1.The Electromechanical Characteristics of Left Atrial Patients with Hypertension Complicated with Paroxysmal Atrial Fibrillation

Objective To investigate the alteration of left atrial(LA) electromechanical characteristics of patients with hypertensivon and patients with hypertension complicated with paroxysmal atrial fibrillation(PAF). Methods By use of two-dimentional echocardiography and Doppler and tissue Doppler imaging(TDI) techniques,we mensurated and calculated the indexes representing LA structure and systolic and diastolic function and electromechanical characteristics in 30 nornal subjects(group1),25 patients with hypertension lasting less than ten years(group2),24 patients with hypertension lasting more than ten years(group3) and patients with hypertension complicated PAF(group4). Indexes representing LA electromechanical characteristics consisted of the difference in time intervals between the QRS onset and the tricuspid A wave,and the QRS onset and the mitral A wave (DQAT),electromechanical time (P-A),electromechanical time dispersion(P-Ad),the maximum P wave duration,the minimum P wave duration,and P wave dispersion (Pd=Pmax-Pmin) calculated from 12-lead surface electrocardiograms. Results The P-A3 in group 4 was much longer than that in group 1,group 2 and group 3. The P-Ad2 in group 4 was much longer than that in group 1,group 2 and group 3.Conclusion There are prolonged P-A3 and P-Ad2 in patients with hypertension complicated PAF. Some indexes,such as P-A3 and P-Ad2,perhaps can predict the onset of PAF in patients with essential hypertension better.
2.Expression profile of microRNAs in lung adenocarcinoma tissue and its expression in CD 133+ lung cancer cells

Objective To study the expression profile of microRNAs in the lung adenocarcinoma tissue and its expression in CD133+ lung cancer cells .Methods The specimens of adenocarcinoma tissue and pericarcinomatous tissue were collected for con-ducting the microRNA expression chip analysis .The flow cytometry(FCM ) was adopted to sort and culture CD133+ A549 cells in serum-free medium for 2 weeks .The positive rate of CD133+CD44+ in those cells was assayed .The expression of miRNA-892b and miRNA-4686 in CD133+ A549 cells were detected by real-time qPCR .Results Compared with the pericarcinomatous tissue ,27 miRNAs expressions in the lung adenocarcinoma tissue were up-regulated(P<0 .05) and 6 miRNAs expressions were down-regula-ted(P<0 .05) .The positive rate of cancer stem like cells CD 133+CD44+ was 75 .0% .The expression of miRNA-892b and miRNA-4686 in CD133+ A549 cells was higher than A549 cells(P<0 .01) .Conclusion Abnormal expression profile of miRNAs in lung ad-enocarcinoma tissue ,and miRNA-892b and miRNA-4686 may play a certain role to maintain the biological characteristics of CD133+ lung cancer cells .
3.Therapeutic Observation of Different Medicinal Forms of Hydro-medicinal Cupping for Discogenic Low Back Pain

Objective To observe the clinical efficacy of hydro-medicinal cupping using Chinese medicinal granules in treating discogenic low back pain. Methods Sixty patients with discogenic low back pain were randomized into a treatment group and a control group, 30 in each group. The two groups were both intervened by hydro-medicinal cupping at Yaoyangguan (GV 3) and two sides of the point, while the Chinese medicinal granules were used in the treatment group and the conventional form of medicine was used in the control group. The pain score was observed before treatment and after 4 treatment courses, and the clinical efficacies were also compared. Results The pain scores were significantly changed after intervention in both groups (P<0.01). After intervention, there was no significant difference in comparing pain score between the two groups (P>0.05). The total effective rate was 93.3%in the treatment group versus 96.7% in the control group, and the difference was statistically insignificant (P>0.05). Conclusions Hydro-medicinal cupping is an effective approach in treating discogenic low back pain, and there is no marked difference in efficacy between the Chinese medicinal granules and conventional form of medicine.
4.Preliminary study of MR elastography for differentiating hepatic focal benign and malignant tumors

Objective To explore the diagnostic value of MRE for differentiating hepatic benign and malignant tumors.Methods 36 patients with liver tumor (a total of 39 lesions,including 20 hepatocellular carcinomas,7 hemangiomas,5 cholangiocellular carcinomas,3 metastases,2 hepatic angiomyolipomas,1 carcinosarcoma,1 castleman’s disease)and 9 healthy volunteers were evaluated with MRE.The elastogram were generated with FUNCTOOL post processing program.The mean value of elasticity of hepatic malignant tumors,hepatic benign tumors,hepatic parenchyma around the malignant tumors,hepatic parenchyma around the benign tumors and the normal liver of healthy volunteers were measured and compared.Results The mean value of elasticity of malignant tumors [(7.39±1.70)kPa]was significantly higher than these of benign tumors [(4.11±0.37)kPa,P < 0.001],peripheral parenchyma around the malignant tumors [(3.50±0.73)kPa,P < 0.001],peripheral parenchyma around the benign tumors [(2.61±0.45)kPa,P < 0.001] and normal liver of healthy volunteers [(2.38±0.24)kPa,P <0.001].The mean value of elasticity of parenchyma around the malignant tumors [(3.50±0.73)kPa]was significantly higher than that of hepatic parenchyma around the benign tumors [(2.61 ± 0.45)kPa,P <0.001].The mean value of elasticity of hepatic parenchyma around the benign tumors [(2.61±0.45)kPa]was slightly higher than normal liver of healthy volunteers [(2.38±0.24)kPa],and there was no significant difference between the two (P >0.05).A cutoff value of 5.08 kPa can accurately differentiate malignant tumors from benign tumors and normal liver parenchyma.Conclusion MRE could be used in diagnosis of hepatic focal tumors,which is helpful for differentiating benign and malignant liver tumors.
5.Study on correlation between plasma homocysteine levels and TCM syndromes of dementia

Objective To investigate the relationship between plasma homocysteine (Hcy) levels and TCM syndromes of dementia.Methods 250 cases were divided into a dementia group (120 cases) and non-dementia group (130 cases).120 cases in the dementia group include 36 Alzheimer dementia(AD) patients and 84 vascular dementia (VaD) patients.The plasma levels of Hcy were measured in all 250 cases.The relationship between Hcy levels,TCM syndromes and dementia was analyzed.Results The level of plasma Hcy was significantly higher in the dementia group (17.13 ± 9.01) nmol/ml than that in the non-dementia group (12.81 ± 7.94) nmol/ml,and it was highest in the VaD patients (P<0.01).The mean of Hcy concentration in obstruction of phlem-turbid syndrome was higher(18.43±7.81)nmol/ml than syndromes [Tanyuzuluo (5.38 ± 8.91)nmol/ml,Shenxusuikui (16.44 ± 9.05)nmol/ml,Shenyinkuixu(16.19 ± 9.24)nmol/ml,Ganjinghuowang (15.92 ± 7.28)nmol/m1,Pishen-Yangxu(12.26 ± 5.17) nmol/ml (P < 0.01)].Conclusion Increased plasma Hcy levels may be one of the risk factors leading to AD or VaD.
6.The physician training of interventional treatment of coronary heart disease in training base sponsored by National Health and Family Planning Commission

[Abstrct] In 2008, Beijing Chaoyang Hospital of Capital Medical University was certified as cardiovascular interventional therapy tralning base of National Health and Family Planning Commission (former Ministry of Health). In accordance with the requirements of the measures for the management of tralning base, and according to the characteristics of tralning students in combination with the clinical teaching experience, skills of teaching for tralning base were summed up as the following: in strict accordance with the syllabus requirements , systematic and standardized tralning were emphasized . Emphasis on theoretical study which combined closely with actual operation was also established. After the tralning, all students who received tralning can do coronary angiography and PCI for simple lesion in accordance with the requirements of the syllabus. All tralnees passed unified national exam organized by the Ministry of Health and got the qualification for interventional therapy.
7.The impact of different doses of atorvastatin on plasma endothelin and platelet function in acute ST-segment elevation myocardial infarction after emergency percutaneous coronary intervention

Objective To investigate the effects of different doses of atorvastatin on plasma endothelin and platelet function in acute ST-segment elevation myocardial infarction (STEMI) patients after emergency percutaneous coronary intervention (PCI).Methods A total of 120 patients with acute STEMI treated with emergency PCI were enrolled and randomly divided into 20 mg of atorvastatin treatment group (standard group,n =60),and 40 mg of atorvastatin treatment group (intensive group,n =60).The blood C reactive protein (CRP),blood lipid profiles,plasma endothelin (ET) were measured before atorvastatin treatment and after 7 days of treatment,respectively.The platelet fibrin clot strength induced by ADP (MAADP) was determined by thrombelastography (TEG).Results Seven days after of atorvastatin treatment,the level of plasma ET in intensive group was significantly lower than that in standard group [(0.49 ± 0.21) pmol/L vs (0.63 ± 0.58) pmol/L,P < 0.05].Moreover,the MAADP in intensive group was significantly decreased compared with the standard group [(38.4 ± 17.4) mm vs (45.7 ± 14.5) mm,P < 0.05].There was a positive correlation between the ET level and MAADP in intensive group after treatment (r =0.378,P < 0.05).However,no significantly differences could be viewed in the CRP and LDL-C levels between the two groups (P > 0.05).Conclusion In patients with acute STEMI,early administration of 40 mg atorvastatin after emergency PCI could significantly reduce the vascular endothelial injury,improve endothelial function,and reduce the residual platelet activity.
8.Preliminary study of whole body MR diffusion weighted imaging in detecting pediatric primary and metastatic malignant tumor

Objective To assess the efficacy of whole body diffusion weighted imaging (WB-DWI) in detecting pediatric primary and metastatic malignant tumor. Methods WB-DWI was performed in 62 healthy pediatric volunteers and 40 pediatric patients with confirmed malignant tumors. The healthy volunteers were divided into three groups: 0 to 12 months, more than 12 months to 5 years and more than 5 to 15 years. The characteristics of WB-DWI imaging were analyzed. McNemar test was used to compare the difference of detection on metastasis between WB-DWI and WB-DWI combined with MRI, CT. The mean apparent diffusion coefficient ( ADC ) values of primary tumors and metastases were measured by using paired t test and compared with those of corresponding body regions of control group. Results WB-DWI imaging shows that signal intensity of metaphysis gradually reduces with increasing age in the normal pediatric group. On WB-DWI primary malignant tumors showed 100% (40/40) high signal intensity and metastases showed high signal intensity in 89.2% (58/65) on WB-DWI, with a positive predictive value of 90. 6% (58/64). The detecting rate for metastases increased to 95.4% (62/65) when WB-DWI was combined with MRL/CT, with a positive predictive value of 95.4% (62/65) there was no statistically significant difference ( x2 = 2. 25, P > 0. 05 ). The ADC values of primary malignant tumor sites in head ( n = 5), liver(n=6), kidney(n=8), adrenal(n=ll) were (0.76 ±0. 19) ×10-3 , (0. 97 ±0.29) × 10-3,(0. 81 ±0. 12) × 10-3 and (0. 93 ±0. 28) × 10-3mm2/s and those of corresponding body regions of control group were (1.02 ±0. 11) × 10-3,(1.57 ±0.58) × 10-3, (1.19 ±0. 15) × 10-3 and (2.03 ±0.42) ×10-3mm2/s respectively, there were statistically significant difference( t values were 3.54,3. 84,7. 02 and 12. 57 ;P < 0. 05 ). The A DC values of metastases sites in head ( n = 9 ), liver ( n = 13 ), kidney ( n = 17 ),bone(n =7) and lymph node(n =6) were (0. 88 ±0. 12) × 10-3, (0. 98 ±0. 10) × 10-3, (0. 89 ±0. 11 ) × 10-3, (0. 96 ±0. 15) × 10-3 and (0. 83 ±0. 14) × 10-3mm2/s, and those of corresponding body regions of control group were (1.01 ±0.09) × 10-3, (1.45 ±0.39) × 10-3, ( 1.31 ±0.27) × 10-3, ( 1.34 ±0. 20) × 10 -3 and ( 0. 99 ± 0. 08 ) × 10 -3 mm2/s, there were statistically significant difference ( t values 4. 09,45.50,6. 95,14. 00 and 9. 27 ;P < 0. 05 ). Conclusions Increased signal intensity is more frequently observed in metaphysis of long bone in normal children on WB-DWI. With a high detection rate for primary and metastatic malignant tumors, WB-DWI combined with conventional CT; MRI can significantly improve their sensitivity.
9.Observations on Characteristic Decoction Cup Hot Compress Dose

Objective To administer a lower dose according to different indoor temperatures to give play to the effect of characteristic decoction cup hot compress and be able to save the cost of medicine. Method Twenty subjects were allocated, according to different indoor temperatures, to four groups, 5 cases each. Each subject received 10 sessions of decoction cupping with different doses. The decoction in all the cups was 45℃ before cupping. The temperature of decoction in the cups was measured after cupping. The average value of temperatures of the same doses at the same indoor temperature was calculated after cupping. The graph of average temperature curve of decoction in the cups was produced according to the indoor temperature after cupping with different doses. Finally obtained were the lowest doses of decoction with hot compress effect in the cup required at different indoor temperatures. Result To produce the effect of characteristic decoction cup hot compress, the decoction in the cup must be more than 55 mL in a indoor temperature of 10℃~14℃, more than 45 mL in a indoor temperature of 15℃~19℃ and more than 25 mL in a indoor temperature of 20℃~24℃. Only 20 mL of decoction was needed in indoor temperatures above 25℃. Conclusion A dose of decoction is flexibly selected according to a seasonal change in indoor temperature. That can not only have the effect of hot compress but also save the dosage.
10.Innovation of characteristic medicinal cupping devices.

OBJECTIVETo compare the differences in the characteristic medicinal cupping therapy between the traditional cupping device and the innovated cupping device.
METHODSFifty patients of neck and low back pain were selected. The self-comparison was adopted. The cupping therapy was applied to the acupoints located on the left or right side with the traditional cupping device and the innovated cupping device. The cupping sites were centered at bilateral Quyuan (SI 13) and Dachangshu (BL 25). The cups were retained for 10 min. The traditional cupping device was the glass with smooth border, 100mL. The innovated cupping device was the vacuum-sucking cup. The operative time, medicinal leakage, comfort and cupping marks were observed for the two different cupping devices.
RESULTSThe operative time with the innovated medicinal cupping device was shorter obviously compared with the traditional one at Quyuan (SI 13) and Dachangshu (BL 25, both P<0. 05). The comfort with the innovated medicinal cupping device was remarkably improved as compared with the traditional one at the two acupoints (both P<0. 05). The medicinal leakage was similar between the two different devices during the cupping operation (both P>0. 05). The cupping marks with the innovated medicinal cupping device were much deeper than those with the traditional one after cupping therapy.
CONCLUSIONThe innovated cupping device is more convenent and comfortable in operation during the characteristic medicinal cupping therapy.
