1.The clinical curative effect of pegylated interferon α-2a combined with ribavirin in treating chronic hepatitis C with positive autoantibodies

Objective To research the clinical curative effect of pegylated interferon α-2a (Peg-IFNα-2a) combined with ribavirin in treating chronic hepatitis C with positive autoantibodies.Methods The autoantibodies were screened in 93 cases of chronic hepatitis C patients and then the patients were divided into positive group and negative group.Both of the two groups were given therapy with Peg-IFN α-2a combined with ribavirin.The capacity of hepatitis C virus (HCV) RNA,virological response rate,and blood concentration of alanine aminotransferase (ALT) and aspartate aminotransferase (AST) were quantified before and in the process of treatment.Results Twenty-eight patients had autoantibodies,with 17 patients of antimitochondrial antibody,8 patients of anti smooth muscle antibody and 3 patients of anti-liver-kidney microsomal antibody-1.The quantity of HCV RNA and the virological response rate had no significant difference before and after treatment (P > 0.05).The concentration of ALT and AST in positive group were higher than those in negative group before and in the process of treatment,while there were obvious differences only after 36 weeks of treatment (P < 0.05).Conclusion Peg-IFN α-2a combined with ribavirin is effective for chronic hepatitis C for all the patients,but liver function of autoantibody negative patients have a better recovery.
2.Anti-inflammatory Effect of Musk Eye Drops on Acute Ceratitis

OBJECTIVE:To explore the anti-inflammatory effect of Musk eye drops on acute ceratitis.METHODS:The model of acute ceratitis resulting from surgical injury was established in rabbits and which were randomly assigned to Musk eye drops groups(0.3%,0.15%,and 0.05%),0.5% Cortisone acetete eye drops group and 0.9% Normal Saline group(control).The corneal epithelium growth at 24,48h,and 72h was observed by fluorescein(2%)staining,levels of protein,Ca2+,IL-1? in the aqueous humor were determined,corneas were weighted and corneas tissues were sampled for histopathologic examination.The anti-inflammatory effect of Musk eye drops on acute ceratitis resulting from surgical injury was discussed.RESULTS:Corneal bedewing was mitigated,transmigration of neutrophilic leukocytes were inhibited,levels of protein and IL-1? in the aqueous humor were decreased,level of Ca2+ in the aqueous humor was altered and the corneal epithelium growth was promoted in Musk eye drops groups(0.3%,0.15%,and 0.05%).CONCLUSION:Musk eye drops had remarkable anti-inflammatory effect on acute ceratistis.

AIM:To investigate the anti-inflammatory effect of Musk Eye Drops(Moschus,Borneolum Syntheticum,etc.) METHODS:Anti-inflammatory effects of Musk Eye Drops on acute ceratitis in mice was observed with the models of mouse auricle edema induced by dimethylbenzene and acute conjunctivitis caused by Croton oil,and acute ceratitis resulting from surgical injury. RESULTS:All groups of Musk Eye Drops could significantly inhibit the edema,decrease the inflammatory of acute conjunctivitis and acute ceratitis.The effect of Musk Eye Drops had statistical significance(P0.05). CONCLUSION:Musk Eye Drops has obvious anti-inflammatory effect.
4.The clinical application of low-weight mesh for tension-free repair of inguinal hernia in the elders

Objective To evaluate low-weight mesh for tension-free repair of inguinal hernia in the elders. Methods 120 old patients ( age at 60 -97 years) underwent tension-free repair of inguinal hernia by using low-weight mesh. The early complications, time of up and about, hospital stay, postoperative chronic pain,foreign body feelings and hernia recurrence were analyzed. Results The operations were successful in all cases and the average time of operation was (43 ± 12 ) min. 9 cases suffered from edema of the scrotum, 5 cases suffered urine retention, 4 cases suffered from incision hematoma. The average time of outof-bed was ( 17.4 ± 1.8 ) h, the average hospital days was ( 7.4 ± 1.9) d. However, there were 1.7%(2/120) patients suffering from slight chronic pain and 9. 1% (11/120) patients complaining foreign body feelings. There was no recurrence after follow-up for 6 to 48 months. Conclusions The clinical application of low-weight mesh for tension-free repair of inguinal hernia in old age is safe and effective, with an additional advantage of low occurrence of chronic pain and foreign body feelings.
5.Prenatal anxiety and its influence on delivery outcome.

OBJECTIVE:
To investigate the factors related to prenatal anxiety, to observe the effect of prenatal anxiety on the mode of delivery, intrapartum hemorrhage, and to determine the correlation between prenatal anxiety and delivery outcome.
METHODS:
A total of 300 pregnant women were randomly enrolled from 3 hospitals of Changsha, China. The instruments of this survey were self-designed general information questionnaire, state-trait anxiety inventory (STAI), pregnancy pressure scale, and social support rating scale (SSRS). The delivery outcome was observed.
RESULTS:
The STAI showed that the state anxiety score of the pregnant women was 43.58±10.79, which was significantly higher than that of usual models (P<0.01). Multiple linear regression analysis showed that those factors, such as the item 2 of pregnancy pressure, relationship with mother-in-law, number of abortions, prenatal health condition, times of attending classes for pregnant women, psychological preparation for childbirth, relationship with mother, relationship with husband, item 3 and item 1 of pregnancy pressure, were all related to prenatal anxiety. The anxiety level between the non-indicative cesarean section group and the normal labor group had statistical difference (P<0.01). Intrapartum hemorrhage of cesarean section in the anxiety group had significant difference compared with the non-anxiety group (P<0.01).
CONCLUSION
Some factors have contributed to the anxiety level of the pregnant, such as psychological stress, relationship with mother-in-law, number of abortions, prenatal health conditions, times of attending classes for pregnant women, psychological preparation for childbirth, relationship with mother, and relationship with husband. Prenatal anxiety may lead to increased the rate of non-indicative cesarean section and intrapartum hemorrhage during the cesarean section.

6.One case report of neonatal onset Crohn disease

ObjectiveTo investigate the diagnosis and treatment of Crohn disease in neonates.Methods The clinical data of one case of neonatal onset Crohn disease were retrospectively analyzed.Results Male infant had intermittent fever, recurrent oral ulcers, skin impetigo and intermittent diarrhea with increased WBC counts from ifve days after birth. He had been diagnosed as sepsis, bacterial enteritis, Behcet's disease, immunodeifciency disease, nutritional anemia, milk intolerance, and inlfammatory bowel disease. Finally he was diagnosed as Crohn disease by rectal mucosa pathology and postoperative bowel pa-thology. He died of intestinal perforation on 8 months.ConclusionsThe infants who have recurrent oral ulcers, skin impetigo, perianal ulcers and fever from neonatal should undergo colonoscopy for the diagnosis.
7.DIAGNOSIS AND TREATMENT OF CYSTICERCOSIS OF CEREBRAL VENTRICLES

31 patients with cysticercosis of cerebral ventricles verified by operation or pathological investigation were reported. All patients were between 7 and 64 years of age and 14 were females. All had a single cyst. Since 29 patients (94%) were without a history of intestinal taeniasis, it was proposed that most patients of cysticercosis of cerebral ventricles were caused by hetero-infection and the entrance of Cysticercus into brain ventricle was through choroid plexus along the cerebro-spinal fluid. This is probably the reason why it occurs mostly in the 4th ventricle. The clinical manifestation of cysticercosis of cerebral ventricles were paroxysmal headache and vomiting caused by increased intracranial pressure. Ventricu-lography and CT scanning have considerable diagnostic value. Removal of Cysticercus by surgical operation is successful (Figs. 1 - 8).
8.Analysis of levels of antibodies against influenza A virus of population in Shanghai during 2009

Objective To know the levels of antibodies against influenza A virus subtypes H1 and H3 of population in Shanghai during 2009, and the detection of antibodies against avian influenza virus subtypes H5 and H9 in population which contacts with avian. Methods The serological survey of the antibodies against influenza A viruses subtypes H1, H3, H5 and H9 in 356 close contacts with avian (professional population) and 332 general subjects (general population) at various age groups were carried out using hemagglutinin inhibit (HI) test. Results The positive rates of antibodies against influenza virus A/Brisbane/59/2007 (H1N1) in general population and professional population were 82.8% (275/332) and 73.9% (263/356), respectively; those of A/Brisbane/10/2007 (H3N2)were 50.6% (168/332) and 54.8% (195/356), respectively. The positive rate of antibodies against influenza virus A/Brisbane/59/2007 (H1N1 )was significantly higher than that of influenza A viruses subtype H3, which was consistent with etiological survey of influenza virus in Shanghai during 2008.The positive rates of antibodies against influenza A virus subtype H5 in professional population and general population were 4.2% (15/356) and 0.3% (1/332), respectively; those of influenza A virus subtype H9 were 34.6% (123/356) and 2.4% (8/332), respectively. The positive rates of antibodies against influenza virus A/Brisbane/59/2007 (H1N1 ) and A/Brisbane/10/2007 (H3N2) in age groups of 6 months-5 years and ≥60 years were lower than other age groups. Conclusions The immune protective response against seasonal influenza A subtype H1 and H3 of population in Shanghai is high,while those of children and the elders were low. The levels of antibodies against influenza A viruses subtype H5 and H9 in professinal population present obviously ascending trend, which indicates that the etiological and serological survey of influenza virus in this population should be enhanced.
9.Immunization coverage of oral live attenuated rotavirus vaccines and its influencing factors among children under 5 years old in Jinshan District of Shanghai

Objective To investigate the immunization coverage of oral live attenuated rotavirus vaccines and its influencing factors among children under 5 years old in Jinshan District of Shanghai in order to provide references for optimizing vaccination management in this area. Methods A cross-sectional study was conducted in 6 community vaccination clinics randomly selected from 12 vaccination clinics in Jinshan District. In each selected clinic,145 children under 5 years old were continuously recruited. Information on immunization status,demographics of children,family socioeconomic status and parents′knowledge of vacci-nation were collected through questionnaire interview. Logistic regression was used to analyze factors influen-cing immunization status by using SPSS22.0. Results Among the 863 participating children, the immuni-zation coverage rate of oral live attenuated rotavirus vaccines was 61.07%. Multivariate analysis revealed that children having an urban registration status (OR=2.21),better family income (OR=1.47) and par-ents with high recognition of rotavirus diarrhea (OR=8.56) and stronger intention to following "doctor′s suggestions" (OR=1.96) were more likely to accept oral live attenuated rotavirus vaccines, while those from families with a medical professional were less likely to be vaccinated (OR=0.57). Conclusion The immunization coverage rate of oral live attenuated rotavirus vaccines among children in Jinshan District were influenced by various factors such as children′s household registration status, household income, parents′knowledge regarding vaccination and doctors′suggestions. Health education on immunization should be strengthened and tailor-made to improve the coverage of oral rotavirus vaccination.
10.Comparison of clinical efficacy between robotic and laparoscopic low anterior resection for rectal cancer:a Meta analysis

Objective To compare the clinical efficacy of robotic low anterior resection (R-LAR) and laparoscopic low anterior resection(L-LAR) for rectal cancer.Methods The case-control studies for comparing the clinical efficacy of R-LAR and LLAR for rectal cancer were retrieved from databases,including PubMed,OVID,Springer,Wanfang and VIP,till June 2016.The studies were selected in accordance with inclusion and exclusion critera,and the data were extracted and evaluated by two researchers.The RevManS.3 software was used for Meta-analysis.Results A total of 7 studies including 1 126 patients with rectal cancer were included,592 cases in the R-LAR group and 534 cases in the L-LAR group.Compared with the L-LAR group,in the R-LAR group the operation time was longer (MD=33.84,95% CI:4.25-63.43,P =0.03),the conversion rate was lower (OR =0.10,95%CI:0.03-0.36,P=0.000 5),and more lymph nodes were dissected (MD=1.24,95%CI:0.31-2.17,P=0.009).No statistically significant difference was found in length of hospital stay,intraoperative blood loss,distance from distal margin to tumor and incidence rate of postoperative anastomotic leakage between the two groups (P > 0.05).Conclusion Compared with L-LAR,RLAR is characterized by longer operation time,lower conversion rate and more thoroughly removing lymph nodes.And the length of hospital stay,intraoperative blood loss,distance from distal margin to tumor and incidence rate of postoperative anastomotic leakage of R-LAR are similar to those of L-LAR.
