1.Study on ligustrazine combined with chemotherapy in patients with relapsed or refractory non-Hodgkin's lymphoma

Objective To explore the efficacy of ligustrazine combined with chemotherapy in patients with relapsed or refractory NHL and the relationships with P-gp expression. Methods 60 Patients were randomized into reversal agent group (ligustrazine + chemotherapy) and control group (chemotherapy) and the outcome was analysed. P- gp expression was analysed by flow cytometry. Subgroup analysis was conducted a ccording to P-gp expression. Results 56 patients could be evaluated. Progression -free survival (PFS) showed no statistical diference in two groups ( P = 0.065 1 ) ; reversal agent group had a higher overall response rate (ORR) than control group(P=0. 048).Subgroup analysis showed 41 patients had P-gp( +). The complete remission rate and complete remission uncertain rate of patients with P-gp( + ) in reversal agent group and control group were 33. 33% and 8. 70% , respectively, reversal agent group had a significant advantage ( P = 0. 048). Patients with P-gp( + ) in reversal agent group had a higher ORR than control group (61. 11% vs 26. 09% ,P =0.024). Kaplan-Meier survival curve and Log-rank test showed patients with P-gp( + ) in reversal agent group had longer PFS than control group (P =0.046 4). Blood pressure in a small number of patients received ligustrazine therapy decreased,and mitigated after reducing the dose. Conclusion Ligustrazine combined with chemotherapy can elevate response rate,prolong PFS with manageable toxicity and correlate to P-gp expression in relapsed or refractory NHL

3.Analysis of the effect of two techniques on sphincter saving procedure for treating low rectal carcinoma

ObjectiveTo investigate the clinical effect of the double stapling technique for anus-preserved (Dixon) and Miles procedure of rectal carcinoma. MethodsFifty-eight cases were divided into 29 cases as control group and 29 cases as observation group by digital table method. The distance of their carcinoma was 4-7 cm to anus. The control group was treated with Miles procedure of rectal carcinoma, the observation group was treated with double stapling technique for Dixon. ResultsThe operation time of observation group was ( 180 ± 56) min, the control group was (240 ± 73)min(P < 0.05). Five-year survival rate of observation group was 77.9%, the control group was 79.3% (P >0.05);the recurrence rate of observation group was 6.9%(2/29),the control group was 6.9%(2/29) (P > 0.05). In the observation group,nobody suffered encopresis after 1 year,postoperative stool frequency was ≤5 times/d,while the control group occurred encopresis gradually. ConclusionIf the distance of carcinoma is 4-7 cm to anus, the way of the double stapling technique for Dixon can get the similar short term effect with the way of Miles, and can get more ability to control it.

Short course chemotherapy combined with local field radiotherapy (IFRT) is an important treatment for limited-stage (Ⅰ -Ⅱ)diffuse large B cell lymphoma patients,but the value of radiotherapy has been disputed.Some patients may be cured by combined chemotherapy which contains anthracycline,but there are some patients still facing death.Therefore,researchers try to improve survival of patients further by increasing the density or intensity of the chemotherapy dose,but attempt to reduce the dose or the use of low toxicity drugs for elderly patients.In addition,researches such as radioimmunotherapy and biological targeted therapy have made certain progress.

MicroRNA (miRNA) is a kind of naturally occurring short non-coding small molecular RNA,which regulates gene expression at the posttranscriptional level through its sequence complementation with target mRNAs.Researches have shown that different malignant tumors,including diffuse large B cell lymphoma,have specific miRNA expression profiles.Therefore,miRNAs expression level of specimens of body fluids and tissues is expected to be the new indicator for assessing diffuse large B cell lymphoma.

The discovery of microRNA (miRNA) has opened up a new train of thought for the diagnosis and treatment of esophagus cancer.let-7 is one of the most widely research of miRNA.In a variety of tumors,the expression of let-7 is down-regulated.let-7 can play the role of tumor suppressor gene by targeting to high mobility group A2 (HMGA2) and inhibit the cell proliferation.Recent studies have shown that the lower the degree of differentiation of cells,let-7 expression level is lower.It is expected as a symbol of poorly differentiated tumors.In addition,let-7 and esophagus cancer's radiation and chemotherapy sensitivity are closely related.
7.Analysis of clinicopathologic characteristics of 520 patients with lymphoma in Shandong

Objective:To analyze the pathologic types and clinical characteristics of lymphoma in Shandong, China. Methods:The clinical data of lymphoma patients admitted from September 2011 to September 2014 in Shandong Cancer Hospital and the Affiliat-ed Hospital of Shandong Academy of Medical Sciences were retrospectively analyzed. Data included age, sex, pathologic classification, and onset locus of lymphoma. Results:The clinical data of 520 lymphoma cases were collected. The ratio of male to female was 1.2:1, and the median age was 51 years old. Among these patients, 67 cases (12.9%) were Hodgkin's lymphoma (HL), whereas 453 cases (87.1%) were non-Hodgkin's lymphoma (NHL). Among NHL cases, 75.9% were B-cell NHL and 24.1% were T/ natural killer-cell NHL. Nodular sclerosis classical Hodgkin's lymphoma was the most common pathologic type of HL, comprising 52.2%. The age of HL onset did not present twin peaks. In NHL, 245 cases (54.1%of NHL) were diffuse large B-cell lymphoma (DLBCL). The constituent ra-tio of DLBCL was higher than in other countries and regions (18%to 42.7%), in which the ratio of male to female was 0.9:1, and the age of onset was slightly earlier (median age was 54 years old). Lymph nodes in the cervical region were the most commonly involved in the disease, and NHL extranodal onset was frequently observed. Conclusion:Among the 520 cases of lymphoma, DLBCL is the most common subtype. Most subtypes show male predominance, but female predominance was observed for DLBCL. The commonly involved sites of extranodal lymphoma are the nasal cavity and gastrointestinal tract.
8.Effect of astrocyte-derived estrogen on the regulation of the synaptic transmission in rats

Objective To study the effect and the mechanism of astrocytes-derived estrogen on synaptic transmission. Methods Based on the model of pure cultures of neonatal cortical neurons, the experimental groups were designed as follows: pure neuron culture (group P), ACM culture (group A), mixed culture of AST and neuron (group M), E_ 2 culture (group E), ACM+tamoxifen (estrogen receptor antagonist) culture (group A+T ), tamoxifen culture (group T). The difference among these groups of synaptic transmission was recorded by patch clamp and presynaptic vesicle releasing kinetics was marked by a dye named FM4-64. Results The mean amplitude and frequency of miniature postsynaptic currents (mPSCs) of group P, A, M, E, A+T and T were: (20.5?2) pA, (13?4) min -1 ; (29.1?3) pA, (73?16) min -1 ; (31.3?3) pA, (78?20) min -1 ; (30.2?3) pA, (76?18) min -1 ; (24.5?2) pA, (35?10) min -1 ; (22.1?2) pA, (37?10) min -1 respectivily. The treatment of pure neuron culture with ACM increased synaptic transmission of pure cultured cortical neuron. Exogenic estradiol added into pure neurons can stimulate the effect of the ACM. Tamoxifen which was the antagonist of estrogen receptor could decrease most effect of ACM on synaptic transmission. The assaying of vesicle release marked by FM4-64 showed astrocytes-derived estrogenic enhancing synaptic transmission was at least mediated by strengthening presynaptic vesicle releasing. Conclusion The estrogen from astrocytes of neonatal rat cortex may enhance neuronal synaptic transmission mainly through estrogen receptor.
9.Expression and clinical significance of Survivin and MHC in non-small cell lung cancer

Objective To explore the expression of Survivin and MHC in non-small cell lung cancer (NSCLC), and their interaction.Methods A total of 40 patients with histologically diagnosed NSCLC were enrolled in this study.Control samples were consisted of normal lung tissues from 15 patients.Expression of Survivin and MHC were detected by flow cytometry in 40 non-small cell lung cancer tissues and 15 normal lung tissues.Results The positive rates of Survivin protein expression in NSCLC tissues classified stages Ⅰ,Ⅱ and Ⅲ were 28.57% ,30.77% and 80.00% respectively.There was a correlation between Survivin protein expres-sion and stages of NSCLC.Survivin protein expression was detected in 19 of 29 patients with lymph node metas-tasis, and 3 of 11 patients with no metastasis.There was a statistically significant difference between the two groups (P<0.030).The loss expression rates of MHC-Ⅰ in NSCLC tissues of low grade,intermediate grade and high grade were 84.62% ,42.10% and 37.50% respectively.There was a correlation in expression be-tween MHC-Ⅰ and tumor grade.The positive rates of MHC-Ⅱ expression in NSCLC tissues was related to pa-thology type (P=0.005).The expression in squamous cancer and non-squamous non-adno cancer was lower than that of adenocarcinoma (P=0.002, P=0.04).There was no obvious correlation between the expression of Survivin and MHC-Ⅰ,MHC-Ⅱ in NSCLC.Conclution The expression of Survivin and MHC could be in-volved in the pathogenesis and development of NSCLC, and the combined detection will predict the prognosis of the patients with NSCLC.
10.Effects of the expression of KAI1 mRNA on patients with gastric cancer

Objective To investigate the relationship between the expression of KAI1 mRNA and the clinicopathological parameters and prognostic outcome of patients with gastric Callcer. Methods The expressions of KAI1 mRNA in 70 samples of gastric cancer tissue and 18 samples of benign gastric lesion were detected by in situ hybridization.Results The positive rates of KAI1 mRNA expression in benign gastric lesion and gastric cancer tissue were 94%(17/18)and 31%(22/70),respectively.The expression of KAI1 mRNA was influenced by tumor difierentiation degree and the state of lymph node metastasis.The positive rate of KAI1 mRNA expression was gradually decreased as the increase of invasion depth and the tumor clinical stages.The 5-year survival rate of patients with positive expression of KAI1 mRNA WaS higher than these with negative expression of KAI1 mRNA.Conclmions The less and down-regulation of KAI1 gene expression may be involved in the occurrence and development of gastric cancer.KAI1 gene might be a valuable indicator for early diagnosis and predicting the malignancy and metastasis potential of gastric cancer and the prognosis of patients with gastric cancer.
