Objective To discuss the technique of supraorbital transeyebrow keyhole approach microsurgery for sellar tumors. Methods A total of 33 cases of sellar tumors were treated in this hospital from July 2001 to June 2004. The resection of the tumor was achieved by way of a supraorbital eyebrow minicraniotomy through a 2.0 cm ? 3.0 cm keyhole approach. The surrounding cisterns of the sellar area were opened to expose underlying structures under microscope at proper angles. Results Of 26 cases of hypophysoma, a total resection was performed 19 cases, and a subtotal resection in 7 cases. Of 4 cases of craniopharyngioma, a total resection was performed 3 cases, and a subtotal resection, 1 case. One case of meningioma received a total resection, and 1 case of optic chiasm glioma underwent a partial resection. Postoperatively, transitory diabetes insipidus was observed in 5 cases, and grand mal epilepsy occurred in 1 case. There were no infection or bleeding after the operation. All the 33 cases were followed for 4~36 months (mean, 27 months). No recurrence of tumors was seen in the 24 cases of total resection. Postoperative radiotherapy was given in 7 cases of subtotal resection of hypophysoma, 1 case of craniopharyngioma, and 1 case of optic chiasm glioma, and no apparent enlargement of tumors was found. Conclusions Supraorbital transeyebrow keyhole approach microsurgery minimizes the unnecessary exposure of the brain and postoperative complications.

Chitosan, a linear polysaccharide, is obtained by chitin N deacetylation. Its ?(1 4) glucosidic bond is not stable and breaks easily in acid condition. In this study, chitosan of different molecular weights(MW) was prepared by acetic acid degradation. Chitosan was dissolved at 3%(w/v) concentration in 1%(v/v) acetic acid and the solution was dried at 50 degree C. We get chitosan membranes of different MW after neutralization with aqueous NaOH. The membranes are transparent, elastic and have good intensity. The permeability studies on chitosan membranes were going on with a permeable install made by our laboratory. It was determined by the transmission concentration of NaCl, L Tyr, glucose and calf albumin. We compared the permeability property with different MW chitosan membranes. The result implied that all of the four samples could permeate through the chitosan membranes. And the permeability property of the membrane increased with the chitosan MW decreaseing.

Objective To introduce the clinical application of the extended anterolateral leg flap, which is based on the superficial peroneal artery and lateral supramalleolar artery. Methods Through anatomic study on the blood supplies of the skin over the anterolateral portion of the leg.The combined superficial peroneal artery and lateral supramalleolar artery could extend the size of the traditional anterolateral leg flap.In addition, the point of pivot of the extended anterolateral leg flap could locate at the level of the lateral malleolus, which decreases the distance between the flap and recipient site.Clinically,we have used the extended anterolateral leg flaps in 60 patients to cover skin defect over the feet. The flap ranged from 16 cm×10 cm(pedicle length 8.0 cm)to 26 cm×7 cm(pedicle length 6.0 cm). Results In the series, the flap survived in 53 patients(88.3%)unevently,and partial necrosis occurred in 7 patients(11.7%).Conclusion Based on the combined superficial peroneal artery and lateral supramalleolar artery, the size of the traditional anterolateral leg flap could be inceased, which enlarges its value on the coverage of skin defects over the feet.
