Objective To explore the advantages of intraoperative NRT measurement(neural response telemetry)during cochlea implant surgery.Methods Intraoperative and postoperative NRT measurements were tested in ten SNHL children with CI24M-Nucleus and four SNHL children with CI24R(CS)-Nucleus, the ECAP threshold of intraoperative and postoperative measurements were compared, and the first maps were set up according to the intraoperative ECAP threshold.Results The average intraoperative ECAP threshold of 22 electrodes was higher than postoperative ECAP threshold, the average intraoperative ECAP threshold of CI24M-Nucleus' 22 electrodes was 11CL greater than postoperative ECAP threshold. It was just five minutes to record ECAP of 10 electrodes, and the beginning of fitting was finished in one hour.Conclusion It is confirmed that intraoperative NRT measurement could test the integrity of implants, and could estimate the first map. It is convenient to use and should be popularized.

Objective To investigate the clinical efficacy of recombinant human epidermal growth factor in the treatment of recurrent oral ulcers in elderly patients.Methods 82 cases of elderly patients in our hospital in the treatment of recurrent oral ulcer were selected,41 cases in the control group,the conventional western medicine treatment,41 cases of the study group,recombinant human epidermal growth factor in treatment before and after treatment,blood determination of immune status,clinical efficacy and adverse reactions of comparison were performed at one-year follow-up.Results the effective rate of treatment group is lower than 78.05％of the effective rate of treatment group 95.12％,study group 3,6 and 12 months recurrence rate(4.88％,7.32％,7.32％)than in the control group the recurrence rate(19.51％,24.39％,29.27％),the study group after the treatment of CD3+ and CD4+/CD8+ and NK were higher than the control group,the CD8+ level is lower than the control group(P<0.05),the two groups had no serious adverse reaction.Conclusion recombinant human epidermal growth factor is effective in the treatment of oral ulcer.

Objective To evaluate the therapeutic effetive,toxicity and survial rate of non-operation esophageal carcinoma patients treated by tegafur plus cisplatin combined with three-dimensional conformal radiotherapy (3D-CRT).Methods 184 cases of non-operation patients with esophageal carcinoma were divided into two groups.The cases of treatment group were treated by cisplatin and tegafur.The cases of control group were treated by cisplatin and 5-Fu.All patients received 3D-CRT,and the total radiation dose was 50-70 Gy.Each group had 14 cases treated with primary chemotherapy for 1 course,28 cases for 2 courses,and 50 cases without any chemotherapy.Results The recent effect rate was 69.57 ％ (64/92) in treatment group and 67.39 ％ (62/92) in control group,there was no statistical difference (x2 =0.101,P =0.874).Radioactive esophagitis and leukopenia were the main side effect of treatment.The treatment group was significantly lower than that of the control group,the difference had statistical significance.The 1-,2-and 3-year survival rates were 74.9 ％,61.6 ％,52.8 ％,respectively in treatment group and 75.8 ％,56.9 ％,48.0 ％,respectively in control group.The control group survival rates were significantly higher than the treatment group,the difference had statistical significance (x2 =7.325,P =0.007).Conclusions Cisplatin plus tegafur or 5-Fu combined with 3D-CRT is effective way of treatment for non-operation of esophageal carcinoma.The toxicity of cisplatin plus tegafur group is significantly lower than cisplatin plus 5-Fu group,therefor it can improve the quality of life of the patients obviously.

The human immunodeficiency virus type 1(HIV-1)can interact with and exploit the host cellular machinery to replicate and propagate itself.Numerous studies have shown that the Mitogen-activated protein kinase(MAPK)signal pathway can positively regulate the replication of HIV-1,but exactly how each MAPK pathway affects HIV-1 infection and replication is not understood.In this study,we used the Extracellular signal-regulated kinase(ERK)pathway inhibitor,PD98059,the Jun N-terminal kinase(JNK)pathway inhibitor,SP600125,and the p38 pathway inhibitor,SB203580,to investigate the roles of these pathways in HIV-1replication.We found that application of PD98059 results in a strong VSV-G pseudotyped HIV-1NL4-3 luciferase reporter virus and HIV-1NL4-3 virus inhibition activity.In addition,SB203580 and SP600125 also elicited marked VSV-G pseudotyped HIV-1NL4-3 luciferase reporter virus inhibition activity but no HIV-1NL4-3 virus inhibition activity.We also found that SB203580 and SP600125 can enhance the HIV-1 inhibition activity of PD98059when cells were treated with all three MAPK pathway inhibitors in combination.Finally,we show that HIV-1virus inhibition activity of the MAPK pathway inhibitors was the result of the negative regulation of HIV-1 LTR promoter activity.

Objective To make spinal cor injury model of rabbit in order to investigate the feasibility and accuracy of diffusion tensor imaging in diagnosis of early ASCI. Methods In 30 Chinese rabbits ,24 rabbits were randomly selected as the experimental group and the other 6 as the control group. Allen′s technique were used to make moderateacute spinal cord injury in the experimental group. In the control group ,the operation only exposed the spinal membrane ,wihthout damage on the spinal cord. All rabbits were performed in routine MRI , DTI ,diffusion tensor tractography and post-processing. After the MRI scan ,the rabbit spinal cord was removed to have pathological examination at 1 ,3 ,5 ,7 days. Results The number of inflammatory cells and spinal motor neurons were closely correlated with FA values and ADC values. DTT images showed the spinal cord defects, curling and breaking. Conclusion The parameters of DTI (FA ,ADC) can reflect the pathological changes of acute spinal cord injury in early stage ,and therefore may be used as an routine evaluation method for clinical acute spinal cord injury.

Objective To analyze the efficacy and debase the toxicity of radiotherapy combined with composite Kushen injection in treatment of esophageal carcinoma patients. Methods A total of 126 cases of esophageal cancer were treated by radiotherapy were analysed. All patients were treated with the continuative hyperfractionated radiotherapy. The total dose was 66 ~ 74 Gy/5 ~ 7 wk, 2.2 ~ 2.4 Gy/d of esophageal carcinoma patients. Two groups were given respectively delivered with the same radioactive dosages and methods.But the synthetic group was treated by radiotherapy with composite Kushen injection. composite Kushen injection was given at the dosage of 20 ml with 500 ml of saline infusion for 20 consecutive days. Results X-ray grades of esophageal cancer after radiotherapy. Objective To explore the relationship between bronchoalveolar lavage fluid (BALF) Mycoplasma load and clinical characteristics in children with Mycoplasma pneumoniae (MP) pneumonia.Methods MP DNA was quantitatively detected by fluorescent real-time PCR in BALF from 67 children with MP pneumonia.They were classified into three groups:low MP load group (＜ 103/ml,21 cases),moderate MP load group (103-106/ml,22 cases) and high MP load group (＞ 106/ml,24 cases).Clinical symptom,main laboratory and imaging results of children among three groups were compared.Results When compared with low MP load group and moderate MP load group,high MP load group had longer fever duration [(7.4 ± 2.6),(10.0 ± 2.4) d vs.(12.4 ± 2.7) d],longer time to normalization of temperature with macrolide administration [(4.2 ± 1.0),(8.5 ± 2.4) d vs.(10.8 ± 4.4) d],more patients with high fever,there was significant difference (P ＜ 0.05).Statistically significant difference existed in C-reactive protein among three groups [(3.0 ± 1.4),(11.5 ±7.6),(34.0 ± 10.1) mg/L] (P =0.004).Large field of consolidation or atelectasis were found in 58.3％ (14/24) of high MP load group,much higher than 22.7％ (5/22) in moderate MP load group and 14.3％ (3/21) in low MP load group.Bilateral or massive pleural effusion was not found in low MP load group,while in moderate MP load and high MP load group,they were 13.6 ％ (3/22) and 25.0％ (6/24)(P =0.033).Conclusions There is a close relationship between MP load in BALF and clinical characteristics in children with MP pneumonia.Those with high MP load have a more severe process.

BACKGROUND: Auditory steady-state responses (ASSR) is an objective method of hearing examination in clinic in recent years. ASSR has the frequency specificity as compared with previous auditory brainstem responses (ABR).OBJECTIVE: To investigate the accuracy of ASSR in objective hearing assessment.DESIGN: A case-control observation.SETTING: Department of Otorhinolaryngology, the First Affiliated Hospital of Sun Yat-sen University.PARTICIPANTS: The subjects in the normal hearing group were the 21 undergraduates (42 ears) were enrolled, they all had not any symptoms of ear disease, without history of noise exposure and disease of vestibule system, and they were normal in otoscopy. The outpatients and inpatients with neurosensory deafness were selected from the Department of Otorhinolaryngology, the First Affiliated Hospital of Sun Yat-sen University. All the children cases worn hearing aids, and had the speech ability, and cooperated in the examination. The main types included 6 ears of sudden deafness,8 ears of presbycusis, and 20 ears of neurosensory deafness due to other unknown causes. Central lesions were excluded by MR examination, and all the patients agreed with the enrollment. The results of pure-tone audiometry were all flat or descending audiogram. According to the severity of hearing damage, the patients were divided into mild deafness group (13ears), moderate deafness group (9 ears) and moderate-to-severe deafness group (12 ears).METHODS: ① The pure-tone audiometry was performed at the frequencies of 0.125-8 000 Hz in a sound insulation room. The auditory threshold grades of the subjects with normal hearing all accorded with the standards of GB-7583-87 expected value distribution. The average value of air-conduction auditory thresholds of pure-tone audiometry at the frequencies of 0.5, 1, 2 and 4 kHz was calculated. ② ASSR measurement was performed with the synchronous stimulation pattern in a sound and electromagnetic shielding room, including 8 points for both ears of the same stimulation intensity and the carrier frequency tones of 0.5, 1, 2 and 4 kHz respectively.③ ABR examination was performed by click sounds with sparse waves in a sound and electromagnetic shielding room, and insert earphones were used.The threshold results were judged according to the minimal stimulation sound intensity of the distinguishable Ⅴ wave. ③ The results of pure-tone audiometry were compared with those of ABR examination, and the results of ASSR measurement in different hearing groups were processed with analysis of variance, multi-classification discrimination based Bayes standard and q test.MAIN OUTCOME MEASURES: The thresholds of pure-tone audiometry, ASSR measurement and ABR examination, and the correct rate analyzed by the multi-classification discrimination based Bayes standard were mainly observed.RESULTS: The indexes of the 42 ears in the normal hearing group, 13, 9 and 12 ears in the mild, moderate and moderate-to-severe deafness groups were all involved in the analysis of results. ① The ABR values were accorded with the actual hearing levels, and the closest to the ASSR thresholds at 1-2 kHz; ASSR reflected induction rates at different frequencies were gradually decreased with the aggravation of hearing damage, and that at each frequency varied with the changes of hearing level, the induction rates of ASSR responses were all 100% for the subjects with normal hearing and patients with mild deafness, but those for the patients with moderate and moderate-to-severe deafness were decreased (0.5 kHz: 77.8%,92.8%; 4 kHz: 88.9%, 85.7%). At different frequencies, the ASSR thresholds in the moderate-to-severe deafness group were significantly higher than those in the normal hearing group (P ＜ 0.05). The ASSR thresholds at 0.5 and 4 kHz in the moderate-to-severe deafness group were significantly higher than those in the mild deafness group (P ＜ 0.05). The ASSR threshold at 2 kHz in the mild deafness group was significantly higher than that in the normal hearing group (P ＜ 0.05). The ASSR thresholds at 4 kHz in the everedeafness group were significantly higher than those in the normal hearing group and mild deafness group. ② The incorrect discriminations of actual pure-tone audiometry were analyzed with the interactive clustering discriminant analysis of ASSR measurement and actual pure-tone audiometry, and the results showed that the correct rate of discrimination was 100% in the normal hearing group; Only 1 of the 12 cases in the mild deafness group was incorrectly judged, and the correct rate was 92%; Only 1 of the 19 cases in the moderate deafness group was incorrectly judged, and the correct rate was 89%; the correct rate in the moderateto-severe deafness group was 83%.CONCLUSION: The results of ASSR measurement can detect the incorrect discrimination of objective hearing condition by taking the results of pure-tone audiometry as the standards. ASSR has an acceptable accuracy for deafness higher than mild level in estimating objective hearing, and it has a better prospect of application in practice.

Objective To study the applied value of multi-slice spiral CT(MSCT)in pulmonary lesions.Methods 68 cases with massive hemoptysis caused by lung tumor or other reasons underwent 16-slice spiral CT scan including plain and contrast-enhanced scan.Volume reconstruction(VR),multiplanar reconstruction(MPR),maximum intensity projection(MIP)and curved planar reconstruction(CPR)were performed using original images.The origin,number and shape of vessels feeding lesions were observed.Results Among 68 cases(two failed),the blood supply of pulmonary lesions was from bronchial artery in 60 cases,from the intercostals artery in 8 cases,from the internal thoracic artery in 5 cases,from the left subclavian artery in 6 cases,from the right inferoir phrenic artery in 5 cases,from the celiac trunk artery in 3 case and from the right renal artery in 1 case.The blood supply was from single artery in 50 cases,double arteriae in 10 cases and three arteriae in 6 cases.The supply arteriae were vascular plexus in 55 cases,meandering in 8 cases,net in 2 cases and aneurysm formation in one cases.Conclusion MSCT can clearly display the vessels feeding pulmonary lesions.
