1.Study on the application of alprostadil and benazepril in patients with diabetes and the effect on blood glucose fluctuation

Objective To analyze the effects of alprostadil combined with benazepril in the treatment of diabetes and the effect on blood glucose fluctuation.MethodsEighty cases with diabetes were randomly divided into the control group and the observation group with 40 cases in each group.The control group was treated with benazepril while the observation group was additionally treated with alprostadil.The treatment effects, the changes of blood glucose, blood lipids and renal function were compared between the two groups, and the incidence of adverse reactions was recorded.ResultsThe total effective rate in the observation group was higher than that in the control group (P<0.05).After treatment, the fasting plasma glucose (FPG), postprandial 2h plasma glucose (2hPG) and glycosylated hemoglobin (HbA1c) were reduced to the normal level but the volatility differences of FPG and 2hPG in the observation group were lower than those in the control group (P<0.05).After 4 weeks of treatment, the total cholesterol (TC), triglyceride (TG) and low-density lipoprotein cholesterol (LDL-C) decreased while high-density lipoprotein cholesterol (HDL-C) increased (P<0.05) but the differences between the two groups showed no statistical significance.After treatment, the improvement of renal function indexes in the observation group was better than that in the control group (P<0.05), and the incidence of adverse reactions showed no significant difference between the two groups.ConclusionThe application of alprostadil combined with benazepril in the treatment of diabetes can reduce blood glucose fluctuation, improve renal function, and reduce the risk of diabetic nephropathy.
2.Identification of Testosterone and Methyltestosterone in Cosmetics by HPLC-MS

Objective To establish an LC-MS method for identification of testosterone and methyltestosterone in cosmetic.Methods Testosterone and methyltestosterone in cosmetic were determined by liquid chromatography mass spectrometry system which can incorporate electrospray ionization interface,post-column addition agent of formic acid.Results The condition of determination was investigated and optimized.The structure of testosterone and methyltestosterone were identified by direct comparison of the observed mass spectra or by the characteristic ions in the selected-ion monitoring as well as MS2 mode.A doubtful testosterone positive cosmetic was identified by this method,it was negative.Conclusion This method has a high sensitivity and a good reproducibility.It is proved that the method is especially suitable for detection of testosterone and methyltestosterone in cosmetics.
3.The clinical observation of hepatic toxicity in hepatitis B virus markers positive cancer patients undergoing chemotherapy

Objective To evaluate the liver damage induced by chemotherapy in patients with cancer and positive for hepatitis B virus (HBV) markers.Methods From January 2005 to January 2012,913 cancer patients were treated by chemotherapy including HBV-positive patients (HBV-positive group,288 cases) and HBV-negative patients (HBV-negative group,625 cases).The changes of hepatic function after chemotherapy between two groups were compared.Results The incidence of hepatic toxicity in HBV-positive group was higher than that in HBV-negative group [24.0% (69/288) vs.11.4% (71/625)],and there was significant difference between two groups (P< 0.01).The incidence of degree 11Ⅲ-Ⅳ hepatic toxicity was 11.4% (14/123) in HBV-DNA-positive patients and 0.6% (4/625) in HBV-negative group.In a variety of chemotherapy,there was significant difference in the incidence of hepatic toxicity in TP(paclitaxel +cisplatin),CAF(cyclophosphamide + doxorubicin + fluorouracil),CHOP(cyclophosphamide + doxorubicin +vincristine + prednisone) between two groups (P < 0.05).The incidence of hepatic toxicity was highest in TP,which was 34.6% (18/52) in HBV-positive group and 16.5% (20/121) in HBV-negative group.Conclusion HBV infection is associated with higher risk of hepatic toxicity in patients with cancer during chemotherapy.

Objective:To observe the effect of 5-aza-2'-deoxycytidine on proliferation and apoptosis of ovarian cancer cell lines(SKOV3 and 3AO)and on expression of mismatch repair(MMR)genes(hMLH1 and hMSH2)in SKOV3 and 3AO cells.Methods:Human ovarian cancer cell lines SKOV3 and 3AO were treated for 3 d with 5-aza-CdR(0.5,5,50 ?mol/L),a specific demethylation agent,and then cultured in RPMI 1640 medium for another 7 d.The cells growth was observed by MTT assay before and after 5-aza-CdR treatment and the cell apoptosis was analyzed by flow cytometry.The expression of hMLH1 and hMSH2 mRNA was examined by semi-quantitative reverse transcription polymerase chain reaction(RT-PCR).Results:5-aza-CdR(0.5,5,50 ?mol/L)obviously inhibited the growth of SKOV3 and 3AO cells compared in a concentration dependent manner.The apoptosis rates of SKOV3 cells were(10.59?1.57)%,(17.52?1.72)%,(34.10?1.45)% after treated with 0.5,5,50 ?mol/L 5-aza-CdR,respectively;and the apoptosis rates of 3AO cells were(11.11?2.21)%,(17.24?1.11)%,and(26.53?2.00)%,respectively,which were all markedly higher than those of control group(P
6.KPC carbapenemases among Enterobacteriaceae with carbapenem non-susceptibility

To investigate the prevalence and gene types of KPC in Enterobacteriaceae strains isolated from 4 tertiary general hospitals in Hainan area ,a total 43 isolates which were resistant or intermediate to imipenem or ertapenem were collected from sterile sites between August 2012 and June 2013 from 4 tertiary general hospitals in Hainan area .Modified Hodge Tests (M HT ) were performed for KPC phenotype screening .PCR amplification and DNA sequence were performed to analyze the encoding genes of KPC .Results showed that in the 43 isolates ,21 strains were positive in M HT .PCR and DNA sequence analysis confirmed that 3 isolates produced KPC‐2 .It's suggested that there were the Enterobacteriaceae carrying KPC in Hain‐an area .The encoding genes were KPC‐2 .The KPC gene could be horizontally transmitted by plasmid among different groups of bacteria .It is important to control the transmission of these Enterobacteriaceae carrying KPC .

Objective To observe the effecacy of modified CHOP regimen in the initial treatment of non-Hodgkin' slymphoma (NHL) patients with low Karnofsky score.Methods Twenty initial treatment NHI.patients with low Karnofsky score were randomly allocated into treatment group and control group,respectively treated with modified and standard CHOP regimen.Results Of 13 cases in treatment group,12 pacients'Karnofsky score were over 60.1 was still below 60,overall response (OR) rate was 92.3 % (12/13).while in control group,3 patients' Karnofsky score were over 60,4 were still below 60.OR rate was 100 % (7/7),rate of marrow toxicity was 57.1 % (4/7).There were no significantly difference for OR rates (P =0.4515),but for improving Karnofsky score there were significantly difference (P =0.0149).Conclusion Modified CHOP regimen can improve Karnofsky score of NHL patients with low Karnofsky score,benefical for further treatment and reducing hospitalization cost.
8.Measurement of gross β activity in urine of normal adults

Objective To measure the total β radioactive background level in the urine of normal adults,and to establish the method which can be universally used and satisfy the needs of rapid screening of samples in large batch.Methods A total of 83 urine samples from healthy adults were prepared by evaporation.And the gross β activity in urine was detected by using low background measuring instrument.Results The optimal experimental conditions were in place.The sampled volume was 200 ml,and the samples were turned to nitric acid salinization,ashed at 300℃ for 2 h,and the measured time was 1000 min.To get a more stable result,the urine residues were put aside for 24 h before measurement.The radioactivity in urine of healthy adults was between 9.40-55.92 Bq/L,and showed no correlation with age and sex.Conclusions When the radioactivity in urine is detected under the conditions mentioned above,the sample preparation process is simple and quickly,which can satisfy the needs of large batch sample screening.

Objective To investigate the whole genomic expression profiles of chronic atrophic gastritis with interleukin(IL)-1β-31CC/-511TT genotype as measured by oligonucleotide microarray technique.Methods Genomic RNA was extracted from gastric biopsies of 12 patients with chronic atrophic gastritis(6 with IL-1β-31CC/-511TT and 6 with IL-1β-31TT/-511CC).The genomic profiles of IL-1β gene polymorphisms 31CC/-511TT and 31TT/-511CC were compared and tested for differential expressed genes associated with 31CC/-511TT using Agilent human whole genomic oligonucleotide microarrays.The results were further analyzed in terms of gene ontology(GO).Results There were 200 differentially expressed genes associated with IL-1β-31CC/-511TT,159 of which were up-regulated and 41 were down-regulated.These genes mainly involved in macromolecule metabolic process,post-translational protein modification,ubiquitin cycle,and protein kinase cascade.Five genes had biological activities,one of which was down-regulated gene(PCSK5)and 4 were upregulated genes(PRKCA,NPLOC4,TRIB3 and MAPKAPK3).Conclusions The chronic atrophic gastritis with IL-1β-31CC/-511TT genotype has molecular phenotypes which is associated with malignance and inflammation.These individuals are needed more intensive preemptive treatment and dynamic surveillance.
10.Accelerated cardiac remodeling of post-infarction was associated with changes of gene expression profile in untreated streptozotocin-induced diabetic rats

AIM: To study the time-dependent effects of diabetes mellitus (DM) on the development of cardiac remodeling in untreated streptozotocin (STZ)-induced rats with acute myocardial infarction (MI). METHODS: The left anterior descending coronary arteries were ligated 10 weeks after DM induction without any therapy. Transmission electron microscopy, echocardiography, heart weight to tibial length ratios, histological examination, microarray analysis, and real time-PCR were utilized to monitor the changes up to 56 d. RESULTS: After MI, the diabetic rats experienced lower survival rate compared to non-diabetic animals. The pathophysiologic changes indicated that DM accelerated the cardiac remodeling post-infarction. In primary examination, 164 genes related to cardiac remodeling were found to be candidates for hierarchical analysis, such as leucine-rich PPR-motif containing (interleukin-6 signaling pathway), procollagen type I and III, fibronectin-1, RT1, and TIMP-1, etc. The gene expression profile at 14 d in diabetic rats were comparably similar to both 14 d and 28 d in non-diabetic rats, while such changes at 28 d and 56 d in diabetic rats was also similar to the ones at 56 d in non-diabetic rats. CONCLUSION: The accelerated cardiac remodeling of post-infarction in STZ-induced untreated diabetic rats seems be associated with the different profile of gene expressions.
