-493X9)].Conclusions COPD,operation time,blood transfusion volume,serum albumin level and age are the main independent risk factors for post-operative severe complications in elderly patients with gastric cancer.

OBJECTIVETo evaluate clinical therapeutic effect of Guan-moxibustion on herpes simplex virus facial neuritis.

METHODSOne hundred and sixty cases were enrolled in 3 centers and 157 cases were completed the study. All he patients were randomly divided into 2 groups, a Guan-moxibustion group and a suspended moxibustion plus acupuncture group. All of them were treated with basic acupuncture, and the Guan-moxibustion group were added with Guan-moxibustion and the suspended moxibustion group with suspended moxibustion. They were treated for 8 weeks, and facial disability index (FDI) and House-Brackmann facial nerve grading system were used to assess therapeutic effects.

RESULTSThe effective rate was 91.0% in the Guan-moxibustion group and 72.2% in the suspended moxibustion group with a significant difference between the two groups (P < 0.05), the Guan-moxibustion being better than the suspended moxibustion group.

CONCLUSIONThe therapeutic effect of Guan-moxibustion plus acupuncture on herpes simplex virus facial neuritis is better than that of suspended moxibustion plus acupuncture.

OBJECTIVETo explore the immunosuppressive effect of local transfection of Molluscum contagiosum virus 148 (MC148) gene to allogenous skin graft against rejection.

METHODSMC148 gene was cloned from molluscum contagiosum virus (MCV), and was employed to construct recombinant adenovirus vector (Ad-MC148). The recombinant Ad-MC148 was then locally transfected into a part of the tail skin of eight Lewis rats, which served as skin donors for grafting. The wounds (1 cm x 1 cm) were produced on the tails of 16 Wistar rats, and they were then randomly divided into control (C, n=8, with grafting of skin from donor rats without transfection), and transfection (T, n=8, with grafting of skin from donor rats with transfection of the recombinant Ad-MC148) groups. The expression of MC148 mRNA gene in T group was detected on 6 post operation hour( POH) and 2, 3, 7 and 10 post operation day (POD), and the results were expressed by the ratio of absorption value (A) between MC148 gene and beta-actin. The survival time of skin grafts in both groups was compared. Gross examination of grafted skin was carried out from 7 POD on in both groups, and the pathomorphological changes were examined in both groups on 7 POD.

RESULTSThe MC148 gene expression in rat skin of T group could be identified in 6 POH, and it reached the peak on 3 POD (A(MC148 mRNA) / A(beta_actin) = 0.86), and then subsided thereafter, but it maintained for 10 days. The survival time of the grafts in T group was (15.0 +/- 2.0) days, and it was significantly longer than that in C group (8.5 +/- 3.4) days, (P < 0.01). Gross and microscopic examination showed that the tail skin of T group appeared ruddy on 7 POD, with little leukocytic infiltration in subcutaneous tissue; it began to turn black after 12 to 20 PODs. On the other hand, the tail skin of C group began to turn black and to shed off on 7 POD, with evident leukocytic infiltration in subcutaneous tissue and dermis.

CONCLUSIONLocal transfection of MC148 gene may promote immunosuppression by inhibiting leukocytic infiltration after allogenous skin transplantation.
