1.Expression and clinical significance of PTEN, Fas and DCR3 in gastric cancer

Objective To study the expression of PTEN,Fas and DCR3 in human gastric cancer and evaluate their effects on carcinogenesis,cell proliferation,invasion and metastasis of gastric carcinoma,as well as clinical significance.Methods The expression of PTEN,Fas and DCR3 were detected by immunohistochemical method (streptavidin-peroxidase) in the tissues of 75 primary gastric carcinomas and 15 gastric ulcers.Results The positive expression rate of DCR3 in gastric carcinoma tissues was significantly higher than that in normal tissues,and the expression of PTEN and Fas was obviously lower than that in normal tissues (P<0.01).The expression of DCR3 was negatively correlated with Fas and PTEN respectively (r=-0.720,P<0.001; r=-0.336,P<0.001).There was a positive correlation between PTEN and Fas.The expression of PTEN,Fas and DCR3 was closely correlated with lymph node metastasis,clinical stage,histological differentiation grade and tumor depth of gastric cancer,but not related to sex,age and tumor size (P>0.05).Conclusion PTEN,DCR3 and Fas have mutual restrictive relationships in the process of invasion and metastasis in gastric cancer.Detecting the expression of PTEN,Fas and DCR3 in gastric cancer is helpful to judge the malignant biological behavior of gastric cancer at a different angle,which may be considered as reliability index in clinic diagnosis,judging curative effect and monitoring the prognosis of gastric carcinoma.

5.Short-term clinical effect of anterolateral minimally invasive approach of total hip replacement

Objective To investigate short-term clinical effect of anterolateral minimally invasive approach of total hip replacement.Methods A total of 35 patients treated by total hip replacement was divided into A and B groups.Group A used anterolateral minimally invasive approach,and group B used standard posterior Moore approach.The clinical data of operation time,amount of bleeding,blood transfusion volume,component position,Harris hip score,and complications of two groups were statistically compared.Results In terms of operation time,group A had certain disadvantages than group B.No significant difference was found between two groups in terms of operation time,annount of bleeding,blood transfusion volume,component position,Harris hip score,and complications.Conclusions In total hip replacement,anterolateral minimally invasive approach was safe and effective compared to standard posterior Moore approach.Doctors should choose their familiar surgical approach in operation.
6.Role of X-box Binding Protein 1 in Acrolein-induced Oxidative Injury of Retinal Pigment Epithelium

[Objective]The oxidative injury of retinal pigment epithelium(RPE)plays a key role in the pathogenesis of age-related macular degeneration(ARMD). This study is to investigate the effects of endoplasmic reticulum stress and the vital transcriptional factor X-box binding protein 1(XBP1)in acrolein-induced oxidative damage of RPE.[Methods]RPE cells were treated with acrolein (75μmol/L)for 2~24 h,expression of glucose regulated protein 78(GRP78)and XBP1 was determined by Western blot analysis. After being transfected with XBP1 siRNA with 24 h,the expression of XBP1 was knocked-down in RPE cells. Protein level of Nrf2 and SOD2 was then determined by Western blot analysis and intracellular Reactive Oxygen Species(ROS)generation was determined by DCF staining. Acrolein was added for 8 h after being transfected with XBP1 siRNA or control siRNA for 24 h. Apoptosis was detected by TUNEL assay before and after the treatment. Subretinal injection of Cre or GFP adenovirus was performed in XBP 1flox mice. After 1 week the mice were sacrificed. Total RNA was extracted from mice eyecups using TRIzol and real-time RT-PCR was performed to determine the two XBP1 down-stream genes,ERdj4 and p58IPK. Cryosectioning and immunofluorescent staining were performed to look at the expression of XBP1,Nrf2 and SOD2 in mice RPE.[Results]Protein level of GRP78 was significantly un-regulated after exposure to acrolein for 2 and 4 h. XBP1 was activated after acrolein treatment for 6 h. Knock-down of XBP1 by siRNA down-regu?lates anti-oxidant genes expression and increased ROS generation in RPE cells. Loss of XBP1 exacerbates acrolein-induced cell apoptosis. XBP1 was knocked-down in the RPE of XBP1flox mice after subretinal injection of Cre adenovirus. Decreased mRNA level of ERdj4 and p58IPK,and decreased Nrf2 and SOD2 expression were seen in the Cre-injected group.[Conclusions]Acrolein induces ER stress and activates XBP1 in RPE cells. Knock-down of XBP1 down-regulates anti-oxidant genes expression ,increases ROS generation,and exacerbates acrolein-induced cell apoptosis. XBP1 plays a role in the anti-oxidant defense in the RPE cells.
7.Application of run-through wire stretch technique in the procedure of endovascular therapy for complex subclavian artery occlusive diseases

Objective To discuss the application of run?through wire stretch technique in the procedure of endovascular therapy for complex subclavian artery occlusive diseases. Methods The clinical data of 22 patients who were diagnosed complex subclavian artery occlusive diseases were retrospectively analyzed from June 2013 to January 2016 in vascular surgery department of Wangjing Hospital .The patients were treated endovascular therapy using run?through wire stretch technique in all procedures successfully. Results The complex subclavian artery occlusive diseases were including long segment occlusion of the left subclavian artery and ostial stenosis or occlusion of the right subclavian artery. The left subclavian artery occlusion patients was 16 and right subclavian artery ostial stenosis or occlusion was 6. The technical success rate was 100%. All patients were using combination of femoral artery and brachial artery approach. Run?through wire stretch technique was used in the procedure of endovascular therapy, which had improved the technical success rate. After the treatment the symptom improvement rate was 88%. The symptoms of upper limb and posterior circulation ischemia were improved. Conlusion Upper limb artery combined femoral artery approachand run?through wire stretch technique can improve the recanalization in the procedure of endovascular therapy for complex subclavian artery occlusive diseases.
8.Study of gender difference on perioperative hidden blood loss of intertrochanteric fracture

Objective To investigate the hidden blood loss of intertrochanteric fractures in different gender. Methods Data of 108 patients with intertrochanteric fractures who were admitted to the department of orthopaedics from January 2012 to De?cember 2015 were retrospectively analyzed. For all patients who were composed of 42 males and 66 females and whose ages ranged from 32 to 95 years old, the average age was 75.4±10.8 years old. With the equation of Gross, the amount of perioperative hidden blood loss (PHBL) was calculated as well as the fraction of PHBL. The lineal regression model was utilized to analyze the correlations between PHBL and those factors including age, time interval between injury and surgery, injured side, Evans classifi?cation of intertrochanteric fractures, patients' ASA grade, and type of anesthesia. The difference of hemoglobin (Hb), hematocrit (Hct), total blood volume, the amount of PHBL, and the percentage of PHBL were respectively compared between male group and female group. Results PHBL had no correlation of lineal regression with age, time interval between injury and surgery, injured side, Evans classification of intertrochanteric fractures, patients' ASA grade, or the type of anesthesia. The share of PHBL did not have correlation of lineal regression with age, time interval between injury and surgery, injured side, Evans classification of inter?trochanteric fractures, patients' ASA grade, or the type of anesthesia as well. Lineal regressive correlation only existed between gender factor and the PHBL fraction of total blood volume, and there was no regressive correlation between gender factor and the amount of PHBL. In the male group, the mean amount of preoperative Hb was 121.8±16.6 g/L and preoperative Hct was averagely 36.7%±5.0%;in the female group, the amount of preoperative Hb was 99.5±13.1 g/L on average and the mean amount of Hct was preoperatively 30.6%± 3.7%. Postoperatively, the mean amount of preoperative Hb was 103.4 ± 19.3 g/L and Hct was averagely 29.8%±4.2%;in the female group, the amount of postoperative Hb was 79.0±10.4 g/L on average and the mean amount of Hct was postoperatively 23.9%± 2.6%. Significant difference of perioperative Hb and Hct was found between two groups. The total blood volume was averagely 4 500.6±191.5 ml in the male group and 3 607.0±235.7 ml in the females group. There was statistical differ?ence between two groups. For male group, the amount of PHBL was 647.9±174.1 ml on average and accounted for 14.4%±4.0%of total blood volume. For the female group, the mean amount of PHBL was 694.8±216.5 ml and making up 19.3%±6.1%of the total blood volume. As for the amount of PHBL, no significant difference was observed between male group and female group, but there was statistical difference of the share of PHBL accounting for total blood volume between two groups. Conclusion Female pa?tients' PHBL fraction of total blood volume is greater than male patients', therefore females' PHBL conditions need to be paid more attention to and ought to be corrected in time.
9.Misdiagnoses and management of the vascular vagovagla reflexes induced by intervention therapy for pe-ripheral arterial diseases

Objective To investigate the misdiagnosed vascular vagovagla reflexes (VVR) and the treat-ments during interventional therapy for peripheral anerial disease (PAD). Methods The clinical data of 5 patients with VVRs from 128 PAD patients undergoing interventional therapy so as to analyse the causes for the misdiag-noses and then render pertinent treatment. Results The VVR in the 5 patients belonged to mixed type and all cas-es occurred after intervention and initially misdiagnosed. All patients were recovered well without adverse reaction. Conclusion Intervention therapy for PAD may be susceptible to VVR. Prompt management may bring in a good prognosis when VVR happens.
10.The influence of bFGF gene transfected BMSCs on inflammatory cytokines expression of COPD rat

Objective To study the influence of bFGF gene transfected bone marrow-derived mesenchymal stem cells (BMSCs) on the inflammatory cytokines of COPD rat. Methods The BMSCs were separated from SD rat and cultured and then bFGF gene was imported to BMSCs by liposome transfection method. The samples were prepared into six groups: normal control group, COPD group (A), BMSCs group (B), pcDNA3.1-BMSCs group (C), bFGF-pcDNA3.1-BMSCs group (D), and bFGF group (E). The expressions of TNF-α and IL-1β by QRT-PCR were detected. Results Compared with COPD group, TNF-α and IL-1β genes from groups B to D dropped significantly (P < 0.05). The changes of TNF-α and IL-1β genes among groups B to D showed no significant difference (P > 0.05). Conclusion BFGF transfected BMSCs, sample BMSCs and pcDNA3.1 transfected BMSCs can inhibit the expression of inflammatory cytokines of TNF-α and IL-1β, but there is no obvious advantage in comparison to bFGF transfected BMSCs and sample BMSCs in respect of inhibiting the expression of inflammatory cytokines of TNF-α and IL-1β.
