Journal of Chinese Physician 2001;0(04):-
Objective To analyze the relationship between the chemotherapeutic efficacy and VEGF expression in the patients with oral squamous cell carcinoma. Methods 51 cases with oral squamous cell carcinoma who received preoperative chemotherapy of a total 80mg pingyangmycin were enrolled in this study. The relationship among the chemotherapeutic effect, VEGF expression, fever, and the duration of fever was analyzed using linear correlation, stepwise regression and student t-test. Results VEGF expression in the cancer cells of 92.1%(47/51) patients decreased after preoperative chemotherapy, and the decreased expression of VEGF was not significantly correlated with chemotherapentic efficacy. But the chemotherapeutic effect was closely related with fever degree in the early stage of chemotherapy. Conclusion There was no significant relationship between chemotherapeutic effects and VEGF expression in oral squamous cell carcinoma. Fever in early stage of chemotherapy may serve as an index for evaluating the patient prognosis.

Objective To investigate the arteries supplying the temporalis. Methods Ten cadavers were examined after embalming with a fluid containing 10% formaldehyde. For detailed examination of the supplying arteries of the temporalis, a mixture of latex and barium sulfate in a ratio of 1∶1 was injected into both carotid arteries of 10 fresh cadavers. The skin and soft tissue were removed from the temporal and lateral regions of the head, and the superficial temporal artery was dissected. As soon as the origin of the medial temporal artery was identified, the zygomatic arch was cut and lifted. The maxillary artery with its branches of the infratemporal region was carefully dissected. After identification of all vessels of the infratemporal region, the temporalis was removed, and then radiographic examination of the injected arteries was performed using 2.5 mA and 40 kV. The injected arteries were clearly identified on the radiographic images, and the area supplied by each artery was determined. Planimetric evaluation of these areas was performed. Results In all specimens, three arteries supplying the temporalis were identified: the anterior deep temporal artery that supplied the anterior part between 11 % and 25% of the temporalis with a mean of 17%, the posterior deep temporal artery that supplied the central part of 27% to 50% of the lateral aspect of the temporalis, with a mean of 38% and a branch of the superficial temporal artery, the medial temporal artery, which supplied the posterior part and in 25% the posterior and the upper part of the temporalis, resulting in 30% to 58% of the muscle's lateral aspect, with a mean of 45%. Conclusion According to a detailed description of the arterial supply of the temporalis, two different pedicled flaps using the anterior or central part of the temporalis can be selectively used for reconstructive surgery.

Objective To report the experience on the modified Abb? flap for correction of mild tightness of the corrected cleft lip deformity. Methods Forty-two patients with a mild or moderately tight lip deformity after primary unilateral cleft lip repair were performed a modified Abb? flap for correcting tight upper lip. The operative technique was that the pocket was creased for the vermilion tubercle by splitting the lip. The skin was incised several millimeters beyond the vermilion-skin border and was moved laterally for a distance equal to the breadth of the vermilion tubercle. The orbicularis oris muscle and the full length of the upper lip vermilion were incised. By releasing the tension, the slight concavity, expressed in profile by the slight pout which a normal upper lip had in cross section, was obtained. The Abb? flap that was taken from the central portion of the lower lip vermilion, was designed to repair the vermilion tubercle and the Cupid's bow. The flap was approximately 8 mm in width. The full length of the lower lip vermilion and the orbicularis oris muscle were incorporated. A ting portion of skin was included, which facilitated closure of the donor site, The flap, based on the labial vessels, was rotated 180?and sutured into the created defect of the upper lip. The pedicle was divided 10 days after operation. Results Forty-two patients all showed an obvious Cupid's bow, Cupid's bow peak, the median tubercle and the height and width of the upper lip. The scarring of the base of the nose was not obvious. Conclusion Use of modified Abb? flap to reconstruct the contour of the upper lip is successful in patients with cleft lip.
