1. Occluded sinus for complex posterior fossa dural arteriovenous fistula

Objective: To evaluate the pathophysiology and therapeutic effect of venous sinuse embolization via Jugular vein for treatment of posterior fossa venous sinus dural arteriovenous fistula (DAVF). Methods: 10 patients with posterior fossa DAVF were diagnosed by angiography, all patients underwent endovascular embolization and cranial venous thrombosis under direct vision to dispose the segment with concentrated fistular feeders in ipsilateral sigmoid and transverse sinus after partial embolization via arterial approach. Results: Circled digit oneThe venous sinus occlusion procedure were successfully finished in 10 cases. Postoperative DSA examination showed that the fistula disappeared entirely in six patients, with more than 80% reduction in four patients. Residual fistulas have no obvious tendence of drainage to cortical veins. Circled digit twoThe murmur was disappeared and intracranial hypertension were relieved in all patients. DSA examination confirmed all the feeders were disappeared in 4 cases with craniotomy. 4 cases had followed-up and no recurrence was found. There was partial recurrence of fistula in 1 of 6 cases who underwent coil embolization and a second embolization was performed. Among five patients of declining of vision, 3 cases had different degrees of recovery after 6-12 months, 2 cases had no obviousy recovery. Five cases with edema of the optic disc were recovered. 3 patients had preoperative CSF pressure higher than 300 mm H2O, the postoperative CSF pressure reduced to 240, 230 and 200 mm H2O. Conclusion: Posterior fossa DAVF is a complicated condition, and transarterial embolization is difficult to achieve good result. The direct cerebral venous sinus occlusion and embolization via femoral vein may have curative effect to cure the posterior fossa DAVF.
3.How to carry out the translational medicine research effectively in gastrointestinal tumors.

As a new concept, the definition of translational medicine remains obscure. The translational medicine connects the bench to bedside, and its importance would be more remarkable. The development of gastrointestinal surgery reflects the idea of translational medicine. To carry out the translational study, the gastrointestinal surgeon must learn how to find subjects from clinical problems, how to collect complete information and tissues, how to collect complete information and tissues, how to collaborate with others from different fields and how to utilize all kinds of resources. By translational studies, gastrointestinal surgeons may further improve the survival of patients with gastrointestinal tumor.

4.Clinical analysis of Gilbert tension-free hernioplasty in the elderly patient under local anesthesia

Objective To summarize the clinical experience of Gilbert tension-free hernioplasty in the elderly patient under local anesthesia. Methods Seventy-six elderly hernia patients operated by Gilbert tension-free hernioplasty under local anesthesia were analyzed in operating time, postoperative complications, hospitalization days and recurrence rate, etc. Results Patients with abdominal hernia were all cured. The average operation time was 25.7 minutes and the time staying in bed after operation was 2-5 hours. The complications of incision pain, foreign body sensation and scrotum hydroceles after operation were observed in 8, 4 and 3 patients respectively. The average hospitalization period was 2.3 days. All patients were followed-up for 1-12 months and no recurrence and infection were observed. Conclusions Gilbert tension-free hernioplasty under local anesthesia is an ideal operation to treat abdominal hernia for elderly patients with less interference to physiological function, quick recovery and less complications.

Objective To evaluate the microRNA role in the pathogenesis of alcohol dependence.Methods By computer retrieval Medline,mirbase,PubMed,Chinese journal full-text database (CNKI),ten thousand database and VIP database,manual retrieval relevant references.Literature retrieval time limit are built from the library until July 2014.English database to Alcoholism,Alcohol Dependence,Alcohol Abstinence,Alcohol Amnestic Disorder Fetal Alcohol Spectrum Disorders,mirna as keywords retrieval;Chinese database to Alcoholism,Alcohol Dependence,Alcohol Abstinence,Alcohol Amnestic Disorder Fetal Alcohol Spectrum Disorders,microRNA,etc.Retrieved from the database to the related literature in July 2014.Results A total of 26 studies were recruited including 8 Chinese literatures and 18 English literatures.Conclusion Abnormal expression of miRNAs will become the next research direction.MicroRNAs may become new biomarker of alcohol dependence on diagnosis,may also makes the molecular drug targets,or the molecular simulation for new drug research and development,this could be for the treatment of alcohol dependence provides a new means.
7.Comparison Analysis of Stress Radiography for the Evaluation of Posterior Knee Laxity

Objective Stress radiography provides an objective tool to measure posterior knee instability.This study was conducted to evaluate the intraobserver and interobserver reliability of measurements using Telos device to quantify posterior knee instability,compared wim KT-1000 and PDT test for consistency analysis.Methods From October 2008 to June 2009,68 stress radiographs in 34 patients with posterior knee instability were taken using Telos device.The amount of posterior displacement on the radiographs was measured independently by 2 surgeons on 2 separate occasions.Changes in intraclass correlation coefficients(ICCs)were examined to assess the intraobserver and interobserver reliability of the measurement,and were compared with those from KT-1000 and PDT test for consistency analysis.Results Intraobserver ICC was 0.992,while interobserver ICC Was 0.991.There was no significant difierence between Telos and KT-1000 in pair-t test.The data from Telos device was consistent with KT-1000.The coincident ratio Of PDT test was 20% while the posterior displacement of the tibia calculated on stress radiography was 5-10 mm.The coincident ratio of the PDT was 71.4% while the posterior displacement of the tibia calculatcd from stress radiography was 10-15 mm.Conclusion Using Telos device for stress radiograph provides a reproducible method to quantify posterior knee instability,and the consistency between Telos divece and KT-1000 was reliable.The coincident ratio of the PDT test with stress radiography increased when the posterior displacement of the tibia from stress view became more severe.
8.Analyzing Financial Security Situation of Disease Prevention and Control System Construction in Jiangxi

Objective: To understand current situation and existing problems in financial security of the center for disease control and prevention system in Jiangxi, to provide scientific references for advancing continuous, fast and healthy development of the system. Methods: combining three methods of questionnaire, group discussions and field surveys to investigate the situation of financial income from 112 centers of disease control and prevention in Jiangxi. Results: The government finance investment increased by year, but the funds of daily work and personnel is insufficient, public and regular business funds of some institution still depend on paid service income. Conclusion: It needs to further enhance financial structure adjustment and disease control investment, increase disease control input, change government appropriation method for disease prevention and control institutions, change the government ’ s funding mode to “purchase system” , reasonably pricing public product, the government on behalf of the people buying services according to quality and quantity from the disease prevention and control institutions.
9.Effects of colon hydrotherapy on lowering body temperature,serum enzymes and inflammatory cytokines of dogs with heat stroke

Objective To investigate the effects of colon hydrotherapy on lowering body temperature and serum enzymes as well as inflammatory cytokines of dogs with heat stroke.Methods 14 dogs were randomly devided into 2 groups,7 for each,as colon hydrotherapy group and conventional therapy group.After the reproduction of heat stroke model was finished(core temperature reached 43℃,maintaing for 10min),cooling therapy was commenced on dogs in circumstance with temperature at 23℃ and 65% of humidity.Conventional therapy group was lowered temperature by ice bags placed on conducting arteries,and the colon hydrotherapy group was by colon way therapeutic machine,the temperature of therapeutic liquid was maintained between 37℃~38℃.Cooling was finished while temperature in dogs reduced to 38℃.The periods of time for lowering body temperature in both groups were compared.Alanine aminotransferase(ALT),aspartate aminotransferase(AST),lactate dehydrogenase(LDH)and creatine kinase(CK)were determined before heat stroke(0h),and 6h,24h and 48h after heat stroke.Interleukin-1?(IL-1?),Interleukin-6(IL-6)and tumor necrosis factor ?(TNF-?)were determined by radioimmunity assay.Results Body temperature lowering time in dogs with colon hydrotherapy were 92.9?9.5min,which was significantly shorter than that of conventional therapy group(123.4?11.6min,P0.05).IL-1? in colon hydrotherapy group at 6h,24h and 48h after treatment were significantly lower than those in conventional therapy group(P
