1.Experiences of PTCA and stents by radial artery from grass roots(attached 55 cases)

Objective Evaluate the feasibility of PTCA and stents by radial artery. Methods we had operated PTCA for 25 cases by radial artery from July 2004 to April 2005. The average age is 63 years old including 40 men and 15 women. Results 67 stents were implanted in 55 patients including 30A cases, 26 B cases and 11C cases. These stents is about 8mm to 33mm in length and 2.5mm to 4.0mm in width. The successful rate is 96%(53/55), and two case was failed, because the guiding wire can not pass by chronic obstruction in LAD all. There is no case by femoral. All the procedures have no heart complications, obstruction of radial artery and vascular complications. Conclusions The way is good for PTCA by radial artery. Its advantages are mild injury, high successful rate and few complications, and the salted hospitals can use the technology successfully.

Chemotherapy completion rate can reflect the tolerance and compliance of patients to chemotherapy. Poor tolerance may result in delay or suspension of the comprehensive treatment plan, thus affect the efficacy of cancer treatment. Evaluating methods to improve the completion rate of chemotherapy and reduce the occurrence of delayed chemotherapy has gained increasing attention and is the significant area of study in the field of cancer treatment. Studies have shown that Chinese medicine combined with chemotherapy could improve the quality of life in patients with stage IIIB/IV non-small-cell lung cancer (NSCLC).
3.Problems and countermeasures for after-sale service of medical equipment

Objective To explore the problems and countermeasures during the implementation of after-sale service of the medical equipment.Methods The common problems and their causes during after-sale service were analyzed,and some countermeasures were proposed from the aspects of signing purchase contract,acceptance check,operation quality,overall evaluation,personnel and etc.Results Strengthened after-sale service contributed to enhancing the medical equipment in operation quality,medical safety and efficiency.Conclusion It is important to take corresponding measures during after-sale service to improve operation quality and decrease operation cost of the medical equipment.
4.Efficacy of Lentinan combined with chemotherapy in advanced non-small cell lung cancer.

BACKGROUNDLentinus edodes polysaccharide (Lentinan) has attracted great attention from both pharmacologists and clinicians as a biological response modifier, and is widely used as an anti-tumor agent in both China and Japan. The aim of this study is to observe the efficacy of Lentinan combined with chemotherapy in stage III-IV non-small cell lung cancer (NSCLC).
METHODSEighty-one patients with stage III-IV NSCLC were randomly divided into two groups: (1)Lentinan + chemotherapy group (group A, 42 cases); (2)Simple chemotherapy group (group B, 39 cases). The peripheral blood T lymphocyte subsets (CD3, CD4, CD4/CD8) and natural killer (NK) cell activity of patients in both groups were measured before and after treatment, while compared with healthy control (30 cases). The immune functions, the effect of treatment, quality of life, and adverse reactions were observed.
RESULTSAfter treatment the objective response rate (CR+PR) was 50% in group A, compared to 33% in group B (P < 0.05). The blood T cell levels(CD3, CD4, CD4/CD8) and NK cell activity in group A increased (P < 0.01), CD8 reduced (P < 0.05), but in group B the value had no obvious change (P > 0.05). Quality of life in group A was higher than that in group B (P < 0.01). The incidence of grade II-IV leukopenia, nausea and vomiting in group B was much higher than those in group A (P < 0.05).
CONCLUSIONSThe therapeutic effect of Lentinan combined with chemotherapy is better than that of chemotherapy alone.
5.Efficacy observation of chronic pelvic inflammation of different differentiated patterns/syndromes treated with acupoint embedding therapy.

OBJECTIVETo compare the clinical efficacy on chronic pelvic inflammation between the acupoint embedding therapy and acupuncture, and to compare the efficacy of different patterns/syndroms in differentiation treated with acupoint embedding therapy.
METHODSTwo hundred and eighteen cases were randomized into an embedding therapy group (115 cases) and an acupuncture group (103 cases). In both groups, Shenshu (BL 23), Guanyuanshu (BL 26), Zigong (EX-CA 1), Yaoyangguan (GV 3), Guanyuan (CV 4) and Qihai (CV 6) were selected as the main points. For qi and blood stagnation pattern/syndrome, Zhongdu (LR 6), Diji (SP 8) and the others were added; for cold and damp stagnation pattern/syndrome, Diji (SP 8) and Sanyinjiao (SP 6) were added; for stasis and phlegm pattern/syndrome due to spleen deficiency, Sanyinjiao (SP 6) and Zusanli (ST 36) were added. In the embedding therapy group, the catgut was embedded at 7 - 13 acupoints each time, once every 10 days. In the acupuncture group, the conventional acupuncture therapy was applied, once a day. The symptom scores were assessed in the aspects of the lower abdominal pain severity and attack frequency, lumbosacral soreness and distention, abnormality of vaginal discharge and the others. The efficacy was compared between the two groups.
RESULTSThe total effective rate was 93.0% (107/115) in the embedding therapy group, which was better than 83.5% (86/103) in the acupuncture group (P < 0.05). The symptom scores were all reduced after treatment in the two groups (all P < 0.05). In the embedding therapy group, the efficacies were not significantly different among different patterns/syndromes (all P > 0.05).
CONCLUSIONThe acupoint embedding therapy achieves the definite efficacy on chronic pelvic inflammation and obtains the similar efficacies among the different differentiated patterns/syndromes of the disease.

7.Correlation between rs4646999 polymorphism of c-jun gene promoter and prognosis of colorectal cancer

Objective To detect the association between rs4646999 polymorphisms in the promoter region of the c-Jun and the prognosis of sporadic colorectal cancer. Methods rs4646999-673C>T genetypes were deter-mined by Taqman-MGB probes in 436 colorectal cancer cases. The survival curve was analyzed by Kaplan-Meier analysis and Cox regression.Western blot was used to analyze the expression levels of c-Jun protein in different gen-otypes. Results Univariate analysis showed that the cumulative survival rate of patients with rs4646999TT geno-type was significantly higher than that of patients with CT and CC genotype. Multivariate Cox regression analysis showed that the differentiation,lymph node metastasis,distant metastasis,TNM stage and rs4646999 genetypes were prognostic factors.Compared with the carriers of TT genotype,CT/CC complex genotypes were associated with poor prognosis of colorectal cancer(P<0.05).Protein expression analysis showed that the expression of c-Jun pro-tein in CC genotype was increased.In contrast,the TT genotype was decreased.Conclusions This study provided the evidence that rs4646999-673C>T genetic variant in c-Jun promoter regions is associated with the poor survival prognosis of colorectal cancer,possibly by elevating the protein expression levels that appeared to up-regulate activ-ity of c-Jun thus tumorigenesis.

Objective To analyze the publication profile and research hotspots of studies at home and abroad on postoperative cognitive dysfunction by bibliometric methods.Methods Litera-tures related to postoperative cognitive dysfunction published in the China National Knowledge Infra-structure(CNKI)and Web of Science databases from 2013 to 2023 were included,and statistical a-nalysis was conducted by CiteSpace software.Results A total of 2 705 Chinese literatures and 2 412 English core literatures were included.The number of literatures on postoperative cognitive dysfunc-tion showed an annual increase from 2013 to 2023;China was the country with the highest number of publications internationally,with Capital Medical University ranking the top among Chinese institu-tions;domestically,postoperative cognitive dysfunction researches predominantly focused on clinical studies,while overseas researches emphasized pathological mechanisms and basic investigations.Re-search hotspot analysis revealed an enhanced interest in the inflammatory mechanisms and anesthetic as-pects of postoperative cognitive dysfunction.Conclusion From 2013 to 2023,the overall research at home and abroad interest in postoperative cognitive dysfunction has shown an upward trend,yet its patho-physiological mechanisms need further exploration.The risk factors for postoperative cognitive impairment and their correlations with anesthesia and mortality are being actively explored,and new methods for prevention and treatment such as percutaneous electrical stimulation are gradually emerging.

10.Effect of high-dose vitamin B6 on stress-induced liver cell death in rats with severe trauma and the possible mechanism

Objective:To investigate the effect of high-dose vitamin B6 on stress-induced liver cell death in rats with severe trauma and its possible mechanism.Methods:Thirty-two male SD rats were selected and divided into sham surgery group, sham surgery+B6 group, trauma group, and trauma+B6 group by using a random number table, with 8 rats in each group. Rat models of severe trauma were established by inducing abdominal wall injury, bilateral femoral fractures, unilateral cranial injury, and withdrawal of 4 ml blood from the femoral artery. The sham surgery+B6 group and trauma+B6 group were treated with saline solution plus high-dose vitamin B6, while the sham surgery group and trauma group with infusion of saline solution only. At 36 hours after injury, rat liver tissues were collected for the following experiments: (1) the genes differentially expressed in the liver tissues of the rats of the trauma group and the trauma+B6 group were screened with next-generation sequencing, followed by an analysis of the possible involvement of cell death pathways; (2) validation was conducted to ascertain whether high-dose vitamin B6 could influence various cell death pathways in the liver cells in the sham surgery group, sham surgery+B6 group, trauma group, and trauma+B6 group: apoptosis was confirmed through terminal-deoxynucleotidyl transferase mediated nick end labeling (TUNEL) staining; necroptosis was verified by mixed lineage kinase domain-like protein (MLKL) immunohistochemical staining; autophagy was examined via transmission electron microscopy; ferroptosis was confirmed by detecting oxidative malondialdehyde (MDA) levels, oxidized glutathione levels, Prussian blue staining with diaminobenzidine (DAB) enhancement, transmission electron microscopy, and immunohistochemical staining for acyl-CoA synthetase long-chain family member 4 (ACSL4); (3) Biological information analyses [Gene Ontology (GO), Kyoto Encyclopedia of Genes and Genomes (KEGG), and Gene Enrichment analysis (GSEA)] were performed for biological processes and signaling pathways represented by liver tissue sequencing results of rats between the trauma group and the trauma+B6 group.Results:(1) In the liver tissues of rats, there were 344 significantly differentially expressed genes between the trauma group and trauma+B6 group, comprising 137 upregulated genes and 207 downregulated genes, of which 18 genes were associated with apoptosis, autophagy, necroptosis, ferroptosis, and pyroptosis. (2) No significant differences were found in TUNEL staining among the sham surgery group, sham surgery+B6 group, trauma group or trauma+B6 group; MLKL protein expression levels in the liver tissues after trauma were improved, of which the trauma+B6 group was lower than that of the trauma group; Electron microscopy showed that autophagic activity in the liver cells were significantly increased after trauma, which was significantly lower of the trauma+B6 group than that of the trauma group; MDA levels in the rat liver tissues were (0.20±0.05)nmol/mg, (0.17±0.07)nmol/mg, (0.69±0.11)nmol/mg and (0.52±0.07)nmol/mg in the sham surgery group, sham surgery+B6 group, trauma group, and trauma+B6 group respectively ( P<0.01), with the trauma group having the highest MDA levels and trauma+B6 group having lower MDA levels than the trauma group; Oxidized glutathione levels in the liver tissues of the four groups were (11.75±2.09)μmol/g, (11.69±1.66)μmol/g, (19.75±3.40)μmol/g, and (14.51±1.46)μmol/g respectively ( P<0.01), with the trauma group having the highest levels and trauma+B6 group having lower levels than the trauma group; Significantly increased iron deposition was observed in the liver tissues after trauma, with lower iron deposition in trauma+B6 group than the trauma group; Electron microscopy revealed significantly lower mitochondrial membrane density in the trauma+B6 group compared to the trauma group. ACSL4 protein expression level was lower in the trauma+B6 group compared to the trauma group; (3) GO, KEGG and GSEA enrichment analyses suggested that high-dose vitamin B6 may enhance cholesterol synthesis metabolism in the liver cells and alleviate oxidative stress to reduce liver cell damage and restore normal liver cell function after trauma. Conclusions:High-dose vitamin B6 attenuates stress-induced liver injury in rats with severe trauma by inhibiting the progression of necroptosis, autophagy and ferroptosis. Its molecular mechanism may be associated with enhanced hepatic cholesterol synthesis metabolism and alleviation of oxidative stress in the liver cells.
