Chinese Journal of Medical Education Research 2013;(8):806-808
Current curriculum provision and teaching patterns of computer science in medical school has been far behind the requirements of clinical and research works, which restricted the develop-ment of medical talents to a large extent. According to the experiences of teaching practice,Department of Computer Science in Chongqing Medical University launched reform on medical computer science from aspects of teaching practice, curriculum provision, teaching methods and evaluation methods. Computer courses were opened according to students' computer knowledge and the profession. Task-based teaching and comprehensive evaluation methods were employed in order to improve the teaching quality.

Objective To investigate the significance of K-ras gene status and ras protein expression in immunophenotypic classification of gastric signet ring cell carcinoma. Methods The expression of ras protein in 180 cases of gastric signet-ring cell carcinoma was detected by tissue microarray immunohistochemistry.Meanwhile, the mutation in codon 12,13 of K-ras gene was determined by using PCR-based DNA direct sequencing analysis. Results The rate of ras protein expression was 27.8%.The rate of ras protein expression in intestinal phenotype was significantly higher than those in gastric and gastrointestinal phenotypes(P<0.05).The rate of ras protein expression in cases with lymph node metastasis was significantly higher than those in cases without nodal involvement(P<0.05).The rate of ras protein expression was significantly higher in cases with deeper invasion(P<0.05).The frequency of K-ras gene mutation was 22(12.2%).All of them were found in codon 12.The types of mutation included GGT→AGT(1 case),GGT→TGT(1 case),GGT→GCT(2 cases),GGT→GTT(8 cases)and GGT→GAT(10 cases).K-ras mutation was significantly associated with intestinal phenotype(P<0.05).The rates of ras protein expression in cases with mutational type of K-ras gene was higher than those in cases with wild type(P<0.05).The ras protein expression was positively associated with K-ras gene mutation(r=0.61,P<0.05).Conclusion The ras protein expression is correlated with nodal involvement and invasion.K-ras gene mutation and expression of ras protein is related to phenotypic classification, and they might influence the phenotypic transformation in gastric signet ring cell carcinoma.

Objective To investigate the resistance to fluoroquinolones and amikacin in escherichia coti isolates. Methods ESBLs-producers were detected by CLSI phenotypic confirmatory test and susceptibilities were tested by agar dilution method. Results 33. 8% of 361 isolates were ESBLs-producers. The resistant rate of ciprofloxacin, levofloxacin, amikaxin was 93.4%, 90.9%, 13.1% in ESBLs-produeers, and that was 69.5%,68.6 %, 19.7 % in non-ESBLs-producers, respectively. Conclusion Amikaxin is active against most escherichia coli isolates; those isolates are highly resistant to fluoroquinolones, especially, in ESBLs-producers.

Objective To investigate the clinicopathological and immunophenotype features of hemangiosarcoma. Methods The clinical, pathologic features of 5 cases of hemangiosarcoma, admitted in our hospital during 2006-2014,were retrospectively analyzed. Results Tumors were located in the spleen (n =3),liver(n =1),scalp(n =1),tumor metastasis to the lung or marrow was found in 4 cases.Preoperative misdiagnosis as lymphoma, cavernous haemangiomas or parasite cyst was suggested by ultrasonography or CT,and final diagnosis was established by typical pathological pictures.CD34,CD31,FⅧ and Vim were positive in tumor cells while CK,EMA were negative. The survival time of the patients receiving resection only was 2 days,4 months and 13 months. The survival time of the patients receiving operation plus chemotherapy was 6 months or 48 months. Conclusions Hemangiosarcoma is a rare and aggressive malignant tumor of soft tissue which has poor prognosis. The additional survival benefits provided by chemotherapy remains unproven.

Objective To study the feature of immunoglobulin complementarity determining region 3 (CDR3) gene in neonates of different gestational age (GA), and the effect of neonatal maturity on the diversity of CDR3 nucleotide sequence. Methods DNA were extracted from cord blood of 10 neonates of very immature (25-30 weeks), 12 immature (31-36 weeks), and 11 mature (37-41 weeks). CDR3 sequence was amplified suing nested PCR technique then cloned and sequenced. Results 1. There was a CDR3 length of 29.4±7.8, 32.4±9.2 and 40.8±10.7bp, including NDN length of 13.5±5.6, 16.1±7.8 and 22.0±8.5bp respectively in very immature, immature and mature neonates. 2. DP73 and DP75 were preferentially used in very immature and immature neonates, VH5, DP73 and DP75 were used in mature neonates. The usage rate of DP73 and DP75 reduced, whereas VH5 was raised with GA increasing. 3. For D gene segment, there was a frequent use of DN, DQ52 and DXP in very immature neonates, whereas DXP, DLR and DN in immature and mature neonates. 4. JH4 usage was preferential, followed by JH6 in all neonates, and their usage rate was decreased with GA increasing. 5. 63.3%, 68.8% and 92.0% of CDR3 sequence respectively in very immature, immature and mature neonates have an open reading frame with 303bp long encoding 101 residues. Conclusion During the early life of neonates, VH-D-JH rearrangement of IgH gene is in an active condition, but the diversity is limited. Humoral immunity is a gradual development with increasing GA. Both heterogeneity and similarity of CDR3 sequence exists in neonates with different GA.

OBJECTIVE To observe the situation of Mycoplasma and Chlamydia trachomatis(Ct) infection in the patients with reproductive tract infection(RTI),and their antibiotic resistance analysis and therapeutic strategies.METHODS To analyze Ct antibody with monoclonal antibody immunogold filtration assay, and to detect Mycoplasma and their drug sensitivity with culture method.RESULTS From 581 specimens there were 42 cases who had Ct(7.2%),and 239 cases who had Mycoplasma(41.1%).The single infection of Ureaplasma urealyticum(Uu) was in 165 cases(28.4%) and of Mycoplasma hominis(Mh) in 18 cases(3.1%),respectively,Uu+Mh were in 39 cases(6.7%).Uu+Ct were in 17 cases(2.9%).The result of drug sensitivity showed that the drug resistance of Uu to erythromycin was 64.8%,to roxithromgcin was 62.6%.Mh was also with the similar tendency. The mixed infections rate was elevated.Doxycycline, minocycline, and josamycin had the higher bacteriostatic ability.CONCLUSIONS With the growing number of mycoplasma infections, and large-scale application of antibiotics themultidrug-resistance was occurred,so it is important to clinical reference to the selection of antibiotic susceptibility test results, and sufficient and adequate course of therapy to reduce the selection of resistant strains.Experimental evidence showed doxycycline, minocycline and josamycin can be used as the first choice for clinical medicine in mycoplasma infection.
