1.Factors influencing dry blood specimens of filter paper in newborn screening assay

Objective To investigate factors influencing thyroid stimulating hormone (TSH) and phenylalanine (phe) assay in dry blood spots on filter paper. Methods The influence on enzyme linked immunoassays and bacterial inhibition assay were studied so as to control their changes. Results Many influencing factors were noted in newborn screening assay. TSH results were stable if specimens were collected form newborns 72 hours after delivery. Different results were obtained when TSH was tested in different seasons. Specimens were air dried at room temperature before transportation. To reduce false positive phe result, specimens were collected from newborns 72 hours after delivery. The phe result was very stable with a plating temperature of 55 60℃ and speradical distribution of blood spots . Conclusion To assure the quality of screening the time of specimens collecting, different cut off, blood spots disposition and best condition should be controlled.

Short course chemotherapy combined with local field radiotherapy (IFRT) is an important treatment for limited-stage (Ⅰ -Ⅱ)diffuse large B cell lymphoma patients,but the value of radiotherapy has been disputed.Some patients may be cured by combined chemotherapy which contains anthracycline,but there are some patients still facing death.Therefore,researchers try to improve survival of patients further by increasing the density or intensity of the chemotherapy dose,but attempt to reduce the dose or the use of low toxicity drugs for elderly patients.In addition,researches such as radioimmunotherapy and biological targeted therapy have made certain progress.
3.Staging treatment of severe complicated injuries in pahnar forearm and wrist

Objective To investigate the therapeutic effect of stage treatment of severe complicated injuries in palmar forearm and wrist.Methods From March 2008 to Novemler 2011, eleven cases with skin lost combined with fracture and lost of flexor tendon, ulnar radial arteries, median and ulnar nerves,and some of them had fracture of ulnar and radius were treated in the first stage with vacuum sealing drainage after emergency debridement and tissue repair. Secondary microsurgical repair was performed after the VSD was removed. The lost of median and ulnar nerves was repaired with transplantation of sural nerve,and the wound was repaired with free flaps and complex tissue flaps. Results Eleven cases were treated with vacuum sealing drainage for 7 to 10 days.Infections were under control,wound areas reduced and most wound areas were covered by granulation tissues.Flaps and skin grafts were survived.After postoperative follow-up for 3 to 36 months (average 13 months),three cases was received flexor tendolysis.As result,the blood circulation of hand was good, the sensor of injured nerve was higher than S3, and there was a good functional rehabilitation of hand muscles. Conclusion Using vacuum sealing drainage after emergency debridement and tissue repair, there was a good repairing effect of secondary repair with flaps and skin grafts for severe complicated tissue defect in palmar forearm and wrist,and it is an effective choice of operation.
4.Application of retrograde intra-enteropexy through appendix stump for adhesive ileus

Objective To investigate operative method for preventing and treatment intestinal obstruction due to relapse adhesion.Methods Sixty patients with recurrent adhesive ileus treated by surgery were randomized into two groups,that was intra-enteropexy group and extra-enteropaxy group.Twenty-three patients in intra-enteropexy group were treated by retrograde intra-enteropaxy through appendix stump.Thirty-seven patients in the extra-enteropexy group were treated by Noble procedure.The effect and complication were compared.Results In the intra-enteropexy group,2 cases (8.70%) were complicated with incision infection,1 case was died (4.35%) and there was no relapsed,abdominal spastic pain and incomplete intestinal obstruction occurred.In the extra-enteropexy group,3 cases (8.11%) were complicated with incision infection,3 cases were died (8.11%),abdominal spastic pain were occurred in 11 cases (29.73%),incomplete intestinal obstruction were occurred in 6 cases(16.22%),and there were 3 cases run into relapse.Conclusion The retrograde intra-enteropexy through appendix stump is an effective method for preventing and treatment to adhesive ileus.
5.Effect of Intensive Insulin Therapy on Inflammatory Level of Biliary Pyemia

Objective To explore the possible anti-inflammatory mechanism of intensive insulin therapy (IIT) by studying the effect of IIT on the levels of TNF-?,IL-6,C-reactive protein (CRP) and APACHE Ⅱ score in biliary pyemia.Methods Twenty eight patients with biliary pyemia who were admitted by our department and given an operation within 24 h form Jan.2005 to Dec.2008 were randomly divided into two groups by using random number table numbers:one group treated with IIT (IIT group,n=14) and another group treated with routine insulin therapy (RIT group,n=14).The inflammatory factors,such as TNF-?,IL-6 and CRP were detected dynamically and the APACHEⅡ score was calculated.Results The level of CRP and APACHEⅡ score on day 5 and 7 and the levels of TNF-? and IL-6 on day 3,5 and 7 after operation in IIT group were significantly lower than those in RIT group (P

Objective To compare the value of transrectal ultrasonography(TRUS) and magnetic resonance(MR) in preoperative estimation of circumferential resection margin (CRM) of mid-low rectal cancer.Methods Both TRUS and MR were performed prior to surgery in 111 patients with mid-low rectal cancer.The CRM was evaluated and compared with the postoperative pathologic findings.Results The accuracy of TRUS and MR for CRM was 91.9%(102/111) and 85.6%(95/111),respectively,there was no statistical difference between them (P =0.137).Their specificities were 94.0% (79/84)and 84.5% (71/84),there was statistical difference between them(P =0.046).The consistency of the results between TRUS and pathology was better than that between MR and pathology(Kappa1 =0.783,Kappas =0.652).Conclusions TRUS is proved to have an important diagnostic value for CRM of mid-low rectal cancer.
7.Fluorescence tracing of umbilical cord blood mesenchymal stem cells in transplantation treatment of scalded wound in mice

Objective Using the previously established mesenchymal stem cells strain derived from human fetal umbilical cord blood (FUCB-MSCs) to culture then label enhanced green fluorescent protein (EGFP) , and to observe skin repair effects of FUCB-MSCs by GFP tracing after exogenous FUCB-MSCs transplantation on to scald wound models of SCID mice.Methods FUCB-MSCs were labeled GFP by transfection with the recombinant retrovirus containing EGFP gen;The established SCID scald mice model were randomLy divided into 3groups, low dose group, high dose group and control group, 6rats each group, 2wounds each mouse, 12wounds in total, then were tail intravenous injected into 0.2mL 1×106, 0.2mL 2×106 GFP-FUCB-MSCs cells, and same volume of medium respectively.On 9days after transplantation, the sections from scald wound area were observed the expression of GFP under the fluorescence microscope and the others were analyzed by the bright-field microscopy after HE staining, and the area of wound surface and the number of wound cells were compared simultaneously.Results After 48h, expression of EGFP in FUCB-MSCs can be seen under the fluorescence microscope, positive rate of GFP was>80%, and after 6weeks GFP expression is still stable, besides, the positive expression of human GFP can be observed after transplantation and there were no fluorescence decay in transplantation after 3weeks.Compared with the control group, there was a significant difference in wound area and wound cell number in the low and high-dose group (P<0.05) .ConclusionGFP can be used as a tracking marker to label FUCB-MSCs during transplantation treatment.It indicates that exogenous FUCB-MSCs can migrate to the scalded wounds via blood circulation system and continuously participate in the repair through SCID mouse.
8.Prevalence of overweight and obesity in Chinese middle-aged populations: Current status and trend of development.

OBJECTIVETo understand the current status on prevalence of overweight and obesity in Chinese middle-aged population, and to quantify the magnitude of the trends of development in the past two decades.
METHODSBody mass index (BMI) was measured on 15 389 middle-aged men and women from 15 natural populations all over China with different geographical, economical (urban/rural) and occupational status. Overweight was defined as BMI >/= 25 while obesity as BMI >/= 30.
RESULTS1) The rates of prevalence on overweight and obesity varied dramatically between populations. 2) In general, the prevalence rates were higher in the northern areas, in urban areas, and in women. 3) The current problem of obesity was related to 'pre-obese' stage, with less than 10% in most populations. 4) The prevalence of overweight and obesity started to show significantly increase from early 80's to early 90's but more prominant in the late 90's.
CONCLUSIONPrevention and intervention of overweight and obesity are in urgent need in the Chinese populations, especially in those economically fast developing areas.

9.Prenatal genetic diagnosis of increased fetal nuchal translucency in two pregnancies of a woman from polycystic kidney disease pedigree

This paper reported a woman with polycystic kidney disease who had increased fetal nuchal translucency (NT) in her two sequential pregnancies. The fetal NT thickness in the first pregnancy was 5.1 mm at 12 +5 weeks of gestation, and the infant was born prematurely at 32 gestational weeks. However, the baby girl died due to respiratory insufficiency and severe asphyxia. The NT thickness in the present pregnancy was 5.7 mm at 12 weeks of gestation. Whole-exome sequencing (WES) and Sanger sequencing confirmed that the dead infant and the current fetus carried compound heterozygous variants of maternal c.4255_4256del and paternal c.18366+2T>C in NEB gene, both were pathogenic variants. The current fetus was diagnosed with arthrogryposis multiplex congenita 6 (AMC6). After genetic counseling, the pregnant woman chose to terminate the pregnancy. The pregnant woman was diagnosed as having polycystic kidney disease type 1 caused by large deletions in exons 25-43 of PKD1 gene by WES combined with multiplex ligation-dependent probe amplification technology.
