1.Assessment of wall motion in myocardial infarction patients with Doppler tissue tracking quantitative score

Objective To measure various parameters with tissue Doppler imaging, quantitatively score left ventricular wall motion, assess longitudinal wall motion in total and regional myocardium of myocardial infarction patients. Methods Tissue tracking was adopted for assessing left ventricular regional myocardium of 30 myocardial infarction patients (MI group) and 30 normal subjects (control group). After collecting dynamic images and analyzing information off-line, maximum displacement along long axis asynchronously in annular, basal, mid and apical segments of different walls were acquired, and left ventricular wall motion score indexes(TT score indexes) were calculated. Results Maximum displacement in different segments of left ventricular regional myocardium was lower in MI group than control group, especially in infarcted segments. 2.Assess dyssynchrony in left bundle branch block by tissue Doppler imaging

Objective To observe longitudinal motion characteristics of regional myocardium in left bundle branch block(LBBB) patients. Methods Left ventricular regional myocardium of 15 LBBB patients and 15 normal subjects were assessed by tissue Doppler imaging(TDI). Peak velocities(V_S,V_E,V_A) during isovolumic contraction, ejection, isovolumic relaxation, early relaxation, atrial systole phases and maximum time velocity integral(TVI_(max)), displacement(D_(max)) in mitral(tricuspidal) annulets of different walls were measured. Furthermore, the different parameters from the beginning of electrocardiogram QRS wave to the beginning of S wave(Q-Sb),to the tip of S wave(TTP), the acceleration time of IVC wave(IVA), the acceleration time of the E wave(Eac),the durations of different phases(IVC,S,IVR,D)and IVC/S and others were acquired. Results Peak velocities(V_S,V_E)and TVI_(max) in left ventricular septal, inferior, anterior, posterior walls were lower in LBBB group than control group; those parameters in different segments of left and right ventricular lateral myocardium were not significantly lower in LBBB group than control group. What is more, D_(max) of mitral annular sites in LBBB group were lower than control group. Q-Sb, TTP and IVC in left ventricular septal, inferior, anterior, posterior walls were longer in LBBB group than in control group; IVA lengthened, Eac and Edc shortened in left ventricular septal, inferior walls; IVR lengthened, diastole shortened, IVC/S increased in left ventricular septal, inferior, and posterior walls. Conclusions Abnormal left ventricular activation sequence during LBBB caused obvious impairment in total and regional systolic and diastolic function. And delayed contraction with different degrees were resulted from different walls of left ventricle with intraventricular dyssynchrony; but contraction in right ventricle was not significantly delayed, with interventricular dyssynchrony.
3.C/EBP? is involved in transcriptional regulation of mVGLUT2 gene expression

AIM:The main purpose of this study is to investigate the regulatory role of C/EBP? in mouse vesicular glutamate transporter 2(mVGLUT2) gene expression. METHODS:The promoter region of mVGLUT2 was cloned to PGL3-basic vector. Site-direction mutation was used to identify the CCAAT-enhancer-binding protein ?(C/EBP?) binding site. The promoter activity was observed by luciferase system. The binding between C/EBP? protein and mVGLTU2 promoter region was determined by EMSA. The C/EBP? gene expression was inhibited by its specific siRNA. RESULTS:mVGLUT2 promoter activity decreased about 50% after mutation of C/EBP? binding site. EMSA showed that C/EBP? protein bound onto mVGLUT2 promoter region. Meanwhile,when C/EBP? gene expression was inhibited by its specific siRNA,mVGLUT2 promoter activity,mRNA level and protein level were decreased about 60%,40% and 45%,respectively. CONCLUSION:C/EBP? is involved in the regulation of mVGLUT2 gene expression.
4.Job characteristics of primary and secondary school teachers between mainland China and Hong Kong

Objective:To discuss the situation and differences of primary and secondary school teachers'job characteristics between mainland China and Hong Kong in order to provide the basis for improvement of job charac-teristics.Methods:A total of 504 teachers (242 in mainland China and 262 in Hong Kong)were chosen to com-plete the Job Characteristics Scale of Primary and Secondary School Teachers (including job demands sub-question-naire of six factors and job resources sub-questionnaire of six factors).Results:Teachers'job demands and job re-sources scores in both two areas were higher than the median 3.The scores of workload,emotional demands, student's misbehavior,role stress,and hard ware conditions,autonomy,work reward of Hong Kong teachers were higher than mainland China teachers(Ps <0.05),while the coworker support score was opposite.In addition,there were gender difference in workload and occupation moral requirements in Mainland China (Ps <0.05),as well as work age difference in workload,occupation moral requirements,organizational support,coworker support,hardware conditions and work reward,while school stage difference in organizational support,hardware conditions and work reward in Hong Kong (Ps <0.05).Conclusion:It suggests that teachers'job characteristics are both high demands-high resourcestype in the two areas,but the highdegree and the demographic characteristics are different.

6.Assessing longitudinal wall motion characteristics at different phases in regional myocardial in patients with myocardial infarction by quantitative tissue velocity imaging

BACKGROUND: At present, Doppler tissue imaging can be used extensively to make a quantitative assessment of left ventricular regional myocardial function. Postsystolic shortening is a delayed relaxation in regional myocardium following acute ischemia.OBJECTIVE: To observe longitudinal wall motion characteristics in left ventricular regional myocardiun and evaluate the clinical significance of postsystolic shortening in isovolumic relaxation period.DESIGN: Case-control observation.SETTING: Heart Center of Beijing Chaoyang Hospital.PARTICIPANTS: We selected 30 myocardial infarction inpatients hospitalized in the Heart Center of Beijing Chaoyang Hospital between April 2003 and September 2003 as myocardial infarction group; another 30 patients with non-cardiac diseases were set as control group. Subjects of the two groups volunteered in the experiment.METHODS: Dynamic images were collected and information was analyzed off-line. Left ventricular myocardial velocity and time velocity integral profiles were acquired along long axis asynchronously in basal and mid- segments of different walls. Peak velocity of isovolumic contraction period (VIC), ejection period (Ye), isovolumie relaxation period (VIR), rapid filling period (VRF) and atrial systole (VAS) and maximum time velocity integral (TVIMAX) in regional myocardium were measured.Doppler in different segments at longitudinal wall of myocardium in the the two groups.RESULTS: Thirty patients were recruited in myocardial infarction group and control group separately and all of them entered the result analysis ocardium was lower in myocardial infarction group than in control group,especially in infarcted segments, suggesting that the function of cardiac contraction and relaxation in patients with myocardial infarction was demyocardial isovolumic relaxation in myocardial infarction group, indicating period and isovolumic relaxation period in TVIMAX in myocardial infarction group.mal movement at isovolumic relaxation phase suggests the presence of postsystolic shortening and abnormal wall motion in regional myocardium.chemic myocardium.
7.Metabolomics characteristics in a rat model of myocardial infarction based on bibiometrics analyses

BACKGROUND:Metabonomics has been proved to analyze and observe the pathological process of rat myocardial infarction and the underlying mechanism. OBJECTIVE:To further analyze the metabolomic pathways of bioinformatics in rat models of myocardial infarction. METHODS:The experimental studies about rat myocardial infarction were retrieved from CNKI, WanFang, CqVip, PubMed and Embase databases. The metabolic products described in the literatures were col ected and summarized. Signaling pathways were analyzed using KEGG database molecular function annotation, the enzymes, translocators and their properties were analyzed by HMDB database. Metabolites pathway were visualized with MetPA. RESULTS AND CONSLUSION:A total of 26 metabolic products were identified in the included literatures and mainly participated in 29 metabolic pathways. Through topology analysis, 5 of the 10 metabolic pathways were selected and regarded as the metabolic pathways of myocardial infarction in rats, including aminoacyl-tRNA biosynthesis;glycine, serine and threonine metabolism;valine, leucine and isoleucine biosynthesis;biosynthesis of unsaturated fatty acids;phenylalanine, tyrosine and tryptophan biosynthesis. In conclusion, the bioinformatics analysis of metabolites in rats with myocardial infarction show that myocardial infarction is related to the metabolism and metabolic pathways of carbohydrates, proteins, fat and RNA.
8.Complete video-assisted thoracoscopes lobectomy for patients with non-small cell lung cancer

Objective To summarize preliminary experiences on complete video-assisted thoracoscopes lobectomy for patients with non-small cell lung cancer.Methods From October 2009 to March 2012,42 patients with non-small cell lung cancer were treated with complete video-assisted thoracoscopes lobectomy.Tumors located in the left upper lobectomy in 7 cases,10 cases of left lower lobectomy,9 cases of upper right lung,4 cases of right lung,12 cases of lower right lobectomy.Preoperative cTNM stage was Ⅰ-Ⅱ (T1N0M0-T2N1M0),the size of tumor was < 5 cm,no obvious lung and mediastinal lymph node enlargement or pleura hypertrophy.Results All operations were successful.There were (9.5 ± 3.2) pieces of lymph nodes removed.Nz lymph node cleaning was more than 3 groups,the average was 3.3 groups.The operation time was 100-400 (220 ± 37) min,blood loss was 120-700 (150 ± 63) ml,duration of drainage was 3-12(4.5 ± 2.1) d.The postoperative hospital stay was 9-31 (12.2 ± 5.0) d.Conclusion Video-assisted thoracoscopes lobectomy is technically feasible and safe,but the operation indications should be paid special attention.
9.Application of vascularized anterolateral thigh perforator flap harvested with free-style approach in oral and maxillofacial reconstruction

Objective To assess the harvest method and application value of free-style anterolateral thigh perforator flap (ALTPF) in oral-maxillofacial reconstruction.Methods Fifty-three patients who suffered from oral and maxillofacial tumor underwent simultaneous reconstruction using free-style ALTPFs after radical resection from August,2013 to May,2014.Perforators of the ALTPF region were marked through hand-held Doppler probe preoperatively.Flaps were designed centered on perforators according to the defect size of the accepting site.Incisions were first made at the inner border of the designed flap.Perforators were exposed above the fascia lata femoris and then dissected retrogradely through the vastus lateralis muscle to harvest a vascular pedicle with desired caliber and length.Flap thinning was applied under microscope in some cases to compromise the need of the accepting site.Results All 53 flaps survived after transplantation while skin exfoliation occurred in 5 cases due to flap thinning.Four cases sustained partial necrosis and was cured by trimming and dressing changes.Five to 14 months' post-operative followup showed satisfactory accepting-site morphology with good speech function and swallowing recovery.All donor sites were closed primarily without skin-grafting,leaving no donor-site complications including incision disruption,scar hyperplasia and muscle strength degeneration of the lower limb.Conclusion Multiple perforators have been accu rately located preoperatively in free-style harvest approach of ALTPFs,thus optimal effects can be reached with decreased donor-site morbidity and improved aesthetic outcome to the uttermost,which accords with the refined,personalized and minimal invasive development concept of modem reconstructive surgery.
10.Management of difficult recipient vessel preparation in microsurgical reconstruction for oral and maxillofacial defects with free flaps

Objective To investigate the management of difficult recipient vessel preparation in microsurgical reconstruction for oral and maxillofacial defects with free flaps and to assess the clinical outcome.Methods A total number of 739 patients with oral and maxillofacial defects underwent consecutive free flap reconstruction with 761 free flaps from May,2012 to May,2015.There were 37 patients who could not find or lack of proper recipient vessels for microvascular anastomosis during operation.Among them,22 were recurrent oral cancer after tumor ablation,10 of them undrewent post-operative radiotherapy;5 were second primary oral cancer,7 diagnosed with osteoradionecrosis,and 3 suffered from oral and maxillofacial defect and deformity caused by trauma or inflammation.Forty free flaps including 23 anterolateral thigh flaps (ALT),10 fibular flaps and 7 radial forearm flaps were harvested.Methods and techniques used during the operation,instant patency rate after anastomosis,the overall survival rate of free flaps,and post-operative complications were recorded.Results Ninety-one anastomoses were performed between 87 pairs of vessels in 37 patients.Fifty recepient vessels were located on ipsilateral side of neck,and the most frequently used recipient vessels were those preserved or not being damaged in former operation and radiation,former transferred free flap vascular pedicles and residual ends of the ligated vessels.Thirty-seven recepient vessels were found on the contralateral side of neck.Thirty-one cases of long-pedicle flap harvesting,2 cases of vein and artery grafting,4 cases of vessel transposition,5 cases of phleboplasties,9 cases of end-to-side anastomoses,and 1 case of flow-through technique were applied in recipient vessels preparation alone or in combination.The patency rate of anastomosis during operation was 100% and the overall survival rate of free flaps was 97.5%.Conclusion Recurrent oral cancer after tumor ablation,second primary oral cancer,osteoradionecrosis and deformity caused by trauma and inflammation are the main reasons of unsuitable recipient vessel conditions in microsurgical reconstruction for oral and maxillofacial defects.Methods including long-pedicle flap harvesting,venous grafting,vessel transposition,phleboplasty,end-to-side anastomosis and flow-through technique applied alone or in combination are still reliable choices for management of neck difficult recipient vessel preparation and anastomosis.
