Chinese Journal of Practical Nursing 2014;30(19):41-43
Objective To comprehend the application of intravenous transfusion instrument in grade three hospitals of GuangZhou, and promote the use of trochar. Methods Using the questionnaire survey,we investigated the application of intravenous transfusion instrument in 12 grade three hospitals within 24 hours on October 10th to 11th in 2012,and then collected all data for analysis. All personnel participated in the investigation were given training to ensure the accuracy of the data collection. Results 9 732 patients needed to receive intravenous therapy, among whom 3 866 patients chose scalp acupuncture and 4 195 patients chose vein detained needle. We divided all the patients who had scalp needle and vein detained needle by age.The utilization of vein detained needle was higher than scalp acupuncture in the patients of <19 years old and >70 years old,but the utilization of scalp acupuncture was higher than vein detained needle in the patients of 19~70 years old.The difference of choosing intravenous transfusion instrument among different age groups had statistical significance. Conclusions Patients of 19~70 years old are in young and maturity phase, middle age,old age.The compliance of them is lower than other patients. They think the vein detained needle influence their daily activities and individual image, and also worry about infection. So they do not actively use vein detained needle, except when having poor situation in peripheral venous.Nurses are lack of health education, and fail to allow patients actively choose vein detained needle.

Objective To investigate the relationship between aggrecan and YKL-40 in knee articular cartilage of Sprague-Daw-lay(SD) rats with osteoarthritis (OA) .Methods Fifty-six healthy SD rats were randomly divided into 7 groups ,8 cases per group . The one side of knee joint was randomly selected for performing the anterior cruciate ligment transection (ACLT) and establishing the OA model .The rats in one group were randomly killed on the day of operation and at postoperative 0 ,2 ,4 ,8 ,12 ,16 ,20 weeks . The femoral condyle cartilage samples at different time periods in the operated side were collected for conducting safranin O /fast green staining and HE staining .Meanwhile ,the OA pathological grade was made out according to the modified Mankin scale .The expression of aggrecan and YKL-40 in the cartilage with different stages of OA were analyzed by the immunohistochemistry meth-od ,and the status of expression were measured by average optical density (AOD) .The correlation between aggrecan and YKL-40 was analyzed .Results With the aggravation of OA ,the expression of aggrecan was gradually reduced and the expression of YKL-40 was gradually increased .The differences during the early ,middle and late phases of OA had statistical significance (P<0 .05) . The expression of aggrecan was negatively correlated with the expression of YKL-40(P<0 .05) .Conclusion The level of aggrecan is gradually reduced with the aggravation of OA .Aggrecan is negatively correlated with the YKL-40 level ,which may reflect the dedifferentiation degree of joint chondrocyte to some extent .

Objective To investigate the changes in cortisol (COR) secretion in the acute phase of traumatic brain injury (TBI) .Method Seventy-five patients admitted to the hospital at 2-24 h after TBI were divided into 3 groups based on the Glasgow Coma Scale score: mild TBI group (group TBI1, n = 30), moderate TBI group (group TBI2, n = 12) and severe TBI group (TBI3, n = 33). Thirteen patients with cervical spondylosis or osteoma of the skull (admitted to the hospital at the same period) were regarded as control group (group C). Venous blood samples were taken on the first day after admission to measure the serum concentrations of total COR, adrenocorticotropin (ACTH) and corticosteroid-binding globulin (CBG). Free COR concentrations and free COR index were calculated. High blood COR was recorded. Result Compared with group C, the serum concentrations of total COR and ACTH, free COR levels and free COR index were significantly increased in TBI1, TBI2 and TBI3groups (P < 0.05). The parameters mentioned above were significantly higher in TBI2 and TBI3 groups than in TBI1 group ( P <0.05). There was no significant difference in serum CBG concentrations among the four groups. The incidence of high blood COR was significantly higher in TBI1, TBI2 and TBI3 groups than in C group, and in TBI3 group thanin TBI1 and TBI2 groups (P <0.05). Conclusion COR secretion is increased in the acute phase of TBI and the level of COR secretion is related to the severity of brain damage.

[ABSTRACT]OBJECTIVETo study the effects of Arsenic Trioxide on the growth of human nasopharyngeal carcinoma cell strain CNE-2Z xenograft in nude mice, and explore the possible mechanisms of the antitumor effect. METHODSThe models of human poorly differentiated nasopharyngeal carcinoma in nude mice were established and randomly divided into 4 groups (control group, As2O3 group, DDP group and As2O3+DDP group). The antitumor effect of every group was studied. Pathological changes of tumor tissues dyed by HE staining were observed with light microscope. The pathological changes of the heart, liver, lung and kidney from nude mice were also observed by HE staining. Blood routine test was examined. Expression of PCNA and Ki-67 were detected by immunohistochemistry method.RESULTSThe growth of tumor in As2O3, DDP and As2O3+DDP were obviously inhibited. No pathological changes and metastases were found in heart, liver, lung and kidney of the nude mice. No obvious toxicity of the treatment to the hematopoietic systems in the nude mice was observed. The expression of PCNA and Ki-67 were down-regulated.CONCLUSIONAs2O3 can inhibit the growth of human nasopharyngeal carcinoma cell strain CNE-2Z xenograft in nude mice, the mechanism of which might be related to the down-regulation of PCNA and Ki-67.

In order to improve the success rate of supraclavicular deep venous catheterization and reduce mechanical complications, we present an auxillary maneuver in regard to supraclavicular subclavian catheterization basing on the relatively fixed anatomy of subclavian vein and its adjacent surroundings, furthermore, we revised the standardized procedure of supraclavicular subclavian catheterization. The maneuver is summarized in the shape of verses (verses: thumb navigation is well designed according to anatomy. Needle penetrated into vein should be parallel to coronal plane. Fine needle in position should be immobilized. Is it difficult for parallel puncture? Pressure determination is required when needle is in place. It is critical to distinguish which vessel has been inserted. Guidewire is advanced smoothly. Check blood return after expansion of skin and catheterization.). For teaching convenience, verses are considered to be more concise and memorable, as well as applicable to clinical practice, in order to provide some help for clinical teaching.
